1.Association of interaction effects of outdoor physical activity and insomnia with depressive symptoms in college students

Objective:
The aim of the study was to investigate the association of outdoor physical activity and insomnia symptoms with depressive symptoms in college students, so as to provide a theoretical basis for preventing and improving depression of the college students.
Methods:
A sample survey of 9 349 college students from 4 colleges and universities in 3 provinces in Hubei, Jiangxi and Chongqing were conducted from November to December 2021. The Patient Health Questionnaire, Insomnia Severity Index and outdoor physical activity questionnaire were used to evaluate the depressive symptoms, insomnia symptoms and outdoor activities of college students, respectively. The χ 2 test was used to compare the difference of the positive rate of depressive symptoms in college students. Logistic regression analysis was used to analyze the association between outdoor physical activity, insomnia symptoms and depressive symptoms. Generalized linear model was used to analyze the association of interaction effect of outdoor activity and insomnia symptoms with depressive symptoms.
Results:
The detection rate of depressive symptoms in college students was 37.4%. Significant differences in the detection rate of depressive symptoms were observed among college students with different majors, nationalities,educational backgrounds, family residence, family type, family economic conditions, study burden, number of friends, father s education level,mother s education level( P <0.05). Logistic regression analysis showed that outdoor physical activity ( OR=0.51-0.69, P <0.01) and insomnia symptoms ( OR=6.15-21.30, P <0.01) were associated with depressive symptoms( P <0.01). Generalized linear model showed that insomnia symptoms( B =0.57), outdoor physical activity ( B =-0.15) and their interaction terms ( B =-0.02) were associated with college students depression( P <0.05).
Conclusion
This study suggests that insufficient time for outdoor physical activity and insomnia symptoms may increase the risk of depressive symptoms in college students. College students with insufficient time for outdoor physical activity and insomnia have a higher risk of developing depressive symptoms.
2.Prevalence and influencing factors of thyroid nodules among students aged 8 - 17 years in Jintan District, Changzhou City, Jiangsu Province, 2018

Objective:To investigate the prevalence of thyroid nodules among students aged 8 - 17 years in Jintan District, Changzhou City, Jiangsu Province, and to explore the risk factors of thyroid nodules among underage students.Methods:In September 2018, two primary and middle schools, one ordinary high school and one vocational high school in Jintan District were selected as the survey sites. Students aged 8 - 17 years were selected as the survey subjects, and questionnaire survey, physical examination and thyroid ultrasound examination were conducted. At the same time, drinking water samples from schools and towns were collected to test water iodine, and urine samples from students and family salt samples were collected to test urinary iodine and salt iodine. Multivariate logistic regression was used to analyze related factors that may affect the occurrence of thyroid nodules.Results:A total of 725 students were surveyed, including 359 (49.5%) boys and 366 (50.5%) girls. Two water samples were collected from each of the four schools, and the mean values of water iodine were 0.3, 8.5, 0.2 and 0.2 μg/L, respectively; two water samples were collected from each of the towns where the four schools were located, and the mean values of water iodine were 6.8, 8.1, 4.8 and 3.7 μg/L, respectively. A total of 725 urine samples were collected, and the median urinary iodine was 209.92 μg/L, ranging from 8.60 to 932.00 μg/L. A total of 725 edible salt samples were collected from students' families, and the salt iodine content was (23.75 ± 7.10) mg/kg, ranging from 0.00 to 60.30 mg/kg, and 95.0% (689/725) of students' families ate iodized salt. A total of 725 students were examined for thyroid gland, and 22 were diagnosed with goiter, with an enlargement rate of 3.0%; and 155 were diagnosed with thyroid nodules, with a detection rate of 21.4%. The detection rates of thyroid nodules in boys and girls were 20.9% (75/359) and 21.9% (80/366), respectively; the detection rates of thyroid nodules in students who participated and did not participate in extracurricular tuition/interest classes in the past month were 28.2% (71/252) and 17.8% (84/473), respectively. The results of multivariate logistic regression analysis showed that girls and participating in extracurricular tuition/interest classes in the past month were independent risk factors for the occurrence of thyroid nodules [odds ratio ( OR) = 2.057, 2.538, 95% confidence interval ( CI): 1.226 - 3.451, 1.532 - 4.204, P < 0.05). Conclusions:The detection rate of thyroid nodules in students aged 8 - 17 years in Jintan District is at a high level. Girls and participating in extracurricular tuition/interest classes in the past month are independent risk factors for the occurrence of thyroid nodules.
3.Analysis of the Effect of Intelligent Regulation of Infusion Sequence on Promoting Intravenous Rational Drug Use

