OBJECTIVE To investigate the effect of pharmaceutical services guided by root cause analysis （RCA） in a problem-oriented manner combined with knowledge-attitude-practice （KAP） theory on reducing the incidence of protocol violations of investigational medicinal products in pediatric clinical trials. METHODS A total of 617 participants from 69 drug clinical trial projects conducted in our hospital from January 2016 to December 2020 were selected as the control group， and 868 participants from 72 drug clinical trial projects from January 2022 to December 2025 as the observation group. RCA was performed on the protocol violations of investigational medicinal product in the control group to identify the types and underlying causes. The control group received routine pharmaceutical services for drug clinical trials， while the observation group was provided with precision pharmaceutical services from the three dimensions of knowledge， attitude and practice on the basis of routine pharmaceutical services， according to the root causes identified by RCA. The occurrence of investigational medicinal products protocol violations was compared between the two groups. RESULTS The total incidence of protocol violations of investigational medicinal products， as well as the incidences of minor and major protocol violations， were all significantly lower in the observation group than in the control group （ P ＜0.001）. The main types of protocol violations in both groups included missed/under-/over-dosing of medications， non-adherence to administration time， failure to adjust dosage as required， and combined medication/vaccination in violation of the protocol. Regarding the responsible subjects of protocol violations， the incidences of protocol violations attributed to participants and their guardians as well as investigators and accidental factors were significantly lower in the observation group than in the control group （ P ＜0.001， P ＜0.001， P ＝0.025）. However， there were no statistically significant differences in the incidences of protocol violations caused by sponsor-related reasons between the two groups （ P ＞0.05）. CONCLUSIONS Pharmaceutical services led by pharmacists， based on problem-oriented RCA and combined with KAP theory， can effectively reduce the protocol violations of investigational medicinal products rate in pediatric clinical trials， thereby safeguarding the safety and rights of study participants.

Objective:To conduct a symptom network analysis in patients with systemic lupus erythematosus (SLE) and identify core symptoms, in order to provide a foundation for the development of precision-based symptom management interventions.Methods:A convenience sampling method was used to recruit 420 SLE patients who received treatment at Shengjing Hospital of China Medical University between November 2022 and July 2023. The SLE Specific Symptom Checklist was administered to these patients. A contemporaneous symptom network was constructed using a Gaussian graphical model, and centrality measures including closeness centrality, betweenness centrality, and expected influence coefficient were calculated to determine the core symptoms within the network.Results:The most commonly reported symptoms among the 420 patients were fatigue (65.7%, 276/420), hair loss (59.8%, 251/420), photosensitivity (56.9%, 239/420), memory impairment (54.5%, 229/420), and facial swelling (52.1%, 219/420). In the symptom network analysis, the most central symptom was photosensitivity (closeness centrality=1.58; betweenness centrality=2.56; expected influence coefficient=1.06), followed by skin fragility (closeness centrality=1.00; betweenness centrality=0.43; expected influence coefficient=1.78), gastrointestinal discomfort (closeness centrality=1.30; betweenness centrality=1.14; expected influence coefficient=0.77), fatigue (closeness centrality=0.50; betweenness centrality=0.70; expected influence coefficient=1.61), and mood changes (closeness centrality=0.65; betweenness centrality=1.14; expected influence coefficient=0.89) .Conclusions:Among the various symptoms experienced by SLE patients, photosensitivity, skin fragility, gastrointestinal discomfort, fatigue, and mood changes emerged as core symptoms that influence and activate other symptoms in the network. Healthcare professionals should tailor symptom management strategies based on patients' self-reported symptom profiles, prioritize intervention targets, and work to reduce the overall symptom burden in individuals with SLE.

Objective To examine the differential expression of plasma circRNA in individuals with type 2 diabetes mellitus(T2DM)and to elucidate the potential role of circRNA in the pathogenesis and progression of T2DM.Methods A total of 33 newly diagnosed T2DM patients who were hospitalized in the Department of Endocrinology of Heze Municipal Hospital and 33 healthy subjects with normal glucose tolerance(NC)were enrolled in this study from March 2022 to March 2023.Three subjects with T2DM and 3 with NC were randomly selected from study population and underwent high-throughput sequencing to identify differentially expressed circRNA.Six circRNA with significant differences were selected,and validated in the remaining study population using Real-Time Quantitative Polymerase Chain Reaction(RT-qPCR).Bioinformatics analyses were conducted on the differentially expressed circRNA-associated genes,and their interaction with microRNA(miRNA)was predicted.Results Microarray analysis revealed 166 significantly differentially expressed circRNA(FC≥2,P<0.05)in the T2DM group compared with NC group.Among them,137 were up-regulated and 29 were down-regulated.RT-qPCR validation of six circRNA showed that the expression level of hsa_circ_0000705 was significantly higher,while the expression levels of hsa_circ_0005362 and hsa_circ_0042839 were significantly lower in T2DM group,consistent with the microarray results.However,hsa_circ_0117392,hsa_circ_0008311,and hsa_circ_0087641 showed no significant changes.Conclusion Differentially expressed circRNA are present in T2DM patients.Hsa_circ_0005362 was significantly down-regulated and may be involved in the development of T2DM through the regulation of related signaling pathways by targeting miR-128-1-5p.

