To analyze the trends in disease burden and risk factors of depression among the elderly population in China from 1990 to 2021, and to provide a theoretical basis for the prevention, treatment, and policy-making of geriatric depression in China. Data on the disease burden of geriatric depression in China from 1990 to 2021, including the number of incident cases, disability-adjusted life years (DALYs), incidence rate, and DALY rate, were extracted from the 2021 Global Burden of Disease (GBD) database. The Joinpoint regression model was used to analyze the trends by calculating the annual percentage change (APC) and average annual percentage change (AAPC).The autoregressive integrated moving average (ARIMA) model was employed to predict the disease burden of geriatric depression over the next five years.Population attributable fractions (PAFs) were used to describe the risk factors for geriatric depression in China in 1990 and 2021. From 1990 to 2021, the number of incident cases and the incidence rate of geriatric depression in China showed an overall upward trend. The most significant increase in incidence was observed in the 60-64 age group, while the prevalence rate increased notably in the ≥ 95 age group.TheDALY rate showed the most pronounced upward trend in the 65-69 age group. The incidence, prevalence, and DALY rates of geriatric depression were higher in women than in men.Major risk factors included child hood sexual abuse and intimate partner violence, with the impact of intimate partner violence being particularly significant among women. The ARIMA model predicted that the incidence, prevalence, and DALY rates of geriatric depression in China would decline over the next five years, with a greater decline observed in women than in men. From 1990 to 2021, the incidence, prevalence, and DALY rates of geriatric depression in China showed an overall upward trend, with higher rates observed in women than in men.Greater attention should be paid to the elderly female population, with a focus on early prevention to reduce the disease burden of geriatric depression.

Objective:To adapt the clinical nursing practice guidelines for myeloproliferative neoplasms (MPN) to form high-quality localized evidence, so as to provide a basis for clinical knowledge transformation, and enable clinical practitioners to care for patients based on standards and evidence. Methods:We carried out guideline adaptation registration on the Practice Guideline Registration for Transparency. Guided by the ADAPTE method, evidence was searched and screened from top to bottom based on the "6S evidence pyramid" model. We conducted quality evaluation and content analysis on the included guidelines to extract, organize, and adapt recommendations on MPN clinical nursing practices. The search period was from September 2011 to September 2022.Results:A total of six evidence-based guidelines, six expert consensus guidelines, and four clinical decisions were included. A total of 40 recommendations were extracted, which were adapted into six Level Ⅰthemes for admission assessment, symptom management, risk factor management, medication management, treatment goal management, and follow-up management, as well as 19 Level Ⅱ themes. Conclusions:The guidelines included are of high overall quality, with rich, specific, and highly consistent recommendations, which can provide resource preparation for later evidence conversion. Some content still needs to be supplemented and expanded. It is recommended that scholars in this field develop comprehensive and specific clinical nursing practice guidelines based on clinical needs.

Objective:To introduce the concept and procedure of CTA guided precision flap surgery and report the preliminary clinical application. Methods:The theory of CTA guided precision flap surgery is: based on CTA high-throughput data, comprehensively evaluate the wound and potential donor sites, and select the best plan for wound repair. This article reviews 177 patients treated under the guidance of CTA guided precision flap surgery from August 2014 to December 2018. Among them, 131 were males and 46 were females; they were 7-72 years old, with an average age of 39.5 years. The tissue defects were: 74 cases of foot and ankle, 64 cases of wrist, 15 cases of calf, 11 cases of forearm and upper arm, 7 cases of thigh and perineum, 5 cases of face and neck, and 1 case of abdomen. The size of the defect was 2 cm×1 cm to 65 cm×50 cm. The recipient and donor sites were assessed based on CTA guided precision flap surgery, then choose the optimal flap to repair the defect. The flap types, outcomes and complications were recorded. The visual analogue scale (VAS), British Medical Research Council sensory rating scale, and revascularization assessments of digital replantation standard by Hand Surgery Branch of Chinese Medical Association was used to assess clinical results. Results:All flaps were uneventfully harvested as planned according to the CTA guided precision flap surgery concept.A total number of 181 flaps were harvested, including 125 free flaps and 56 pedicled flaps. The flap area rangedfrom 4 cm×3 cm to 40 cm×8 cm. After surgery, 165 cases were survival completely, 7 cases were total necrosis, 9 cases were partial necrosis. A total of 174 cases of flaps survived. The mean follow-up was 9.2 months (range, 6-60 months). After surgery 53 flaps were bulky, and 7 cases with other complications. At the last follow-up, only 6 patients had mild pain, the VAS score was 1-3. The sensation of the flaps recovered partially. According to the British Medical Research Council sensory rating scale: 68 cases were grade S2 and 106 cases were grade S3. According to the revascularization assessments of digital replantation standard by Hand Surgery Branch of Chinese Medical Association, 171 cases were excellent and 3 cases were good. Conclusion:The concept of precision flap surgery based on CTA can effectively guide flap surgery and achieve precise wound repair.

