1.Clinical efficacy of Quhan Tongluo Formula combined with celecoxib against knee osteoarthritis

Objective To investigate the clinical efficacy and safety of Quhan Tongluo Formula in treating knee osteoarthritis(KOA)of cold-dampness obstruction pattern.Methods A retrospective study was conducted on 231 patients with KOA of cold-dampness obstruction pattern admitted to Dongfang Hospital,Beijing University of Chinese Medicine from December 2022 to December 2023.The patients were divided into an exposed group(105 cases)and a non-exposed group(126 cases)based on whether Quhan Tongluo Formula was applied.The exposed group received Quhan Tongluo Formula combined with celecoxib,while the non-exposed group received only celecoxib.Using propensity score matching based on the baseline data of the medical records,76 pairs(a total of 152 cases)were matched.The changes in Visual Analog Scale(VAS)scores,Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)total scores,and traditional Chinese medicine(TCM)syndrome scores before treatment and at 2,4,and 6 weeks after treatment were compared between the two groups,and safety was evaluated.Results Before treatment,there were no significant differences in VAS scores or WOMAC pain scores between the two groups.After treatment,both groups exhibited significant reductions in VAS scores and WOMAC pain scores,with the exposed group demonstrating significantly greater reductions compared to the non-exposed group(P<0.01).The total clinical effective rate was significantly higher in the exposed group than in the non-exposed group(P<0.01).Conclusion The clinical application of Quhan Tongluo Formula combined with celecoxib demonstrated superior efficacy over celecoxib monotherapy in treating knee osteoarthritis of cold-dampness obstruction pattern.The combined regimen significantly reduced VAS scores,total WOMAC scores,and TCM syndrome scores,with marked improvements in long-term efficacy.
2.Preparation and evaluation of in-house Factor Ⅷ inhibitor-positive quality-control samples

Objective To prepare in-house coagulation factor Ⅷ(F Ⅷ)inhibitor-positive control material and evaluate its perform-ance.Methods Frozen plasma samples from hemophilia A patients with positive factor Ⅷ inhibitors were pooled,and diluted with Owren's Veronal Buffer(OVB)to 1 BU/mL of the inhibitor concentration in the mixture,then aliquoted and freeze-stored.The homo-geneity and stability of the in-house quality control material were verified,and its suitability was further assessed through intra-laborato-ry reproducibility among different technologists and inter-laboratory comparisons.Results Twenty-one aliquots were randomly tested for homogeneity assessment,yielding an average of 1.05 BU/mL(range 0.9-1.15 BU/mL),with a standard deviation(SD)of 0.083 and coefficient of variation(CV)of 7.90%.The freshly prepared inhibitor-positive control samples contained a concentration of 1.03 BU/mL.After storage at-80℃ for 24 hours,1 week,1 month,2 months,3 months,4 months,5 months,6 months,7 months,8 months,and 9 months,thawed the samples showed relative deviations of 9%,0%,10%,9%,14%,15%,6%,0%,-10%,-5%,and 2%,respectively.The intra-laboratory CV value from different technologists at this center was 7.28%,and the inter-labora-tory CV across different centers was 18.75%.Conclusion The prepared in-house positive control material of Factor Ⅷ inhibitor ex-hibited adequate uniformity and stability.

4.Research progress in atrial fibrillation with cerebral small vessel disease.

Non-valvular atrial fibrillation is a common arrhythmia and a major risk factor for cardioembolic stroke. Small cerebral vascular disease is a syndrome of clinical, cognitive, imaging, and pathological manifestations caused by intracranial small vascular lesions. The imaging findings on cranial magnetic resonance usually shows recent subcortical small infarction, vascularised lacunae, white matter hypersignal, perivascular space enlargement, cerebral microhemorrhage, and brain atrophy. It is a major cause of neurological loss and cognitive function decline in the elderly. Current studies suggest that atrial fibrillation may increase the imaging load of cerebral small vessel disease through a series of mechanisms such as microembolization, hypoperfusion, inflammation, endothelial dysfunction, and lymphoid system dysfunction. The imaging of cerebral small vessel disease with atrial fibrillation has a potential relationship with cognitive function decline and is related to the occurrence and prognosis of stroke, even more has a potential role in suggesting the etiology and secondary prevention strategies of ischemic stroke.

