1.Application and Advances on Circulating Tumor DNA in Esophageal Cancer

Esophageal cancer is one of the most prevalent malignant tumors globally.Its clinical management is challenged by difficulties in early diagnosis,significant inter-individual variability in therapeutic efficacy,and high rates of tumor residuals and recurrence.Circulating tumor DNA(ctDNA),a novel liquid biopsy biomarker,reflects tumor burden and genetic characteristics.It offers advantages such as non-invasiveness,repeatability,and real-time dynamic monitoring,pro-viding a promising approach for precision diagnosis and treatment of esophageal cancer.In recent years,ctDNA has shown remarkable progress in the early detection,efficacy prediction,prognos-tic assessment,and monitoring of tumor residuals and recurrence in esophageal cancer,emerging as an essential tool in the clinical management of the disease.This review summarizes the latest advancements in ctDNA research in esophageal cancer,providing valuable insights and inspiration for future studies and clinical applications.
2.Photoplethysmography signal smoothing technology based on locally orthogonal weighted polynomial fitting

To address the issue of reduced signal quality of photoplethysmography caused by local fluctuation,an approach called locally orthogonal weighted polynomial fitting(LOWPF)is proposed for signal smoothing.After determining the positions of the fluctuation sequences using the forward-backward difference XOR method,weighted polynomial fitting is applied to these sequences,and the fitted values are used to replace the fluctuation sequences to achieve signal smoothing.By constructing orthogonal basis functions,the condition number of the coefficient matrix is reduced,and the stability of the equation system solution for higher-order fitting is improved.Simulation results demonstrate that the smoothed signal's XOR smoothness of the proposed method surpasses that of the moving average algorithm and the empirical mode decomposition reconstruction algorithm.The smoothing results on 241 sets of measured PPG signals show that LOWPF achieves an efficiency of smoothness of 89.10%,significantly higher than the 78.05%of empirical mode decomposition and the 59.13%of the 5-point moving average algorithm.LOWPF has promising application prospects for smoothing signals with significant local fluctuations.

5.Epidemiological characteristics of sexually transmitted diseases in Dehong Prefecture, Yunnan Province from 2010 to 2022

ObjectiveTo understand the epidemiological characteristics and trends of sexually transmitted diseases (STDs) in Dehong Prefecture, Yunnan Province from 2010 to 2022, so as to provide a basis for the prevention and control of STDs in Dehong Prefecture. MethodsThe 2010‒2022 epidemic cards of STD cases in Dehong Prefecture were downloaded from the China Disease Control and Prevention Information System, and descriptive analysis of the incidence rate and demographic characteristics by disease type was conducted. The syphilis screening data of various populations in Dehong Prefecture between 2014 and 2022 were obtained from the National STD Control and Management Information System, and the syphilis positivity rates of key populations were calculated. ResultsA total of 18 225 STD cases were reported in Dehong Prefecture from 2010 to 2022, and the reported incidence rate increased from 17.1/105 in 2010 to 172.0/105 in 2022, showing an increasing trend (χ2trend=42.9, P<0.001). The number of reported cases of gonorrhea, syphilis, condyloma acuminatum, genital chlamydia infection, and genital herpes were 7 801 (42.8%), 4 563 (25.0%), 3 462 (18.8%), 1 660 (9.1%), and 775 (4.3%), respectively. The majority of the reported STD cases were males (12 336 cases, 67.7%), young adults aged 15 to <45 years (15 839 cases, 87.2%), and farmers (9 230 cases, 50.7%). The elderly group aged 65 years and over accounted for 10.5% of syphilis cases. Among different types of key populations, the highest syphilis positivity rate was found among men who have sex with men (10.1%), followed by STD clinic attendees (8.1%), and the syphilis positivity rates among clandestine prostitutes, voluntary counseling and testing population, drug addicts, and drug rehabilitation center/re-education through labor center population were 2.2%, 1.6%, 1.4%, and 1.3%, respectively. ConclusionFrom 2010 to 2022, the STD epidemic in Dehong Prefecture showed a rapidly increasing trend, with a higher incidence of gonorrhea and syphilis, and a higher syphilis positivity rate among men who have sex with men, drug addicts, clandestine prostitutes, and STD clinic patients. In the future, publicity, education and behavioral interventions for these groups should be strengthened to reduce the prevalence and transmission of STDs.
6.Characteristics of HIV-infected persons and their spouses who became HIV-positive in HIV discordant couples in Dehong Prefecture, Yunnan Province

