1.Evaluation of gastric emptying after preoperative oral carbohydrate administration in obese women:a non-inferiority study

Objective To investigate whether the gastric juice volume(GV)of obese women who took carbohydrate 2 hours before operation was equal to or less than that of those who are not routine drinking.Methods Obese patients undergoing elective laparoscopic gynecological surgery under general anesthesia with BMI of 28-32 kg/m2 and ASA grade of Ⅰ-Ⅱ,aged 18-65 years old were selected.The patients were divid-ed into the carbohydrate group(CHO group)and the fasting group(Control group)by random number table method,with 51 cases in each group.The Control group was fasted from 11 pm before surgery,while the CHO group was given 300 mL of carbohydrate orally 2 hours before anesthesia.The main outcome measure was compared with a non-inferiority limit(△)of 17 mL and the mean difference in gastric juice volume between the two groups in the right lateral decubitus position(RLDP)examined by gastric ultrasound.The secondary outcome measures included CSA in right lateral position under gastric ultrasound,Perlas classification,GV/kg,gastric juice pH value,patient satisfaction and gastric emptying time.Results The mean difference of gas-tric juice volume was 7.18 mL(95%CI:-0.06 to 14.43),and the upper limit of 95%CI was lower than the preset non-inferiority limit(△=17.05 mL).There was no statistically significant difference in the mean(standard deviation)gastric juice volume between the control group and the CHO group[40(26,66)mL vs.58(34,65)mL;P=0.43].The gastric volume GV/kg was similar in the two groups(P=0.66),and there was no significant difference in the number distribution of Perlas grades between the two groups(P>0.05).There was significant difference in patient satisfaction between the two groups(P<0.05).Conclusion Ac-cording to the evaluation of gastric ultrasound,compared with fasting at midnight,300ml carbohydrate intake at 2 hours before surgery is not inferior to fasting at midnight in terms of gastric fluid volume in obese women as assessed by gastric ultrasound.
2.Preventive effect of Gehua Jiecheng Decoction in IL-6/JAK2/STAT3 pathway in"inflammation-cancer transformation"of ulcerative colitis with spleen deficiency and damp-heat

Objective:To investigate effect of Gehua Jiecheng Decoction on IL-6/JAK2/STAT3 signaling pathway in colon tissue of ulcerative colitis with"inflammatory to cancer transformation"(UC-UCAC)mice of spleen deficiency and damp-heat type.Methods:Ten from 80 SPF C57BL/6 male mice were randomly selected as blank group and other 70 mice were selected as model group.After establishment of spleen deficient damp-heat model,mice were randomly divided into model group(1st,2nd and 3rd cycle),Gehua Jiecheng Decoction high,medium and low doses groups and mesalazine group,with 10 pieces per group.UC-UCAC transformation model was further established with azo methane oxide solution(AOM)/sodium glucan sulfate(DSS).Each group was treated with corresponding drugs for 4 weeks.General state of mice was observed.Score of disease activity index(DAI)was calculated.HE staining was used to observe colonic mucosa pathology of mice.EGFR,IL-6,JAK2,STAT3 and p-STAT3 proteins and genes expressions in mice colon tissues were detected by Western blot,IHC and RT-qPCR,respectively.Results:Compared with blank group,mice in model group(3rd cycle)were generally in a worse state,colon mucosal tissue was cancerous,DAI score,target proteins and genes expressions were significantly increased(P<0.01).Compared with model group(3rd cycle),general state of mice in all treatment groups were restored and colonic tissues pathology were improved to some extent.Target proteins and genes expressions in other treatment groups were significantly decreased except for Gehua Jiecheng Decoction low-dose group(P<0.01).Conclusion:Gehua Jiecheng Decoction may destroy tumor inflammatory microenvironment,repair damaged colonic mucosa tissue,delay inflamma-tion-cancer transformation process and prevent UCAC by inhibiting activation of IL-6/JAK2/STAT3 signaling pathway.

Osteoporotic vertebral compression fracture (OVCF) can lead to lower back pain and may be even accompanied by scoliosis, neurological dysfunction and other complications, which will affect the daily activities and life quality of patients. Vertebral augmentation is an effective treatment method for OVCF, but it cannot correct unbalance of bone metabolism or improve the osteoporotic status, causing complications like lower back pain, limited spinal activities and vertebral refracture. The post-operative systematic and standardized rehabilitation treatments can improve curative effect and therapeutic efficacy of anti-osteoporosis, reduce risk of vertebral refracture, increase patient compliance and improve quality of life. Since there still lack relevant clinical treatment guidelines for postoperative rehabilitation treatments following vertebral augmentation for OVCF, the current treatments are varied with uneven therapeutic effect. In order to standardize the postoperative rehabilitation treatment, the Spine Trauma Group of the Orthopedic Branch of Chinese Medical Doctor Association organized relevant experts to refer to relevant literature and develop the "Guideline for postoperative rehabilitation treatment following vertebral augmentation for osteoporotic vertebral compression fracture (2022 version)" based on the clinical guidelines published by the American Academy of Orthopedic Surgeons (AAOS) as well as on the principles of scientificity, practicality and advancement. The guideline provided evidence-based recommendations on 10 important issues related to postoperative rehabilitation treatments of OVCF.
4.A Five-step Systematic Therapy for Treating Plugged Ducts and Mastitis in Breastfeeding Women: A Case-Control Study

