1.Analyses of the epidemiological and clinical characteristics of 21 confirmed monkeypox cases in a district of Chengdu City

ObjectiveTo analyze the epidemiological and clinical characteristics of the 21 confirmed monkeypox cases in a district of Chengdu City, and to provide scientific guidance for the prevention and control of subsequent monkeypox epidemics. MethodsData of confirmed monkeypox cases residing in this district were collected from the Disease Control and Prevention Information System of China. A retrospective descriptive epidemiological analysis was used to analyze the demographic, distributional and behavioral characteristics of the cases. ResultsThe first confirmed case of monkeypox was reported on July 5, 2023. Up to April 30, 2025, a total of 21 confirmed cases of monkeypox have been reported. All cases were male, with a mean age of (30.9±6.2) years. The highest proportion of cases(47.62%) was in the 30‒40 years age group. The majority were men who have sex with men (MSM) population (90.48%, 19/21). The results showed that 19.05% of cases were co-infected with HIV, and 19.05% had a history of syphilis infection. Within 21 days prior to symptom onset, 19 cases (90.48%) self-reported engaging in male-to-male sexual contact, among whom 10 cases (52.63%) reported having taken protective measures, while 9 cases (47.37%) did not take safety precautions. Thirteen cases (61.90%) had no travel history to areas with reported monkeypox cases during the 21 days before symptom onset. The predominant manifestation was exanthem (100%, 21/21), followed by fever (57.14%, 12/21) and lymphadenectasis (47.62%, 10/21). Among febrile cases, 50.00% (6/12) had low-grade fever (37.3‒38.0 ℃). All cases were identified through active medical consultation. The median interval from symptom onset to the first medical visit was 3 (2, 6) days, with a maximum interval of 14 days. The median interval from symptom onset to laboratory confirmation was 7 (5, 9) days. Six cases (28.57%) had two or more visits to the hospital, with bacterial infection being the primary initial diagnosis. ConclusionMonkeypox prevention and control efforts in a district of Chengdu City should prioritize MSM population and young and middle-aged adults aged 30 to <40 years. It is recommended to establish an integrated monkeypox epidemic prevention and control network by leveraging existing HIV/AIDS prevention and control network. Concurrently, accelerating the deployment of the national intelligent infectious disease monitoring and early warning front-end software will strengthen early detection capabilities and be beneficial for the overall effectiveness of epidemic prevention and control efforts.

5.Research development on genetic background and immune characteristics of latent autoimmune diabetes in adults

Latent autoimmune diabetes in adults (LADA) is a type of mixed diabetes in WHO2019 Diabetes classification. LADA has both type 1 diabetes and type 2 diabetes genetic susceptibility genes. It is difficult to distinguish LADA from type 1 diabetes mellitus and type 2 diabetes mellitus. This paper systematically reviewed the genetic background and immune characteristics of LADA, providing new ideas for the study of LADA and theoretical support for the precise diagnosis and treatment of LADA and immune intervention strategies.
6.Exploration on the Role of Autophagy in Diabetic Cardiomyopathy Based on the Theory of Healthy Qi and Pathogenic Factors and Chinese Materia Medica Intervention

The struggle between healthy qi and pathogenic factors runs through the whole process of the formation and development of diabetic cardiomyopathy(DCM),and autophagy is the main regulator of cardiac homeostasis and function.Based on the theory of healthy qi and pathogenic factors,the autophagy state of DCM was understood.In the early stage of DCM,it is mainly characterized by deficiency of healthy qi and supplemented by pathogenic excess.In the middle stage,it is characterized by deficiency of healthy qi and pathogenic excess.In the late stage,it is mainly characterized by pathogenic excess and supplemented by deficiency of healthy qi.TCM clinical practice adheres to the principle of"strengthening the healthy qi and eliminating pathogenic factors",which aims to restore the dynamic balance of autophagy in the body by adjusting the balance between healthy qi and pathogenic factors,and ultimately improve the prognosis of DCM.This article took the theory of healthy qi and pathogenic factors as the starting point,and elaborated on the role of autophagy in the development of DCM from macro and micro perspectives.It explored Chinese materia medica intervention plans for different stages of DCM,providing theoretical reference for the regulation of autophagy in DCM by TCM.
7.Imaging factors affecting early prognosis of syrinx after posterior fossa decompression surgery for Chiari malformation type Ⅰ combined with syringomyelia

