Objective·To evaluate the relationships between disc-condyle distance and anterior disc displacement,as well as between disc-condyle distance and disc morphology,in patients with temporomandibular disorders(TMD)using magnetic resonance imaging(MRI)of the temporomandibular joint(TMJ).Methods·From September 2023 to March 2024,90 patients(180 TMJs)who visited the TMJ clinic of Department of Stomatology,The Second Affiliated Hospital of Anhui Medical University,with clinical symptoms of TMD and were diagnosed via MRI with either anterior disc displacement or no significant displacement,were included.Clinical data were collected,and MRI images were used to measure the angle of disc displacement,disc-condyle distance,disc length,and thickness.The degree of disc deformation was assessed.The relationships between clinical symptoms and anterior disc displacement,between anterior disc displacement and both disc morphology and disc-condyle distance,and between disc-condyle distance and disc morphology were analyzed.Results·Among the 90 patients,there were 16 males and 74 females,with a mean age of(28.1±14.5)years.Among the 180 TMJs,175 had clinical symptoms and 5 were asymptomatic.There were 40 joints with no displacement,78 with reducible anterior disc displacement,and 62 with irreducible anterior disc displacement.In the joints with irreducible anterior disc displacement,the proportion of those with two or more symptoms was slightly higher at 62.9%,but the difference was not statistically significant compared with the joints with no displacement or reducible anterior disc displacement.MRI assessment revealed that in the joints with irreducible anterior disc displacement,the proportion of disc deformation type Ⅲ or higher was significantly higher compared with the non-displaced joints(P<0.001).The disc length was significantly shorter(P<0.001),and the intermediate zone thickness was significantly greater(P<0.001)compared with the non-displaced joints.The disc displacement angles at centric closure and maximum opening were also significantly larger(P<0.001).The disc-condyle distance was 3.10(2.70,3.70)mm for non-displaced joints,3.40(3.00,4.00)mm for joints with reducible anterior disc displacement,and 6.60(4.78,7.90)mm for joints with irreducible anterior disc displacement,with significant differences(P<0.001).The disc-condyle distance was 3.10(2.80,3.60)mm for type Ⅰ discs,3.70(3.10,4.60)mm for type Ⅱ discs,5.10(4.00,7.30)mm for type Ⅲ discs,and 6.80(4.98,8.20)mm for type Ⅳ/Ⅴ discs,with significant differences(P<0.001).The disc-condyle distance was negatively correlated with disc length(rs=-0.469,P<0.001),positively correlated with intermediate zone thickness(rs=0.319,P<0.001),and positively correlated with disc displacement angle at centric closure(rs=0.626,P<0.001).Conclusion·With increasing severity of disc deformation,intermediate zone thickness,and disc displacement angle at centric closure,as well as decreasing disc length,the disc-condyle distance increases.The disc-condyle distance is an important indicator for MRI assessment of pathological changes in TMD.

Objective:To build a big data analysis model of comprehensive performance of medical equipment,so as to improve the management efficiency and fine management level for medical equipment of hospital.Methods:The big data analysis model of comprehensive performances of medical equipment was developed through building a data integration platform,and integrating multi-business system data of hospital,and including quantified social benefit data and relative data of the process of procurement and maintenance,which established index system of performance analysis with multi-dimensions and multi-layers.It realized the presentation of evaluation results of performance of medical equipment in the form of visual data reports.Results:The big data analysis model for the comprehensive performance of medical equipment has set a system with more than 10 indicators,which included social-benefit indicators,evaluation indicators of comprehensive performance,income indicators,and the indicator of recovery period of investment,and visual report with 35 pages.Each layer of the performance of equipment was analyzed from 35 angles,which realized a comprehensive,multi-dimensional and refined evaluation for the performance of medical equipment of hospital.Its'agile,easy-to-use,efficient response and automation features can significantly improve work efficiency of business.The model realized automatic data collection through interfaces and other means,and reduced manual input errors,which data accuracy rate was＞99%.The results of testing the performance data of equipment for three consecutive years by using this model indicated that the consuming time of single calculation of business system of hospital was shortened from＞20 min to 5-8 s.Conclusion:The big data analysis model of comprehensive performance of medical equipment can help hospital to identify,prevent and control risks in advance,and improve the efficiency of internal control,and enhance management efficiency and fine management level for hospital,and provide support for management decision for medical equipment of hospital.

