1.Evaluation of Taraxacum mongolicum Germplasm Resources Based on Characteristic Profiles and Chemical Markers

ObjectiveBased on the established characteristic profiles, quantitative analysis of multiple components, and chemometric analysis of Taraxacum mongolicum, the quality of different T. mongolicum germplasms was evaluated at the chemical level, thereby providing a reference for the screening of high-quality germplasms and the rational utilization of wild resources. MethodsAn ultra-performance liquid chromatography (UPLC) was employed to establish characteristic profiles. Principal component analysis (PCA) and partial least squares-discriminant analysis (PLS-DA) were then adopted to screen and comprehensively rank marker compounds. ResultsThe UPLC fingerprint of T. mongolicum germplasm identified 13 chromatographic peaks corresponding to gallic acid, coumaric acid, neochlorogenic acid, monocaffeoyltartaric acid, chlorogenic acid, cryptochlorogenic acid, caffeic acid, p-coumaric acid, cichoric acid, luteoloside, isochlorogenic acid B, isochlorogenic acid A, and isochlorogenic acid C. Combined with chemometric analysis such as PCA and PLS-DA, eight core markers (cichoric acid, luteoloside, cryptochlorogenic acid, isochlorogenic acid B, chlorogenic acid, caffeic acid, isochlorogenic acid C, and isochlorogenic acid A) were screened for distinguishing wild and cultivated germplasms. Additionally, eight core markers (cichoric acid, caffeic acid, luteoloside, chlorogenic acid, cryptochlorogenic acid, isochlorogenic acid A, monocaffeoyltartaric acid, and neochlorogenic acid) were selected for the evaluation and screening of different T. mongolicum germplasms. ConclusionThis study establishes a UPLC analysis method capable of simultaneously determining 13 characteristic components in T. mongolicum, such as cichoric acid and chlorogenic acid, as well as their precursor compound contents in the biosynthetic pathway. Based on the above methods, three T. mongolicum germplasms (PGY-004, PGY-009, and PGY-010) with promising medicinal potential are selected for subsequent research on variety breeding. The present study provides a reference for quality control of Taraxacum mongolicum, germplasm screening, and the rational development and utilization of wild resources.
2.Research progress of red light therapy for dry eye and visual fatigue

Dry eye disease(DED)is a common ocular surface disorder worldwide, primarily characterized by a loss of homeostasis of the tear film, and frequently associated with meibomian gland dysfunction(MGD), decreased tear film stability, ocular discomfort, and visual impairment. In recent years, factors such as the widespread use of digital devices,the aging population, and environmental changes have contributed to a significant increase in its global prevalence, making it a major public health concern. Red light therapy(RLT), also known as low-level laser therapy(LLLT)or photobiomodulation(PBM), is a non-invasive treatment that utilizes low-energy red or near-infrared light to irradiate tissues. It exerts photobiomodulatory effects to promote cellular repair and functional recovery. This therapy has demonstrated considerable potential in treating various ocular conditions. Its broader clinical application could improve therapeutic outcomes, alleviate patient discomfort and financial burden, and reduce the consumption of healthcare resources, thereby yielding significant socio-economic benefits. This paper systematically reviews the multifaceted mechanisms and application prospects of RLT in managing DED, including its anti-inflammatory effects, improvement of meibomian gland function, promotion of conjunctival goblet cell repair, and alleviation of visual fatigue, aiming to provide a theoretical foundation and practical reference for its clinical adoption.
3.Effect of Pibai Yucuo Formula (枇柏愈痤方) on Inflammatory Response in Lesional Tissue and Skin Barrier Damage in Acne Model Mice

