Objective To assess the effects of different nutritional support methods on postoperative recovery in patients with gastric cancer. Methods 98 patients who received radical gastrectomy were divided into three groups: parenteral nutrition group (PN group) (n=36), early enteral nutrition group (EEN group) (n=33) and early oral feeding group (EON group) (n=29). Tolerance of enteral nutrition, postoperative recovery and economic indicators were compared. Results The number of laparoscopic?assisted surgeries was 18, 17 and 25 in PN group, EEN group and EON group, respectively. There was no significant difference in sex, age and body mass index ( BMI) among the three groups. Gastrointestinal function recovered slowly in 3 cases, including 2 cases in EEN group and 1 case in EON group. 1 case in EON group had abdominal hemorrhage. Median postoperative hospital stay in PN,EEN and EON group was 11.0, 11.0 and 8.0 days respectively, and significant reduction can be found in EON group(P<0.001). The complication rates were 30.5%(11 cases), 12.1%(4 cases), and 13.8%(4 cases), respectively, with no statistically significant difference ( P=0.102). The median nutritional support costs for PN group, EEN group, and EON group were 4 543.3, 974.2, and 265.0 yuan, respectively. The median albumin consumption was 90.0, 40.0, and 0 g, respectively. The EON groups were significantly lower ( P<0.001). The results of the laparoscopic assisted subgroup and the ones of whole group were consistent. Conclusion Compared with parenteral nutrition and early enteral nutrition, early oral feeding can reduce the amount of albumin consumption, decrease the cost of nutrition support and shorten the average hospital stay after surgey without increasing the incidence of complications.

4.Should albumin be a conventional medication after gastrectomy?

A 61-year-old male patient with gastric cancer received routine albumin supplementation after surgery,which resulted in hypoalbuminemia,increased abdominal drainage fluid and pleural effusion.Clinical pharmacists and surgeons did literature reviews and discussions,and finally reached consensuses on albumin administration of patients after gastric cancer surgery.The consensuses were as follows:(1) it is not recommended to conduct albumin supplementation routinely after gastric cancer surgery;(2) albumin is not an effective nitrogen source and it cost too much to be used as a source of postoperative nutrition or protein;(3) for patients with postoperative hypoproteinemia (<30 g/L),albumin supplementation should be used after the surgical stress response period (day 4 after the surgery) and discontinued when the serum albumin is >30 g/L;(4) for patients with peritoneal and/or pleural effusion after surgery,the nature of the effusion should be defined firstly,and albumin can be supplemented only when the transudate is determined.

6.Study on the Stability Evaluation of 12 Kinds of Submicro Emulsion in Market and the Test Method for the Stability

OBJECTIVE:To evaluate the stability of 12 kinds of submicro emulsion in market,and screen the test method for the stability. METHODS:12 kinds of submicro emulsion in market were selected,high pressure sterilization (121 ℃,30 min), high speed centrifugation(4000 r/min,15 min),accelerated test [placing 6 months under temperature of(40±2)℃,relative hu-midity of (75 ± 5)%] were conducted to investigate the pH,particle size and other indexes,and SPSS 22.0 was used to analyze the distribution variance and chi-square test,and investigate the correlation of 3 evaluation methods. RESULTS:In terms of stabili-ty investigation,the pH value of 12 kinds of submicro emulsion decreased to some extent after accelerated test,average particle size of 6 kinds of submicro emulsion samples were greater than 300 nm,the variance of the particle size distribution of 9 kinds ap-peared in 0.05-0.15,the chi-square test results of 8 kinds distributed below 1. The average particle size of 4 kinds of submicro emul-sions changed more than 10 nm after accelerated test. In terms of stability test method,Pearson chi-square progressive significance of high pressure sterilization and accelerated test was 0.665,which was higher than 0.05,indicating there was no correlation (no significance),the stability results of high pressure sterilization can not represent the results of accelerated test;that of high speed centrifugation and accelerated test was 0.004,which was lower than 0.05,indicating stability results between high speed centrifuga-tion and accelerated test results were significantly correlated. CONCLUSIONS:The submicro emulsion in market can meet the re-quirements of stability. To a certain extent,high speed centrifugation can replace the acceleration test.
7.Chemical constituents of n-BuOH extract of Comastoma pedunculatum.

Thirteen compoumds were isolated from the n-BuOH portion of the 70% ethanolic extract of Comastoma pedunculatum by a combination of various chromatographic techniques including silica gel, macroporous adsorbent resin, Sephadex LH-20, and preparative HPLC, of which nine were triterpenoid saponins and four were flavone C-glycosides. Their structures were elucidated by spectroscopic data as saikogenin F (1), 3-O-beta-D-fucopyranosylsaikogenin F (2), clinoposaponin XV (3), saikosaponin A (4), 6"-acetylsaikosaponin A (5), clinoposaponin I (6), bupleuroside I (7) , clinoposaponin XII (8) , saikoponin b3 (9), isovitexin (10) , swertisin (11) , isoorientin (12), 3',4',5-trihydroxy-7-methoxy-6-C-beta-D-glucopyranosyl-flavone (13). Compounds 1-10, 12-13 were all isolated from Comastoma genus for the first time.