Objectives:To investigate the imaging factors influencing the early prognosis of syrinx in patients with Chiari malformation type Ⅰ(CM Ⅰ)and syringomyelia(SM)who underwent posterior fossa decompression(PFD).Methods:Clinical data of 60 patients with CM Ⅰ and SM treated in the Department of Neurosurgery of Peking University Third Hospital from March 2013 to November 2022 were analyzed retrospectively.There were 18 males and 42 females with an average age of 48.8±9.1(range,27-65)years old.All the patients un-derwent PFD.Imaging assessments were performed preoperatively by cervical spine MRI and X-ray,and pa-rameters were collected,including maximum anterior-posterior and transverse diameters of syrinx,spinal cord anterior-posterior diameter,longitudinal distances of bulbo-pontine sulcus,fourth ventricle and cerebellar ton-sil,length of McRae line,length of clivus,clivus-canal angle,C0-C2 and C2-C7 Cobb angles as well as cervical sagittal vertical axis.The maximum syrinx/cord ratio and maximum syrinx area were calculated.With-in 1 month postoperatively,the maximum anterior-posterior diameter of syrinx was measured again by cervical spine MRI and its change rate was calculated to evaluate the early prognosis of syrinx.The patients were di-vided into improved(the change rate≥20%)and unimproved(the change rate<20%)groups.The imaging find-ings were compared between the two groups,and a multi-variate logistic regression analysis was employed to identify significant predictors.Results:All the patients were followed up for 12.2±7.1d(range,5-31d).After PFD surgery,24 cases were improved in syrinx(the improved group),while 36 cases were not improved(the u-nimproved group).Age and gender were not significantly different between the two groups(P>0.05).Postopera-tively,there were 2 cases with fever,2 cases with transient occipital pain,4 cases with CSF leakage,1 case with poor incision healing,and there was no significant difference of overall complication ratio between the two groups(P>0.05).The clivus-canal angle and cervical sagittal vertical axis of CM Ⅰ patients in improved group(153.47°±9.99°,22.10±11.10mm)were significantly greater than those of unimproved patients(147.98°±9.55°,15.83±9.00mm)(P<0.05).The maximum transverse diameter of syrinx in improved group(8.34±3.20mm)was significantly smaller than that of unimproved group(10.36±3.71mm,P<0.05).No significant differences were found in other preoperative imaging parameters(P>0.05).The multi-variate logisitic analysis revealed that the maximum transverse diameter of syrinx before operation was a risk factor of syrinx size reduction(OR=1.281,95%CI 1.042-1.576,P<0.05),while the offset distance of cervical sagittal vertical axis acted as a protective factor of syrinx size reduction(OR=0.916,95%CI 0.853-0.984,P<0.05).Conclusions:In patients with CM Ⅰand SM,preoperative maximum transverse diameter of syrinx and offset distance of cervical sagittal vertical axis are independent influencing factors for early postoperative prognosis of syrinx.
8.Application of mesoporous silica nanoparticle as drug carrier in treating of osteoarthritis

Objective To study the application of mesoporous silica nanoparticle(MSN)as drug carriers in the treatment of osteoarthritis(OA).Methods Core-cone structured MSN(MSN-CC)was synthesized by sol-gel method,modified by polyethyleneimine(PEI),and loaded with hyaluronic acid synthase 2(HAS2).Forty OA model rats were randomly assigned to OA group,MSN-CC-PEI group,hyaluronic acid(HA)group,or MSN-CC-PEI-HAS2 group.Ten healthy rats were assigned to control group.Normal saline 100 μL was injected into the knee joint in the control group and OA group.MSN-CC-PEI 100 μL,HA solution 100 μL,and MSN-CC-PEI-HAS2 100 μL were injected in MSN-CC-PEI group,HA group,and MSN-CC-PEI-HAS2 group,respectively.The levels of HA,interleukin-1β(IL-1β),and prostaglandin E2(PGE2)in the joint effusion and the foot swelling degree were measured at 1,2,and 3 weeks after injection.Results HA level in the MSN-CC-PEI-HAS2 group was significantly lower than that in the HA group at 1 week after intervention(P<0.05),while HA level in the MSN-CC-PEI-HAS2 group was significantly higher than that in the HA group at 2 and 3 weeks after intervention(P<0.05).With the time going on,HA level in the HA group was gradually decreased(P<0.05),but HA level in the MSN-CC-PEI-HAS2 group was gradually increased(P<0.05).At 1 and 2 weeks after intervention,compared with the HA group,IL-1β,PGE2 and foot swelling degree in the MSN-CC-PEI-HAS2 group were increased(P<0.05).At 3 weeks after intervention,compared with the HA group,IL-1β,PGE2 and foot swelling degree in the MSN-CC-PEI-HAS2 group were decreased(P<0.05).With the time going on,IL-1β,PGE2 and foot swelling degree in the HA group and MSN-CC-PEI-HAS2 group were gradually decreased(P<0.05).Conclusion MSN carrier can stably stimulate the secretion of endogenous HA,reduce the inflammatory response within joint cavity of OA rats,and improve the joint morphology and soft tissue pathological changes.
9.PIK3CA Somatic Mutations Are Associated With Lymph Node Metastasis in Endometrial Cancer

