1.Study on the equivalence of different decoction processes for a shell-based traditional Chinese medicine drug pair based on rat model of hypertension with liver-yang hyperactivity

OBJECTIVE To investigate the equivalence of different decoction processes based on rat model of hypertension with liver-yang hyperactivity. METHODS Inductively coupled plasma mass spectrometry (ICP-MS) was used to compare the dissolution differences of inorganic elements in the powder-directly-decocted decoction versus the pieces-decocted-first decoction of Ostreae Concha- Haliotidis Concha- Margaritifera Concha drug pair. Six SD rats were included in the normal group. The spontaneously hypertensive rats were given Aconite decoction for six weeks to induce the hypertension model with liver-yang hyperactivity. After successful modeling, 48 rats were randomly divided into the model group, the captopril group [positive control, 8 mL/(kg·d) ] , as well as low-, medium-, and high-dose groups of pieces decocted first or directly powder decocted [2.02, 4.05, 8.10 mL/(kg·d) ] , with 6 rats in each group. Each group received the corresponding drug or equal volume of pure water intragastrically, once a day, for two consecutive weeks. After the last administration, the degree of irritability, facial temperature, pressure pain threshold, blood pressure, and pathological changes of the thoracic aorta were observed in each group. Serum nitric oxide (NO) and plasma angiotensin Ⅱ (Ang Ⅱ), renin, and aldosterone (ALD) levels were also measured. RESULTS ICP-MS analysis results showed statistically significant differences in the contents of macroelements Li, Na, Mg, Ca, Mn, Ga, Sr, Mo, Cd, Sn, and Sb, between the powder-directly-decocted decoction and the pieces-decocted-first decoction ( P <0.05) ,the elements P, Cr, Fe, Ni, Zn, Hg, Tl, and Pb were not detected in either decoction. Animal experiments showed that after two weeks of administration, compared with the model group, the facial temperature, and blood pressure decreased in all treatment groups, while the pressure pain threshold increased; plasma levels of Ang Ⅱ, renin and ALD, as well as the serum level of NO were all decreased, and thoracic aortic media thickness was significantly reduced, most of the differences in the above indicators were statistically significant ( P <0.05 or P <0.01 or P <0.001). Pathological observation showed improvement in thoracic aortic pathological injury. CONCLUSIONS The powder-directly-decocted process for the Ostreae Concha- Haliotidis Concha- Margaritifera Concha drug pair significantly promotes the dissolution of key elements such as Ca, Mg, and Sr without increasing the dissolution of harmful elements. It is equivalent to the traditional pieces-decocted-first in alleviating liver-yang hyperactivity syndrome, lowering blood pressure, and protecting the vascular endothelium, and even shows better performance in some indicators.
2.Sanren Runchang Formula Regulates Brain-gut Axis to Treat IBS-C: A Randomized Controlled Trial

ObjectiveTo observe the clinical efficacy of Sanren Runchang formula in treating constipation-predominant irritable bowel syndrome (IBS-C) by regulating the brain-gut axis and the effects of the formula on serum levels of 5-hydroxytryptamine (5-HT), vasoactive intestinal peptide (VIP), and substance P (SP). MethodsA randomized controlled design was adopted, and 72 IBS-C patients meeting Rome Ⅳ criteria were randomized into observation and control groups (36 cases).The observation group received Sanren Runchang formula granules twice daily, and the control group received lactulose oral solution daily for 4 weeks. IBS Symptom Severity Scale (IBS-SSS), IBS Quality of Life Scale (IBS-QOL), and Bristol Stool Form Scale (BSFS) were used to assess clinical symptoms, and bowel movement frequency was recorded. The Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) were employed to evaluate psychological status. ELISA was employed to measure the serum levels of 5-HT, VIP, and SP. ResultsThe total response rate in the observation group was 91.67% (33/36), which was higher than that (77.78%, 28/36) in the control group (χ2=4.50, P<0.05). After treatment, both groups showed increased defecation frequency and BSFS scores, decreased IBS-SSS total score, abdominal pain and bloating scores, IBS-QOL health anxiety, anxiety, food avoidance, and behavioral disorders scores, SAS and SDS scores, serum 5-HT and VIP levels, and increased SP levels (P<0.05, P<0.01). Moreover, the observation group showed more significant changes in the indicators above than the control group (P<0.05, P<0.01). The SP level showed no significant difference between the two groups. During the 4-week follow-up, the recurrence rate was 5.88% in the observation group and 31.25% in the control group. No adverse events occurred in observation group, and 2 cases of mild diarrhea occurred in the control group. ConclusionSanren Runchang formula demonstrated definitive efficacy in alleviating gastrointestinal symptoms and improving the psychological status and quality of life in IBS-C patients, with a low recurrence rate. The formula can regulate serum levels of neurotransmitters such as 5-HT and VIP, suggesting its potential regulatory effect on the brain-gut axis through modulating neurotransmitters and neuropeptides. However, its complete mechanism of action requires further investigation through detection of additional brain-gut axis-related biomarkers.
3.Targeted Regulation of Inflammation-related Signaling Pathways by Traditional Chinese Medicine for Prevention and Treatment of Atherosclerosis: A Review

