Objective:To investigate the reference intervals of serum uric acid levels in normal-weight children and analyze the factors influencing these levels. Methods:In this cross-sectional study, clinical data were collected from 7 910 normal-weight children, aged 1 month to 15 years, who underwent health check-ups at the Children′s Hospital of Zhejiang University School of Medicine between August 2013 and August 2023. Data included sex, age, pubertal signs, blood pressure, and serum uric acid levels. The participants were categorized into 4 age groups: 1-<12 months, 1-<6 years, 6-<11 years, and 11-<16 years, and were further analyzed by sex. The P5 and P95 percentiles of uric acid values were defined as the lower and upper limits of the reference interval, respectively. Correlation analysis and partial correlation analysis were used to assess the relationship between uric acid and other variables such as age, body mass index Z value, and Tanner stage. Multivariate linear regression was used to compare uric acid levels across gender and age groups, respectively. Results:Among the 7 910 children, the distribution across age groups was as follows: 562 (317 boys) in 1-<12 months, 4 120 (2 366 boys) in 1-<6 years, 2 357 (1 432 boys) in 6-<11 years, and 871 (536 boys) in 11-<16 years, the uric acid levels in boys were significantly higher than those in girls ( P<0.05). Uric acid levels exhibited a positive correlation with age ( r=0.47 , 0.20, both P<0.001), and a weak positive correlation with BMI Z-scores(both r=0.11, P<0.001). Among participants aged 6-<11 years and 11-<16 years, uric acid levels in boys were positively correlated with Tanner stage ( r=0.10, 0.27, both P<0.05), but no significant correlation was observed in girls (all P>0.05). The uric acid levels were significantly higher in boys than in girls in the 1-<12 months, 1-<6 years and 11-<16 years age groups (all P<0.05), but no significant gender difference was found in the 6-<11 years groups ( P>0.05). Uric acid levels exhibited statistically significant variations among age groups ( P<0.001). The reference intervals of uric acid values were as follows in 1-<12 months age group, 157-335 μmol/L for boys and 160-315 μmol/L for girls; in 1-<6 years age group, 180-359 μmol/L for boys and 180-355 μmol/L for girls; in 6-<11 years group, 190-375 μmol/L; in 11-<16 years age group, 237-480 μmol/L for boys and 218-410 μmol/L for girls. Conclusions:Reference intervals for uric acid varying significantly across different pediatric age groups. Sex, and pubertal development status are closely related to uric acid levels.

Objective:To investigate the association between serum ferritin levels, erythrocyte parameters, and the risk of developing type 2 diabetes mellitus. Methods:A total of 11 408 subjects aged 40 to 70 years who underwent physical examinations at a hospital in Qingdao from 2014 to 2015 were enrolled in the study. Data were collected through questionnaires, physical examinations, and assessments of blood biochemical indices, serum ferritin, and related erythrocyte parameters. A multivariate logistic regression model was employed to analyze the relationship of serum ferritin and red cell parameters with type 2 diabetes mellitus, while a linear regression model was utilized to examine the association between serum ferritin, red cell parameters and fasting blood glucose levels. Results:The median ( Q1, Q3) age of the subjects was 51 (45, 58) years, with 55.3% (6 305) being male. The prevalence of type 2 diabetes in women was 5.9% (300/5 103), which was lower than that in men [13.0% (817/6 305)] ( P<0.001).After adjusting for age, sex, body mass index (BMI), smoking status, alcohol consumption, and family history of diabetes, serum ferritin levels were found to be positively associated with both type 2 diabetes and fasting blood glucose levels, with OR (95% CI) and β(95% CI) values of 1.70 (1.37, 2.11) and 0.002 (0.002, 0.003), respectively. Additionally, hemoglobin, red blood cell count, and hematocrit were positively correlated with type 2 diabetes and fasting blood glucose, with OR (95% CI) values of 1.72 (1.32, 2.24), 1.91 (1.51, 2.41), and 1.52 (1.17, 1.97), and β (95% CI) values of 0.008 (0.006, 0.011), 0.365 (0.286, 0.445), and 2.543 (1.564, 3.521), respectively. Conversely, mean erythrocyte volume was negatively associated with type 2 diabetes and fasting blood glucose, with OR (95% CI) and β (95% CI) values of 0.54 (0.45, 0.66) and -0.017 (-0.023, -0.011), respectively (all P values<0.001). Conclusion:The findings indicate that serum ferritin and related erythrocyte parameters are significantly correlated with the risk of type 2 diabetes mellitus and fasting blood glucose levels.

