1.Bibliometrics and Visualization Analysis of Podophyllotoxin Literature Based on Web of Science Database

Objective To provide reference for future basic research and clinical research,the research status and trend of podophyllotoxin were explored.Methods Based on Citespace and R software,the co-occurrence map,keyword clustering and burst analysis of the English literature on podophyllotoxin in the Scientific Core Collection(WoSCC)were carried out to explore the research hotspots and analyze the frontier progress.Results A total of 1220 papers on podophyllotoxin published by 531 institutions from 77 countries and regions were retrieved.The number of published papers showed an overall upward trend.The analysis of the author 's cooperation network map showed that there was cooperation and communication between the authors,but it was relatively scattered,and the cooperation was not close.This phenomenon also existed in the relationship between research institutions.The country with the largest number of papers was China,and the institution was Lanzhou University.869 authors participated in the research in this field,and 422 academic journals published papers on podophyllotoxin.Among them,podophyllotoxin,derivatives,analogues,biological evaluation and etoposide had a high frequency of occurrence,indicating that these fields had high attention and great research potential.Conclusion In recent years,the research on podophyllotoxin and its derivatives in the treatment of tumors has developed rapidly.In addition,the pharmacological effects and gene regulation of podophyllotoxin are also new research trends.The research hotspots show a trend of diversification,which provides strong evidence for further exploring the molecular mechanism of podophyllotoxin in the treatment of various diseases.However,the lack of relatively in-depth basic research has limited the development of clinical research.
2.Research progress on the pathogenesis of central retinal vein occlusion

Central retinal vein occlusion(CRVO)is a retinal vascular disorder that significantly impairs vision, with its underlying mechanisms involving complex interactions across multiple biological systems. This article provides a systematic review of the pathological mechanisms associated with CRVO, emphasizing critical factors such as endothelial dysfunction, arteriosclerosis, thrombophilia, inflammation, and oxidative stress. The pathological mechanisms of CRVO are characterized by arteriosclerosis, which obstructs venous return through a dual mechanism involving mechanical compression and endothelin-1-mediated contraction; endothelial dysfunction, which exacerbates disturbances in blood flow; genetic and acquired coagulation abnormalities that disrupt hemostatic balance and promote thrombosis; and the synergistic effects of inflammation and oxidative stress that activate cytokines, thereby aggravating ischemia and vascular leakage. Innovatively, this review explores emerging mechanisms such as miRNA-mediated vascular regulation via exosomes, gut microbiota-retina crosstalk through the “gut-eye axis,” and systemic metabolic interactions that link local retinal lesions to broader dysregulation of CRVO. These insights underscore the importance of integrated eye-system interventions and provide a theoretical foundation for advancing early biomarker discovery, multitarget therapeutics, and personalized treatment paradigms. By bridging localized pathology and systemic mechanisms, this work promotes a transformative shift toward an integrative medicine model in the diagnosis and management of CRVO.
3.Incidence and epidemic characteristics of tsutsugamushi disease in Ankang City, Shaanxi Province from 2018 to 2023

Objective:To study the incidence and epidemiological characteristics of tsutsugamushi disease in Ankang City, Shaanxi Province.Methods:From 2018 to 2023, data on tsutsugamushi disease cases reported by various medical institutions at all levels in Ankang City were collected through the China Disease Prevention and Control Information System. At the same time, individual epidemiological investigation of reported cases was carried out, and electronic medical records of patients were retrieved to verify their diagnosis. In 2023, the rodents and chigger mites were monitored in 3 townships with high incidence of tsutsugamushi disease in Ankang City by night clamping method, and the chigger mites rate and chigger mites index were calculated. Blood, liver, spleen and kidney samples of rodents were collected, DNA of blood and organ mixture was extracted, and quantitative real-time PCR was used to detect Orientia tsutsugamushi. Results:A total of 330 cases of tsutsugamushi disease were reported in Ankang City from 2018 to 2023, and the reported incidence rate increased from 0.56/100 000 in 2018 to 5.02/100 000 in 2023. Among them, 139 were males and 191 were females, with a sex ratio of 0.73∶1.00. The age was mainly 45 - 70 years old, accounting for 81.52% (269/330). The occupation was mainly farmers, accounting for 86.97% (287/330). The onset time was mainly concentrated from April to October, with a total of 317 cases, accounting for 96.06% (317/330). The main clinical manifestations were fever (93.20%, 274/294), ulcer or eschar (75.85%, 223/294), headache (44.56%, 131/294), and lymph node enlargement (44.22%, 130/294). The misdiagnosis rate at the first diagnosis was 73.81% (217/294). A total of 452 rodents were captured, of which 91 rodents carried chigger mites, and chigger mites rate was 20.13% (91/452). A total of 1 463 chigger mites were captured, and the chigger mites index was 3.24 (1 463/452). Quantitative real-time PCR was performed on blood and organ mixture samples collected from 452 rodents, and one of the organ mixture samples tested positive for Orientia tsutsugamushi. Conclusions:The incidence of tsutsugamushi disease in Ankang City is relatively high, and the overall incidence is on the rise. The cases are mainly female, middle-aged and elderly, and farmers, with a high misdiagnosis rate at the first diagnosis. There are chigger mites in rodents in townships with high incidence of tsutsugamushi disease.

With the wide application of artemisinin and its derivatives,its safety has become particularly important.Previous studies have shown that artemisinin and its derivatives have adverse reactions such as nausea,vomiting and diarrhea in clinical use,but they are all within the controllable range.In animal experiments,it has toxic effects on kidney,liver,heart,nerve,blood,embryo and DNA at a high dose.Toxicity and adverse reactions can be alleviated or eliminated by combining medication or changing the drug administration mode,solvent and preparation type.This article mainly discusses the drug safety,toxicity mechanism and attenuation countermeasures of artemisinin and its derivatives,in order to improve the understanding of the potential toxicity of artemisinin and its derivatives and provide reference for the safe use of the drug in clinic.