ObjectiveTo determine the characteristics, viral load and immunological status of HIV-infected persons and their spouses who became HIV-positive, and the reasons for HIV seroconversion in 55 HIV discordant couples in Dehong Dai and Jingpo Autonomous Prefecture (Dehong Prefecture), Yunan Province. MethodsData on the 55 couples meeting the criteria of having a previously positive spouse were retrieved from the AIDS Integrated Prevention and Control Data Information System of the China Disease Control and Prevention Information System during 2015-2021. General socio-demographic information, age at diagnosis, exposure history, CD4+T lymphocyte count, and antiviral treatment were collected. Descriptive analysis and chi-square test were used to compare the distribution of pre-HIV-positive spouses and their HIV seroconverted spouses. ResultsA total of 55 spouses from HIV discordant couples had HIV seroconversion. Of them, 72.7% (40/55) of pre-HIV-positive spouses were husbands. The most recent CD4+T lymphocyte count in the pre-HIV-positive spouses was (328.31±246.27) cells·μL-1 at the time of diagnosis of their seroconverted spouses, of which 36.3% (20/55) had a CD4+T lymphocyte count of less than 200 cells·μL-1. Furthermore, of those pre-HIV-positive spouses with low CD4+T lymphocyte count, 45.0% (9/20) had an undetectable viral load, 15.0% (3/20) <400 copies·mL-1, and 25.0%(5/20) ≥400 copies·mL-1. Additionally, 16.4% (9/55) of the pre-HIV-positive spouses did not have a viral load test. The main reasons for HIV seroconversion among HIV-negative spouses in the discordant couples were poor condom use, poor compliance with antiviral therapy, and treatment discontinuation. ConclusionThe follow-up management of HIV discordant couples should be strengthened in Dehong Prefecture, especially the monitoring of viral load levels and immunological status of pre-HIV-positive spouses, to improve their compliance with antiviral therapy and reduce treatment discontinuation, which would effectively prevent and control HIV transmission between spouses.
7.Buyang Huanwutang Alleviates Inflammatory Injury After Cerebral Ischemia-reperfusion in Rats via Activating Adiponectin Pathway

ObjectiveTo investigate the effects of Buyang Huanwutang (BYWHT) on reducing inflammatory injury and improving neurological function after cerebral ischemia-reperfusion in rats via activating the adiponectin (APN) pathway. MethodMale SD rats were randomized into sham, model, BYHWT (16 g·kg-1, twice a day), and BYHWT + APN inhibitor (GW9662) groups. In the sham group, blood vessels were isolated. The rat model of middle cerebral artery occlusion (MCAO) was established. The rats in the BYHWT+GW9662 group was treated with subcutaneous injection of GW9662 at 4 m·kg-1 30 min before MCAO surgery and BYHWT at 16 g·kg-1 by gavage after MCAO surgery, once in the morning and once in the evening. The immunofluorescence (IF) assay was employed to observe the expression of adiponectin receptor 1 (AdipoR1) and the colocalization of AdipoR1 with neuronal nuclei (NeuN) and ionized calcium binding adaptor molecule 1 (Iba1) in the brain. Enzyme-linked immunosorbent assay (ELISA) was employed to detect the expression of APN in the serum and brain. The balance beam test was carried out to examine the balance ability, and the grasping test to assess the recovery of limb strength. The immunofluorescence assay was used to detect the expression of myeloperoxidase (MPO). Western blot was employed to determine the expression of tumor necrosis factor-alpha (TNF-α), interleukin (IL)-1β, IL-6, and IL-10 and in the brain. ResultCompared with the sham group, the modeling promoted the expression of AdipoR1 (P<0.01), lowered the APN levels in the serum and brain (P<0.05, P<0.01), increased the score in the balance beam test (P<0.01), and decreased the grasping strength of forepaw (P<0.01), which were accompanied with increased MPO, TNF-α, IL-1β, and IL-6 levels (P<0.01) and decreased IL-10 level (P<0.01). Compared with the model group, BYHWT promoted the expression of AdipoR1 (P<0.01), elevated APN levels in the serum and brain (P<0.05, P<0.01), and increased the grasping strength of forepaw (P<0.01), which were accompanied with lowered MPO, TNF-α, IL-1β, and IL-6 levels (P<0.01) and elevated IL-10 level (P<0.01). All the above effects were partially blocked by GW9662. ConclusionBYHWT can reduce inflammatory injury and improve neurological function in the rat model of cerebral ischemia-reperfusion by activating the APN pathway.
8.Effect of hand hygiene comprehensive intervention on incidence of pediatric healthcare-associated infection