OBJECTIVE:To evaluate th e eff ects of infusion sequence regulation on the promotion of intravenous rational drug use. METHODS :The intelligent control of infusion sequence was realized with Markov chain machine learning method on the basis of intelligent reminder manual labeling infusion sequence. Selected from Binhu Hospital of Hefei during Jan. to Sept. 2017, 164 800 inpatients’medication orders were collected as data before intelligent control of infusion sequence ,and 264 600 inpatients’ medication orders were collected as data after intelligent control of infusion sequence. The correct rate of incompatibility ,storage time limit ,administration frequency ,chronopharmacology,preventive drugs ,drug interaction ,auxiliary drugs and infusion sequence for intravenous irritant drugs were compared before and after regulation ;the times of flushing tube ,the cost of flushing tube infusion and the change of liquid property in the infusion tube were compared before and after regulation. RESULTS :The correct rate of incompatibility ,storage time limit ,administration frequency ,chronopharmacology,drug interaction and infusion sequence for auxiliary drugs after regulation were significantly higher than before regulation ;the correct rate of infusion sequence for preventive drugs and intravenous irritant drugs ,the times of flushing tube ,the cost of flushing tube infusion and the case number of the liquid change (proportion) in infusion tube were significantly lower than before regulation (P<0.001). CONCLUSIONS:Intelligent regulation of infusion sequence can improve the rationality of intravenous medication sequence to a certain extend.
4.Study on the Practice of Regulating Individualized Infusion Sequence in PIVAS of Our Hospital

ABSTRACT OBJECTIVE:To establish the infusion sequence regulation procedure of PIVAS,and implement individual regulation. METHODS:By combining factors such as liquid volume of each batch,incompatibility between groups and specific solvents of specific drugs,intelligent prompt system and the artificial intelligent regulation system of infusion sequence were successively established to sequence the therapeutic drugs and adjuvant drugs used for the patients with different diagnoses and different purposes in different inpatient areas in a day,so as to realize intelligent control of individual infusion sequence. From Jan. to Sept. in 2017 and from Jan. to Sept. in 2019 as research stages before and after the intelligent control of individualized infusion sequence,the control effect was evaluated with the proportion of reasonable number of infusion priority cases,the proportion of appropriate number of elderly patients’batch liquid volume cases,the proportion of incompatibility cases between separate groups, the number and cost of flushing tubes as index. RESULTS:After regulating infusion sequence individually,the proportion of reasonable infusion priority rose from 5.56% to 98.72% ;the proportion of batch liquid quantity appropriate cases in elderly patients rose from 9.58% to 98.10%;the proportion which separated the incompatibility between groups rose from 41.03% to 99.12%;the number of washing tube dropped to(0.95±0.43)times/ward/d from(12.95±0.57)times/ward/d;the cost of washing tube could be saved 85 800 yuan/year. CONCLUSIONS:The implementation of individualized infusion sequence regulation promote safe and effective infusion,and provide reference for pharmacists of PIVAS in China to carry out precise pharmaceutical care.
5. The predictive value of CTCs characterization for time to castration resistance of high-volume metastatic castration sensitive prostate cancer

