1.Application of 5 kinds of sarcopenia assessment scales in patients with bone tumors

Objective To explore the application effect of five kinds of sarcopenia assessment scales in the sarcopenia screening in the patients with bone tumors.Methods The convenience sampling method was a-dopted to select 198 patients with bone tumor in this hospital from August 2023 to February 2024 were select-ed as the study subjects.The Simplified Five-Item Scoring Questionnaire(SARC-F),the Modified Simplified Five-Item Scoring Questionnaire(SARC-Calf),the Simplified Five-Item Scoring Questionnaire Combined with Age and BMI(SARC-F+EBM),the Mini Sarcopenia Risk Assessment 7-Item Questionnaire(MSRA-7)and the Mini Sarcopenia Risk Assessment 5-Item Questionnaire(MSRA-5)were used for conducting the screen-ing.The Asian Sarcopenia Working Group-2019(AWGS-2019)screening criteria for sarcopenia were used as diagnostic criteria.The receiver operating characteristic(ROC)curve and area under the curve(AUC)were used to analyze the predictive efficiency.The sensitivity,specificity,positive predictive value(PPV),negative predictive value(NPV)and Kappa value were compared among various screening tools.Results According to the AWGS-2019 screening criteria for sarcopenia,there were 144 cases in the non-sarcopenia group and 54 ca-ses in the sarcopenia group,and the incidence rate of sarcopenia was 27.27%.SARC-Calf and MSRA-5 had the highest sensitivity(81.48%).SARC-F+EBM had the highest specificity(91.67%).SARC-F+EBM had the highest PPV(70.00%).SARC-Calf had the highest NPV(91.49%);SARC-F+EBM had the highest AUC(0.890),the corresponding cut-off value was 8.5 points,the sensitivity was 83.30%and specificity was 80.60%.The Kappa values of SARC-F,MSRA-5 and MSRA-7 were 0.206,0.336 and 0.324 respectively,pos-sessing the ordinary consistency,while the Kappa values of SARC-Calf and SARC-F+EBM were 0.544 and 0.474 respectively,possessing the medium consistency.Conclusion SARC-F+EBM could be used as the best screening tool of clinical medical staff for sarcopenia in the patients with bone tumor.
2.A fragile X syndrome family with epilepsy

Fragile X syndrome(FXS)is caused by abnormal duplication and amplification of the FMR1 gene CGG.This article reports a pair of brothers diagnosed with FXS by genetic testing.Two patients,aged 15 and 14 years old respectively,both had clinical manifestations such as language disorders,intellectual disabilities,attention deficit disorder,autism spectrum disorder,and FXS's characteristic facial features.The proband had a rare late-onset epileptic seizure,which was well treated with levetiracetam,while his younger brother had no electroencephalogram abnormalities after repeated follow-up.This pair of cases suggests that the clinical phenotype of FXS has diversity and heterogeneity.
3.E-cigarette use among high school students in Wuhan City

Objective:
To investigate the use of e-cigarette and analyze its influencing factors among high school students in Wuhan City, so as to provide insights into developing control measures for adolescents.
Methods:
From September to November 2019, high school students in Wuhan City were selected by the multi-stage stratified cluster random sampling method, and basic information, e-cigarette use and second-hand smoke exposure were collected through questionnaire surveys. Proportions of e-cigarette current use and attempt to use were analyzed, and factors affecting the current use of e-cigarettes were identified using a multivariable logistic regression model.
Results:
Totally 5 700 questionnaires were allocated, and 5 602 valid questionnaires were recovered, with an effective rate of 98.28%. The respondents included 2 925 males (52.21%) and 2 677 females (47.79%); 4 033 high school students (71.99%) and 1 569 vocational high school students (28.01%). The proportion of attempt to use e-cigarettes was 9.23%, and the proportion of current e-cigarette use was 2.03%. Multivariable logistic regression analysis identified male (OR=2.112, 95%CI: 1.285-3.471), vocational high school (OR=1.967, 95%CI: 1.214-3.186), private high school (OR=9.684, 95%CI: 5.648-16.605), family second-hand smoke exposure (OR=3.064, 95%CI: 1.741-5.392), second-hand smoke exposure in public places (OR=4.402, 95%CI: 1.687-11.484) and having close friends who were smokers (OR=6.432, 95%CI: 3.219-12.852) as factors affecting current e-cigarette use among high school students.
Conclusions
The proportion of current e-cigarette use among high school students in Wuhan City was 2.03%. Male, vocational high school, private high school, second-hand smoke exposure and having close friends who were smokers may be promoting factors for e-cigarette use.
4.Genetic analysis for a female carrying idic(Y)(p11.32) with Disorders of sex development