ObjectiveTo investigate the possible mechanism of Pibai Yucuo Formula (枇柏愈痤方, PYF) in treating acne from the perspective of skin barrier damage. MethodsThirty-two mice were randomly divided into blank group, model group, minocycline group, and PYF group, with 8 mice in each group. Except for the blank group, mice were induced by intradermal injection of Cutibacterium acnes (C.acnes) combined with topical application of artificial sebum to establish acne model. The blank group and model group received intragastric administration of 0.2 ml of distilled water, while the PYF group received intragastric administration of 22.75 g/(kg·d)of PYF, and the minocycline group received 0.013 g/(kg·d)of minocycline suspension, all once daily for 5 consecutive days. On day 0 and day 6 of the experiment, the body weight of mice in each group was recorded, and the absolute value of the body weight difference during the experiment was calculated. Skin conditions were assessed with multifunctional skin imaging system on the 2nd, 4th and 6th day of the experiment. Skin barrier function indicators including transepidermal water loss (TEWL), and the water content of the stratum corneum and epidermis on day 0, 2, 4 and 6 of the experiment. Optical coherence tomography (OCT) was used to observe stratum corneum and skin thickness on the 1st, 3rd and 5th day of the experiment. Hematoxylin-eosin (HE) staining was performed to observe histopathological changes, while ELISA was used to detect interleukin-17A (IL-17A) levels, and immunofluorescence staining was used to assess skin barrier-related proteins filaggrin (FLG) and loricrin (LOR) levels of skin lesions on day 6 of the experiment. ResultsCompared to the blank group, the model group showed a decrease in body weight on day 6, and an increase in the absolute value of the difference in body weight before and after the experiment (P<0.05). On day 4 and 6, TEWL values increased, while water content in the skin stratum corneum and epidermis decreased (P<0.05), accompanied by elevated IL-17A level and reduced immunofluorescence intensity of FLG and LOR proteins (P<0.05). The model group mice showed papules or pustules at the skin modeling site with progressively worsening desquamation under multifunctional skin imaging system. OCT revealed focal epidermal protrusions, blurred epidermal-dermal boundaries, and disorganized structural layers. HE staining showed significant epidermal hyperkeratosis and incomplete keratinization in the skin, with keratin plug formation in hair follicles and glandular lumens, thickened stratum corneum, hyperplasia of the stratum spinosum, as well as dense dermal inflammatory cell infiltration, and capillary dilation. Compared to the model group, both the minocycline group and the PYF group showed a reduced difference in body weight before and after experiment (P<0.05). On day 4 and 6, the TEWL value decreased, and water content of the skin stratum corneum increased (P<0.05); on day 6, the IL-17A level in the skin lesions decreased and immunofluorescence intensity of FLG and LOR proteins increased (P<0.05). On day 4 and 6, the severity of the skin lesions and range of redness and swelling were lighter than those in the model group, with reverted epidermal thickness, smoother surface and clearer epidermis-dermis boundary. HE staining showed that the degree of skin keratinization was reduced, and the inflammatory infiltration and vascular dilation in the dermis were improved compared to the model group. The PYF group showed better results than the minocycline group in reducing TEWL value on day 4 (P<0.05). ConclusionPYF may improve inflammation and skin barrier damage by downregulating IL-17A levels in lesion tissue and increasing skin barrier-related proteins, which could be one of the potential mechanism of action on acne.
4.Preparation and properties of degradable and high-porosity sodium alginate/organic composite calcium salt porous scaffolds for tissue engineering

A porous composite scaffold is prepared through freeze-drying method using sodium alginate as the matrix and calcium citrate double salt as the functional enhancer.To thoroughly investigate the structure and properties of the composite scaffold,advanced instruments including Fourier transform infrared spectrometer,X-ray diffractometer,scanning electron microscope,and thermogravimetric analyzer are utilized for comprehensive structural characterization.Meanwhile,key properties such as porosity,mechanical performance,swelling ratio,gel fraction,and in vitro degradation rate are systematically tested and analyzed.The results show that the composite of sodium alginate and calcium citrate double salt is a physical complex.The porosity of the scaffold mainly ranges from 200 to 600 μm,and as the addition of double salt increases,the pore size firstly increases and then slightly decreases,while the pore distribution become more uneven.In terms of mechanical properties,with the gradual increase of the content of double salt in the scaffold,the mechanical properties of the scaffold are significantly improved.However,its swelling ratio and gel fraction exhibit decreasing trends.Additionally,the degradation rate of the alginate scaffold is relatively fast,while the addition of calcium citrate double salt does not significantly improve the degradation rate.This study not only provides valuable insights into the preparation and property research of porous composite scaffold composed of sodium alginate/calcium citrate double salt,but also offers important reference for their potential applications in biomedicine.
5.Observation on the therapeutic effect of transcutaneous electrical nerve stimulation combined with mobilization with movement in the treatment of knee osteoarthritis

