1.Association between obstructive sleep apnea-hypopnea syndrome and reflux esophagitis: a cross-sectional study

Objective:This study aimed to evaluate the association between obstructive sleep apnea-hypopnea syndrome (OSAHS) and reflux esophagitis (RE).Methods:This cross-sectional study retrospectively analyzed 218 patients diagnosed with OSAHS by polysomnography (PSG) and who also had undergone gastroscopy at the First People′s Hospital of Yunnan Province from January 2021 to December 2021. The cohort comprised 91 males and 127 females, aged from 19 to 78 years (40.7±13.2). Clinical data, PSG parameters, and gastroscopy findings were collected. The prevalence of RE among OSAHS patients was calculated, potential risk factors for RE were evaluated. Differences in PSG parameters between patients with and without RE were analyzed. Statistical analyses were conducted using SPSS 26.0.Results:The prevalence of RE in OSAHS patients was 20.6% (45/218). Males had a significantly higher RE prevalence than females (31.9% vs. 12.6%, χ2=12.02, P<0.05). The difference remained significant after adjusting for confounding factors (34.9% vs. 11.1%, χ2=10.08, P<0.05). No significant variation in RE prevalence was observed across age groups. However, after adjusting for confounding factors, a significant difference was found between overweight and obese BMI groups (12.5% vs. 29.2%, χ2=4.04, P<0.05). When stratified by apnea-hypopnea index (AHI) severity, RE prevalence increased progressively in mild (7.1%), moderate (18.8%), and severe (30.1%) groups, with statistically significant differences ( χ2=11.45, P<0.05). Positive correlations were found between RE and male sex, AHI, longest apnea time (LAT), and time spent with oxygen saturation below 90% (TS90%) ( rs=0.24, 0.18, 0.17, 0.14, respectively, P<0.05). Regression analysis showed that identified male sex was the primary independent predictor of RE. Patients with RE exhibited higher AHI, TS90%, and LAT compared to those without RE ( P<0.05) .Conclusion:This single-center hospital-based study revealed a relatively high prevalence of reflux esophagitis (20.6%) among patients with OSAHS. Male sex was identified as the main independent factor associated with RE. Furthermore, RE prevalence increased with greater AHI, BMI, LAT and TS90%.
2.A cohort study of lipid levels and recurrence risk of ischemic stroke in a community-based natural population in Songjiang District, Shanghai

ObjectiveTo investigate the recurrence of ischemic stroke (IS) and to analyze the association between four indices of total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and triglycerides (TG) and the risk of IS recurrence by analyzing the follow-up data related to IS in the community-based natural population of Songjiang District, Shanghai, so as to provide a scientific basis for improving the prognosis of stroke patients in the community and controlling IS recurrence. MethodsA prospective follow-up study was conducted among the IS patients in the community-based cohort population, collecting data about patient’s age, gender, disease history, biochemical indicators, and etc. Cox regression model and restricted cubic spline model were used to analyze the relationship between different levels of plasma lipids and the recurrence of IS in these patients. ResultsA total of 1 368 patients with IS were included. The total follow-up duration was 7 171.46 person-years, with a median follow-up time of 6.24 years. There were 420 cases of IS recurrence, resulting in a cumulative recurrence rate of 30.70%. The results of multivariate Cox regression analysis showed that the recurrence risk of IS was reduced when the baseline TC and LDL-C levels of IS patients were in the ranges of 4.65‒5.67 mmol·L-1 and 2.52‒3.46 mmol·L-1, respectively. The results of restricted cubic spline analysis showed a U-shaped relationship between baseline TC and LDL-C levels and the recurrence risk in IS patients. ConclusionThe cumulative recurrence rate of patients with IS in the community of Songjiang District in Shanghai is high, and the levels of TC and LDL-C at baseline survey are correlated with the recurrence of IS in these patients. It is suggested to pay more attention to the levels of LDL-C and TC in patients with IS, so as to improve the prognosis.

5.Dendrobine interferes with diabetic kidney disease by up-regulating HSP90AA1 to regulate immune cells