A novel technology is proposed for non-contact and real-time detection of atrial fibrillation using millimeter-wave radar.A 60 GHz PCR millimeter wave radar is used to continuously detect the chest echo signal of the subject.After signal acquisition,I-Q signal is generated through I-Q demodulation,and the signal phase information is extracted using effective points phase trend evaluation for obtaining the signals from oscillations in the chest wall,from which the respiratory signals and cardiac signals are extracted through digital filtering for the analysis of cardiac movement.Whether the atrial fibrillation occurs or not is determined by the characteristics of atrial fibrillation wave in the time domain.The effective points phase trend evaluation for extracting more accurate signal phase information and the time-domain method for real-time atrial fibrillation detection are the innovations of the study.The experimental results show that the proposed method achieves a detection accuracy of 99.2%in clinic.

Objective To explore the mechanism of miR-421 affecting the occurrence and development of depression.Methods A depressive rat model was established by single intraperitoneal injection of lipopolysaccharide(LPS),and depressive behavior was detected by glucose preference test and open-field test.miRNA microarray chips and RT-PCR were used to analyze the expression level of miR-421 in hippocampus of the depressed rats.TargetScan database and mi RDB database were used to predict the target genes of miR-421.Dual luciferase reporter gene assay was used to observe the binding of miR-421 to the target genes.The impact of over-expression and inhibition of miR-421 on target genes was observed,then the influence of over-expression and inhibition of target genes on downstream factors was observed,and the related mechanism of miR-421 on depression was explored.Results miRNA microarray chips and RT-PCR assay showed that miR-421 was highly expressed in the hippocampus of the depressed rats(P<0.001),Inhibition of miR-421 expression could significantly restore the body weight and exercise ability of the depressed rats(P<0.001).Binding targets of Menin and miR-421 were predicted by TargetScan database,and interaction between Menin and miR-421 was demonstrated by dual-luciferase reporter gene assay.Menin expression was down-regulated while miR-421 was overexpressed(P<0.001),whereas it was up-regulated as miR-421 was inhibited(P<0.001).qPCR indicated that expressions of Caspase-3 and NF-κB in the hippocampus of the depressed rats was significantly increased(P<0.001),and IL-1β expression in the hippo-campus was significantly increased(P<0.01).When the expression of Menin was inhibited,the expressions of Caspase-3,NF-κB and IL-1β were increased(P<0.001),while the expressions of Caspase-3,NF-κB and IL-1β were decreased when Menin was overexpressed(P<0.001).Conclusions Inhibition of miR-421 expression can increase Menin expression,decrease Caspase-3 content,and reduce neuroinflammatory response,thereby improving depressive symptoms.

Objective:To explore the relationship between the degree of devascularization and clinical outcomes after interventional embolization and sclerotherapy for peripheral arteriovenous malformations.Method:A retrospective analysis was conducted on the data of 37 patients with peripheral arteriovenous malformations admitted at Department of Cardiovascular Surgery, China-Japan Friendship Hospital from July 2021 to June 2023. All patients received the treatment of "nidus" and/or outflow veins embolization combined with sclerotherapy injection. Two experienced physicians evaluated the degree of devascularization before and after treatment, and conducted a correlation study with clinical outcomes after follow-up.Result:All 37 patients were symptomatic. Swelling and pain accounted for 75.7% of all the cases. Twenty-six patients received only one procedure, 3 patients received re-interventional treatments. The average follow-up time was（13.3±5.0）months. Clinical symptoms were completely relieved in 14 patients, and partial relief in 22 patients. The overall effective rate was 97%. There were 6 patients with degree of de vascularization<50% during procedure, 16 patients with degree of 50%-75%, and 5 patients with degree of 75%-90%, 10 cases with degree over 90%. Patients with devascularization degrees less than 60% can not achieve clinical symptom relief.Conclusions:There is a positive correlation between the degree of devascularization and clinical outcomes in the interventional embolization and sclerotherapy of peripheral arteriovenous malformations, and 60% of the degree of devascularization can serve as the "threshold" for effectiveness of treatment.