Objective:To explore the genetic basis for a patient with Disorders of sex development (DSD).Methods:A female patient who had presented at the Linyi People′s Hospital due to primary amenorrhea on April 6, 2022 was selected as the study subject. Conventional chromosomal karyotyping, fluorescence in situ hybridization (FISH), chromosomal microarray analysis (CMA), fluorescence quantitative PCR and Sanger sequencing were carried out for the patient. Results:The patient, a 14-year-old female, had featured short statue, multiple nevi, and primary amenorrhea. She was found to have a karyotype of 46, X, idic(Y)(p11.3)[59]/45, X[39]/47, X, idic(Y)(p11.3)×2[2]. The result of FISH assay was 46, X, der(Y).ish idic(Y)(p11.3)( SRY+ )[59]/45, X[39]/47, X, der(Y)×2.ish idic(Y)(p11.3)( SRY+ )[2]. That of CMA was arr[GRCh37](X)×1, (Y)×0-1, arr[GRCh37]Yp11.32(118552_472090)×1. The patient had no deletion in the AZF region of Y chromosome, and was negative for variant of SRY gene. Combining the above results, her molecular karyotype was determined as mos 46, X, idic(Y)(p11.32)[59]/45, X[39]/47, X, idic(Y)(p11.32)×2[2].ish 46, X, idic(Y)(p11.32)( DXZ1+, DYZ1+ +, DYZ3+ +, SRY+ )[59]/45, X( DXZ1+, DYZ1-, DYZ3-, SRY-)[39]/47, X, der(Y)×2.ish idic(Y)(p11.32)( DXZ1+, DYZ1+ +, DYZ3+ +, SRY+ )[2].arr[GRCh37](X)×1, (Y)×0-1, arr[GRCh37]Yp11.32(118552_472090)×1. The patient was diagnosed with mosaicism DSD with idic(Y)(p11.32). Conclusion:The abnormal mosaicism karyotype probably underlay the DSD in this patient.
5.Correlation between serum leptin, adiponectin levels and bone metabolism indexes in pregnant women with gestational diabetes mellitus complicated with osteoporosis

Objective:To analyze the correlation between serum leptin (LEP), adiponectin (ADP) levels and bone metabolism indexes in pregnant GDM women complicated with osteoporosis.Methods:A total of 120 GDM patients admitted to our hospital from Dec. 2020 to Dec. 2023 were included as the observation group, and 30 healthy pregnant women were chosen as the control group. The levels of serum LEP, ADP, bone metabolism indexes [parathyroid hormone (PTH), alkaline phosphatase (ALP), 25-hydroxyvitamin D, amino terminal and middle osteocalcin (N-MID) ] were compared. The relationship between serum LEP, ADP and PTH, ALP, 25-hydroxyvitamin D and N-MID was analyzed by Pearson correlation, and receiver operating characteristic curve (ROC) was drawn to evaluate the diagnostic value of serum LEP and ADP levels in gestational diabetes mellitus complicated with osteoporosis.Results:The observation group had higher LEP level, but lower ADP, PTH, 25-hydroxyvitamin D and N-MID level ( t=10.08, 7.19, 12.70, 15.05, 20.54, P<0.05), and there was no significant difference in ALP between the two groups ( t=0.78, P>0.05) ; 25 had osteoporosis and 95 did not. LEP levels in the osteoporosis subgroup were significantly higher than those in the non-osteoporosis subgroup, and ADP, PTH, 25-hydroxyvitamin D and N-MID levels were significantly lower ( t=4.44, 3.06, 3.51, 3.18, 3.00, P<0.05). There was no significant difference in ALP level between the two groups ( t=0.38, P>0.05). Pearson correlation analysis showed that serum LEP and ADP were significantly correlated with PTH, 25-hydroxyvitamin D and N-MID in pregnant GDM women ( P<0.05). The area under the curve (AUC) of serum LEP, ADP and combined test in pregnant GDM women complicated with osteoporosis were 0.723, 0.650 and 0.755, respectively. Conclusion:Serum LEP level increases and ADP level decreases significantly in pregnant GDM women complicated with osteoporosis, which is closely related to bone metabolism indexes, and can assist in early diagnosis of gestational diabetes mellitus complicated with osteoporosis.
6.Analysis of the current situation and influencing factors of the fatalism of disabled elderly people in the hospital