Purpose:
This study aimed to describe the clinical response to five-step systematic therapy (FSST) in the management of plugged ducts and mastitis. FSST was a comprehensive milk stasis dredging treatment, which contained five steps to make the milk out of the plugged duct.
Methods:
This retrospective study included 922 breastfeeding women, 714 with plugged ducts, and 208 with mastitis who received FSST from June to September 2017. The breast pain score, swelling degree, and range of breast induration were recorded pre-FSST and post-FSST.
Results:
After a single FSST, pain score and swelling degree were significantly improved (both p < .001) in all cases. After FSST, the mean breast pain relief score was 1.69 ± 0.70, whereas the mean swelling fade away degree was 1.61 ± 0.62. In the subgroup analysis, pain score and swelling degree were significantly improved (both p < .001) in the plugged ducts group and the mastitis group. The score of pain relief in the plugged ducts group was less than that in the mastitis group (1.63 ± 0.68 vs. 1.91 ± 0.70, t = 5.30; p < .001), whereas improvement of swelling fade away was greater in the plugged ducts group than the mastitis group (1.65 ± 0.64 vs. 1.48 ± 0.56, t = 3.49; p = .001). The composition ratio of changes in induration range between the two groups was statistically different (Pearson χ2 = 137.87, p < .001), of which more obvious improvement in the plugged ducts group than the mastitis group (χ2 = 25.65, p < .001).
Conclusion
FSST can relieve pain, reduce breast swelling and range of induration, and for plugged ducts or mastitis varied degree differently.

6.Low-dose rituximab combined with dexamethasone in the treatment of refractory ITP in children: Clinical analysis of 31 cases

【Objective】 To investigate the clinical efficacy and safety of low-dose rituximab combined with dexamethasone in the treatment of refractory ITP (RITP) in children. 【Methods】 A total of 31 RITP children, admitted to the Hematology Department of Kunming Children′s Hospital from January 2016 to December 2019 and agreed to receive low-dose rituximab (100 mg/ time, once a week, for 4 successive weeks) combined with dexamethasone (0.6 mg/kg, once a day, for 4 successive days) were enrolled and studied. Blood routine was monitored every other day during treatment, and adverse drug reactions were recorded. The influence of gender, disease course and age on prognosis was compared by χ2 test. 【Results】 1) Among the 31 cases, 11 (35.5%) had platelets >100×109/L after 4 weeks and had no recurrence in 6 months; 9 (29%) had platelets >30×109/L but <100×109/L and had no recurrence in 6 months; 11 (35.5%) showed no recovery of platelets, which were consistently lower than 30×109/L. 2) Rituximab was used in 4 cases (12.9%), 1 case (3.2%) presented with severe drug-induced rashes; Headache, vomiting and elevated blood pressure occurred in 2 cases (6.4%). 1 case (3.2%) presented with laryngeal edema. 3) There was no difference in the total effective rate among different gender, age and disease course (P >0.05). 【Conclusion】 The total effective rate of low-dose rituximab combined with dexamethasone for children with refractory ITP in 6 months is 64.5%, and the adverse reactions are tolerable, so it can be used as a treatment option for children with refractory ITP.
7.Effects of not monitoring gastric residual volume on the risk of ventilator-associated pneumonia: a meta-analysis

Objective:To assess the effects of not monitoring gastric residual volume compared to the routine monitoring gastric residual volume on the risk of ventilator-associated pneumonia.Methods:A systematic literature search of PubMed, EMBASE, Cochrane Library, CNKI, CBM and WanFang Database was conducted from inception up to January 2019.Two independent reviewers screened potentially eligible articles, selected eligible studies and abstracted pertinent data. Relative risk ( RR), weighted mean difference ( MD), and 95% confidence interval (95% CI) were calculated and heterogeneity was assessed with the I2test. Results:Four studies with a total of 785 patients were included in this meta-analysis. No monitoring gastric residual volume did not significantly increase the incidence of ventilator-associated pneumonia ( RR=1.30, 95% CI 0.78-2.16, P=0.32), compared with monitoring gastric residual volume. However, the incidence of vomiting in the no monitoring gastric residual volume group is higher ( RR=1.52, 95% CI 1.20-1.91, P=0.000 4). Not monitoring gastric residual volume decreased the rate of feeding intolerance in critically ill patients ( RR=0.61, 95% CI 0.51-0.72, P<0.01).There were no differences in the duration of mechanical ventilation ( MD=0.39, 95% CI -0.70-1.47, P=0.49) and ICU length of stay ( MD=-0.19, 95% CI -1.55--1.16, P=0.78). Conclusion:The absence of monitoring gastric residual volume did not increase the risk of ventilator-associated pneumonia in critically patient with mechanical ventilation. Gastric residual volume monitoring can not be used as a maker to prevent aspiration and to assess feeding intolerance. We still need to conduct large-scale,well-desighed clinical trials to verify whether gastric residual volume monitoring can be eliminated.
8.Progress on atherosclerosis vaccine

