As a physiologic pacing technique,conduction system pacing(CSP)can capture the intrinsic conduction system and rebuilt cardiac electrical synchronization.CSP including His bundle pacing(HBP)and left bundle branch pacing(LBBP),which played novel and important roles in patients with heart failure with reduced ejection fraction(HFrEF)resulted from intraventricular or interventricular asynchrony.This review aims to demonstrate the current status and prospects CSP for cardiac resynchronization therapy in patients with HFrEF.
2.A survey on ketosis in 79 large-scale dairy cattle farms in China

To clarify the current situation of ketosis in dairy cattle on large-scale pastures in China and provide new insights,a questionnaire survey was conducted to analyze the incidence,preven-tion,treatment methods,and associated costs of ketosis in 79 large-scale pastures.The results showed that the average incidence of ketosis in dairy cows was 3.97%,with a cure rate of 92.40%.The order of importance of methods for preventing and controlling ketosis was as follows:feed for-mulation optimization>blood ketone monitoring>negative energy balance monitoring>feed in-take monitoring>milk yield monitoring.The most important treatment methods are intravenous glucose>propylene glycol butyl phosphate>vitamins>choline.The most important diagnostic methods are blood ketone testing>milk ketone testing>negative energy balance testing>clinical symptoms>blood glucose testing.Economic analysis revealed that treatment costs were lower on larger farms and higher milk yields farms.Continuous optimization of feeding management,preven-tion,and control measures should be implemented on large-scale farms in China to reduce the oc-currence of ketosis in dairy cows.Additionally,more effective diagnostic and treatment methods should be employed to improve the cure rate and overall farm income.

4.Short-term clinical efficacy of transcatheter edge-to-edge repair for moderate to severe mitral regurgitation.

OBJECTIVES:
To evaluate the short-term clinical efficacy of transcatheter edge-to-edge repair (TEER) in patients with moderate to severe mitral regurgitation.
METHODS:
Clinical data of patients with moderate to severe mitral regurgitation who underwent TEER in the Department of Cardiology, the First Affiliated Hospital of Xi'an Jiaotong University from April 2021 to May 2024, were retrospectively analyzed, including preoperative baseline clinical and echocardiography data, intraoperative data and 6-month postoperative follow-up data.
RESULTS:
A total of 67 patients' (47 males and 20 females) data were included, of whom 62 completed 6-month follow-up. The immediately postoperative success rate was 88.1% (59/67), and 83.9% (52/62) patients exhibited mitral regurgitation ≤2+ at 6 months postoperatively, showing significant improvement compared with preoperative (P<0.05). The proportion of patients with mitral regurgitation ≤2+ at 6 months was significantly higher in the degenerative mitral regurgitation (DMR) group than that in the functional mitral regurgitation (FMR) group (P<0.05). The mean mitral valve gradient (MVG) in DMR group was increased from (3.1±1.2) mmHg (1 mmHg=0.133 kPa) to (3.7±1.2) mmHg 6 months after operation (P<0.05), while there was no significant change in FMR group (P>0.05). Compared with those before operation, the N-terminal pro-B-type natriuretic peptide levels in both FMR and DMR groups were significantly lower at 6 months postoperatively (all P<0.05), and the left atrial volume index and left atrial anteroposterior diameter were also significantly lower (all P<0.05). The left ventricular end-diastolic diameter and left ventricular end-systolic diameter were significantly reduced 6 months after operation in the FMR group (all P<0.05), but no significant changes were observed in the DMR group (all P>0.05). The ejection fraction was not significantly changed before and after operation in both groups (all P>0.05). The mitral regurgitation, tricuspid regurgitant, and pulmonary artery pressure were significantly reduced in both groups at 6 months postoperatively (all P<0.05).
CONCLUSIONS
TEER is effective for moderate to severe mitral regurgitation. The improve-ments in left ventricular remodeling are more pronounced in patients with FMR while the degree of mitral regurgitation is more significant in DMR patients. However, MVG elevation is more common during the follow-up.

