1.Status and influencing factors of post-traumatic stress disorder in postoperative breast cancer patients

Objective:To investigate the status of post-traumatic stress disorder (PTSD) and its influencing factors in postoperative breast cancer patients.Methods:A convenience sampling method was used to select 410 postoperative breast cancer patients from the Provincial Hospital of the First Medical University of Shandong between January 2023 and December 2024. The PTSD Checklist-Civilian Version (PCL-C), the Connor-Davidson Resilience Scale (CD-RISC), and the Simple Coping Style Questionnaire (SCSQ) were used for assessment. Binary Logistic regression analysis was performed to analyze influencing factors.Results:A total of 410 questionnaires were distributed, and 405 valid questionnaires were returned, with an effective recovery rate of 98.78% (405/410). Among them, 150 patients were PTSD-positive. Binary Logistic regression analysis showed that educational level, monthly family income, psychological resilience, and coping style were influencing factors for PTSD in postoperative breast cancer patients ( P<0.05) . Conclusions:Low education level, low income, and negative coping style are risk factors for PTSD in postoperative breast cancer patients; positive coping style and high psychological resilience are protective factors. Clinical practitioners should develop preventive or intervention measures based on these influencing factors to reduce the occurrence of PTSD in postoperative breast cancer patients.
2.Molecular epidemiological characteristics of group B Streptococcus from puerpera based on whole genome sequencing

OBJECTIVE To understand the drug resistance,serotypes,virulence-associated genes and epidemiologi-cal characteristics of group B Streptococcus(GBS)isolated from puerpera in this area so as to provide bases for prevention of mother-to-infant infections.METHODS Totally 67 strains of GBS were isolated from obstetrics out-patient department of The First Affiliated Hospital of Nanchang University from Jan.2023 to Dec.2023.The spe-cies of the strains were identified by VITEK MS,the drug susceptibility testing was carried out by disc diffu-sion method.Multilocus sequencing types,capsular types,virulence genes and drug resistance genes were analyzed by means of whole genome sequencing technique.RESULTS The 67 strains of GBS were sensitive to penicillin,vancomycin,ceftriaxone and linezolid;the drug resistance rates to erythromycin and clindamycin were 76.12%and 55.22%,respectively.All strains fell into 7 serotypes,with serotype V predominant;21 sequence types were in-volved,with ST529 most prevalent;8 clonal complexes(CCs)were involved,with CC12 most common.Totally 17 types of drug resistance genes were identified,and the carrying rate of macrolide resistance gene ErmB was highest.Among all the virulence genes except for the adhesion genes fbsA and fbsB,the carrying rates of 18 genes involving in invasion,adhesion,and immune evasion-associated virulence genes were more than 86.57%;67.16%of the strains co-expressed both PI-1 and PI-2a pilus islands.CONCLUSIONS The drug resistance rate of the GBS strains isolated from the puerpera is high,and the strains carry multiple drug resistance genes and viru-lence genes and present with molecular clonal diversity.The serotype V/ST529 is the predominant clone,for which the prevention and control should be strengthened.
3.Five new meroterpenoids from Rhododendron anthopogonoides and their anti-inflammatory activity.

Five meroterpenoids, rhodonoids K-M (1-2), daurichromene E (3), and grifolins A-B (4-5), together with seven known compounds (6-12), were isolated from Rhododendron anthopogonoides. The chemical structures of these compounds were elucidated through comprehensive analysis of high-resolution electrospray ionization mass spectrometry (HR-ESI-MS), ultraviolet (UV), infrared spectroscopy (IR), and nuclear magnetic resonance (NMR) data. Their absolute configurations were determined by comparing experimental electronic circular dichroism (ECD) spectra with computed values. Notably, compounds 1 and 3 demonstrated significant inhibitory effects on lipopolysaccharide (LPS)-induced inflammation in RAW264.7 cells. These compounds markedly suppressed the mRNA expressions of inflammatory factors, including interleukin (IL)-1β, IL-6, and tumor necrosis factor-α (TNF-α) while also down-regulating the protein expressions of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2).

4.Comparative effect of ARNI and ACEI/ARB on left ventricular diastolic function in patients with hypertensive heart disease