Objective:
To explore the predictive value of circulating tumor cells (CTCs) characterization for time to castration resistance of newly diagnosed high volume metastatic castration sensitive prostate cancer (mCSPC) patients.
Methods:
Newly diagnosed high volume mCSPC patients were prospectively enrolled in this study from September 2015 to February 2017. The inclusion criteria include that the patients' age should be between 18 to 85 years old. The Prostate cancer should be diagnosed by biopsy or cytopathology. No endocrinological therapy, radiative therapy or chemotherapy was used before the study. High-volume metastatic lesion was confirmed by imaging. Those patients who accepted previous endocrinological therapy, radiative therapy or chemotherapy were excluded in this study. Those patients combined with concomitant tumor were also excluded. The health males were enrolled in the control group. All patients received androgen deprivation therapy (ADT) with goserelin plus bicalutamide (goserelin 3.6 mg subcutaneous injection, once a month plus bicalutamide 50mg orally, once a day). CanPatrol system was used to count CTCs in peripheral blood of patients and characterize CTCs based on expressions of epithelial markers(EpCAM and CK8/18/19) and mesenchymal markers(vimentin and twist). Primary endpoint was time to castration resistance. Survival analysis was conducted using Kaplan-Meier method and log-rank test was used to assess the difference of survival between groups, and univariate and multivariate analyses of prognostic factors were conducted using the Cox proportional hazards model.
Results:
A total of 108 newly diagnosed high volume mCSPC patients were enrolled in this study. The median age of enrolled patients was 68 years old (ranging 51-85 years old), and median PSA was 196.2 ng/ml(ranging 5.8-5 011.9 ng/ml). The median level of hemoglobin was 32 g/L(ranging 9-172 g/L). The median level of LDH was 179 U/L(ranging 49-630 U/L). The ECOG scores was 0-1 score in 94 cases(87.0%), 2 scores in 14 cases (13.0%). The Gleason scores was 6-7 in 20 cases (18.5%) and more than 8 in 88 cases (81.5%). All patients had bone metastatic lesions, among which 41 (38.0%) patients had more than 10 metastatic lesions and 6 (5.6%) patients with visceral metastasis, 30(27.8%) patients with limb bone metastasis. The median CTCs count was four, and ranging 0-35. Mesenchymal CTCs positive and negative (negative included CTCs negative, epithelial CTCs positive and biophenotypic CTCs positive) patients were 58(53.7%) and 50, respectively. There was no correlation between CTCs characterization with age, baseline PSA, Gleason score, ALP and other clinical parameters (
6. Exploration of regulating infusion administration sequence by an intelligent prompt system in pharmacy intravenous admixture services

Eight sub-databases of pharmacological information on rational use of medicines were designed and established. These sub-databases cover dosing frequency, therapeutic drug priority to auxiliary medication, infusion shelf-life after admixture, incompatibility of two continuous infusion in the vein infusion tube, chronopharmacology, drug interaction involved in administration sequence , venous irritative drugs and preventive medicine. Once the standing order delivery to PIVAS involves any of these sub-databases, the system will prompt in turn " Auxiliary" , " Time limit" , " Taboo" , " Chronopharmacology" , " Interaction" , " Irritative" and " Preventive (therapeutic)" , starting from therapeutic drug priority to auxiliary medication. Pharmacists would make a reasonable sequence of the corresponding infusion and label the administration sequence on the infusion tag according to the prompt.Compared to the circumstance prior to this system, the wards with the infusion sequence labeling and equipped with intelligent prompt increased from 2 to 43; the percentage of such inpatients increased from 0.50% to 66.33%; the percentage of medical orders increased from 0.72% to 78.94%. The time spent by pharmacists for the same workload reduced from 73.44 h per day to 1.94 h per day. The times of flushing infusion tube reduced from 34.42 to 1.49 per ward and per day. The cost of flushing infusion tubes reduced by a big margin. PIVAS has established a method of regulating infusion administration sequence by an intelligent prompt system, based on improved safety and effectiveness of medication.
7.In-vitro study of epithelial-mesenchymal transition mediated by HBX protein and M2 macrophages in hepatocellular carcinoma cells

Objective To explore the synergetic effect of HBX protein and M2 macrophages in inflammatory microenvironment on invasion and metastasis of hepatocellular carcinoma cells.Methods Hep3B cells were infected with recombinant lentivirus carrying HBx gene,following co-culture with THP-1 original M2 macrophages.The cells were divided into six groups:two infected groups (Hep3B +and Hep3B + + M2),four non-infected groups (Hep3B-,Hep3B-+ LV5,Hep3B-+ M2,Hep3B-+LV5 + M2).Western blot (WB) was used to assess the expression changes of E-cadherin and N-cadherin,markers of epithelial-mesenchymal transition (EMT).The cellular location of EMT markers was observed by immunofluorescence confocal microscopy.Transwell assay was used to evaluate the invasion ability of Hep3B cells.Results HBX protein overexpressed in Hep3B cells by lentivirus infection.After 72 h co-culture with M2 macrophages,WB results showed that E-cadherin descreased significantly in Hep3B+ (0.42 ±0.11) when compared with Hep3B-(1.00 ±0.18) (t =4.762,P <0.05),while N-cadherin was significantly higher in Hep3B + (2.85 ± 0.44) than in Hep3B-(1.00 ± 0.17) (t =4.762,P < 0.05).M2macrophages decreased E-cadherin expression in Hep3 B + + M2 (0.1 ± 0.13) compared with Hep3 B + (t =3.255,P <0.05),while N-cadherin expression increased in Hep3B+ + M2 (4.18 ± 0.52) (t=10.009,P < 0.05).Non-Infected groups didn't change the markers of E-cadherin and N-cadherin.It was suggested that invasion ability of Hep3B increased by HBx overexpression.Conclusions HBX protein and M2 macrophages synergetically mediated the invasion and metastasis of hepatocellular carcinoma cells by EMT.
8.Correlation between Inelligence and Speech Development of School Age Children with Cerebral Palsy

