1.Diagnostic value of exhaled volatile organic compounds in pulmonary cystic fibrosis: A systematic review

Objective To explore the diagnostic value of exhaled volatile organic compounds (VOCs) for cystic fibrosis (CF). Methods A systematic search was conducted in PubMed, EMbase, Web of Science, Cochrane Library, CNKI, Wanfang, VIP, and SinoMed databases up to August 7, 2024. Studies that met the inclusion criteria were selected for data extraction and quality assessment. The quality of included studies was assessed by the Newcastle-Ottawa Scale (NOS), and the risk of bias and applicability of included prediction model studies were assessed by the prediction model risk of bias assessment tool (PROBAST). Results A total of 10 studies were included, among which 5 studies only identified specific exhaled VOCs in CF patients, and another 5 developed 7 CF risk prediction models based on the identification of VOCs in CF. The included studies reported a total of 75 exhaled VOCs, most of which belonged to the categories of acylcarnitines, aldehydes, acids, and esters. Most models (n=6, 85.7%) only included exhaled VOCs as predictive factors, and only one model included factors other than VOCs, including forced expiratory flow at 75% of forced vital capacity (FEF75) and modified Medical Research Council scale for the assessment of dyspnea (mMRC). The accuracy of the models ranged from 77% to 100%, and the area under the receiver operating characteristic curve ranged from 0.771 to 0.988. None of the included studies provided information on the calibration of the models. The results of the Prediction Model Risk of Bias Assessment Tool (PROBAST) showed that the overall bias risk of all predictive model studies was high, and the overall applicability was unclear. Conclusion The exhaled VOCs reported in the included studies showed significant heterogeneity, and more research is needed to explore specific compounds for CF. In addition, risk prediction models based on exhaled VOCs have certain value in the diagnosis of CF, but the overall bias risk is relatively high and needs further optimization from aspects such as model construction and validation.
2.Effects of unilateral biportal endoscopic transforaminal lumbar interbody fusion on paraspinal muscles

BACKGROUND:With the development of unilateral biportal endoscopic technology,endoscopic lumbar fusion surgery has become an increasingly popular development trend in spinal surgery.Unilateral biportal endoscopic transforaminal lumbar interbody fusion shows significant advantages for preserving paravertebral muscle and reducing intraoperative bleeding in spinal stenosis and lumbar spondylolisthesis in the lumbar spine with degenerative changes.OBJECTIVE:To compare the effects of unilateral biportal endoscopic lumbar interbody fusion and traditional open transluminal lumbar interbody fusion on paraspinal muscles.METHODS:A retrospective analysis was conducted on the clinical data of 60 patients who visited the Department of Spine Surgery,Second Affiliated Hospital of Soochow University from October 2019 to November 2022 and underwent single segment unilateral decompression fusion.They were divided into two groups according to different surgical procedures.Group A received unilateral biportal endoscopic lumbar interbody fusion.Group B received traditional open transluminal lumbar interbody fusion.All patients underwent magnetic resonance imaging examination before and 3 months after surgery.Basic patient information,last follow-up time,magnetic resonance imaging images,visual analog scale scores for lower back and leg pain,and Oswestry Disability Index were collected.RESULTS AND CONCLUSION:(1)Three months after surgery,the cross-sectional area of the bilateral multifidus muscle in group A was significantly larger than that of group B(P<0.05),and the degree of fat infiltration was lower(P<0.01).(2)There was no significant difference in the cross-sectional area and degree of fat infiltration of bilateral erector spinae muscles between the two groups 3 months after surgery(P>0.05).(3)It is concluded that unilateral biportal endoscopic lumbar interbody fusion is more effective than traditional open transluminal lumbar interbody fusion in protecting multifidus muscle,reducing multifidus muscle atrophy and fat infiltration.Both surgical methods did not have a significant impact on the erector spine muscle.
3.Research progress on postoperative hypoglycemia in pheochromocytoma and paraganglioma

Pheochromocytoma and paraganglioma(PPGL)are rare neuroendocrine tumors characterized not only by hemodynamic instability but also by fluctuations in blood glucose levels during the perioperative period.These features are closely associated with significant variations in catechromamine secretion.Hypoglycemia is a common postoperative complication in patients with PPGL and is primarily attributed to rebound insulin secretion and enhanced insulin sensitivity following tumor resection.Postoperative hypoglycemia requires heightened clinical attention due to its often subtle presentation and potential for serious complications.Known risk factors include the presence of an epinephrine-secreting tumor,prolonged operative time,larger tumor size,and a history of end-stage renal disease.Therefore,it is essential to identify high-risk patients preoperatively,ensure meticulous intraopera-tive monitoring,and implement timely postoperative interventions.This article provides a comprehensive review of recent advances in the epidemiology,underlying mechanisms,clinical diagnosis,management strategies,and prognosis of postoperative hypoglycemia in PPGL.Importantly,it introduces novel research directions and concep-tual frameworks for the first time,including the optimal timing and dosage of α-receptor antagonists,potential ap-plications of micro-dose glucagon therapy,and molecular mechanisms with targeted interventions in glucose me-tabolism regulation.This review aims to address the current lack of standardized perioperative management proto-cols and to offer innovative and clinically relevant guidance for healthcare professionals.
4.Improvement of the content determination method for isosorbide dinitrate tablets