Objective To explore the mechanism of dendrobine in the treatment of diabetic kidney disease(DKD)by intervening immunity using network pharmacology,bioinformatics and Western blot.Methods The targets of dendrobine were screened by Pharmmaper,Superpred and other databases.The disease targets of DKD were screened from GeneCards,OMIM and other databases.The immune-related targets were searched in Immport,In-nateDB and other databases.The intersection of the above three was obtained via Venn diagram.The PPI map of intersection targets was constructed by STRING software combined with Cytoscape software,and the core targets in the interaction network were mined by CytoHubba plug-in.The data set GSE142025 was obtained from GEO,and immune infiltration and WGCNA analysis were performed using the R language CIBERSORT package.GSE1009,GSE30122 and GSE142025 were used for gene differential expression analysis and ROC verification.The correla-tion between core targets and immune cell phenotype was analyzed.Molecular docking and molecular dynamics sim-ulation analysis were used to test the binding force of dendrobine to key targets.Western blot was used to detect the expression of HSP90AA1 in HK-2 cells.Results A total of 128 intersecting genes,including HSP90AA1,LCK and EGFR,were obtained,and the core targets involved HSP90AA1,LCK and STAT3;molecular docking and molecular dynamics simulation analyses showed that the protein of HSP90AA1 could bind well with dendrobine.Western blot experiments showed that compared with the control group,HSP90AA1 was significantly down-regula-ted in the high glucose group.Compared with the high glucose group,HSP90AA1 was significantly up-regulated in the dendrobine group.Conclusion Dendrobine can intervene in diabetic kidney disease by regulating the expres-sion of HSP90AA1.

Diabetic kidney disease(DKD)is one of the major complications of diabetes mellitus(DM),which significantly affects the survival and quality of life of DM patients.Currently,existing first-line treatment approaches for DKD are ineffective in effectively preventing the occurrence and progression of DKD.Traditional Chinese medicine(TCM)has the advantages of"ultiple pathways,multiple targets,and multiple mechanisms"in treating DKD,and has a broad prospect in the treatment of DKD.Aquaporin-2(AQP2)is a membrane channel protein widely expressed in the kidneys,and its physiological function is highly similar to the function of kidney filtration and reabsorption.Studies have found that various effective components and related formulae of Chinese medicine can improve symptoms in DKD patients by inhibiting the abnormal expression of AQP2.This article provides a review on the role of AQP2 in DKD and the research progress of TCM intervention,aiming to provide insights and references for the development of drugs related to the prevention and treatment of DKD.

Diabetes mellitus(DM)is a disease syndrome characterized by chronic hyperglycaemia.A long-term high-glucose environment leads to reactive oxygen species(ROS)production and nuclear DNA damage.Human umbilical cord mesenchymal stem cell(HUcMSC)infusion induces significant antidiabetic effects in type 2 diabetes mellitus(T2DM)rats.Insulin-like growth factor 1(IGF1)receptor(IGF1R)is important in promoting glucose metabolism in diabetes;however,the mechanism by which HUcMSC can treat diabetes through IGF1R and DNA damage repair remains unclear.In this study,a DM rat model was induced with high-fat diet feeding and streptozotocin(STZ)administration and rats were infused four times with HUcMSC.Blood glucose,interleukin-6(IL-6),IL-10,glomerular basement membrane,and renal function were examined.Proteins that interacted with IGF1R were determined through coimmunoprecipitation assays.The expression of IGF1R,phosphorylated checkpoint kinase 2(p-CHK2),and phosphorylated protein 53(p-p53)was examined using immunohistochemistry(IHC)and western blot analysis.Enzyme-linked immunosorbent assay(ELISA)was used to determine the serum levels of 8-hydroxydeoxyguanosine(8-OHdG).Flow cytometry experiments were used to detect the surface markers of HUcMSC.The identification of the morphology and phenotype of HUcMSC was performed by way of oil red"O"staining and Alizarin red staining.DM rats exhibited abnormal blood glucose and IL-6/10 levels and renal function changes in the glomerular basement membrane,increased the expression of IGF1 and IGF1R.IGF1R interacted with CHK2,and the expression of p-CHK2 was significantly decreased in IGF1R-knockdown cells.When cisplatin was used to induce DNA damage,the expression of p-CHK2 was higher than that in the IGF1R-knockdown group without cisplatin treatment.HUcMSC infusion ameliorated abnormalities and preserved kidney structure and function in DM rats.The expression of IGF1,IGF1R,p-CHK2,and p-p53,and the level of 8-OHdG in the DM group increased significantly compared with those in the control group,and decreased after HUcMSC treatment.Our results suggested that IGF1R could interact with CHK2 and mediate DNA damage.HUcMSC infusion protected against kidney injury in DM rats.The underlying mechanisms may include HUcMSC-mediated enhancement of diabetes treatment via the IGF1R-CHK2-p53 signalling pathway.

Objective To study the application effect of quality control circle(QCC)in reducing the dissatisfaction rate of physical examination clients in health management center.Methods To establish QCC,selected the health check-up popula-tion in our hospital in September-2019 and March-2020,through the questionnaire investigation and analysis,compare the dis-satisfaction of the clients before and after the quality control circle.Results After carrying out QCC activities,the dissatisfaction of physical examination clients was significantly lower than that before QCC,and the difference was statistically significant(P<0.05).Conclusion The activities of QCC in the health management center can effectively improve the quality of the physical examination work and reduce the dissatisfaction of the customers in the physical examination.It is of great significance to the health management.