Objective To investigate the expression levels and mutation status of phosphatidylinositol-4,5-bisphosphate3-kinase catalytic subunit alpha(PIK3CA)in endometrial cancer(EC)and evaluate its association with lymph node metastasis in EC.Methods We retrosepctively collected and analyzed EC genetic mutation testing data submitted to the Molecular Detection Center,The First Affiliated Hospital of Chongqing Medical University between July 2020 and June 2022.The mutation rate of PIK3CA gene was calculated based on the sequencing results of EC patients,and the correlation between PIK3CA mutations and clinical pathological parameters,as well as protein expression consistency,was analyzed accordingly.Results A total of 97 EC patients were enrolled in this study,and PIK3CA mutations were identified in approximately 48.5%(47 out of 97 cases).The rate of lymph node metastasis in patients with PIK3CA mutations was higher than that in patients with wild-type PIK3CA(21.3%vs.6.0%,P=0.027).Findings from univariate and multivariate logistic analyses indicated that histological subtype Ⅱ(odds ratio[OR]=5.51;95%CI,1.08-28.06;P=0.040),positive result for lymphovascular space invasion(LVSI)(OR=7.96;95%CI,1.37-46.44;P=0.021),and PIK3CA mutation(OR=8.58;95%CI,1.51-48.84;P=0.015)were independent risk factors for lymph node metastasis in EC.In addition,the receiver-operating characteristic(ROC)curves demonstrated that the combined use of clinicopathological parameters and PIK3CA mutations could more accurately predict lymph node metastasis in EC,with an area under the curve of 0.824(95%CI,0.678-0.970).It is noteworthy that there was a high consistency between PIK3CA mutations and its protein expression,and EC patients with positive expression of PIK3CA protein had a higher rate of lymph node metastasis(53.8%vs.9.1%,P=0.078).Conclusion PIK3CA somatic mutations are strongly correlated with lymph node metastasis in EC.
10.IL-17A collaborating with TGF-β1 in regulating benign tracheal stenosis after tracheal injury in experimental dogs

Objective To discuss the effect of interleukin-17A(IL-17A)and transforming growth factor-β1(TGF-β1)on the benign tracheal stenosis after tracheal injury in experimental dogs.Methods The trachea stenosis model of healthy Beagle dogs was established by burning the middle part of trachea with electric snare under bronchoscopy guidance.A total of 21 dogs were divided into normal group(n=3,receiving normal feeding),molding group(n=12,after airway modeling every 3 dogs were sacrificed each week for 4 weeks),IL-17A suppression group(n=3,receiving Secukinumab after airway modeling),and IL-17A inhibitor+TGF-β1 inhibitor group(n=3,receiving Secukinumab and SB43154 after airway modeling).Bronchoscopy and CT scan were performed once a week,and the stenosis degree was calculated.RT-qPCR,immunohistochemistry,and HE staining of the obtained tracheal tissues were performed.Results Within 1-4 weeks after molding,in module-making dogs the degree of stenosis of the injured trachea gradually increased,and the expressions of ECM-related proteins,TGF-β1 and IL-17A were up-regulated.After treatment with IL-17A inhibitors,the inflammatory infiltration and granulation tissue hyperplasia were reduced and the early tracheal stenosis was improved(P<0.05).The combination use of IL-17A inhibitor and TGF-β1 inhibitor had a better remission effect(P<0.05).Conclusion IL-17A and TGF-β1 may synergistically affect the formation of tracheal stenosis.