Objective: To observe the clinical efficacy of Tuina (Chinese therapeutic massage) plus oxiracetam in treating mild vascular dementia (VD) and seek its underlying mechanism. Methods: Ninety-six patients with mild VD were randomized into an observation group and a control group, with 47 cases in the observation group and 49 cases in the control group. The control group received oral oxiracetam capsules for treatment, and the observation group was given additional Tuina treatment. Before and after treatment, the mini-mental state examination (MMSE) was adopted to assess the patient's cognitive function; the activities of daily living (ADL) scale was used to evaluate their ability to conduct daily activities; changes in the serum inflammatory factors and oxidative stress indicators were also detected. Results: After treatment, the serum content of malondialdehyde (MDA) decreased in both groups (P<0.05) and was lower in the observation group than in the control group (P<0.05); the serum contents of superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) increased in both groups (P<0.05) and were higher in the observation group than in the control group (P<0.05); the serum contents of interleukin (IL)-1, tumor necrosis factor (TNF)-α, IL-6, and IL-8 declined in both groups (P<0.05) and were lower in the observation group than in the control group (P<0.05). After the intervention, the levels of systolic velocity (Vs) and mean velocity (Vm) of the middle cerebral artery elevated, and the pulsatility index (PI) dropped in patients in the two groups, showing significant intra-group differences (P<0.05); the levels of Vs and Vm in the observation group were higher than those in the control group, and the PI was lower in the observation group than in the control group, showing significant between-group differences (P<0.05). The MMSE and ADL scores increased in both groups after the intervention (P<0.05) and were higher in the observation group than in the control group (P<0.05). Conclusion: In the treatment of mild VD, Tuina plus oxiracetam can improve the cerebral blood supply, ADL, and cognitive function; the mechanism may be associated with the reduction of oxidative stress damages and inflammatory reactions.

The coronavirus disease 2019 (COVID-19) pandemic is a major public health event caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). COVID-19 has spread widely all over the world. A high proportion of patients become severely or critically ill, and suffer high mortality due to respiratory failure and multiple organ dysfunction. Therefore, provid-ing timely and effective treatment for critically ill patients is essential to reduce overall mortality. Convalescent plasma therapy and pharmacological treatments, such as aerosol inhalation of interferon-α (IFN-α), corticosteroids, and tocilizumab, have all been applied in clinical practice;however, their effects remain controversial. Recent studies have shown that extracorporeal therapies might have a potential role in treating critically ill COVID-19 patients. In this review, we examine the application of continuous renal replacement therapy (CRRT), therapeutic plasma exchange (TPE), hemoadsorption (HA), extracorporeal membrane oxygenation (ECMO), and extracorporeal carbon dioxide removal (ECCO2R) in critically ill COVID-19 patients to provide support for the further diagnosis and treatment of COVID-19.

Chinese Critical Care Medicine 2020;32(4):504-507
Sepsis is a life-threatening systemic inflammatory response syndrome (SIRS) caused by the host's maladjustment response to infection, which eventually leads to septic shock and multiple organ failure. Pancreatic injury was found to be an important pathological change in sepsis. Autophagy is a crucial way to maintain the normal metabolism of cell substances and energy, which plays an important role in many diseases. Recent studies have found that autophagy plays a dual role in pancreatic injury in sepsis. Moderate autophagy can protect the pancreas and reduce the injury, while excessive autophagy can cause apoptosis-related autophagic cell death and aggravate the pancreatic injury. In sepsis, activated nuclear factor-κB (NF-κB) has a promoting effect on autophagy, and lysosome associated membrane protein (LAMP) degradation can result in impaired autophagy flux and aggravate pancreatic injury. The exploration of the mechanism of autophagy in pancreatic injury of sepsis will help to restore the normal autophagy function, so as to find a new target for the treatment of pancreatic injury of sepsis.

Objective:To evaluate the clinical performance of postoperative deformity after the release of simple syndactyly, and to discuss the corresponding method for repair. Methods:Clinical data of 25 children with postoperative deformities after simple syndactyly releasing were reviewed retrospectively in the past 5 years, including 13 males and 12 females, with an average age of 3 years and 2 months. The possible causes leaded to postoperative deformity were analyzed by investigating the data of the first operation, including the age of the first operation, whether there was skin graft and whether there was postoperative infection. The common types of postoperative deformities were summarized through morphological observation, and the main methods of reoperation repair were reviewed. During follow-up, the improvement of postoperative deformities after repair was evaluated from the four aspects of webbed shape, finger shape, fingernail shape and skin color difference, so as to propose an effective repair plan for such postoperative deformities. Results:The average age of the first operation was 1 year and 5 months, 15 cases had no skin graft with tense skin, and 4 cases had a history of wound infection. The postoperative deformities of simple syndactyly can be summarized in four main categories: web deformity in 27 webs, finger deformity in 53 fingers, nail deformity in 46 nails and skin color difference in 15 fingers. During the revision surgery, for web deformity, Z-plasty technique were used to widen narrow webs, the flaps were used to reconstruct the recurred webs and the double wing flap were used in 13 webs. For finger deformity, in all cases, most of scar skin was reserved and released by multiple Z-plasty incisions to extend and straighten the fingers, while full-thickness skin grafting was employed if lacking of enough skin to close wound. For nail deformity, using the adjacent flap pushed forward to wrap the nail margin in 14 crooked nail cuticles and others remained untreated. For skin color difference, using z-plasty incision to break the large piece of dark skin into small one and remove the overly dark skin as much as possible. The average follow-up time after reoperation was 33 months, and all webs reached the normal depth and width. Except for the incomplete correction of the lateral deviation in 1 finger, the deformity of the other fingers hand was corrected completely. There was no improvement in other nail deformity except for 10 skew nail cuticle being improved. The skin color difference were improved in all cases. Conclusions:The occurrence of postoperative deformity of simple syndactyly may be related to the tight stitching (should have skin grafting) and the wound infection during primary surgery. The reconstructive operation should be performed about one year after the initial operation when the scar is softened. The flaps for construction of web space, especially double wing flap, can be used widely in all kinds of web deformities, which could result in excellent web shape. The area of skin grafting can be reduced dramatically by reserving softened scar skin. Multiple Z-plasty technique can make use of the transverse excess skin to extend the longitudinal skin and corrected enlarged finger bodies. When should be used to reduce the color difference. But the repair of most of nail deformity were too difficult to improve.