Objective:To investigate the current situation of the fatalism of the disabled elderly in hospital, and to analyze the influencing factors, so as to provide a basis for the later development of targeted intervention programs.Methods:This was a cross-sectional survey study. From December 2 to 29, 2023, the convenience sampling method was used to investigate the disabled elderly hospitalized in 7 tertiary general hospitals in Sichuan Province by the general information questionnaire, the Activity of Daily Living scale and the Fatalism Scale. And its influencing factors were conducted by the multivariate linear regression.Results:A total of 280 questionnaires were distributed in this survey, and 269 questionnaires were effectively recovered, with an effective recovery rate of 96.07%(269/280). There were 136 males and 133 females, aged 60-69 years old with 61 cases, 70-79 years old with 103 cases, ≥80 years old with 105 cases. The score of the fatalism in hospitalized disabled elderly was (48.87 ± 10.02) points, of which the pre-determined dimension score was (18.73 ± 4.17) points, the luck dimension score was (12.75 ± 3.42) points, and the pessimistic dimension score was (17.39 ± 4.72) points. Multivariate linear regression showed that the frequency of visits to relatives and friends and the level of education were protective factors for the occurrence of fatalism (t=-3.02, -2.81, both P<0.05), and economic pressure was a risk factor for the occurrence of fatalism (t=4.35, P<0.01).Conclusions:The fatalistic view of the disabled elderly in the hospital is at a medium level.Medical staff can start from the influencing factors, formulate relevant intervention measures, and improve the level of the patient's fatalism.
7.Correlation analysis of functional constipation and its related factors with acute appendicitis in children

Objective:To explore the correlation between functional constipation (FC)and its related factors with acute appendicitis(AA)in children,so as to provide a reference for the prevention and clinical diagnosis and treatment of children with AA. Methods:A case-control study was conducted on 170 children diagnosed with AA in the Department of Pediatrics,the Second Affiliated Hospital of Air Force Military Medical University,from August 2022 to March 2023,and 170 non-AA children during the same period were selected as control objects.The clinical data,incidence of FC,symptoms related to FC,Bristol stool classification,past constipation history and other information were compared between two groups.Results:The incidence of FC in 170 children with AA was 22.9%(39/170),which was significantly higher than 10.6%(18/170)in the non-AA group( P<0.01);For children under four years old,the proportion of faecal retention in AA group was higher than that in non-AA group(25.6% vs. 9.3%, P<0.05);For children ≥ 4 years old,the proportion of faecal retention and dyschezia in AA group were higher than those in non-AA group(28.2% vs. 6.9%,29.0% vs. 16.4%,respectively,all P<0.05).The proportion of past constipation history in AA group was higher than that in non-AA group(29.4% vs.14.1%).The duration of constipation in AA group was longer than that in non-AA group [0.00(0.00,1.25)month vs. 0.00(0.00,0.00)month,all P<0.01].The proportion of children with low Bristol stool classification in AA group was higher than that in non-AA group( P<0.01).Multivariate Logistic regression analysis showed that faecal retention[ OR=6.186(95% CI 2.336~16.380)] and long constipation time [ OR=1.310(95% CI 1.095~1.567)]were independent risk factors for AA in children(all P<0.05). Conclusion:The incidence of faecal retention in children with AA is higher than that in children without AA,and the median duration of constipation is longer than that in children without AA.Fecal retention and long-term constipation are independent risk factors for AA in children.
8.Current situation and influencing factors of stigma among disabled elderly inpatients

Objective:To explore the current situation of stigma among disabled elderly inpatients and analyze its influencing factors, so as to provide reference for formulating targeted intervention measures.Methods:This study was a cross-sectional survey. In December 2023, convenience sampling was used to select 428 disabled elderly inpatients from 12 ClassⅢ Grade A hospitals in Sichuan Province as research subjects. General Information Questionnaire and Social Impact Scale (SIS) were used to conduct surveys on patients. Multiple linear regression analysis was used to explore the influencing factors of stigma among disabled elderly inpatients.Results:A total of 428 questionnaires were distributed, and 407 valid questionnaires were collected, with a valid response rate of 95.09% (407/428). SIS score of 407 disabled elderly inpatients was (56.55±8.38). Multiple linear regression analysis showed that the number of diseases, educational level, and self-rating economic pressure were the influencing factors of stigma among disabled elderly inpatients ( P<0.05) . Conclusions:The stigma among disabled elderly inpatients is at a moderate level. Medical and nursing staff should establish a comprehensive prevention and management mechanism based on influencing factors.