Atherosclerosis is an inflammatory disease which mediated by the immune system of the arterial wall. Specific and non-specific immune systems of arterial wall showed positive immunore action with many endogenous and exogenous antigens.Studies have shown that the immune system promotes atherosclerosis on the one hand and delays atherosclerosis on the other. Therefore,the progress of atherosclerosis can be regulated through activating immune regulation. In this review,the double effects of specific and non-specific immune systems on the process of atherosclerosis were discussed. The studies of those antigen which have been made into vaccines and successfully delayed the process of atherosclerosis in laboratory models were introduced. And the prospects and challenges for future clinical application of atherosclerosis vaccines were discussed in this review.
9.Application of six sigma management model in orthopedic robotic surgery training for new nurses

Objective To explore the effects of six sigma management model in orthopedic robotic surgery training for new nurses of Operating Room. Methods This study selected 20 nurses with entry of 2016 of Operating Room at Henan Provincial People's Hospital in control group,20 nurses with entry of 2017 in experimental group. Control group carried out the orthopedic robotic surgery training with the routine method, while experimental group implemented the training based on the six sigma management model. Examination for new nurses of two groups was carried out with the same examination forms and contents. The investigation adopted the surgeon satisfaction questionnaire and the training satisfaction questionnaire. Results After orthopedic robotic surgery training based on the six sigma management model,the scores of theory knowledge, operational skills and clinical practice of experimental group were(94.72±3.63),(96.95±2.32)and (95.96±2.15)higher than those[(86.41±4.44),(90.15±4.61)and(88.72±4.28)]of control group with statistical differences(P<0.01). After training,surgeon satisfaction with experimental group and control group was(95.56±2.26)and(88.32±4.83)respectively with a statistical difference(P < 0.01). In experimental group,new nurses' satisfaction with training arrangement,training mode,examination,acceptance and total score of satisfaction were(24.08±1.03),(23.78±0.77),(23.95±0.83),(23.71±0.73)and(95.45±1.64) respectively better than those[(21.15±1.14),(21.31±1.21),(21.08±1.04),(20.65±1.23)and(84.22±2.16)] in control group with statistical differences(P< 0.01). Conclusions Six sigma management model can help new nurses of Operating Room to grasp the robotic surgery-related knowledge,increase operation cooperation ability,satisfaction of surgeon and new nurses so as to improve the effects of orthopedic robotic surgery training.
10.Investigation of perceived stress and coping style based on SWOT analysis among nurses of Operating Room in the duration of second pregnancy

Objective:To explore the current situation of perceived stress and coping style based on SWOT analysis among nurses of Operating Room in the duration of second pregnancy and their influencing factors so as to give advice on reducing the stress of nurses of Operating Room in the duration of second pregnancy.Methods:From January to August 2019 , a total of 162 nurses of Operating Room in the duration of second pregnancy at all ClassⅢ Grade A hospitals in Zhengzhou were investigated with the Perceived Stress Scale (PSS) and the Simplified Coping Style Questionnaire (SCSQ) . SWOT analysis was used to assess the advantages, disadvantages, opportunity and threat of external environment, as well as the systematic summary analysis to formulate the effective coping strategy.Results:Among those nurses of Operating Room in the duration of second pregnancy, the total score of perceived stress was (27.43±7.74) ; the scores of perceived stress tension and stress out of control were (13.01±5.10) and (14.42±4.53) respectively. The total score of coping style was (32.30±6.98) ; the scores of positive and negative coping were (21.91±5.55) and (10.40±3.90) respectively. Perceived stress total score had a negative correlation with the coping style total score ( r=-0.295, P<0.01) . Hierarchical regression analysis showed that coping style could independently explain 31.6% of the total variation of perceived stress with a statistical difference ( F=4.130, P<0.01) . Conclusions:Nurses of Operating Room in the duration of second pregnancy have the stress of health risk and take positive coping style when facing stress. Managers should carry human-based management and health education, pay attention to mental health, seek family support and actively guide to reduce the stress of nurses of Operating Room in the duration of second pregnancy.
