1.Expert Consensus on Clinical Application of Qidong Yixin Oral Liquid

The prescription of Qidong Yixin oral liquid is derived from the experience of national medical master Ren Jixue in treating viral myocarditis (VMC). It has the functions of tonifying Qi, nourishing the heart,calming the mind, and relieving palpitations. It is used to treat VMC and angina pectoris of coronary heart disease caused by deficiency of both Qi and Yin. However,the understanding of its efficacy evidence, advantageous aspects, dosage and administration, and medication safety remains insufficient in clinical practice. Therefore,the development of the Expert Consensus on the Clinical Application of Qidong Yixin Oral Liquid (hereinafter referred to as consensus) was initiated. Consensus strictly followed the process and methods of the expert consensus on the clinical application of Chinese patent medicines of the China Association of Chinese Medicine,successively completing multiple tasks such as the consensus project initiation,determination of clinical problems,evidence search and evaluation,formation of recommendation opinions and consensus suggestions,solicitation of opinions,peer review, submission for review and release, and so on. Consensus formed a total of 10 recommendation opinions and 12 consensus suggestions,clarifying the clinical positioning,efficacy advantages,syndrome differentiation,dosage and administration,combination therapy,timing of medication,adverse reactions,contraindications, and precautions of Qidong Yixin oral liquid,indicating that it has good clinical advantages and safety in the treatment of VMC and angina pectoris of coronary heart disease,providing norms and references for physicians to safely and rationally apply Qidong Yixin oral liquid. Consensus was reviewed and approved for release by the Standardization Office of the China Association of Chinese Medicine on December 23, 2024. Standard number:GSCACM-376-2024.
2.Wumeiwan Promotes M1 Polarization of Tumor-associated Macrophages to Treat Metastatic Colorectal Cancer

ObjectiveTo explore the effects of Wumeiwan on liver metastasis and lung metastasis of colorectal cancer and its potential mechanism. MethodsFirstly, mice were randomized into control, low-dose (20 g·kg-1) Wumeiwan, high-dose (40 g·kg-1) Wumeiwan, and paclitaxel (10 mg·kg-1) groups. Secondly, liver metastasis and lung metastasis models of colorectal cancer were established in mice. After 4 weeks of intervention, the body weight of each mouse was recorded, and the lung weight, liver weight, and survival time of mice with metastatic colorectal cancer were determined. Hematoxylin-eosin (HE) staining was employed to detect the effects of Wumeiwan on liver metastasis and lung metastasis. Real-time PCR was employed to determine the mRNA levels of M1 and M2 macrophage markers in the liver tissue. Finally, the content of M1 macrophage markers CD80 and CD86 in the liver tissue was measured by flow cytometry. ResultsCompared with the control group, Wumeiwan and paclitaxel reduced the body weight (P<0.01) and liver weight (P<0.01) and prolonged the survival of the mouse model of liver metastasis of colorectal cancer (P<0.01). In the mouse model of lung metastasis of colorectal cancer, Wumeiwan and paclitaxel also reduced the body weight (P<0.01) and lung weight (P<0.01) and extended the survival time (P<0.01). Histopathological results showed that compared with the control group, Wumeiwan inhibited the liver and lung metastases of colorectal cancer. Real-time PCR results showed that compared with the control group, Wumeiwan upregulated the mRNA levels of M1 macrophage markers IL-1β, IL-6, tumor necrosis factor-α (TNF-α), inducible nitric oxide synthase (iNOS), and prostaglandin-endoperoxide synthase 2 (PTGS2) in the liver and lung tissue of mice with liver metastasis and lung metastasis of colorectal cancer (P<0.01). Meanwhile, Wumeiwan downregulated the mRNA levels of M2 macrophage markers Arg1, CD163, and CD206 (P<0.01). Meanwhile, the flow cytometry results showed that compared with the control group, Wumeiwan increased the content of CD86 and CD80 (P<0.01). In addition, immunohistochemical results showed that Wumeiwan promoted the expression of CD86 and inhibited the expression of CD206 in the liver and lung tissue of mice with liver metastasis and lung metastasis. ConclusionWumeiwan can inhibit the liver metastasis and lung metastasis of colorectal cancer by promoting the M1 polarization of macrophages in the liver and lung of the model mice.
3.Clinical doctor-patient shared decision-making: the “collision” between Western theories and Chinese culture

This paper reviewed the development history of doctor-patient shared decision-making (SDM) at home and abroad, emphasizing the importance of cross-cultural analysis in constructing a Chinese doctor-patient SDM model. It also delved into the relationship between Western “individualistic” sociocultural values and doctor-patient SDM, as well as the influence of China’s “collectivist” sociocultural values on doctor-patient SDM, revealing significant disparities in doctor-patient SDM models under distinct sociocultural contexts. Although the doctor-patient SDM theory in China originated from the West, this theory requires profound “collision” and adaptation with local Chinese culture to form a localized theory suited to China’s national conditions. Through cross-cultural adaptation and integrating China’s familism tradition and medical ethics concepts, the future construction of the doctor-patient SDM model in China should emphasize family members’ involvement and seek cultural balance to facilitate its widespread application in clinical practice.
4.Impact of blood donation scenario difference on donor characteristics and blood quality in Xi'an

