1.Correlation between Periostin, IL-33, and chronic cough after thoracoscopic lobectomy in patients with coronary artery bypass grafting combined with lung cancer: A prospective cohort study

Objective To study the correlation between Periostin, interleukin-33 (IL-33), and chronic cough after thoracoscopic lobectomy in patients with coronary artery bypass grafting (CABG) combined with lung cancer. Methods A total of 102 lung cancer and coronary heart disease patients at Tianjin Chest Hospital from January 2022 to January 2024 were prospectively enrolled, and they were divided into a chronic cough group (n=42) and a non-chronic cough group (n=60) based on whether chronic cough occurred after surgery. Serum levels of Periostin and IL-33 were measured on the 1st, 7th, and 14th days post-lobectomy. The Pearson method was employed to analyze the correlation between Periostin and IL-33 levels and the severity of cough. Univariate and multivariate logistic regression analyses were conducted to identify factors influencing the occurrence of chronic cough. Additionally, receiver operating characteristic (ROC) curve analysis was utilized to assess the potential value of serum Periostin and IL-33 levels in predicting postoperative chronic cough. Results In patients with chronic cough, the peripheral blood Periostin and IL-33 levels measured on days 7 and 14 were significantly higher than those in patients with non-chronic cough, and the interactions between the two groups and at different time points were significant (P<0.001). The degree of cough was positively correlated with the levels of Periostin and IL-33 on days 7 and 14 (P<0.05), but had no significant correlation with the levels on day 1 (P>0.05). In patients with lung cancer, after thoracoscopic lobectomy, Periostin [OR=1.619, 95%CI (1.295, 2.025)] and IL-33 [OR=1.831, 95%CI (1.216, 2.758)] on day 7 and Periostin [OR=1.952, 95%CI (1.306, 2.918)] and IL-33 [OR=1.742, 95%CI (1.166, 2.603)] on day 14 were identified as risk factors for chronic cough. ROC curve analysis showed that the sensitivity of Periostin on day 7 was 69.05%, the specificity was 71.67%, and the area under the curve (AUC) was 0.756 [95%CI (0.616, 0.893)]. The sensitivity of Periostin on day 14 increased to 71.43% and the specificity was 76.67%, AUC was 0.762 [95%CI (0.633, 0.898)]. At the same time, the critical value of IL-33 on day 7 was 45.03 pg/mL, the sensitivity and specificity were both 83.33%, the AUC was 0.884 [95%CI (0.789, 0.980)], and the critical value of IL-33 on day 14 was 56.01 pg/mL, the sensitivity was 85.71%, the specificity was 80.00%, and the AUC was 0.899 [95%CI (0.799, 0.999)]. Joint logistic regression analysis of Periostin and IL-33 levels on days 7 and 14 showed showed that the sensitivity was 95.24%, the specificity was 95.00%, and the AUC reached 0.993 [95%CI (0.979, 1.000)]. Conclusion Periostin and IL-33 levels, measured at various time points, are abnormally elevated following thoracoscopic lobectomy in patients with combined CABG and lung cancer. These levels significantly correlate with cough severity. Given their predictive potential for chronic cough, these markers are deemed valuable biomarkers.
2.Clinical characteristics and prognosis of 41 neonates with congenital idiopathic vocal cord paralysis