Objective:To develop college students' proactive aggression questionnaire (CPAQ) and test its reliability and validity.Methods:From March 2023 to April 2024, a questionnaire survey was conducted among 1 552 college students(618 individuals in Sample 1, 783 individuals in Sample 2, 151 individuals in Sample 3 (retested after a 6-month interval), and 49 individuals in Sample 4 (randomly selected from Sample 3)).The reliability and validity of CPAQ were assessed using SPSS 24.0 and Mplus 8.3 software through item analysis, exploratory factor analysis, confirmatory factor analysis, internal consistency reliability, test-retest reliability, compatibility validity, discriminant validity, and empirical validity.Results:The results of the project analysis showed that the 33 items in the CPAQ were significantly correlated with the total score( r=0.25-0.78, P<0.05), and item scores between the high and low group were significantly different (all P<0.001). Exploratory factor analysis retained 28 items, which belonged to three dimensions of proactive aggression (instrumental motivation, moral inhibition, and moral disengagement). The results of confirmatory factor analysis showed that the three-dimensional model of the questionnaire was well fitted ( χ2/ df=3.25, CFI=0.96, TLI=0.95, RMSEA=0.05, SRMR=0.06). The Cronbach's α coefficients for the total score of CPAQ and its sub dimensions were 0.93, 0.95, 0.89 and 0.75, respectively(all P<0.05). The test-retest reliability was 0.64, 0.63, 0.67 and 0.60, respectively(all P<0.01). In the compatibility validity test, the instrumental motivation dimension score was positively correlated with the proactive aggression dimension score of reactive-proactive aggression questionnaire(RPQ)( r=0.50, P<0.05), while moral inhibition dimension was negatively correlated with empathy and guilt scores( r=-0.30, -0.39, P<0.05). The dimension of moral disengagement was positively correlated with the score of the moral disengagement scale( r=0.58, P<0.05). In the discriminant validity test, the correlation of CAPQ and RPQ reactive aggression dimension score was not significant( r=0.08, P=0.16). In the criterion related validity test, the correlation coefficients between the score of CAPQ and each calibration questionnaire and scale were 0.70-0.83. In the empirical validity test, there was a significant difference in the different CPAQ score groups for proactive aggressive behavior(high group: (1.16±0.18), low group: (1.04±0.08), t=2.60, P<0.05). Conclusion:CPAQ has good discrimination, reliability, and validity, which could be used as a tool to measure proactive aggressive behavior among college students.

OBJECTIVE To explore the extraction process of volatile oil from the stems， leaves and roots of Glehnia littoralis， analyze the chemical components of the volatile oil from the stems， leaves and roots of G. littoralis， and preliminarily evaluate its in vitro antifungal activity. METHODS Based on the steam distillation method， single factor test and orthogonal experiment were conducted to optimize the extraction method of volatile oil from the stems， leaves and roots of G. littoralis. The chemical components of the volatile oil from the stems， leaves and roots of G. littoralis were identified by using gas chromatography-mass spectrometry （GC-MS) technology and their relative contents were calculated. The antifungal activity of volatile oils from the stems， leaves and roots of G. littoralis against Fusarium solani， Fusarium incarnatum， Fusarium oxysporum， Aspergillus parasiticus and Aspergillus flavus was determined by paper diffusion method. RESULTS The optimal extraction process of G. littoralis was solid-liquid ratio of 1∶15， distillation time of 5 hours， and KCl concentration of 15%. Eleven components were identified from the volatile oil of the stems and leaves of G. littoralis， and a total of eight components were identified from the volatile oil of the roots. Ginsenethinol was a common component in the volatile oil from the stems， leaves and roots of G. littoralis， its contents in the stems and leaves， roots were 38.21% and 74.02%， respectively. The volatile oil from the stems， leaves and roots of G. littoralis had a certain E-mail：zwhjzs@126.com inhibitory effect on F. solani， F. incarnatum， F. oxysporum， A. parasiticus and A. flavus， especially volatile oil from the stems and leaves. CONCLUSIONS There is a significant difference in chemical components of the volatile oil between the roots， stems and leaves of G. littoralis， both of which have certain in vitro antifungal activity.

OBJECTIVE To provide reference for clarifying improvement effects of Anemarrhena asphodeloides on sepsis- induced myocardial injury and potential material basis. METHODS Water extract of A. asphodeloides was extracted by thermal reflux method. Total xanthone and total saponins in A. asphodeloides were separated by macroporous adsorption resin. The mice model of sepsis-induced myocardial injury was established by intraperitoneal injection of lipopolysaccharide. The effects of the location of three extraction fractions and the monomers of A. asphodeloides as mangiferin， timosaponin AⅢ and timosaponin BⅡ on the survival rate of the model mice were explored. HE staining was used to observe the effects of mangiferin， timosaponin AⅢ and timosaponin BⅡ on myocardial morphology in model mice. The effects of mangiferin on mRNA expressions of inflammatory cytokines [interleukin-6 （IL-6）， IL-1β and tumor necrosis factor-α （TNF-α）] and the level of reactive oxygen species （ROS） in myocardial tissue of model mice were detected. RESULTS Compared with the model group， the survival rate of mice in the intervention group of total xanthone， total saponins and water extract was increased to different extents， especially total xanthone fraction. Mangiferin， timosaponin AⅢ and timosaponin BⅡcould improve the degree of myocardial cell swelling and muscle bundle arrangement disorder in model mice， especially mangiferin. Compared with model group， mRNA expressions of IL-6， IL- 1β and TNF- α， ROS level in myocardium of mice after mangiferin intervention were decreased to different extents. CONCLUSIONS The different extraction fractions of A. asphodeloides can improve survival rate of mice with sepsis-induced myocardial injury， especially total xanthone fraction. Mangiferin is the best among the three monomers of A. asphodeloides to improve sepsis-induced myocardial injury， which may play a role in anti-sepsis myocardial injury by anti-inflammation and antioxidantion.