Objective: To explore the impact of different blood donation scenarios on population characteristics and blood donation behaviors, and to provide a basis for precise blood donation recruitment. Methods: Eligible voluntary blood donors with complete records from four scenarios in Xi'an: medical institutions, cultural tourism SITES, commercial superstores, and blood centers. After a preliminary analysis of overall donor characteristics across the four scenarios, three core subgroups were defined based on scenarios and donor population differences: the medical subgroup (aged 30-39 years), the blood center subgroup (aged 30-39 years), and the cultural tourism and commercial superstore subgroup (aged 18-29 years). Baseline characteristics, blood donation behaviors, and blood test results were compared among the subgroups. Results: The blood donors in all four scenarios were mainly male, but the proportion of female donors in the medical scenario was the highest (26.4%, 8 878/33 634). In terms of age distribution, the cultural tourism and commercial superstore scenarios were dominated by donors aged 18-29, while the blood center and medical scenarios were dominated by those aged 30-39, with the highest proportion of donors over 40 in the medical scenario. The occupational and educational composition of blood donors in the blood center scenario spanned a wide range, exhibiting a clear bipolar distribution. Significant differences were observed in baseline characteristics, blood donation behaviors, and blood test results among the three core subgroups (P<0.001). The medical subgroup was mainly composed of enterprise/industrial workers and married individuals, with high proportions of first-time blood donors, and the highest unqualified rate of infectious indicators (2.0%, 274/13 509). The cultural tourism and commercial superstore subgroup was mainly unmarried and students, featured high proportions of large-volume donations and evening donations, and had a relatively high unqualified rate for ALT (0.4%, 130/31 443). The blood center subgroup had a complex population structure, a high proportion of repeat blood donors (45.3%, 6449/14 225), and moderate results in all test indicators. Conclusion: There is an inherent correlation between the attributes of blood donation scenarios, population characteristics, and blood donation behaviors. The differences among the three core subgroups provide a clear basis for precise scenario operation. By constructing transformation channels between different scenarios and optimizing scenario-specific strategies, the structure of blood donors can be improved, thereby supporting the sustainable development of voluntary blood donation.
5.Serum levels of basic helix-loop-helix transcription factor 40, serine protease inhibitor family E member 1, and biliverdin reductase B in patients with acute cerebral infarction and their association with the degree of carotid atherosclerosis and short-term prognosis

目的 探究急性脑梗死(ACI)患者血清碱性螺旋-环-螺旋转录因子40(BHLHE40)、丝氨酸蛋白酶抑制剂家族E成员1(SERPINE1)、胆绿素还原酶B(BLVRB)水平与其颈动脉粥样硬化(CAS)程度、近期预后的关系。方法 选择2023年4月—2024年8月邯郸市第一医院(以下简称“本院”)收治的112例ACI患者(研究组),另选取在本院通过健康体检的110例健康人为对照组。根据患者颈动脉内膜中层的厚度(IMT)结果,将研究组患者中的22例纳入正常组,38例纳入增厚组,52例纳入斑块组。以预后评估结果为基准,将研究组患者中的68例纳入预后良好组,44例纳入预后不良组。用ELISA法测定BHLHE40、SERPINE1、BLVRB在ACI患者血清中的表达水平;多因素Logistic回归分析ACI预后不良危险因素;ROC曲线分析BHLHE40、SERPINE1、BLVRB的预测价值。结果 与对照组比较,研究组患者血清BHLHE40、SERPINE1、BLVRB水平明显升高(P<0.05);与正常组对比,增厚组、斑块组患者血清BHLHE40、SERPINE1、BLVRB水平明显升高,且斑块组显著高于增厚组(P<0.05);与预后良好组比较,预后不良组血清BHLHE40、SERPINE1、BLVRB水平及NIHSS评分明显提高(P<0.05);在ACI患者中,血清BHLHE40、SERPINE1、BLVRB水平升高,NIHSS评分提高是其预后不良的危险因素(P<0.05);BHLHE40、SERPINE1、BLVRB联用,预测ACI预后不良的AUC为0.939;联合预测AUC优于单独预测(Z值分别为3.804、3.197、3.240,P<0.05)。结论 ACI患者血清BHLHE40、SERPINE1、BLVRB水平升高,三者不仅与病情进展相关,亦是预后的影响因素;联合检测可有效提升对ACI不良预后的预测效能。
6.Advances in the role of ketone body metabolism in the pathogenesis of diabetic retinopathy