Objective:To analyze the reasons for the low results obtained by the determination of isosorbide dini-trate tablets content using the methods in the Chinese Pharmacopoeia and to improve the content determination method.Methods:The content of isosorbide dinitrate in 10 tablets was determined by high-performance liquid chromatography(HPLC)using a Phenomenex Luna C18 column(4.6 mm ×250 mm,5 μm)for separation,with a mobile phase of methanol-water(54∶46),a flow rate of 1.0 mL·min-1,and a UV detector set at a wavelength of 230 nm.The average content of 10 tablets was taken as the result of the content determination.Results:Isosor-bide dinitrate showed good linearity in the range of 49.22 to 1 230.5 μg·mL-1(r=0.999 96);the relative stand-ard deviation was 0.11%,the recovery rate was 96.0%,and the RSD was 1.5%(n=9).Conclusion:This method simplifies the experimental operations and steps,can eliminate the interference factors introduced by the original method,and improves the accuracy of the detection results.
5.The correlation between serum miR-21-5p,miR-1224-5p,miR-181a-5p expression with postoperative delayed healing in patients with femoral neck fracture

Objective To investigate the correlation between the expression of serum microRNA-21-5p(miR-21-5p),microRNA-1224-5p(miR-1224-5p),and microRNA-181a-5p(miR-181a-5p)with postoperative delayed healing in patients with femoral neck fracture(FNF).Methods From October 2021 to April 2024,371 patients with FNF admitted to our hospital were included as subjects.The patients were assigned into a study group(delayed healing,68 cases)and a control group(normal healing,303 cases)based on whether the fracture ends healed 4 months after surgery.Real-time fluorescence quantitative PCR(qRT-PCR)was applied to detect the expression levels of serum miR-21-5p,miR-1224-5p,and miR-181a-5p.Multivariate Logistic regression was applied to analyze the influencing factors of postoperative healing delay in FNF patients.ROC curve was applied to analyze the predictive value of serum miR-21-5p,miR-1224-5p,and miR-181a-5p for postoperative healing delay in FNF patients.Results The levels of serum miR-21-5p,miR-1224-5p,and miR-181a-5p in the study group were lower than those in the control group(P<0.05).Multivariate Logistic analysis showed that age ≥ 60 years old,diabetes,and smoking were risk factors for postoperative delayed healing in FNF patients(P<0.05),and miR-21-5p,miR-1224-5p and miR-181a-5p were protective factors for postoperative delayed healing in FNF patients(P<0.05).ROC curve showed that the AUC values of miR-21-5p,miR-1224-5p,and miR-181a-5p for predicting postoperative healing delay in FNF patients were 0.845,0.809,and 0.797,respectively.When the three were combined for prediction,the AUC increased to 0.933.The combined detection effect of the three indicators was better than that of any individual indicator(Zcombination vs.miR-21-5p=3.444,Zcombination vs.miR-1224-5p=4.401,Zcombination vs.miR-181a-5p=4.279,P<0.05).Conclusion The levels of serum miR-21-5p,miR-1224-5p,and miR-181a-5p are reduced in FNF patients with postoperative delayed healing.The combined detection of the three may have a certain predictive value for postoperative delayed healing in FNF patients.

10.Sequencing analysis of whole genome of one strain of bovine Akabane virus and es-tablishment of fluorescence quantitative PCR for virus detection

In order to understand the prevalence of Akabane disease(AKAD)in Guizhou Province and the molecular characteristics of the isolates,the whole-genome sequence of a strain of Akabane virus(AKAV)from a bovine AKAD-positive sample was determined and analyzed.The genotype and genetic variation of the strain were also explored.Based on the conserved S sequence,a fluores-cence quantitative PCR(qPCR)detection method was established and applied for the investigation of AKAV infection status in four large-scale beef cattle farms of Guizhou.Results showed that the S,M and L fragments of the bovine strain were highly homologous to the Tianjin strain(TJ2016/China/2016)and the Australian strain(JaLAB39/Australia/1959),where they were in the same evolutionary branch and belonged to genotype Ⅱ.Sensitivity assay found that the lowest detection limit was 2.5 X 101 copies/μL.Specificity assay showed the established method detected only AKAV with no amplification on bovine bluetongue virus(BLUV),Pasteurella multocida(PM),bovine infectious rhinotracheitis virus(IBRV)and bovine Mycoplasma bovis.The variation coefficients of inter-and intra batches in the repeatability test were both lower than 2.26%.These findings illus-trated that the established qPCR method had high sensitivity,good specificity and repeatability.A total of 298 serum samples from 4 large-scale beef cattle farms in Qianxi City and Huangping County of Guizhou Province were collected and tested for AKAV by the method.Out of 298 sam-ples,25 positive samples(25/298)were detected as positive with a positive rate of 8.39%.In sum-mary,this work provided the reference data for a deep understanding of the molecular prevalence of AKAV in Guizhou Province and laid foundation for the prevention and control of AKAD.