Objective To investigate the effects of transcutaneous electrical nerve stimulation(TENS)combined with mobilization with movement(MWM)on clinical efficacy,musculoskeletal ultrasound findings and bone metabolism indicators in patients with knee osteoarthritis(KOA).Methods Ninety KOA patients were collected and randomly divided into the control group(conventional rehabilitation+TENS,n=45)and the observation group(conventional rehabilitation+TENS+MWM,n=45)using a random number table.Clinical efficacy,knee joint function,musculoskeletal ultrasound indices and bone metabolism indices were compared between the two groups.Results The clinical efficacy of the observation group was superior to that of the control group(P<0.05).At 2 and 6 months post-treatment,Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)were significantly lower in the observation group than those of the control group,and Lysholm Knee Scoring Scale scores were higher in the observation group than those of the control group(P<0.05).Additionally,articular cartilage thickness,bone gla protein(BGP)and bone-specific alkaline phosphatase(BALP)were higher in the observation group than those of the control group,and synovial thickness,suprapatellar effusion and tartrate-resistant acid phosphatase 5b(TRACP-5b)were lower in the observation group than those of the control group(P<0.05).Conclusion TENS combined with MWM in the treatment of KOA patients can not only improve knee joint function but also optimize musculoskeletal ultrasound indices and regulate bone metabolism indices.
6.Evaluation of the therapeutic effects of peroral super minimally invasive incision for esophageal diverticulum

Objective:To evaluate the efficacy of peroral super minimally invasive incision for esophageal diverticulum.Methods:The clinical data of patients with esophageal diverticulum who underwent super minimally invasive surgery (SMIS) at the First Medical Center of the PLA General Hospital from April 2022 to September 2024 were retrospectively analyzed. These data include clinical baseline data, endoscopic surgical parameters, preoperative and postoperative Eckardt scores, surgical costs, and the duration of hospitalization.Results:Thirteen patients successfully completed submucosal tunneling endoscopic septum division (STESD) surgery without any postoperative adverse events. The duration of operation was (37.00 ± 5.82) min, the application time of proton pump inhibitors (PPI) was 4 (4, 4) d, the application time of antibiotics was 3 (2, 4) d, the surgical cost was 22 580 (27 044, 34 255) yuan, and the hospital stay was 12 (10, 22) d. The Eckardt scores before and after the operation were 3 (2, 4) scores and 1 (0, 1) score respectively, the Eckardt score after the operation decreased significantly compared with that before the operation.Conclusions:STESD is a safety and efficient operation for the treatment of esophageal diverticulitis. It has the advantages of short term curative effect and obvious improvement of the patient′s symptoms.
7.Evaluation of Health Resource Allocation Efficiency in Private Ophthalmic Medical Institutions in Shanxi

Objective:To study the allocation efficiency of private ophthalmology health resources in Shanxi,and to provide references for improving the allocation efficiency of health resources in Chinese private ophthalmology medical institutions.Methods:The resource allocation and services of 70 private ophthalmic medical institutions in Shanxi were collected through a questionnaire survey,and Data Envelopment Analysis(DEA)was used to evaluate the efficiency of health resource allocation in medical institutions of Shanxi.Results:The average values of technical efficiency,pure technical efficiency,and scale efficiency of health resource allocation in private ophthalmic medical institutions in Shanxi were 0.963,0.980,and 0.982,respectively.Among the 70 private ophthalmology institutions,7 institutions were DEA-strongly efficient in health resource allocation,26 institutions were DEA-weakly efficient,37 institutions were non-DEA efficient,15 institutions had constant return to scale,40 institutions had increasing return to scale,and 15 institutions had decreasing return to scale.The allocation of health resources in 7 cities,including Taiyuan,Datong,and Shuozhou,etc.were DEA-strongly efficient;Changzhi and Jincheng were DEA-weakly efficient,both with increasing return to scale;and Linfen was non-DEA efficient with increasing return to scale.Conclusion:The efficiency of health resource allocation in some municipalities of Shanxi needs to be improved;the level of inter-organization varied,and the problems of insufficient resources and wasted inputs coexisted.In the future,ophthalmic resources should be rationally allocated,and input and output indicators should be adjusted according to the actual situation.
8.Study on mechanism of Ermiao powder in treatment of collagen-induced arthritis rats through ChAT/α7nAChR/NF-κB pathway