Objective To explore the correlation between inelligence and speech development of school age children with cerebral palsy.Methods 46 school age children with cerebral palsy were evaluated with China Rehabilitation Research Centre Sign-Significance test (S-S)and Wechsler Intelligence Scale. The correlation between S-S and Wechsler Intelligence Scale was analyzed. Results The intelligence quotient decreased in 36 cases (78.26%), mostly mild to moderate. The development of speech delayed in 36 cases (78.26%). The outcomes of Wechsler Intelligence Scale significantly correlated with those of S-S Test (P<0.001). Conclusion There is a correlation between intelligence and speech development of school age children with cerebral palsy.
9.Risk factors of rebleeding in patients with obscure gastrointestinal bleeding

ObjectiveTo investigate the long-term (>1 year) rebleeding rate after capsule endoscopy (CE)-guided intervention in patients with obscure gastrointestinal bleeding (OGIB) and to identify the risk factors of rebleeding.MethodsA total of 307 consecutive patients who underwent CE for OGIB in our hospital from June 2002 to October 2010 were enrolled.Follow-up data were obtained by reviewing medical records,CE database and contacting the patients or their relatives by telephone.We evaluated the rebleeding rates and analyzed risk factors predictive of rebleeding by means of COX ratio hazard model.ResultsThe medium follow-up was 52 months (range13-112 months).Significant lesions were found in 202 patients (65.8%).The overall rebleeding rate after interventional therapy induced by CE findings was 28.0% (86/307).CE positive patients had higher rebleeding rate than CE negative patients (37.6% vs 9.5%,log-rank test,P=0.000),while specific therapy could prevent rebleeding,compared with nonspecific therapy (32.9% vs 23.0%,P=0.042).95.3% (82/86) rebleeding occurred within 24 months after CE.Multivariate analysis performed by using COX proportional hazards model showed that age over 50 years,CE positive findings,lowest hemoglobin (Hb) level 3 months before CE ≤7 g/dl,receiving nonspecific therapy after CE,hypertension,administration of anticoagulants,antiplatelet medicine or NSAIDs after CE were six risk factors associated with rebleeding.Conclusion Clinicians should be aware of these risk factors for OGIB rebleeding,which can reduce the occurrence of rebleeding and improve OGIB patients' prognosis.Those high risk OGIB patients should be followed up for at least 24 months after CE.
10.Efficacy of implantation of a modified temporary metallic stent for cardia achalasia

Objective To study the efficacy and safety of implantation of a modified temporary self-expanding metallic stent for cardia achalasia.Methods A total of 30 patients diagnosed as having cardia achalasia were randomly divided into 2 groups,group A (traditional stents) and group B (modified stents)(n =15 in each group).Two days after stent implantation at the cardia by endoscopy,stents were withdrawn with endoscope guided by X-ray.LES Pressure,X-ray images ( including the diameter of the most dilated part of esophageal and the most narrow part of cardia) and the symptoms of dysphagia were compared before and half year after the treatment.The width changes of the most narrow stenting part on the point of stenting and 2 days after removal were compared.The side effects and complications during the treatment were recorded.Results All thirty stents were successfully implanted and removed.Stent dislocation occured in 2 cases in group A,but none in group B.Dysphagia had significant improvement after the treatment in both groups ( P < 0.05 ),but the recurrence rate of group A ( 26.7% ) was significantly higher than group B (6.67% ) in 6 months ( P < 0.05 ).LES pressure and X-ray images of both groups significantly improved after treatment ( P < 0.001 ),and those of group B were superior to group A ( P < 0.05 ).There was no difference in adverse reaction between the two groups.No perforation occured in any group.Conclusion Self-expanding metallic stents is safe for patients with cardia achalasia,with implantation convenience,symptomatic improvement,low recurrence,and few complications or dislocation.