Objective This paper summarizes the research progress of biosynthesis and pharmacological effects of podophyllotoxin drugs,in order to provide reference for the wide application of these drugs.Methods The plant source,chemical structure,action mechanism and synthesis pathway of podophyllotoxin drugs were summarized.The pharmacological and toxicological characteristics and mechanisms of these drugs were discussed,and the research progress of new dosage forms of podophyllotoxin drugs was reviewed.Results Podophyllotoxin and its derivatives are important chemical constituents in the plants of the podophyllum,such as Dysosma versipellis,Diphylleia sinensis,Sinopodophyllum hexandrum.This kind of drug has few natural sources and can be artificially produced through chemical and biosynthetic pathways.Podophyllotoxin drugs have pharmacological effects such as anti-inflammatory,anti-tumor and anti-virus,but their toxic effects need to be paid attention to.Drugs can be transported by carriers to reduce toxicity and increase effect.Conclusion Podophyllotoxin drugs have broad clinical application prospects,and will be further studied and applied in the fields of anti-inflammatory,anti-tumor and anti-virus in the future.
8.To Explore the Harmful Outcomes Induced by Traditional Chinese Medicine Based on the Mechanism of Programmed Cell Death and Inflammatory Response

Objective In this paper,the harmful effects and mechanisms induced by traditional Chinese medicine were expounded by combining the mechanism of programmed cell death and inflammatory reaction.It is expected to provide data support and theoretical reference for the rational use of traditional Chinese medicine and toxicology research.Methods By systematically analyzing the correlation between cell death and inflammatory response,the molecular mechanisms of pyroptosis,programmed cell necrosis and ferroptosis at home and abroad were summarized,and the harmful outcomes induced by traditional Chinese medicine were further discussed.Results Different toxic traditional Chinese medicines and their active ingredients have different tolerances to biological organisms.Programmed cell death is an important molecular mechanism for the toxicity of traditional Chinese medicines.Its signaling pathways are complex and diverse and are often accompanied by inflammatory reactions.Both of them have a certain effect on the toxic effects of traditional Chinese medicine.Conclusion Traditional Chinese medicine can provide inflammatory mediators for inflammatory response pathways through cell death or change the physiological balance of cells,thereby inducing inflammatory cascade reactions and promoting harmful outcomes in various systems of the body under the dual effects of cell death and inflammatory response.

Diabetic cardiomyopathy is one of the leading causes of heart failure and death in diabetic patients.There is classic evidence of ferroptosis in the development of diabetic cardiomyopathy,which proves that ferroptosis is correlated with the diabetic cardiomyopathy.Oxidative stress caused by excessive production of reactive oxygen species and loss of antioxidant capacity is considered to be the main mechanism leading to diabetic cardiomyopathy.As one of the key regulatory factors of oxi-dative stress,nuclear factor E2-related factor 2(NRF2)and its target gene play an important role in the prevention and treat-ment of diabetic cardiomyopathy.This article summarizes the mechanism of ferroptosis and the pathogenesis of diabetes cardio-myopathy,and expounds the relationship between them,and focuses on the molecular mechanism of NRF2 in the development of diabetic cardiomyopathy and its targeted treatment effect.
10.Analysis of assisted reproduction outcomes for infertility patients with multiple morphological abnormalities of the sperm flagella caused by DNAH1 gene mutation

Objective:To investigate the clinical outcomes of infertility patients with multiple morphological abnormalities of the sperm flagella (MMAF) caused by DNAH1 gene mutation after intracytoplasmic sperm injection (ICSI). Methods:A retrospective cohort study analyzed the clinical data and genetic test results of 39 MMAF infertility patients who were treated in the Center for Reproductive Medicine of Henan Provincial People's Hospital from February 2018 to January 2020. Twelve MMAF patients caused by DNAH1 mutations were acted as DNAH1 positive group and 27 MMAF patients with no DNAH1 mutations were acted as DNAH1 negative group. Totally 100 cases of infertility patients with normal sperm morphology and their spouses who were age-matched by both men and women for ICSI during the same period were selected as control group. The outcomes of assisted pregnancy treatment in the three groups were analyzed. Results:All 39 MMAF patients underwent whole-exome sequencing. Among them, 12 patients had DNAH1 gene mutations, 10 cases of compound heterozygous mutations and 2 cases of homozygous mutations, and the other 27 cases were not detected the currently known DNAH1 mutations. The patients of three groups were treated with ICSI, and the differences in the number of oocytes obtained and the number of M II oocytes in the DNAH1 gene positive group, DNAH1 gene negative group and control group were statistically significant (17.08±5.32, 9.59±3.98, 10.44±6.33, P=0.001; 14.58±5.18, 6.78±3.38, 8.32±5.31, P<0.001). There were no statistically significant differences in the embryo implantation rate, the clinical pregnancy rate, the embryo miscarriage rate and the live birth rate (all P>0.05). Among them, 12 couples of male infertility caused by DNAH1 mutation received a total of 12 cycles of oocyte extraction, forming 79 day 3 embryos, 12 times of the first fresh or frozen embryo transplantation, and 10 biological offspring were obtained. Conclusion:For patients with MMAF caused by DNAH1 gene mutation, ICSI can help them to give birth to their own offspring, and has a higher clinical pregnancy rate and live birth rate.