Objective To explore the effect of 5M1E quality analysis tool-based hand hygiene(HH) comprehensive intervention measures on pediatric healthcare-associated infection(HAI).Methods HH status of 29 health care workers(HCWs) in the pediatric department of a hospital was monitored, April-June 2015 was baseline survey stage, 5M1E quality analysis tool was adopted to analyze various factors affecting the compliance rate of pediatric HH, intervention measures began to be taken in June 2015, and compared with HH in July 2015-March 2016.The occurrence of HAI between July 2014-March 2015 and July 2015-March 2016 was compared.Results HH compliance rate of HCWs increased from 30.86% in April-June 2015 to 81.94% in January-March 2016;HH correct rate increased from 68.14% to 93.75%;HH compliance rate and correct rate of HCWs gradually increased(X2=2 608.626, 630.798,respectively, both P<0.001).Qualified rate of detection of HCWs' HH sampling increased from 20.69% in April-June 2015 to 89.66% in January-March 2016(X2=31.957,P<0.001).Incidence of HAI decreased from 7.74% in July 2014-March 2015 to 3.62% in July 2015-March 2016(X2=46.717,P<0.001).Conclusion Application of 5M1E quality analysis tool in the investigation, analysis, and comprehensive intervention in HCWs' HH in pediatric department has greatly enhanced HCWs' HH compliance rate, and reduced the incidence of HAI in pediatric patients.
9.Effects of extracellular regulated protein kinases protein and impairment of blood testis barriar stucturein of mice with exposure to decabromodiphenyl ether

Objective To study changes in expression of claudin-11 and proteins related to mitogen-activated protein kinase (MAPK) signaling pathways, as well as the ultrastructure of the blood testis barrier(BTB), in male ICR mice exposed to decabromodiphenyl ether (BDE-209). Methods Fifty-two mice, 4 weeks of age, weighing 15-21 g, were provided with adaptive feeding for 1 week. Mice were randomly divided into 4 groups, named control, low-dose, medium-dose and high-dose groups. The treated groups received BDE-209, by intragastric gavage, at doses, respectively, of 100, 300 and 500 mg/kg. Mice were sacrificed after 6 weeks and organs harvested on ice, weighed and stored at-80 °C. The ultrastructure of testicular tissues was examined by electron microscopy. Western blotting was used to detect proteins related to the MAPK pathway, including p38 mitogen activated protein kinase (p38), phosphorylated p38 (p-p38), extracellular regulated protein kinase 1/2 (ERK1/2) , phosphorylated ERK1/2 (p-ERK1/2) , c-jun N-terminal kinase (JNK), phosphorylated JNK (p-JNK) and the BTB tight junction protein claudin-11. Analyze the difference between each groups. Results At sacrifice, the body weights in each treated group were compared with those in the control group weighing (41.14 ± 0.60) g. Compared with controls, body weights were significantly different (P<0.05) in the middle dose, at (39.97 ± 0.66) g and high dose, at (39.98± 0.55) g in control group. The coefficients of the testis were significantly lower (P<0.05) in each treated group than in controls, with values of (0.37±0.0)%, (0.31±0.05)% and (0.31±0.04)% for low- dose, medium-dose and high-dose groups, respectively. The epidymus coefficient values were also significantly lower than controls (P<0.05), with values of (0.16±0.06)%, (0.11±0.05)% and (0.07±0.03)%, respectively in the same three dose groups. Electron microscopy ultrastructure showed that, compared with the control group, the testes in the middle and high dose groups had closely connected fractures, cell edema and more vacuoles. Compared with in the control group, levels of p-p38 and p-JNK in testicular tissue were significantly increased (P<0.05). In the control group and in low-, medium- and high-dose groups, the p-p38/p38 ratios were 1.35±0.13, 3.46±0.10, 5.71±0.26 and 4.79±0.21, respectively. The corresponding p-JNK/JNK ratios were 2.07±0.0, 4.77±0.18, 3.63±0.06 and 4.85±0.15. Claudin-11 levels were significantly lower (P<0.05) than control values in each dosed group. The corresponding values in control, low-dose, medium-dose and high-dose groups were 8.33±0.36, 2.06±0.27, 3.37±0.27 and 1.55±0.19, respectively. Conclusion BDE-209 increased expression of proteins in the MAPK pathway and decreased expression of the BTB tight junction protein claudin-11 in testicular tissue. It also caused ultrastructural damage to the Sertoli cell BTB tight junctions.This suggested that BDE-209 might damage Sertolicells BTB through effects on the MAPK pathway.