OBJECTIVE To explore the effect of Shiquan Dabu Decoction combined with vacuum sealing drainage(VSD)on treatment of the Gustilo Ⅲ open fracture patients with postoperative chronic refractory wound(CRW)infection and observe the changes of nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)/Caspase-1/interleukin-1β(IL-1β)signaling pathways.METHODS A total of 82 Gustilo Ⅲ open fracture patients with postoperative CRW infection who were treated in Shandong Wendeng Orthopedic Hospital from Jan.2020 to Oct.2023 were randomly divided into the sodium chloride irrigation group(the conventional debridement,VSD plus 0.9％sodium chloride injection irrigation)and the Shiquan Dabu Decoction irrigation group(the conventional debridement,VSD plus Shiquan Dabu Decoction irrigation),with 41 cases in each group.Both groups were trea-ted for 14 days and were followed up for 1 month.The indexes were observed and compared between the two groups.RESULTS The total effective rate of clinical treatment of the Shiquan Dabu Decoction irrigation group was higher than that of the sodium chloride irrigation group after the treatment for 14 days(P＜0.05).The average healing time of wound and length of hospital stay were shorter in the Shiquan Dabu Decoction irrigation group than in the sodium chloride irrigation group(P＜0.05).The scores of wound pain,exudation,edema and granulation and the expression levels of NLRP3 mRNA,Caspase-1 mRNA and IL-1β mRNA were lower after the treatment for 14 days than before the treatment,and the above indexes of the Shiquan Dabu Decoction irrigation group were lower than those of the sodium chloride irrigation group(P＜0.05).There were no significant differences in the levels of serum bone morphogenetic protein-2(BMP-2),insulin growth factor-1(IGF-1),tartrate-resistant acid phosphatase 5b(TRACP5b),bone gla protein(BGP),bone-specific alkaline phosphatase(BALP)and total procol-lagen type Ⅰ N-terminal propeptide[PINP]between before the treatment and after the treatment for 14 days or be-tween the two groups.CONCLUSIONS The Gustilo Ⅲ open fracture patients with postoperative CRW infection show the down-regulated expression levels of NLRP3/Caspase-1/IL-1β signaling pathways after the treatment with Shiquan Dabu Decoction combined with VSD,which may relieve the wound pain,exudation and edema and promote the wound healing.It can achieve remarkable curative effect without affecting the bone metabolism indexes.

Purpose To compare the HRCT findings of lung damage in patients with idiopathic inflammatory myopathies(IIMs)with positive and negative anti-Ro-52 antibody.Materials and Methods Fifty patients with IIMs admitted to Beijing Shijitan Hospital,Capital Medical University from January 2015 to July 2023 were retrospectively analyzed.The patients were divided into anti-Ro-52 antibody negative group(18 cases)and positive group(32 cases),the lung CT findings of the two groups were compared,the distribution of lung lesions and related signs were analyzed.Results The incidence of interstitial lung disease in the anti-Ro-52 antibody-negative group was 11.1%(1 case),which was lower than that in the positive group 75.6%(31 cases)(P=0.001).Among the interstitial lung disease-positive patients,the proportion of lesions in the anti-Ro-52 positive group was more symmetrical(28 cases,87.5%,P=0.022)and peripheral distribution(20 cases,62.5%,P=0.039)than that in the anti-Ro-52 negative group.Non-specific interstitial pneumonia-like lesions were the main lesions in the two groups,including 14 cases(43.8%)in the anti-Ro-52 positive group and 7 cases(38.9%)in the negative group.However,9 cases(28.1%)of the anti-Ro-52 positive group had acute interstitial pneumonia-like lesions and 1 case(5.6%)of the negative group had acute interstitial pneumonia-like lesions.Imaging accompanying signs:anti-Ro-52 positive combination and consolidation(14 cases,43.8%,P=0.009),nodular shadow(17 cases,53.1%,P=0.025),and reverse halo sign(12 cases,37.5%,P=0.021)were more than those in the anti-Ro-52 negative group.Conclusion IIMs patients with positive anti-Ro-52 antibody have a higher incidence of interstitial lung disease.HRCT findings are of great significance for diagnosis and treatment of IIMs.

Objective:To explore the application effect of the milestone evaluation system based on ACGME core competencies in the standardized training of residents in the department of rheumatology and immunology.Methods:A modified Milestone Evaluation Scale was developed for the department of rheumatology and immunology. Self evaluation and mentor evaluation were conducted on 150 residents who received standardized training in grades 1-3 before May 2023. The results were collected and statistically analyzed. One-way analysis of variance and t-test were performed using SPSS 27.0. Results:The self-evaluation and mentor evaluation scores of all residents increased with the training years. Self-evaluation scores were significantly higher than mentor evaluation scores ( P<0.05) for the medical knowledge ( t=2.95, P=0.006), diagnosis and treatment ability ( t=2.57, P=0.015), and learning ability ( t=2.67, P=0.015) of grade 2 residents and the learning ability ( t=2.87, P=0.007) of grade 3 residents. There were no significant differences in the baseline score and ability improvement of residents from different institutions ( P>0.05). Conclusions:The milestone evaluation system is an effective tool for training, assessing, and providing feedback to residents in rheumatology and immunology, and provides essential feedback to resident training bases regarding the training effectiveness. The method is worth promoting.