Objective:To investigate the clinical features and prognosis of neonatal patients with congenital idiopathic vocal cord paralysis (VCP).Methods:A retrospective analysis was conducted on the general condition,clinical manifestations,auxiliary examinations,complications,treatment,and follow-up of 41 cases of congenital idiopathic VCP at the Neonatal Center,Xi'an Children's Hospital,from January 2018 to June 2023.Patients were divided into unilateral vocal cord paralysis (UVCP) and bilateral vocal cord paralysis (BVCP) groups based on the side of the affected vocal cord.The Fisher exact test or Mann-Whitney U test was employed to compare the follow-up indicators between the two groups. Results:A total of 41 cases (26 males,15 females) were included.All were full-term infants with a mean birth weight of (3 240.0±553.9) g.The median age at first visit was 3.0 (1.0,12.5) days.All 41 cases exhibited varying degrees of hoarseness,with 33 (80.5%) experiencing inspiratory stridor,10 (24.4%) having feeding difficulties,and 9 (22.0%) presenting with dyspnea and cyanosis.Serum vitamin D levels were found to be deficient or insufficient in 19 of 24 (79.2%) cases.Eighteen (43.9%) cases were misdiagnosed,and 4 (9.8%) cases were missed.Seven (17.1%) cases were complicated by laryngomalacia.Among the 33 (80.5%) cases in the UVCP group,30 (73.2%) exhibited left VCP and 3 (7.3%) had right VCP.Eight (19.5%) cases were classified in the BVCP group.Nine patients required respiratory support,with 2 undergoing tracheotomy.Ten patients with feeding difficulties received tube feeding,and some underwent swallowing and sucking training.A total of 37 (90.2%) cases were followed up for a median of 2 (1,5) months; 29 (78.4%) had no symptoms,4 (10.8%) showed symptom improvement,2 (5.4%) had persistent symptoms,and 2 (5.4%) died due to treatment interruption.The rates of symptom disappearance and improvement in the UVCP group were significantly higher than those in the BVCP group,while the rate of symptom persistence and mortality were significantly lower in the UVCP group (83.3% vs.57.1%,13.3% vs.0,0 vs.28.6%,3.3% vs.14.3%, P<0.05).No statistically significant difference was found between the two groups regarding the age of symptom disappearance ( P>0.05). Conclusion:Neonatal congenital idiopathic VCP is more prevalent among full-term infants,with most cases presenting hoarseness,vitamin D deficiency,and some associated with laryngomalacia.The condition is often misdiagnosed or missed,but most cases demonstrate a favorable prognosis.
3.Dosimetric study of radiotherapy synchronized with 3D printing-based tumor treating fields for glioblastoma

Objective To investigate the dosimetric effects of tumor treating fields(TTFields)patches on different radiotherapy modes for glioblastoma(GBM)patients who wear TTFields patches during radiotherapy,thereby providing dosimetric guidance for determining the appropriate radiotherapy mode.Methods With the TTFields data from GBM patients,artifact-free radiotherapy CT images were obtained utilizing 3D-printed TPU TTFields patches(3D-Print-TTFields)and anthropomorphic phantoms,and then a TTFields-synchronized radiotherapy image model was constructed.Furthermore,the treatment planning system was used to construct a dosimetric calculation model for TTFields-synchronized radiotherapy by simulating and fitting the ray attenuation rate of TTFields patches measured by accelerators.Using these models,3 kinds of radiotherapy plans were simulated and developed.Specifically,P1 simulated the conventional radiotherapy mode;P2 simulated the TTFields-combined radiotherapy mode(TTF-Com-RT),in which patients underwent radiotherapy using the P1 plan while wearing TTFields patches;and P3 simulated the TTFields-synchronized radiotherapy(TTF-Syn-RT)mode where the TTFields patches were worn throughout the entire radiotherapy process.The paired t-test was used to analyze dosimetric parameters such as target dose(D95),average scalp dose(D-skin),conformity index(CI)and homogeneity index(HI)in 3 plans(P1,P2,and P3),as well as the D95 and D-skin parameters for intensity-modulated radiotherapy(IMRT)and volumetric modulated arc therapy(VMAT)techniques in the P3 plan.Results The D95 simulated by P2 decreased by 1.35%as compared with P1(P<0.05),and the D95 simulated by P3 was 1.31%higher than that in P2(P<0.05).Compared with P1,P2 and P3 increased the D-skin by 12.56%and 14.30%,respectively(P<0.05),and the D-skin simulated by P3 increased by 1.55%as compared with P2(P<0.05).However,there were trivial differences in D95 between P3 and P1,CI and HI among all plans,D95 and D-skin between IMRT and VMAT techniques in P3 plan(P>0.05).Conclusion Based on GBM patient data,CT simulation images obtained from 3D-Print-TTFields combined with anthropomorphic phantom are artifact-free and meet radiotherapy requirements.The target and scalp dose differences between TTF-Com-RT and TTF-Syn-RT are less than 2%,and the dosimetric difference of TTF-Syn-RT using IMRT/VMAT techniques is insignificant.Therefore,clinicians can choose radiotherapy modes and techniques according to actual needs.
4.Forced normalization: a case report and literature review