5.Simultaneous Determination of 4 Components in Huanshao Capsules by HPLC

OBJECTIVE:To estab lish a me thod for simultaneous determination of morroniside ,loganin,echinacoside and acteoside in Huanshao capsules. METHODS :HPLC method was adopted. The determination was performed on Zhongpuhong RD-C18 column with mobile phase consisted of acetonitrile- 0.1% formic acid solution (gradient elution )at the flow rate of 1.0 mL/min. The detection wavelength was set at 240 nm (morroniside,loganin) and 330 nm (echinacoside,acteoside). The column temperature was set at 35 ℃,and sample size was 10 μL. RESULTS:The linear range were 5.29-105.80 μg/mL for morroniside, 4.49-89.88 mg/L for loganin ,16.26-325.25 mg/L for echinacoside and 16.31-326.25 mg/L for acteoside ,r values were 0.999 9. RSDs of precision ,stability (24 h),reproducibility and durability tests were all lower than 2.0% . The recoveries were 94.34% -96.23%(RSD=0.81% ,n=6),97.04% -98.89%(RSD=0.73% ,n=6),96.23% -98.08%(RSD=0.82% ,n=6), 95.40%-98.47%(RSD=1.23%,n=6),respectively. The contents of above 4 components in 11 batches of Huanshao Capsules were 0.190-0.704,0.439-0.857,2.723-4.475 and 0.589-1.035 mg/g,respectively. CONCLUSIONS :Established method is specific , precise and can be used for content determination of 4 components in Huanshao capsules.
6.Associated factors of screened myopia of junior middle school students in six provinces of China

Objective:
To understand the current situation and associated factors of myopia in junior middle school students, and to provide scientific basis for prevention and control of myopia in junior middle school students.
Methods:
A total of 5 393 junior middle school students were selected from middle schools in Guangdong, Guangxi, Guizhou, Liaoning, Shandong, Shanxi provinces. The visual acuity of middle school students was examined, and the data of general population, economy, sociology and natural environment were obtained through statistical yearbook of each province. The influencing factors of myopia of middle school students were analyzed by univariate and multivariate Logistic regression.
Results:
The results of single factor analysis showed that the myopia rate of junior high school students was different by gender, grades, parents average wage, sunshine duration, temperature, altitude, longitude and latitude(χ2=47.76,59.05,10.79,106.19,53.56,85.02,76.23,107.07,P<0.05). The results of multi factor analysis showed that gender, grade, average wage, temperature and latitude was positively associated with myopia vision; sunshine duration and longitude were negatively associated with the risk for myopia(OR=1.54,1.34,1.62,7.58,27.10,0.42,0.39,P<0.05).
Conclusion
The myopia of junior high school students is affected by a variety of factors, economic and social factors and natural environmental factors have an impact on the screening of sexual myopia in junior high school students. Economic and social factors and natural environmental factors should be taken into account in the formulation of myopia prevention and control measures.
7.Analysis of influencing factors of screened myopia in primary school students in seven provinces

Objective:
To understand the current situation and associated factors of myopia among primary school students, and to provide scientific basis for prevention and control of myopia among primary school students.
Methods:
In Gansu, Guangdong, Guangxi, Guizhou, Liaoning, Shandong, Shanxi and other provinces, 1 prefecture was selected, and a number of primary schools were selected from each region. All the students in the class were selected as the object of this survey. A total of 8 365 middle school students were examined for their eyesight, and the data of general population economic indicators and natural environment indicators were obtained through the statistical yearbook of various provinces and cities. The influencing factors of primary school students myopia were analyzed by chi-square test and multivariate Logistic regression.
Results:
Univariate analysis showed that different provinces and different sex, different nationalities, different grade, parents average salary, sunshine duration, air temperature, altitude, longitude, latitude, different economic zone(χ2=116.22, 18.08, 26.33, 1 059.04, 14.86, 10.28, 16.95, 10.01, 23.15, 29.43, 88.14, P<0.05). Multivariate analysis results showed that gender, grade, sunshine duration, longitude were risk factor for poor vision(OR=1.31, 1.71, 1.45, 1.54, P<0.05); Economic zone and parents salary were protective factors for poor eyesight of students (OR=0.65, 0.86, P<0.05).
Conclusion
Myopia of primary school students is affected by a variety of factors, economic and social factors and natural environmental factors have an impact on the screening.
8.Efficacy of the traditional Chinese medicine NAOAN capsule for primary prevention of stroke in high-risk population