Atherosclerosis (AS) is the main pathological basis of cardiovascular diseases and seriously threatens human quality of life. Its prevention and treatment urgently need breakthroughs. The inflammatory response, which runs through the physiological and pathological evolution process of AS, is one of the important mechanisms for AS occurrence. Currently, the treatment methods for AS in Western medicine are relatively mature. However, they have adverse reactions such as abnormal liver and kidney function, drug tolerance, target vessel restenosis, and stent thrombosis, which remain the key bottleneck restricting clinical efficacy. Traditional Chinese medicine (TCM), characterized by multiple components, multiple targets, and multi-pathway synergy, shows unique clinical application potential and efficacy advantages in the intervention of AS. This article reviewed the research progress of TCM in intervening in AS by regulating inflammatory-related signaling pathways, such as nuclear factor-κB (NF-κB), Toll-like receptors (TLRs), mitogen-activated protein kinase (MAPK), and Janus kinase/signal transducer and activator of transcription (JAK/STAT), in the past five years. It summarized the combined mechanism of action of TCM monomers, TCM pairs, and compound preparations in inhibiting the inflammatory cascade reaction through multiple targets, regulating lipid metabolism disorders, and improving vascular endothelial dysfunction and the imbalance of the microenvironment. It deepened the research on the molecular mechanism of TCM in anti-AS, so as to provide a scientific basis for the clinical transformation application and related theoretical research of TCM in anti-AS.

5.Advances in detection techniques for congenital blood group chimerism

Congenital blood group chimerism refers to the coexistence of two or more distinct blood types within an individual, resulting from the presence of hematopoietic cell populations with different genotypes. Consequently, red blood cells in such individuals may express different blood group antigens. Based on the timing and mechanism of formation, blood group chimerism can be classified as either congenital or acquired. Although congenital blood group chimerism is rare and involves complex mechanisms, it holds significant implications in transfusion medicine, transplantation, and obstetrics. This article reviews the formation mechanisms, detection methods, and clinical significance of congenital blood group chimerism in transfusion medicine. Particular emphasis is placed on the principles, advantages, and limitations of various detection techniques. Furthermore, the potential applications of these technologies in clinical diagnosis are discussed, providing a technical foundation for the development of precise transfusion strategies.
6.Novel therapeutic advances for uveal melanoma

Uveal melanoma(UM)is the most common primary intraocular malignancy in adults, characterized by high invasiveness and unique metastatic biological features. Although local treatments(such as proton beam therapy and brachytherapy)can effectively control the primary lesion, approximately 50% of patients eventually develop distant metastasis, with the liver being the primary target organ(occurring in 90% of cases). This highlights a paradigm shift in treatment focus from mere local control to systemic prevention and management. For metastatic UM(mUM), current treatment strategies encompass biomarker-guided molecular targeted therapy, immunotherapy(including Tebentafusp, vaccines, and oncolytic virus therapy), and liver-directed therapy. Focusing on the synergy between local and systemic prevention and control, this article systematically elaborates on the precision local treatment for primary UM, the decision-making pathway for systemic treatment of metastatic UM based on molecular subtyping, the integration of local and systemic therapies for liver metastases, and the translational value of nanomedicine in addressing therapeutic bottlenecks. It provides insights for optimizing clinical management of mUM and developing novel therapeutic strategies.
7.Analysis of Clinical and Phenomics Characteristics of Patients with Phlegm-Stasis Binding Syndrome and Its Accompanied Patterns in Stable Angina Pectoris of Coronary Heart Disease