Objective:To investigate therapeutic effect of Ermiao powder on collagen-induced arthritis(CIA)rats through ChAT/α7nAChR/NF-κB pathway.Methods:Fifty-six SPF female Wistar rats were randomly divided into normal control(NOR)group,CIA model(CIA)group,vagotomy(VAO)group,sham operation(SHO)group,Ermiao powder treat CIA rats(EMS)group,Ermiao powder treat vagotomy(EVAO)group and Ermiao powder treat sham operation(ESHO)group.CIA rats model was established in all groups except NOR group.Administration of drug began 7 days after unilateral vagotomy and sham operation,continued for 35 days.Body weight and joint condition of rats were recorded,and pathological morphology of spleen and joint tissues of rats were observed.Serum levels of IL-1,IL-6 and TNF-α were detected,mRNA and protein expressions of ChAT/α7nAChR/NF-κB pathway core genes in joints were detected,and analyzed by immunohistochemical localization.Results:Compared with NOR/VAO group,body weight of rats in CIA group/SHO group was significantly decreased(P<0.05,P<0.01),arthritis score was significantly increased(P<0.01),ankle joint was obviously swollen(with deformity),lymphoid white pulp and main germinal center hyperplasia in spleen were obvious,joint lacuna reduction(destruction)with inflammatory cell infiltration,serum levels of IL-1,IL-6 and TNF-α were significantly increased(P<0.05,P<0.01),mRNA and protein levels of CHRNA7,ChAT,IκBα and NF-κB p50/p65 were significantly increased and expressed in articular surface(P<0.05,P<0.01).Compared with CIA group/SHO group,body weight of rats in EMS group/ESHO group was significantly increased,arthritis score was significantly decreased(P<0.05),swelling degree of ankle joint was significantly reduced,white pulp and germinal center of spleen were significantly reduced,infiltration of inflammatory cells in articular surface was reduced,joint space was increased,serum levels of IL-1,IL-6 and TNF-α were decreased,mRNA and protein expressions of CHRNA7,ChAT and IκBα were increased,while mRNA and protein expressions of NF-κB p50/p65 were decreased,which were mainly located in articular surface.Compared with VAO group,rats in EVAO group had a significant reduction in body weight(P<0.05),serum level of IL-1 was significantly increased(P<0.01),and no significant difference in IL-6 and TNF-α contents(P>0.05),mRNA and protein expressions of CHRNA7,ChAT and IκBα were decreased,NF-κB p50/p65 expression was increased,and mainly expressed in articular surface.Conclusion:Ermiao powder may down-regulate NF-κB p50/p65 by up-regulating expression of α7nAChR by ChAT in CIA rats,thereby significantly reducing expression of inflammatory factors,relieving degree of inflammation and joint lesions,and finally achieving an effective treatment of CIA.
9.Comprehensive clinical evaluation of Shuangdan Mingmu Capsule in diabetic retinopathy treatment

