Background/Aims:
Natural killer (NK) cell function is generally considered dampened in chronic hepatitis B virus (HBV) infection; however, the NK cell pool exhibits phenotypic and functional heterogeneity, and the antibody--mediated effect of NK cells remains less characterized. This study evaluated the dynamic changes in antibody-mediated NK cell responses and the involvement of distinct NK subsets across disease stages and during antiviral treatment.
Methods:
A T-cell receptor-like antibody specific for the HBV core 18–27 peptide (cTCRL-Ab) was used to determine the antibody-mediated effect of NK cells, and an array of NK cell surface markers were analyzed in cross-sectional and longitudinal cohorts of patients with chronic HBV infection. Single-cell RNA sequencing (scRNA-seq) was performed to identify the heterogeneity of NK subsets.
Results:
The cTCRL-Ab enabled the detection of NK cell cytolytic activity and IFNγ production. Notably, cTCRL-Ab-mediated NK cell responses were compromised in chronically HBV-infected patients, particularly in those receiving pegylated interferon-α (Peg-IFNα), which was associated with the downregulation of CD16 expression. Correspondingly, Peg-IFNα inhibited cTCRL-Ab-mediated NK cell function by reducing CD16 expression in vitro. scRNA-seq revealed that CD16 downregulation occurred mainly within a dysfunctional CD16hi NK subset exhibiting exhaustion properties. In contrast, an activated CD16hiNK subpopulation (CX3CR1⁺KLRC2–CD16hi) with high cytotoxicity was enriched in patients who experienced favorable treatment responses. Furthermore, the intrahepatic CX3CR1+KLRC2–CD16hi subset tended to exhibit functional restoration in HBsAg-loss individuals.
Conclusions
Our data contribute to the understanding of antibody-mediated responses of NK cells in chronic HBV infection, and highlight a previously unappreciated functional CX3CR1+KLRC2–CD16hiNK subset as a potential therapeutic target.

Objective To explore effect of pre-programmed guidance of functional exercise on exercise compliance and accuracy of patients after lumbar surgery. Methods From January to March 2016, 60 patients who had undergone the surgery of lumbar spine internal fixation were selected as the research object. All patients were divided into intervention group and control group randomly, 30 patients in each group. Patients in the control group underwent routine postoperative functional exercises. Patients in the intervention group received preoperative functional exercise instruction before 2 days. According to the self-designed questionnaire, we evaluated the compliance and accuracy of functional exercises of the two groups by asking, checking and demonstrating. Results At first day after operation, the proficiency on ankle pump exercises and straight-leg-raising movement showed no significant difference between two groups (P>0.05); However, on supine siding leg movement to right-left, compared with the control group, the intervention group were significantly better (Z=-5.100,P<0.001). At first day after removing wound drains, the proficiency on ankle pump exercises and raising buttock with five positions showed no significant difference between two groups (P>0.05), but on straight-leg-raising movement (Z=-2.316,P=0.021) , supine siding leg movement to right-left (Z= -4.238,P<0.001), lateral siding leg movement to front-back (Z=-3.027,P=0.002) and lateral straight-leg-raising movement (Z=-3.027,P=0.002), the intervention group was significantly better than the control group. Moreover, as far as adherence of exercise, patients in the intervention group were obvious better (Z= -2.156,P=0.031).Conclusions The pre-programmed guidance of functional exercise can improve the patient's awareness and confidence of functional exercise on rehabilitation, reduce the patients' psychological anxiety, so as to improve the compliance and the correct rate of functional exercise, promote their recovery. It also can save time for propaganda, improve work efficiency.

Objective To evaluate regional right ventricular function by ultrasonic strain rate imaging in postoperative children with conotruncal defects(CTD). Methods All 27 postoperative CTD children had magnetic resonance imaging evaluation of right ventricular end-diastolic volume and end-systolic volume to calculate right ventricular ejection fraction (RVEF). The ultrasonic parameters of peak systolic strain rate (SRs), peak early diastolic strain rate (SRe), peak late diastolic strain rate(SRa) and strain (S) were obtained at the basal, middle and apical segment in curve of right ventricular anterior wall in 27 postoperative CTD children and 27 normal children. The correlation between SRs,S and RVEF were evaluated. Results Compared with healthy children,SRs,S,SRe,SRa were significantly reduced in postoperative CTD children (P < 0.01 ), there were different distribution rules between postoperative CTD children and normal children, and all indexes had no statistical descrepancy among the basal, middle and apical segment ( P >0.05). SRs in basal segment of the right ventricular anterior wall correlated well with RVEF ( r = 0.89,P < 0. 01 ). Conclusions Regional right ventricular function still was reduced in postoperative CTD children, and could be accurately evaluated by ultrasonic strain rate imaging.

ObjectiveTo evaluate the relationship between the level of thoracic complete spinal cord injury(SCI) and ambulatory function wearing Reciprocating Gait Orthosis(RGO) through three dimentional gait analysis, and to explore the quantitative indicators of reconstructing walking capacity of thoracic complete SCI patients.Methods10 patients with thoracic complete spinal cord injury of lesion level from T4 to T12 who had experienced RGO gait training for at least 3 months. Three dimentional gait analysis system of Vicon Nexus 1.2 was used to test and examine the gait speed, cadence, stride length, pelvic angle of rotation, hip range of motion(ROM), crutch force, angular velocity of hip flexion and extension phases, etc. Pearson's product moment correlation coefficient and Spearman rank correlation coefficient were used to examine the relationship between the level of spinal cord injury and the kinematic and kinetic values.ResultsThe mean cadence and stride length were (37.4±2.15) steps/min and (91.6±9.09) cm. The mean hip ROM, angular velocity of hip flexion and extension phases were (42.57 °±5.43 °), (20.88 °±2.18 °)/s and (124.75 °±9.31 °)/s respectively. The gait speed, stride length, peak crutch force, hip ROM, mean crutch force and angular velocity of hip extension phase all had significant pertinence with the level of spinal cord injury.ConclusionThe limitation of hip ROM and excessive load of upper limbs mainly result in ambulatory disorder in higher thoracic complete SCI patients who should be undertaken some rehabilitation training to reduce excessive physiological load in order to improve their ambulatory capacity.