ObjectiveTo explore the clinical and phenomics characteristics of patients with phlegm-stasis binding syndrome and its accompanied patterns in stable angina pectoris (SAP) of coronary heart disease (CHD). MethodsA multicenter cross-sectional study design was adopted. A total of 300 patients with SAP of CHD were enrolled and classified into 120 cases of phlegm-stasis binding syndrome, 125 cases of qi deficiency-accompanied syndrome, 38 cases of qi stagnation-accompanied syndrome, and 17 cases of toxin accumulation-accompanied syndrome according to traditional Chinese medicine (TCM) patterns. Data of patients with different TCM patterns were collected, including general condition, TCM symptoms, blood lipids, coagulation function, immune indicators, and serum metabolomics. Metabolic pathway enrichment analysis was used to compare differences in phenomics characteristics among groups. Metabolites with variable importance in projection (VIP) ≥1 and fold change (FC) ≥2 were considered as representative differential metabolites. ResultsPatients in each TCM pattern type were most commonly in the 60-75 years age group, with a relatively high proportion of males. Regarding TCM symptoms, patients with phlegm-stasis binding syndrome most commonly presented with wiry-choppy or wiry-slippery pulse, chest pain, and chest tightness; in patients with qi deficiency-accompanied syndrome, fatigue, chest pain, and weak pulse were most common; in patients with qi stagnation-accompanied sydnrome, wiry-choppy or wiry-slippery pulse, chest pain, and symptoms that increase or decrease with emotional changes, belching, or flatulence were most common; in patients with toxin accumulation-accompanied syndrome, bitter taste in the mouth, chest tightness, irritability, restlessness or manic delirium, and dry and hard stools or foul-smelling diarrhea were most common. Comparisons among the different TCM patterns showed statistically significant differences in coagulation parameters (P<0.05), whereas no statistically significant difference was found in blood lipid levels and immune indicators (P>0.05).Metabolomics analysis suggested that disorders of glycerophosphate metabolism and valine, leucine, and isoleucine metabolism are characteristic features of phlegm-stasis binding syndrome and its accompanied patterns. The representative differential metabolites between phlegm-stasis binding syndrome and qi deficiency-accompanied syndrome were 7,8-dihydrobiopterin (FC = 3.58) and oxypurinol (FC = 124.50). Those between phlegm-stasis binding syndrome and qi stagnation-accompanied syndrome were cytidine 5′-diphosphocholine (FC = 2.62) and D-mannosamine (FC = 2.99). Those between phlegm-stasis binding syndrome and toxin accumulation-accompanied syndrome were anserine (FC = 7.83) and canrenone (FC = 8.94). ConclusionThere are certain differences in the clinical characteristics and phenomics characteristics among patients with SAP due to CHD exhibiting phlegm-stasis binding syndrome and its accompanied patterns. The phenomics characteristics mainly involve biological alterations such as lipid metabolism disorders, amino acid metabolism abnormalities, and multiple immune indicators activation, which may provide a reference for precise differentiation and treatment of SAP of CHD in TCM clinical practice.