5.A Three-Dimensional Framework Analysis of High-Quality Development Policies for Public Hospitals in China

Objective To explore the characteristics,potential challenges,and optimization paths of high-quality development policies for public hospitals in China,and to provide reference for policy formulation and implementation.Methods The 31 policy documents on high-quality development of public hospitals issued at the central and provincial levels between 2021 and 2024 were coded using Nvivo 11 software,and a three-dimensional analytical framework was constructed on the basis of the policy tools,stakeholders,and system dimensions,combining both quantitative and qualitative methods for content analysis.Results It found that the distribution of policy content in the three dimensions was characterized by stage imbalance:policy tools were mainly environmental,followed by supply,and demand;stakeholder attention was focused on public hospitals and healthcare administrations,and patient attention was low;The system dimension focused mainly on the macro level,with less distribution at the meso and micro levels.Although the unbalanced distribution of policy instruments is relevant at certain stages,the long-term structural imbalance may lead to insufficient systemic policies and weakened stakeholder synergies,and there is an urgent need to optimize the structure of instruments.Conclusions It is recommended to increase the proportion of demand-based policy instruments in policy design and dynamically adjust the synergistic application of the three types of instruments;to strengthen the attention to vulnerable stakeholders,such as patients;and to optimize the design of policies at the meso-levels and micro-levels in order to enhance the systemicity and sustainability of policy implementation.

8.Effect of modified attention and interpretation therapy on fatigue and sleep quality in patients with post-stroke hemiplegia

Background Patients with post-stroke hemiplegia are likely to experience fatigue and sleep disorder.Attention and interpretation therapy(AIT)has been shown to promote psychological flexibility,thereby alleviating their stress,improving emotional management and somatopsychic adaptation.Currently,there is a lack of application of AIT in patients with post-stroke hemiplegia.Objective To explore the effect of modified AIT on fatigue and sleep quality in patients with post-stroke hemiplegia,so as to provide references for physical and mental rehabilitation nursing of post-stroke hemiplegia patients.Methods A total of 120 patients with post-stroke hemiplegia attending the rehabilitation medicine department of North China University of Science and Technology Affiliated Hospital from August 1,2021 to June 30,2022 and fulfilling the Chinese guidelines for diagnosis and treatment of acute ischemic stroke 2018 and the Chinese guidelines for diagnosis and treatment of acute intracerebral hemorrhage 2019 were enrolled.Patients were assigned into two groups using random number table methods,each with 60 cases.Control group was given chronic illness trajectory model-based nursing,while study group received modified AIT intervention.All participants were evaluated using the Chinese version of Neurological Fatigue Index for Stroke(NFI-Stroke)and Pittsburgh Sleep Quality Index(PSQI)at the baseline,after 4 weeks and 8 weeks of intervention.Results The repeated measures ANOVA on PSQI total score and scores on sleep quality,sleep time,sleep duration,sleep efficiency,sleep disturbance and daytime dysfunction revealed a significant group effect,time effect and interaction effect(Fgroup=4.281,5.598,8.814,4.351,4.302,4.091,7.608,Ftime=122.230,19.698,19.289,16.403,15.421,28.879,90.876,Finteraction=19.508,3.227,11.586,5.237,5.507,5.799,3.338,P<0.05).Significant group effect,time effect and interaction effect were reported on NFI-Stroke total score and scores on physical subscale and cognitive subscale(Fgroup=7.140,5.466,5.547,Ftime=362.192,309.817,70.888,Finteraction=21.079,13.430,13.354,P<0.05).Conclusion Modified AIT may help alleviate fatigue and improve sleep quality in patients with post-stroke hemiplegia.
9.Severe acute respiratory syndrome coronavirus 2-specific T-cell responses are induced in people living with human immunodeficiency virus after booster vaccination