Objective To investigate whether there exist differences in the improvement of left ventricular diastolic function between angiotensin receptor-neprilysin inhibitor(ARNI)and angiotensin-converting enzyme inhibitor(ACEI)/angiotensin receptor blocker(ARB)in patients with hypertensive heart disease(HHD).Methods A retrospective cohort study was conducted on the HHD patients admitted in Department of Cardiovascular Diseases of our hospital from January 2021 to December 2024.The general information,echocardiographic parameters before and after treatment,and results of routine tests were collected.Finally,517 HHD patients were subjected,including 117 receiving ARNI treatment(ARNI group)and 400 getting ACEI/ARB treatment(ACEI/ARB group).A 1∶1 propensity score matching(PSM)was performed with a caliper value of 0.02,resulting in 89 matched cases for each group.Electrocardiography was performed to assess left ventricular diastolic dysfunction(LVDD),with various parameters,including left atrial volume index,early diastolic peak velocity at the interventricular septal portion of mitral annulus(septal e'),early diastolic peak velocity at the lateral wall portion of mitral annulus(lateral e'),tricuspid regurgitation velocity,and E/e'.These parameters were followed up and reassessed during the treatment period.Kaplan-Meier survival curve was plotted to compare the incidence of LVDD between the 2 groups.Multivariable logistic regression model was employed to identify the risk factors contributing to LVDD.Results The median follow-up time was 412(309,736)d in the whole cohort,and was 409(300,729)d for the patients after PSM.Kaplan-Meier survival analysis demonstrated that the incidence of LVDD was lower in the ARNI group than the ACEI/ARB group both before and after PSM(P<0.05).After treatment,the ARNI group obtained lower lateral e'[8.00(7.00,9.40)vs 9.00(7.10,10.30)cm/s,P<0.001],thinner left ventricular posterior wall thickness[12.20(10.80,12.80)vs 12.30(11.20,12.90)mm,P<0.048]when compared with the ACEI/ARB group.After adjusting for confounding factors,multivariable logistic regression analysis revealed that advanced age(OR=1.082,P<0.001),increased systolic blood pressure(OR=1.009,P=0.005),thicker left ventricular posterior wall thickness(OR=1.462,P<0.001),left atrial enlargement(OR=1.081,P<0.001),and use of calcium channel blocker(OR=1.548,P=0.006)were independent risk factors for LVDD,and positively correlated with the risk of LVDD.While,male(OR=0.709,P=0.043)and BMI(OR=0.933,P=0.006)were protective factors,which were negatively correlated with LVDD risk.Conclusion In HHD patients,ARNI is superior to ACEI/ARB in reducing the incidence of LVDD.
5.Identification of diagnostic biomarkers for metastatic lymph nodes in oral squamous cell carcinoma using spatial metabolomics

Objective:To uncover alterations in the metabolic microenvironment of lymph node metastasis (LNM) in oral squamous cell carcinoma (OSCC) and identify potential metabolic biomarkers for the early diagnosis of LNM using desorption electrospray ionization mass spectrometry imaging (DESI-MSI) spatial metabolomics.Methods:Six OSCC patients with LNM, who underwent neck dissection surgery at the Department of Oral and Maxillofacial Surgery, Affiliated Hospital of Jiangsu University between October 2020 and October 2022, were enrolled. Matched metastatically involved (positive) and benign (negative) lymph node tissue samples were collected and analyzed using DESI-MSI. Univariate and multivariate statistical analyses were employed to identify differentially abundant metabolites. The diagnostic efficacy of these metabolites was evaluated using receiver operating characteristic (ROC) curve analysis. Finally, Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis was performed to determine the implicated metabolic pathways.Results:A total of 62 and 29 differentially abundant metabolites were identified in the metastatically involved lymph nodes compared to benign lymph nodes under positive-ion mode and negative-ion mode, respectively. These metabolites were predominantly amino acids and lipids. Four metabolites common to both ionization modes were selected for ROC curve analysis: phenylalanine, phosphoethanolamine, histidine, and taurine. The area under the curve values were 0.861, 0.802, 0.729, and 0.722, respectively, indicating promising diagnostic performance. Metabolic pathway analysis revealed significantly heightened activity in Amino acid metabolism ( P=0.469) and Glycerophospholipid metabolism ( P=0.006) within the LNM microenvironment. Conclusions:This DESI-MSI-based study identified disruptions in amino acid and glycerophospholipid metabolism within OSCC metastatic lymph node tissues. The associated differentially abundant metabolites represent potential candidate molecules for diagnosing OSCC LNM.

10.Study on the influencing factors of outpatient workload of doctors in a tertiary public hospital

Objective:To investigate the workload of doctorsand its influencing factors in the outpatient department of a tertiary public hospital in Beijing, in order to provide references for further improvement of outpatient management.Methods:Data on outpatient workload and basic information of doctors in clinical departments of a tertiary public hospital in Beijing in 2022 were collected. Independent sample t-tests was used to analyze the differences in workload among doctors with different characteristics. Multiple linear regression analysis was conducted to identify the main factors influencing doctors′ outpatient workload. Results:In 2022, the number of patients treated by doctors in clinical departments per half day was 18.4±8.1, the duration of half-day treatment was 3.08±0.40 h, and the treatment time per visit was 11.93±5.19 min. The type of department was found to be a key factor affecting the number of patients, the duration of treatment, and the time per visit in outpatient care. The workload of surgical and internal medicine departments was significantly higher than that of other departments. The workload of doctors with less experience was significantly higher than that of more experienced doctors.Conclusions:The outpatient workload in tertiary comprehensive hospitals is heavy, and it is mainly influenced by the type of department and professional title. Different measures should be implemented for different department types by hospitals and health management departments to optimize the allocation of medical resources and effectively reduce the burden on doctors in tertiary public hospitals.