9.Asociation between intolerance of uncertainty and severity of obsessive-compulsive symptom: the mediating role of cerebellar ALFF value

BackgroundObsessive-compulsive disorder (OCD) carries a considerable disability burden. Intolerance of uncertainty (IU) is associated with the severity of obsessive-compulsive symptoms, yet its neural underpinnings remain unclear. The cerebellum has been implicated in uncertainty processing and demonstrates structural and functional abnormalities in OCD, but direct evidence linking its spontaneous activity to the relationship between IU and obsessive-compulsive symptoms is still lacking. ObjectiveTo explore the pathway through which spontaneous cerebellar neural activity mediates the relationship between IU and obsessive-compulsive symptoms in OCD patients, with the aim of providing a new perspective for understanding the neural mechanisms of OCD. MethodsA total of 51 patients with OCD who visited the Mental Health Center outpatient clinic of the General Hospital of Ningxia Medical University from November 2022 to November 2024 and met the diagnostic criteria for OCD according to the International Classification of Diseases, tenth edition (ICD-10). During the same period, 43 community-based volunteers matched for gender and age were enrolled as the control group. Resting-state functional magnetic resonance imaging data were collected from two groups. Amplitude of low-frequency fluctuation (ALFF) analysis was used to compare whole-brain spontaneous neural activity between two groups. Clinical assessments were conducted using the Yale-Brown Obsessive Compulsive Scale (Y-BOCS) and the Intolerance of Uncertainty Scale-12 (IUS-12). Pearson correlation analysis was used to examine correlations between cerebellar ALFF values and scale scores in OCD patients. Using IU as the independent variable, obsessive-compulsive symptoms as the dependent variable, and ALFF values from between-group difference brain regions as the mediator, Process 4.0 for SPSS 24.0 (model 4) was used to examine the mediating role of these ALFF values between IU and obsessive-compulsive symptoms. The Bootstrap method was used to test the mediation effect. ResultsA total of 47 OCD patients and 40 controls completed this study. Compared with the control group, the OCD group showed decreased ALFF values in the right cerebellar Crus Ⅱ (t=-5.254, FWE-corrected P<0.01). Correlation analysis revealed that ALFF value in the right cerebellar Crus Ⅱ was negatively correlated with Y-BOCS total score (r=-0.851), compulsive behavior subscale score (r=-0.647), IUS-12 total scores (r=-0.617), and prospective IU subscale scores (r=-0.717) in OCD group (P<0.01). Prospective IU subscale scores were positively correlated with Y-BOCS total scores (r=0.811) and compulsive behavior subscale score (r=0.535) (P<0.01). Mediation analysis showed that ALFF value in the right cerebellar Crus Ⅱ region partially mediated the effect of prospective IU on obsessive-compulsive behavior, with a mediation effect of 0.064 (95% CI: 0.004–0.136), accounting for 24.57% of the total effect. ConclusionPatients with OCD showed reduced spontaneous neural activity in the right cerebellar Crus Ⅱ region, which was significantly associated with IU and obsessive-compulsive symptoms. This region partially mediated the effect of IU on compulsive behaviors. 10.Research on decision-making method of updating medical equipment based on intuitionistic fuzzy TODIM

Objective:To construct a updating decision model of medical equipment based on multi-criteria,so as to provide scientific guidance for decision of management departments for medical equipment.Methods:To establish an evaluation index system for updating medical equipment,and evaluate the importance of expert,and transform weight language of expert index into intuitionistic fuzzy numbers,and use the Intuitionistic Fuzzy Weighted Average(IFWA)operator to aggregate qualitative indicators,and construct an initial decision matrix by integrating quantitative data,and apply the Tomadade Decis?o Iterativa Multicritério(TODIM)method to conduct prioritization for medical equipment that was waiting update.Four B-ultrasound equipment that were waiting for update in Peking University First Hospital from January 2023 to December 2023 were selected,and they were labeled respectively as A1,A2,A3 and A4 for priority ranking to conduct update.Results:The obtained comprehensive scores of A1,A2,A3 and A4 were respectively 0.617,0,0.714 and 1.000 through intuitionistic fuzzy theory and TODIM calculations.The priority ranking for updated equipment was ε(a4)>ε(a3)>ε(a1)>ε(a2),indicating that medical equipment A4 was the highest-priority equipment for update.The result was consistent with actual operation that A4 equipment had been scrapped for disposal as scheme in April,2024.Conclusion:The model of updating decision for medical equipment on the basis of the intuitionistic fuzzy TODIM,can effectively avoid the subjectivity of medical institutes in updating equipment and solve the problem of multi-criteria decision-making in updating medical equipment.