Objective To monitor the antifungal resistance of clinical isolates of Candida spp.in the East China region.Methods MALDI-TOF MS or molecular methods were used to re-identify the strains collected from January 2018 to December 2022.Antifungal susceptibility testing was performed using the broth microdilution method.The susceptibility test results were interpreted according to the breakpoints of 2022 Clinical and Laboratory Standards Institute(CLSI)documents M27 M44s-Ed3 and M57s-Ed4.Results A total of 3 026 strains of Candida were collected,65.33%of which were isolated from sterile body sites,mainly from blood(38.86%)and pleural effusion/ascites(10.21%).The predominant species of Candida were Candida albicans(44.51%),followed by Candida parapsilosis complex(19.46%),Candida tropicalis(13.98%),Candida glabrata(10.34%),and other Candida species(0.79%).Candida albicans showed overall high susceptibility rates to the 10 antifungal drugs tested(the lowest rate being 93.62%).Only 2.97%of the strains showed dose-dependent susceptibility(SDD)to fluconazole.Candida parapsilosis complex had a SDD rate of 2.61%and a resistance rate of 9.42%to fluconazole,and susceptibility rates above 90%to other drugs.Candida glabrata had a SDD rate of 92.01%and a resistance rate of 7.99%to fluconazole,resistance rates of 32.27%and 48.24%to posaconazole and voriconazole non-wild-type strains(NWT),respectively,and susceptibility rates above 90%to other drugs.Candida tropicalis had resistance rates of 29.55%and 26.24%to fluconazole and voriconazole,respectively,resistance rates of 76.60%and 21.99%to posaconazole and echinocandins non-wild-type strains(NWT),and a resistance rate of 2.36%to echinocandins.Conclusions The prevalence and species distribution of Candida spp.in the East China region are consistent with previous domestic and international reports.Candida glabrata exhibits certain degree of resistance to fluconazole,while Candida tropicalis demonstrates higher resistance to triazole drugs.Additionally,echinocandins resistance has emerged in Candida albicans,Candida glabrata,Candida tropicalis,and Candida parapsilosis.
10.Clinical observation of 9 cases of rituximab followed by belimumab in the treatment of severe systemic lupus erythematosus

OBJECTIVE To investigate the clinical efficacy and safety of rituximab (RTX) followed by belimumab (BLM) in patients with severe systemic lupus erythematosus(SSLE). METHODS Nine SSLE patients, who were treated with RTX followed by BLM for more than 6 months in the Department of Rheumatology and Immunology of the First Affiliated Hospital of Zhengzhou University from October 2020 to June 2021, were enrolled. Baseline clinical data of patients, laboratory examination results and basic treatment status at weeks 0, 4, 12, and 24 of medication were collected retrospectively. The patients’ systemic lupus erythematosus disease activity index (SLEDAI) score, glucocorticoid dosage and serological indicators (complement C3, complement C4, serum albumin, and 24-hour urine protein quantification) level were analyzed. At the same time, the occurrence of adverse drug reaction was collected. RESULTS All 9 patients completed more than 24 weeks of RTX followed by BLM therapy. All patients suffered from renal impairment, of which 7 (77.8%) had renal pathology support, 3(33.3%) had blood system damage and 2 (22.2%) had nervous system damage. During treatment, with the prolongation of treatment time, the SLEDAI score, 24- hour urinary protein quantification, and glucocorticoid dosage of patients showed a significant downward trend, and ultimately decreased to the normal index level (P<0.05); serum albumin, complement C3 and complement C4 all showed a significant upward trend, eventually rose to the normal index level (P<0.05). During treatment and follow-up, 1 patient developed herpes zoster, 1 patient developed upper respiratory tract virus infection, and 1 patient developed urinary system bacterial infection. All patients recovered after symptomatic treatment. CONCLUSIONS In sequential use of RTX followed by BLM for SSLE, early administration of RTX can quickly stabilizethe condition, significantly alleviate clinical symptoms, and gradually normalize specific serological indicators; subsequent administration of BLM can reduce the type and dosage of basic treatment drugs; there is no increase in the incidence of adverse drug reactions.
