1.Severe diarrhea and hypokalemia caused by gefitinib

An 83-year-old female patient received gefitinib 0.25 g once daily due to right lung adenocarcinoma with left lung metastasis and suspicious liver metastasis. She developed severe diarrhea with nausea and vomiting 27 days after treatment. Laboratory test showed that blood potassium was 2.1 mmol/L. The results of fecal culture were negative, and infectious diarrhea was excluded. Gefinib was continued. After 4 days of treatments with potassium supplement and antidiarrheal, the patient recovered. Four days later, the patient developed the above symptoms again, and the laboratory test showed that the blood potassium was 2.96 mmol/L. Diarrhea and hypokalemia were considered to be related to gefitinib. The drug was withdrawn and the symptomatic treatments such as potassium supplement, antidiarrheal, antiemetic, and stomach protection were given. Nine days of drug withdrawal, her symptoms of diarrhea, nausea, and vomiting were relieved, and laboratory test showed that blood potassium was 3.52 mmol/L; 22 days of drug withdrawal, the diarrhea, nausea, and vomiting disappeared, and the laboratory test showed that blood potassium was 4.69 mmol/L.

3.Anaphylactic shock caused by an intravenous infusion of etimicin

A 54-year-old female patient was scheduled to undergo laparoscopic segmental resection for hepatic hemangioma. Thirty minutes before operation, an IV infusion of etimicin sulfate and sodium chloride injection 100 mg was given to prevent infection. After 2 minutes of medication, the patient developed general numbness, apathy, redness of the skin, cold sweating, and dyspnea. Her breath rate was 22 times per minute, heart rate was 110 beats per minute, blood pressure was 45/32 mmHg, and pulse oxygen saturation (SPO 2) was undetectable. Anaphylactic shock due to etimicin was considered. Etimicin was discontinued immediately and treatments such as oxygen inhalation, epinephrine, methylprednisolone sodium succinate, norepinephrine, and intravenous volume expansion were administered. Twenty minutes later, the patient′s symptoms were basically relieved, with breath rate 18 times per minute, heart rate 88 times per minute, blood pressure 108/60 mmHg, and SPO 2 0.99. Thirteen hours later, all symptoms disappeared.

5.The effects of anti-inflammatory on activation of nuclear factor-kappaB and expression of cell adhesion molecules in patients with ulcerative colitis.

The purpose of this study is to assess the effects of anti-inflammatory on activation of nuclear factor-kappaB and mRNA and protein expression of intercellular adhesion molecule-1 (ICAM-1) and vascular cell adhesion molecule-1 (VCAM-1) in intestinal mucosal biopsy specimens from patients with ulcerative colitis (UC). A total of 27 cases with UC were investigated. 15 cases received sulfasalazine (SASP) treatment or SASP and glucocorticoid treatment, 12 cases did not receive any medication related with UC. Normal mucosa from 9 colon cancer cases served as control. Ten pieces of intestinal mucosal biopsy specimens were obtained from each patient. The mRNA expression of ICAM-1 and VCAM-1 were determined by reversal transcription-polymerase chain reaction (RT-PCR). The protein levels of ICAM-1 and VCAM-1 were measured by enzyme linked immunosorbent assay (ELISA). NF-kappaB DNA binding activity was evaluated by electrophoretic mobility shift assay (EMSA). The results showed that NF-kappaB DNA binding activity, mRNA and protein expression of ICAM-1 and VCAM-1 were increased significantly in patients with UC, compared with normal control (P<0.05). Glucocorticoids and SASP markedly inhibited NF-kappaB activation and significantly decreased mRNA and protein expression of ICAM-1 and VCAM-1 (P<0.05). Adhesion molecules (ICAM-1 and VCAM-1) gene activation had significant positive correlation with the NF-kappaB DNA binding activity (r=0.8652 P<0.05, r=0.7902, P<0.05, respectively). We concluded that NF-kappaB is a major and essential factor in regulating the expression of adhesion molecules, it plays an important role in the pathogenesis of UC. SASP and glucocorticoids ameliorate UC via inhibition of NF-kappaB activation and reduction of adhesion molecules expression.

6.Observation on Moving Patterns of Oncomelania Snails in Rivers and Canals

Objective To understand the moving patterns of Oncomelania snails, intermediate host of S.japonicum, in the water bodies. . Methods. Based on the biological features of the snails, methods and techniques in relation to hydraulics and silt engineering were adopted to investigate the active scrawl ability and passive movement of the snails. . Results and Conclusion . The scrawl speed of the snails themselves was very low, 2.^45 mm per minute only. The active movement in the water current was therefore almost negligible. The major moving patterns of the snails in rivers and canals were that: 1. Snails adsorbed on different kinds of carriers drifted on the water. 2. Snails suspended on the water and drifted with the current, these were the young snails under 7 weeks of age. Adult snail showed a strong ability of adhesion, 12 times higher than its body weight.
