1.Clinical Observation on Huatan Quyu Formula Improving Cerebral Small Vessel Disease with Phlegm and Blood Stasis Blocking Collateral Pattern via Regulating Glymphatic System Circulation

ObjectiveTo observe the clinical efficacy and safety of Huatan Quyu formula in treating cerebral small vessel disease (CSVD) with phlegm and blood stasis blocking collateral pattern via randomized controlled trial, and explore its mechanism of improving CSVD by regulating glymphatic system (GS) circulation. MethodsSixty-eight CSVD patients with phlegm and blood stasis blocking collateral pattern in the Department of Encephalopathy, Affiliated Hospital of Shaanxi University of Chinese Medicine from April to December 2024 were selected and randomly divided into an experimental group (34 cases) and a control group, with 34 cases in each group. Both groups received basic Western medicine treatment, while the experimental group additionally received Huatan Quyu formula. After a course of 12 weeks, the following parameters were compared between the two groups before and after treatment. Clinical outcomes were assessed using the Tinetti performance-oriented mobility assessment (POMA), Montreal Cognitive Assessment (MoCA), Scales for Outcomes in Parkinson's Disease-Autonomic (SCOPA-AUT), and traditional Chinese medicine (TCM) syndrome scores of phlegm and blood stasis blocking collateral pattern. Perivascular space (PVS) in the frontal lobe/basal ganglia and cerebrospinal fluid (CSF) flow parameters in the cerebral aqueduct were evaluated by 3.0T brain MRI, cerebrospinal fluid flow imaging, and phase-contrast magnetic resonance imaging (PC-MRI). Then, safety indicators were monitored, and SPSS 25.0 was used for statistical analysis. ResultsSixty-four patients completed the study (32 in each group). ①Baseline data: No statistically significant difference was found between the two group. ②Efficacy indicators: After treatment, the experimental group exhibited significantly improved total POMA, SCOPA-AUT, and TCM syndrome scores (P<0.01), outperforming the control group (P<0.05). No significant change was observed in MoCA scores between the two groups. ③Imaging indicators: The experimental group showed a reduced PVS area alongside significantly increased CSF flow parameters (including downward flow during the systolic period, and upward flow during the diastolic period) (P<0.01), which were superior to the control group (P<0.01). ④Safety: The laboratory indicators were normal in both groups, with no drug-related adverse reactions. ConclusionFor CSVD patients with phlegm and blood stasis blocking collateral pattern, Huatan Quyu formula can safely and effectively improve motor function, autonomic nerve function, and TCM syndromes, with potential mechanisms related to pulsatile CSF flow enhancement and GS circulation efficiency improvement.
2.CT diagnosis and differential diagnosis of perivascular epithelioid cell tumor

Objective To explore the CT diagnosis and differential diagnosis of perivascular epithelioid cell neoplasms (PEComa), improve the accuracy of PEComa diagnosis, and reduce misdiagnosis. Methods CT findings of 8 cases of PEComa confirmed by pathology in Jining First People’s Hospital from January 2020 to April 2024 were retrospectively analyzed for the location, shape, size, boundary, plain scan density, and enhancement characteristics of the lesions. Results All 8 tumors were solitary, with 5 located in the kidney, 1 in the liver, 1 in the extraperitoneal space, and 1 in the retroperitoneal space. The tumors were round in 3 cases, oval in 1 case, and irregular in 4 cases. Seven cases were benign with clear boundaries and 1 case was malignant with unclear boundaries. On plain CT, 2 cases showed slightly low density, 3 cases showed slightly high density, and 3 cases showed low density. One tumor had uniform density, and 7 tumors had nonuniform density with internal necrosis and cystic changes. Contrast-enhanced CT revealed diverse enhancement patterns. Four cases showed a “fast in and fast out” enhancement pattern, with significant arterial-phase enhancement and reduced portal venous-phase enhancement. Three cases showed a “fast-in and slow-out” enhancement pattern, with significant enhancement in the arterial phase, persistent enhancement in the portal venous phase, and slightly reduced density in the delayed phase. One case showed mild enhancement in the arterial phase and significant enhancement in the portal venous phase. In 3 cases, multiple tortuous and thickened blood vessels were observed around the tumors, while 3 cases showed tortuous vascular shadows within the tumors. Conclusion PEComa demonstrates characteristic CT features, predominantly with “fast in and fast out” or “fast in and slow out” enhancement patterns. When thickened and tortuous blood vessels are observed within or around the tumor, PEComa should be considered in combination with clinical findings.
3.Study on the ADC value of deep gray matter nuclei in patients with Parkinson's disease by RESOLVE DWI

