Parkinson's disease (PD) is a complex neurodegenerative disease involving multiple systems and neurotransmitters. Due to the high clinical heterogeneity of PD,it is urgent to establish a comprehensive and long-term traditional Chinese medicine (TCM) management model. In this paper, the conceptual framework of full-cycle management of PD is preliminarily constructed:based on the evolution of the pathophysiological mechanisms of protein deposition and neurotransmitter disorder in PD,the three-stage syndrome characteristics of the prodromal stage (predominant healthy Qi with subtle pathogenic factors),the early clinical stage (declining healthy Qi with growing pathogenic factors) and the middle and late stages (overwhelming pathogenic factors with deficient healthy Qi) are longitudinally described. Through the syndrome differentiation of visceral manifestations, the etiology and pathogenesis of PD motor and non-motor symptoms were comprehensively analyzed, while the matching treatment methods and prescriptions were inferred, and the modular scheme of the combining main symptoms, accompanying symptoms and secondary symptoms was proposed. The conceptual gap of TCM regarding motor complications ('variable syndrome') and PD-related hyperpyrexia syndrome ('critical syndrome') was explained. This framework reflects the characteristics of combination of disease and syndrome and overall constant motion, and provides new theories and research ideas for individualized and whole-process management of PD in TCM.

Objective To explore the clinical manifestations and ultrasound features of primary high-grade serous carcinoma of the fallopian tube (PHGSC-FT) under the new pathological criteria. Methods A retrospective study was conducted on 45 PHGSC-FT patients diagnosed by postoperative pathology at Obstetrics & Gynecology Hospital of Fudan University from April to June 2023. Clinical data, ultrasound characteristics, and prognosis were analyzed. Results The age of patients was (56.2±10.1) years, with postmenopausal women accounting for 68.9%. The main clinical symptoms were abdominal pain and (or) distension (53.3%), pelvic mass (31.1%), and abnormal vaginal discharge (11.1%). Preoperative ultrasound revealed that 40 cases presented mainly as solid pelvic or abdominal masses (82.5%, 33/40), while 5 cases involved widely disseminated small lesions not describable by ultrasound; 28 lesions had irregular shapes 70.0%. Intraoperative findings showed that the major lesions were located in the ovary in 27 cases (60.0%), fallopian tube in 7 cases (15.6%), extra-adnexal pelvic in 6 cases (13.3%), and widespread disseminated lesions in 5 cases (11.1%). The accuracy rate of ultrasound localization was 82.2%, and the accuracy rate for benign and malignant differential diagnosis was 75.6%. Serum carbohydrate antigen 125 (CA125) levels were significantly lower in patients whose primary lesion was located in the fallopian tube than in those with lesions outside the fallopian tube (P=0.001). FIGO stage Ⅰ: 4 cases (8.9%), stage Ⅱ: 10 cases (22.2%), stage Ⅲ: 23 cases (51.1%), and stage Ⅳ: 8 cases (17.8%), with 3-year survival rates of 75.0%, 80.0%, 56.5%, and 50.0%, respectively. Conclusions PHGSC-FT predominantly occurs in postmenopausal women, and its biological behavior is characterized by “small primary lesion–large metastatic lesion”. Prognosis of these patients is related to FIGO stage; Ultrasound demonstrates high accuracy in localization of large lesions and in determining benign versus malignant nature.

Aneurysmal subarachnoid hemorrhage(aSAH)is a type of hemorrhagic stroke with high morbidity and mortality. After treatment, most aSAH patients can completely recover their neurological func-tion,about 50%of aSAH patients will have cognitive disorder, mainly manifested as executive function,lan-guage and memory dysfunction, more than half of aSAH patients can not recover cognitive function, which brings great pressure to individuals and their families. This article aims to discuss the characteristics,influen-cing factors, assessment tools, potential mechanisms and drug therapy of post-ASAH cognitive disorder,in or-der to provide prevention and treatment strategies for clinical post-ASAH cognitive disorder.

Objective:To separate and purify the polysaccharides from Polygonatum filipes, characterize their primary structure and investigate the immunomodulatory effects on RAW264.7 macrophages. Methods:Crude polysaccharides from Polygonatum filipes were extracted by ultrasound assisted method, then Polygonatum filipes polysaccharides(CSPFPs)were obtained after elimination of the proteins with combined papain-Sevag method. The total sugar content was determined by phenol-sulfuric acid method.Structures of CSPFPs were analyzed by fourier transform infrared spectroscopy(FT-IR),high performance gel permeation chromatography(HPGPC)and high performance liquid chromatography(HPLC).Effects of CSPFPs on cell viability, pinocytic activity,TNF-α secretion,MAPK and NF-κB signaling pathways of RAW264.7 cells were explored by MTT,Neutral red,ELISA and Western blot, respectively. Results:Extraction rate of CSPFPs by ultrasound-assisted method was 41.61%,which contained total sugar content of 94.00%.CSPFPs with Mw of 3 125 Da was composed of arabinose(1.85%),galactose(6.14%),glucose(56.41%)and mannose(35.60%).The in vitro experiments showed that CSPFPs were non-cytotoxic and enhanced the pinocytic activity,TNF-α secretion and phosphorylation levels of p38,ERK,JNK,p65,IκB and IKK,indicating the activation of MAPK and NF-κB signaling pathways under the concentra-tion of 2.5~200 μg/ml.Conclusion:The ultrasound-assisted method can efficiently isolate CSPFPs with immunomodulatory activity, which provides basic data for the development and application of CSPFPs as an immunostimulant.

BACKGROUND:Intervertebral disk degeneration is a pathological change caused by a series of complex molecular mechanisms that result in the aging and damage of intervertebral discs, ultimately leading to severe clinical symptoms.Traditional Chinese medicine has unique advantages in the treatment of intervertebral disk degeneration due to its low cost,non-addictive nature,multi-target effects, minimally toxic and side effects, and high patient acceptance. OBJECTIVE:To review the latest research results of traditional Chinese medicine monomer intervention-related signaling pathways in the treatment of intervertebral disk degeneration, describe and analyze the action mechanism of traditional Chinese medicine monomer on intervertebral disk degeneration, and provide a new approach and theoretical basis for future basic research and clinical treatment. METHODS:The first author searched for relevant literature from January 2018 to February 2023 in CNKI,PubMed,VIP,and WanFang using the search terms"intervertebral disc, signal pathway".The articles that did not meet the criteria were excluded after preliminary screening of the titles and abstracts.Finally,72 articles were selected for review and analysis. RESULTS AND CONCLUSION:Traditional Chinese medicine monomers can regulate multiple classical signaling pathways such as Wnt/β-catenin,PI3K/Akt,mTOR,NF-κB,and MAPK.They achieve this by regulating oxidative stress, adjusting the expression of pro/anti-apoptotic proteins in cells, stimulating cellular autophagy function, reducing stimulation of cell inflammatory factors, increasing the expression of extracellular matrix markers, reducing the production of matrix-degrading enzymes, maintaining the synthesis and stability of extracellular matrix, inducing differentiation of mesenchymal stem cells in the nucleus pulposus into nucleus pulposus cells, promoting endogenous repair and reconstruction, controlling apoptosis and aging of nucleus pulposus cells, and increasing the activity of nucleus pulposus cells. These actions improve the microenvironment within the intervertebral disc, maintain the normal physiological function of the intervertebral disc, and delay intervertebral disc degeneration.