Objective:To explore the effect of ultrasound-guided stellate ganglion block (SGB) combined with cognitive behavioral therapy for insomnia (CBT-I) in the treatment of chronic insomnia. Methods:A total of 69 patients with chronic insomnia admitted to the Nanjing First Hospital from January 2023 to February 2024 were selected and randomly divided into the CBT-I alone group (simple group, 34 cases) and the SGB combined with CBT-I group (combined group, 35 cases) using a random number table. Both groups received treatment for 8 weeks. The simple group was treated with CBT-I twice a week, 1 hour each time. The combined group, on the basis of the simple group, received ultrasound-guided SGB with 4 ml of 1% lidocaine, which was performed once a day, alternately on the left and right sides, for 7 consecutive days each time, with a total of 14 injections in the 1st and 5th weeks. One week before treatment and 8 weeks after treatment, the Pittsburgh Sleep Quality Index (PSQI) and sleep diary were used to record patients′ subjective sleep quality; Actigraph was used to record objective sleep status; the Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) were used to assess patients′ emotional status. Adverse reactions such as hoarseness, pneumothorax, bleeding, and infection were recorded. Results:After 8 weeks of treatment, the effective rate was 76.5%(26/34) in the simple group and 94.3%(33/35) in the combined group, with a statistically significant difference (χ 2=8.433, P<0.05). Before treatment, there was no significant difference in PSQI scores between the two groups ( P>0.05). After 8 weeks of treatment, the PSQI scores of both groups were lower than those before treatment, and the PSQI score of the combined group was lower than that of the simple group, with a statistically significant difference ( P<0.05). Before treatment, there were no significant differences in sleep parameters from sleep diaries and Actigraph between the two groups (all P>0.05). Compared with before treatment, after treatment, the total sleep time and time in bed were prolonged, and sleep efficiency, the proportion of deep sleep, and the proportion of rapid eye movement sleep were increased in both groups; these indicators in the combined group were better than those in the simple group, with statistically significant differences (all P<0.05). Compared with before treatment, the sleep onset latency was shortened and the number of awakenings was reduced in both groups after treatment, and the combined group was better than the simple group, with statistically significant differences (all P<0.05). Before treatment, there were no significant differences in SAS and SDS scores between the two groups (all P>0.05). After treatment, the SAS and SDS scores of both groups were lower than those before treatment, and the SAS and SDS scores of the combined group were lower than those of the simple group, with statistically significant differences (all P<0.05). During treatment, 2 cases in the combined group developed hoarseness, which relieved spontaneously after 2 hours, and no other complications occurred. Conclusions:Ultrasound-guided SGB combined with CBT-I can significantly improve the nighttime sleep quality of patients with chronic insomnia.

Objective To investigate the feasibility, efficacy and safety of cold snare polypectomy(CSP)and endoscopic mucosal resection(EMR)in treatment of colonic adenoma in proximity to the orifice of appendix. Methods Clinical data of 41 cases of colonic adenoma in the orifice of appendix treated with CSP(CSP group)and 15 cases treated with EMR(EMR group)from January 2018 to January 2023 were collected. The clinicopathological features and outcomes were analyzed. The main outcome was postoperative complications, including bleeding, perforation and postoperative appendicitis, while the secondary outcome was the total resection rate, block resection rate and local recurrence rate. Results The operation time and treatment cost of CSP group were shorter than EMR group, and the differences were statistically significant(P<0.05).The diameter of applicable lesions in CSP group was smaller than that in EMR group, and the proportion of lesions with tubular adenoma was significantly higher than that in EMR group. There were no significant differences in the time of admission, intestinal preparation score, length of stay, lesion morphology and lesion type between the two groups(P>0.05).Perforation and postoperative appendicitis were not observed in both groups, and delayed postoperative hemorrhage occurred in 1 case in the CSP group, but it could heal itself after surgery without endoscopic intervention. The complete resection rate was 100.0%in both groups. The total removal rate of CSP group was 100.0%,which was significantly higher than that of EMR group(86.7%),and the difference was statistically significant(P<0.05).The local recurrence rate was 2.4%(1/41)in CSP group, and no local recurrence was observed in EMR group(0/15),with no statistical significance(P>0.05).Conclusion CSP and EMR are safe and effective in the treatment of colonic adenoma in proximity to the orifice of appendix, and they are trustworthy.Compared with EMR,CSP is more applied to small diameter tubular adenomas, which has the characteristics of economy and time saving, but at the same time there is the risk of recurrence.EMR obviously has a wider application range, but there is a disadvantage in price and time, and individual cases need to be segmented resection.