Ketone body metabolism plays a significant role in the development and progression of diabetic retinopathy(DR), which closely related to the system and local metabolic disorders as a major microvascular complication of diabetes mellitus. Previous research has established a close relationship between dyslipidemia and DR progression. Ketone bodies, comprising β-hydroxybutyrate, acetoacetate, and acetone, are metabolic products generated from fat breakdown when glucose metabolism is impaired. Studies have revealed that ketone body metabolism is intricately linked to multiple pathophysiological processes in DR, including oxidative stress, inflammatory responses, and neurodegeneration within retinal cells. This article provides a review exploring the impact of ketone body metabolism on the pathogenesis of DR, and systematically reviews the latest research progress on the impact of ketone bodies on the core pathological links such as retinal vascular barrier destruction, glial cell activation and angiogenesis through metabolic reprogramming, epigenetic modification and cell signal transduction, so as to provide a theoretical basis for in-depth understanding of the metabolic driving mechanism of DR.
7.Preventive suggestions and development trajectories of symptom clusters in 286 patients with acute pancreatitis

Objective To explore the occurrence and development trajectories of symptoms at different time points in patients with acute pancreatitis (AP), and to analyze the influencing factors and preventive measures of development trajectories of AP symptom clusters. Methods A convenient sampling method was used to select AP who were admitted from January 2023 to December 2023 were selected and included in the study. The symptoms at different time points were recorded. The severities of symptom clusters in AP patients were explored, and the development trajectories of main symptom clusters were analyzed. Univariate and multivariate logistic regression analyses were used to analyze the influencing factors of development trajectories of symptom clusters in AP patients. Results The incidence rates of abdominal pain, dry mouth, abdominal distension and lack of energy were higher in AP patients during hospitalization. The incidence rates of lack of energy, anxiety, abdominal pain and sleep disturbance were higher on the 1st month after discharge. The incidence rates of abdominal distension, abdominal pain, sleep disturbance and anxiety were higher on the 3rd month after discharge. The incidence rates of anxiety, abdominal pain and irritability were higher on the 6th month after discharge. The fatigue symptom cluster, psychological symptom cluster and gastrointestinal symptom cluster were extracted during hospitalization and on the 1st month and the 3rd month after discharge, and the psychological symptom cluster and gastrointestinal symptom cluster were extracted on the 6th month. The severity scores of symptom clusters at each time point were statistically different (P<0.05). The development of gastrointestinal symptom cluster in AP patients was mainly low decline. The development of psychological symptom cluster was mainly high decline. Drinking history and diabetes mellitus were the influencing factors of development trajectory of gastrointestinal symptom cluster in AP patients (P<0.05). High disease severity, drinking history and biliary tract disease were the influencing factors of development trajectory of psychological symptom cluster in AP patients (P<0.05). Conclusion The symptom clusters of AP patients changes over time, with digestive, fatigue, and psychological symptoms being the main groups in the early stage, and psychological and digestive symptoms persisting in the later stage. Early identification and intervention are crucial for improving the prognosis of AP patients.
8.Teaching practice of the problem-based hands-on inquiry-based comprehensive experiment of blood-borne protozoa infections and diagnosis

Cultivating and inspiring students’ interests in performing experiments and improving students’ diagnostic skills and scientific research capability for infectious diseases like malaria are critical to comprehensive experimental teaching of morphology. Consequently, Soochow University initiated a problem-based hands-on inquiry-based comprehensive experiment program of blood-borne protozoa infections and diagnosis, which took students in the “5 + 3” integrated program of clinical medicine as the teaching targets, and it consisted of three parts: pre-class, in-class, and post-class. Before the experimental curriculum, students learned the theoretical knowledge and the process of modeling Plasmodium berghei and Babesia microti infections in mice through online course and virtual simulation experiments, and during the experimental curriculum, students performed exploratory experiments on differential diagnosis of P. berghei and B. microti infections with pathogenic and serological tests. After the experimental curriculum, students performed molecular biological testing and extracurricular scientific research project training through open experiments. A questionnaire survey was conducted among 99 students in the “5 + 3” integrated training program of clinical medicine in batch 2021, and a total of 93 valid questionnaires were retrieved, with a questionnaire recovery rate of 93.94%. Questionnaire survey showed that 70.97% (66/93), 70.97% (66/93), 77.42% (72/93), 70.97% (66/93), and 83.87% (78/ 93) of the students strongly agreed with the five statements in the questionnaire respectively, namely “high interest in learning during the experiment”, “reasonable experimental content settings and good classroom atmosphere”;, “teachers were good at guiding students’ practice and thinking”, “students were the main body of the classroom during the experiment” and “Comprehensive experiments had better teaching effects than traditional verification experiments”, indicating that the problem-based hands-on inquiry-based comprehensive experiment teaching has enhanced students’ learning interest, spirit of inquiry, innovative thinking, and teamwork ability.