Objective To evaluate the efficacy of the traditional Chinese medicine NAOAN capsule in primary prevention of stroke in high?risk populations. Methods A multicenter, randomized controlled study was performed in community setting, involving 1 088 individuals at high risk of stroke, with cerebrovascular function scores<75 and 10?year Framingham stroke risk ≥6%. Subjects were recruited in communities at Beijing, Shanghai, Changsha, and Chengdu communities, through resident committees or the village unit. A total of 559 subjects were randomized into a group treated with the NAOAN capsule, and 529 subjects in an aspirin treatment group. Follow?up was performed every 2 months for 2 years. At the mid?point and end of the intervention, we compared cerebrovascular function differences between the 2 groups. Results During the 2?year community trial, 531 subjects in the NAOAN capsule group and 465 in the aspirin group followed the protocol. Cerebrovascular function scores increased from 45.2±19.7 at baseline to 61.7±26.5 after the 2?year trial (t=-12.931, P<0.001) in the NAOAN capsule group, and from 47.2±18.9 at baseline to 53.7 ± 25.1 (t=-5.058, P<0.001) in the aspirin group; greater increases in cerebrovascular function scores were found in the NAOAN capsule group than that in the aspirin group (t=4.906, P<0.001). Conclusions Cerebrovascular function in individuals at high risk of stroke was improved by taking NAOAN capsule. Cerebrovascular function scores improved more with NAOAN capsules than with aspirin.
9.Investigation on the detection of transient ischemic attack in cerebrovascular disease surveillance in Hunan Province

Objective To determine the incidence and prevalence of transient ischemic attack (TIA) and to evaluate its epidemiological situation in Hunan province.Methods Seven monitoring points were randomly selected from the province,a total of 8 311 subjects aged≥50 years were then chosen by stratified sampling.The cases counted in prevalence was defined as patients diagnosed before 24:00 o'clock August 31st,2013,and the new diagnosis for incident counting was defined as those diagnosed between 00:00 September 1st,2012 and 24:00 August 31st,2013.Results Among all 8 311 screened subjects,the number of TIA patients was 24 (288.8 per 100 000 people),the incidence of TIA was 7 (85.2 per 100 000 people).Standardized prevalence and incidence were 283.2 and 82.4 per 100 000 respectively using 2010 China census population.Among them,the standardized incidence rate of female was higher than that of male (114.8 per 100 000 person-years vs.48.8 per 100 000 person-years),and the prevalence rate of males was higher than that of female (288.2 per 100 000 people vs.273.2 per 100 000 people).Hypertension is the most important risk factor for TIA (55.2%).Conclusion The incidence and prevalence of TIA in Hunan province are higher than the national average.Hypertension is the main risk factor.
10.Risk assessment of hyperperfusion induced intracranial hemorrhage after stenting in patients with severe carotid stenosis

Objective To assess the risk of hyperperfusion induced intracranial hemorrhage (HICH) after carotid artery stenting (CAS) in patients with symptomatic severe carotid stenosis.Methods From June 2009 to June 2015,the clinical data of 210 patients with symptomatic severe carotid stenosis (70%-99%) treated with CAS at Department of Neurosurgery,Changhai Hospital,the Second Military Medical University,were analyzed retrospectively.Seven of them (3.3 %) developed HICH after operation.The relationship between the clinical baseline and imaging characteristics and HICH were assessed.All patients received the evaluation of cerebral CT perfusion examination.The time to peak (TTP) index of cerebral blood flow was defined as the TTP ratio of the affected and contralateral sides,t test was used to conduct the comparison of measurement data,and Poisson test was used to conduct the comparison of the enumeration data.Results There was significant difference in the TTP index between the HICH group and non-HICH group (1.15 ± 0.10 vs.1.30 ± 0.15,t =4.461,P < 0.01).The receiver operating characteristic (ROC)curve analysis results suggested that the TTP index > 1.22 could be used as a risk factor for predicting HICH (sensitivity 100%,specificity 75.9%).Conclusion Under the condition of prompting the preoperative TTP index > 1.22,the risk of HICH may be higher after CAS in patients with severe carotid stenosis.