Forced normalization (FN) is a rare epileptic psychiatric disorder that usually characterized by the disappearance of seizures and acute psychosis in patients with paradoxical normalization of the electroencephalogram following a change in the dose of antiseizure medication (ASM) or the initiation of a new medication. This article reports a case of a young female patient with Lennox-Gastaut syndrome who developed FN twice after a change in the ASM regimen, which improved after ASM reduction and olanzapine treatment. Further literature review summarizing the clinical features of FN found that there were slightly more females than males in patients with FN, the onset was more common in young adults, and most patients had refractory epilepsy. The psychiatric and behavioral abnormalities included delusions, hallucinations, bizarre behavior, mania, depression, and dissociation. The changes in ASM were the main inducing factor. Most patients improved by adjusting ASM or adding antipsychotic drugs. By reviewing this case, this article aims to increase awareness of the clinical features, characteristics of mental behavioral abnormalities, treatment and prognosis of FN and to improve the clinical management of the disease.

Objective:To assess the efficacy and safety profile of antaitasvir phosphate combined with yiqibuvir in the treatment of chronic hepatitis C (CHC) of various genotypes, without cirrhosis or with compensated cirrhosis.Methods:394 cases with CHC from 22 centers were collected from October 2021 to April 2023. They were randomly assigned to receive either the experimental drugs (antaitasvir phosphate 100 mg+yiqibuvir 600 mg) or placebo treatment in a 3∶1 ratio. The patients were administered drugs once a day for 12 consecutive weeks, and then followed up for 24 weeks after treatment cessation. All subjects were unblinded at the four-week follow-up following drug discontinuation, with the experimental drug group continuing to complete subsequent post-discontinuation follow-up. The placebo group was switched to receive the experimental drugs for a repeated 12-week treatment period and followed up for another 24 weeks after discontinuation of the drug (placebo delayed treatment phase).The sustained virologic response rate (SVR12) was observed for subjects in the double-blind phase and the placebo delayed-treatment phase at 12 weeks after treatment cessation.Virological resistance analysis was performed on subjects who failed treatment. Alzheimer's disease (AD) is a common neurodegenerative disorder among the elderly, and BuChE has emerged as a potential therapeutic target. In this study, we reported the development of compound 8e, a selective reversible BuChE inhibitor (eqBuChE IC50 = 0.049 μmol/L, huBuChE IC50 = 0.066 μmol/L), identified through extensive virtual screening and lead optimization. Compound 8e demonstrated favorable blood-brain barrier permeability, good drug-likeness property and pronounced neuroprotective efficacy. Additionally, 8e exhibited significant therapeutic effects in zebrafish AD models and scopolamine-induced cognitive impairments in mice. Further, 8e significantly improved cognitive function in APP/PS1 transgenic mice. Proteomics analysis demonstrated that 8e markedly elevated the expression levels of very low-density lipoprotein receptor (VLDLR), offering valuable insights into its potential modulation of the Reelin-mediated signaling pathway. Thus, compound 8e emerges as a novel and potent BuChE inhibitor for the treatment of AD, with significant implications for further exploration into its mechanisms of action and therapeutic applications.
8.Study on the Distribution Pattern and Driving Factors of Health Poverty among Middle-aged and Elderly People with Chronic Diseases

Objective Based on the assumption of spatial heterogeneity,the distribution pattern and risk characteristics of health poverty in middle-aged and elderly people with chronic diseases are described from the perspective of spatial differentiation.In order to providing a theoretical basis for the optimization of subsequent poverty reduction policies and a model policy for other countries.Methods It used factor detector and interaction detector to capture the role of single-factor and multi-factor interactions on the spatial differentiation of health poverty,and risk detectors were utilized to explore the high-risk factors in risky areas Results The single factor explanation of medical assistance and health education activities is prominent,and the factors such as PM2.5,old-age dependency ratio and urban unemployment rate have strong interaction.Furthermore,it identified high-risk factor characteristics in areas at high risk of health poverty.Conclusion The spatial differentiation pattern of health poverty among the middle-aged and elderly chronic disease population in China is the result of the synergistic driving effect of multidimensional factors,and there is variability in the risk characteristics among regions.The government should establish a contextual optimization strategy and pay attention to the joint effect of multiple factors to establish a synergistic management system.
