Calcified lesions increase the difficulty of interventional therapy for coronary heart disease, and increase the risk of perioperative and long-term complications.Pretreatment of calcified lesions is very important.Coronary lithotripsy(IVL)is used more and more in calcified lesions, and many clinical trials have proved its effectiveness and safety.Stent underexpansion is an important risk factor for stent thrombosis and restenosis, which increases the incidence of complications. At present, there is no effective coping strategy or clear consensus or guidelines for the treatment of stent underexpansion caused by calcified lesions. There are few reports about the treatment of stent under expansion by IVL,and most of them are case reports and small sample studies. In this paper, two cases of stent under expansion were reported. After stent implantation, stent under expansion was found, and IVL was used to treat the cases, which achieved good results. This paper reports 2 cases of stent under expansion to explore the efficacy and safety of IVL in the treatment of such lesions.

Objective:To explore the barriers and facilitators of exercise rehabilitation in patients with atrial fibrillation (AF) after radiofrequency ablation. Methods:From March to August 2022, 285 patients with atrial fibrillation who underwent radiofrequency ablation within three months were recruited from the "Cloud Ward" of the Department of Cardiovascular of the First Affiliated Hospital with Nanjing Medical University by convenience sampling. The Cardiac Rehabilitation Barriers Scale (CRBS) and the Self-efficacy for Exercise Scale (SEE) were used to evaluate the barriers and self-efficacy of patients participating in exercise rehabilitation, respectively. Among them, 22 patients were subjected to semi-structured interviews to extract themes. Results:Among 285 patients with atrial fibrillation, 202 (70.88%) participated in exercise rehabilitation programs. Work/time conflicts, lack of cardiac rehabilitation knowledge, and health/physical function limitations affected the participation of patients in exercise rehabilitation ( P<0.05). Qualitative research extracted three themes that affected the patient's exercise rehabilitation, including capability, opportunity, and motivation. Conclusions:The participation of atrial fibrillation patients in exercise rehabilitation is influenced by multiple factors such as family responsibility, capability, motivation, and opportunity. Medical and nursing staff should comprehensively consider the above factors, develop targeted interventions from all aspects and multiple angles, so as to effectively improve the participation of patients in exercise rehabilitation.

Chinese Journal of Organ Transplantation 2024;45(5):329-336
Objective:To explore risk factors of early acute kidney injury (AKI) after pediatric living donor liver transplantation (LT) and examine the effects on the prognosis of recipients. Methods:From January 2018 to December 2019, the relevant clinical data were retrospectively reviewed for 201 pediatric recipients of elective living donor LT. Post-LT AKI recipients were diagnosed and categorized according to Kidney Disease: Improving Global Outcomes (KDIGO) criteria (2012). Based upon the presence or absence of AKI within 7 days post-LT, they were assigned into two groups of AKI (64 cases) and non-AKI (137 cases). Baseline profiles, preoperative results of major laboratory tests and operation-related parameters were compared between two groups. Univariate variables with statistical differences were included into binary Logistic regression model for multivariate analysis to identify the independent risk factors of early AKI post-LT. Prognostic data of recipients such as postoperative mechanical ventilation time, intensive care unit (ICU) stay time, total hospitalization stay, in-hospital mortality and 3-year postoperative mortality were compared between two groups. Survival analysis was conducted for pediatric recipients with different AKI grades. Results:The incidence of AKI within 7 days post-LT was 31.8% (64/201). Univariate analysis revealed significant inter-group differences in age, preoperative PELD score, diagnosis of biliary atresia, total bilirubin, cystatin C, operative duration and volume of blood loss ( P<0.001, P<0.001, P=0.002, P<0.001, P<0.001, P<0.001& P<0.001). Multi-factorial analysis showed that total bilirubin ( OR=1.154, 95% CI: 1.068-1.248, P<0.001), cystatin C ( OR=2.532, 95 % CI: 1. 627-3.939, P<0.001), operative duration ( OR=1.174, 95% CI : 1.064-1.295, P=0.001) and volume of blood loss ( OR=1.210, 95% CI : 1. 095-1.337, P<0.001) were independent risk factors of AKI within 7 days post-LT. As compared with non-AKI group, postoperative mechanical ventilation time and ICU stay time became markedly extended (178 vs 389 min, P<0.001 ; 2 vs 3 day, P<0.001) and mortality during hospitalization rose sharply (0.7% vs 7.8%, P=0.002) in AKI group. The survival rates of recipients during hospitalization in group non-AKI/AKI were 99.3% (136/137) and 96.8% (30/31, grade 1), 92.9 % (13/14, grade 2), 78.9% (15/19, grade 3 ). The survival rates of recipients 3 years post-LT in group non-AKI/AKI were 94.2% (129/137) and 96.8% (30/31, grade 1), 78.6% (11/14, grade 2), 73.7% (14/19, grade 3). Results of survival analysis indicated that, in group non-AKI and AKI (geade 1, 2, 3), survival rate of recipients during hospitalization and 3 years post-LT declined gradually ( χ2=21.102, P<0.001 ; χ2=13.316, P=0.004) . Conclusion:As one common complication after pediatric living donor LT, AKI adversely affects the prognosis of recipients. Elevated preoperative levels of total bilirubin and cystatin C, prolonged operative duration and greater volume of intraoperative blood loss may boost the postoperative risk of early AKI in pediatric recipients.

AIM:To explore the impact of histidine triad nucleotide-binding protein 2(HINT2)on atheroscle-rosis(AS),and to elucidate its underlying mechanisms.METHODS:Peripheral blood mononuclear cells(PBMCs)were isolated from patients with chest pain and induced to differentiate into macrophages for assessing HINT2 expression. In vitro,RAW264.7 mouse macrophages were cultured to evaluate inflammatory cytokine levels and cell death in superna-tants.An apolipoprotein E gene knockout(apoE-/-)mouse model of AS was developed to analyze plaque formation, lipid metabolism and macrophage foaminess in the aortic root.Pyroptosis-related protein expression in cultured RAW264.7 cells and the aorta of apoE-/-mice was determined by RT-qPCR and Western blot.RESULTS:Significantly reduced HINT2 expression was observed in PBMCs from patients with acute coronary syndrome, which was negatively correlated with pro-inflammatory and positively correlated with anti-inflammatory serum factors, suggesting HINT2's potential role in mitigating AS-related inflammation. In vitro experiments demonstrated that HINT2 overexpression inhibited lipid accumu-lation and foam cell formation in macrophages induced by oxidized low-density lipoprotein(ox-LDL).It also reduced se-cretion of inflammatory cytokines, including interleukin-6(IL-6),IL-1β,IL-18 and tumor necrosis factor-α,and de-creased cell death and pyroptosis-related protein expression. In vivo experiments in apoE-/-mice confirmed that HINT2 overexpression lessened plaque burden in the aortic root, reduced macrophage foaminess, and inhibited the expression of pyroptosis-related proteins such as nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3),caspase-1,gasdermin D(GSDMD),IL-1β and IL-18.CONCLUSION:HINT2 potentially inhibits ox-LDL-induced macrophage pyroptosis, attenuates inflammatory responses in AS,and may slow the progression of this disease.