Objective To evaluate the clinical evidence and overall value of Shuangdan Mingmu Capsule in treating diabetic retinopathy(DR),highlight its advantages and characteristics,clarify the exact clinical positioning for DR,and propose a reference that can aid the National Medical Management Department in its decision-making.Methods This study combined qualitative and quantitative evaluation methods.Multiple method ologies were used to analyze the"6+1"dimensions.Each dimension was measured using the Multi-criteria Decision Analysis Model and calculated using CSC software V2.0.Results SRS monitored since the launch of Shuangdan Mingmu Capsule were general adverse reactions and those reported in the literature;phase Ⅱ,Ⅲ,and Ⅳ clinical studies primarily focused on the gastrointestinal system,no serious adverse reactions were reported,and the safety was rated as A.When Shuangdan Mingmu Capsule was combined with calcium hydroxybenzenesulfonate or laser photocoagulation in the treatment of DR in a meta-analysis,it further enhanced ophthalmologic parameters such as macular thickness,hemorrhagic spot area,leakage area,best-corrected visual acuity,and the degree of visual field defect.It also increased serum vascular endothelial growth factor and platelet-derived growth factor levels,the scores from the low vision quality of life scale and ultrasound indexes,and the validity was rated as grade A.A pharmacoeconomic evaluation study of Shuangdan Mingmu Capsule combined with calcium hydroxybenzenesulfonate compared to calcium hydroxybenzenesulfonate alone for DR treatment indicated that assuming the per capita disposable income in 2023 was applied as the willingness-to-pay threshold for patients,with an ICER of 3,730.98—which is approximately 2.85%of the per capita disposable income in 2023—the treatment regimen of Shuangdan Mingmu Capsule combined with calcium hydroxybenzenesulfonate was more economical than the calcium hydroxybenzenesulfonate treatment plan alone.The economy was rated as grade B.Therefore,Shuangdan Mingmu Capsule can meet clinical needs,and the formula includes medicines to tonify the liver and kidney,activate blood,and clear the channels,which is a combination of both symptomatic and fundamental treatments.Shuangdan Mingmu Capsule can also tonify the kidney,nourish the liver,stimulate blood circulation,and improve eyesight,and it is rated as grade A for innovativeness.Therefore,the drug is widely used,has no special technical and administrative requirements,is convenient to carry,and does not need to be administered at special times.The comprehensive questionnaire survey indicated that the appropriateness of the drug is good(grade B),that the production capacity is sufficient,and that the accessibility has been rated as grade B.This drug originates from ancient prescriptions of Er Zhi Pill and Liuwei Dihuang Pill,which have a long-standing reputation in traditional medicine.The combination of disease and evidence embodies the Chinese medicine characteristics of holistic concept and identification of evidence-based treatment,and the special features of Chinese medicine are rated as grade A.Conclusion The clinical value of Shuangdan Mingmu Capsule was evaluated as Class A by synthesizing the"6+1"dimensions and calculated using CSC V2.0 software.The clinical value of Shuangdan Mingmu Capsule is Class A.However,clinical needs must be focused upon and transformed into relevant clinical medication policies,providing a strong reference for the efficacy research of clinical trials.
10.Stress characteristics of the main joints of femur and lower limb bones of Tai Chi with different steps based on Anybody simulation

BACKGROUND:Anybody musculoskeletal modeling system uses mathematical modeling techniques to simulate the relationship between human bones,muscles,and the environment,allowing for the study of inverse dynamics of the human body and obtaining indicators such as lower limb joint forces.OBJECTIVE:To analyze the stress distribution patterns of lower limb joints during the practice of Tai Chi movements,thereby exploring the scientific training and exercise value of Tai Chi.METHODS:Eight Tai Chi master-level athletes were selected from the Wushu College of Beijing Sport University for data collection,including seven sets of stepping movements and CT scans of the right femur.The BTS infrared capture system and Kistler three-dimensional force platform were used to collect kinematic and mechanical data of the seven sets of stepping movements in Tai Chi(Eight Methods and Five Steps).The Anybody 7.2 musculoskeletal model's multi-body dynamics simulation technology was utilized to calculate lower limb joint dynamic parameters,and Workbench 19.2 was used to perform stress analysis on the femur.RESULTS AND CONCLUSION:(1)Using Workbench software,the stress results of the femur for seven sets of movements were obtained.The peak stress values of the seven sets of movements in descending order were:Retreat and Rollback(22.00 MPa),Retreat and Pluck(19.379 MPa),Left and Right Shift Step Squeeze and Press(9.35 MPa),Left and Right Shift Step Elbow Lean(6.30 MPa),Forward Step and Expand(4.68 MPa),Forward Step and Pull(2.57 MPa),and Middle Fixed Standalone Position(0.31 MPa).(2)In the seven sets of stepping movements,the two backward stepping movements resulted in the greatest stress on the femur(P<0.05),and the maximum stress positions on the femur were different during the movement of the seven sets of actions.(3)It is concluded that during the seven sets of stepping movements in Tai Chi(Eight Methods and Five Steps),stress threshold and maximum stress position of the femur will vary with different movements in five directions(seven sets of movements).Continuous training can comprehensively stimulate the femoral body.Forward stepping movements have a greater impact on the front and upper lateral side of the femoral body,while backward stepping movements have a greater impact on the back and inner side of the femoral body.Left and right lateral stepping movements mainly involve symmetrical stress on both sides of the femoral body.(4)Beginners should train targeted according to the stress characteristics of different stepping movements.During forward and backward stepping movements,attention should be paid to the rotational force of Tai Chi,and during left and right lateral stepping movements,attention should be paid to the medial counterforce.Beginners should focus on the Tai Chi training steps that correspond to their own weaknesses to achieve better exercise outcomes.