Background::T-cell-mediated immunity is crucial for the effective clearance of viral infection, but the T-cell-mediated immune responses that are induced by booster doses of inactivated severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccines in people living with human immunodeficiency virus (PLWH) remain unclear.Methods::Forty-five PLWH who had received antiretroviral therapy (ART) for more than two years and 29 healthy controls (HCs) at Beijing Youan Hospital were enrolled to assess the dynamic changes in T-cell responses between the day before the third vaccine dose (week 0) and 4 or 12 weeks (week 4 or week 12) after receiving the third dose of inactivated SARS-CoV-2 vaccine. Flow cytometry, enzyme-linked immunospot (ELISpot), and multiplex cytokines profiling were used to assess T-cell responses at the three timepoints in this study.Results::The results of the ELISpot and activation-induced marker (AIM) assays showed that SARS-CoV-2-specific T-cell responses were increased in both PLWH and HCs after the third dose of the inactivated SARS-CoV-2 vaccine, and a similar magnitude of immune response was induced against the Omicron (B.1.1.529) variant compared to the wild-type strain. In detail, spike-specific T-cell responses (measured by the ELISpot assay for interferon γ [IFN-γ] release) in both PLWH and HCs significantly increased in week 4, and the spike-specific T-cell responses in HCs were significantly stronger than those in PLWH 4 weeks after the third vaccination. In the AIM assay, spike-specific CD4 + T-cell responses peaked in both PLWH and HCs in week 12. Additionally, significantly higher spike-specific CD8 + T-cell responses were induced in PLWH than in HCs in week 12. In PLWH, the release of the cytokines interleukin-2 (IL-2), tumour necrosis factor-alpha (TNF-α), and IL-22 by peripheral blood mononuclear cells (PBMCs) that were stimulated with spike peptides increased in week 12. In addition, the levels of IL-4 and IL-5 were higher in PLWH than in HCs in week 12. Interestingly, the magnitude of SARS-CoV-2-specific T-cell responses in PLWH was negatively associated with the extent of CD8 + T-cell activation and exhaustion. In addition, positive correlations were observed between the magnitude of spike-specific T-cell responses (determined by measuring IFN-γ release by ELISpot) and the amounts of IL-4, IL-5, IL-2 and IL-17F. Conclusions::Our findings suggested that SARS-CoV-2-specific T-cell responses could be enhanced by the booster dose of inactivated COVID-19 vaccines and further illustrate the importance of additional vaccination for PLWH.
10.Association between cognitive function and anterior cingulate cortex gamma-amino-butyric acid concentrations in patients with depression before and after treatment

Objective:To explore the association between cognitive function and the level of gamma-amino-butyric acid(GABA)in anterior cingulate cortex(ACC)before and after treatment in patients with major depres-sion disorder.Methods:Totally 31 medication-naive patients with major depression disorder meeting the criteria of the Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition(DSM-5)and 33 normal controls were col-lected.Each eligible patient received treatment with selective serotonin reuptake inhibitor agents for 8 weeks.The MATRICS Consensus Cognitive Battery(MCCB)was used to evaluate the cognitive function.By means of 1H magnetic resonance spectroscopy,anterior cingulate cortex GABA concentrations were measured.Results:At base-line,the concentration of ACC GABA relative to water(GABA+/W)was lower in the patient group than in the control group(P<0.05)and increased after treatment(P<0.05).ACC GABA+/W was negatively associated with verbal learning and visual memory score in patient group at baseline(correlation coefficient and P value were r=-0.40,P<0.05;r=-0.42,P<0.05,respectively).The ACC GABA+/W difference resulted of treatment in patient group was positively associated with the difference of working memory score and the difference of reasoning and problem-solving score(correlation coefficient and P value were r=0.58,P<0.05;r=0.66,P<0.05,respec-tively).Conclusion:The cognitive dysfunction of patients with major depression disorder may not be related to the degree of depression and anxiety.And improvement of cognitive function may be associated with increase of ACC GABA concentrations.