Objective Readout segmentation of long variable echo-trains diffusion weighted imaging(RESOLVE DWI)was used to analyze the difference of apparent diffusion coefficient(ADC)value in deep cerebral nucleus and its correlation with clinical characteristics in patients with Parkinson's disease(PD).Methods Clinical data of 60 patients with PD were retrospectively analyzed as PD group,and were divided in-to the tremor group(n=30)and the bradykinesia group(n=30)according to symptom type,middle-aged(≤65 years old)group(n=23)and elderly(>65 years old)group(n=37)according to age,and 60 healthy vol-unteers were selected as the control group during the same period.ADC values were measured on the ADC map of RESOLVE DWI sequence,and the ADC values of bilateral putamen,pallidus,substantia nigra,rubra,and dentate nucleus were analyzed and their correlation with UPDRS-Ⅲ score and H&Y grading.Results Compared with the control group,the ADC values of both putamen,globus pallidus,globus pallidus,red nucleus,right substantia nigra and right dentate nucleus were increased in the PD group,the ADC values of both putamen,globus pallidus,red nucleus and right substantia nigra in the tremor group were increased,and the ADC values of right putamen,globus pallidus and bilateral red nucleus in the bradykinesia group were increased(P<0.05).Compared with the bradykinesia group,the values of ADC in the tremor group were similar.Compared with the middle-aged group,the ADC values of right putamen,bilateral globus pallidus,left substantia nigra and left red nucleus were higher in the elderly group(P<0.05).In the PD group,the ADC values of the right putamen,globus pallidus,and dentate nucleus were positively correlated with H&Y grade and UPDRS-Ⅲscore,and the ADC value of the left putamen was positively correlated with H&Y grade.Conclusion RE-SOLVE DWI can be used to evaluate the differences in deep gray matter nuclei in PD patients,and its ADC value may be used to evaluate and predict the severity of nuclear mass damage and motor symptoms in PD pa-tients.
4.Hereditary Angioedema: a Report of Ten Cases

Hereditary angioedema (HAE) is a rare autosomal dominant genetic disorder characterized by recurrent episodes of skin and/or mucosal edema, with laryngeal edema posing a life-threatening risk. This paper reports the general conditions, diagnostic and therapeutic processes, and current status of 10 patients diagnosed with HAE at the First Affiliated Hospital of Kunming Medical University between October 2022 and March 2025. Eight cases were type 1 and two were type 2. Patient ages ranged from 17 to 55 years, with a median age at diagnosis of 36 years and a median age at onset of 17 years. The median diagnostic delay was 13.5 years, with the longest delay being 40 years and the shortest 4 years. Clinical presentations were diverse, primarily involving recurrent skin and/or mucosal edema. All 10 patients presented with upper respiratory tract mucosal edema, five cases exhibited prominent gastrointestinal edema accompanied by abdominal pain and distension. By reporting the clinical characteristics of 10 confirmed cases of HAE, this paper aims to provide a reference for clinicians in their diagnosis and treatment.
5.Influencing factors of work-related musculoskeletal disorders among medical staff: a Bayesian network modeling analysis

Objective To understand the current situation and influencing factors of work-related musculoskeletal disorders (WMSDs) in medical staff in Beijing City. Methods A total of 2 687 medical staff were selected as the research subjects using the multi-stage sampling method. The current situation of WMSDs and occupational stress, anxiety symptoms, depressive symptoms, and insomnia symptoms were investigated using the Musculoskeletal Disorders Questionnaire, the Core Occupational Stress Scale, the Generalized Anxiety Disorder Scale, the Patient Health Questionnaire Depression Scale, and the Self-Sleep Management Questionnaire. The Max-Min Hill-Climbing algorithm was used to construct a Bayesian network model to analyze the influencing factors and internal relationships of WMSDs and to conduct reasoning and prediction of the model. Results The prevalence of WMSDs among the research subjects was 88.9%. Binary logistic regression analysis was used to identify age, educational level, personal monthly income, anxiety symptoms, depressive symptoms, insomnia symptoms, prolonged forward-head desk work, and prolonged static posture work to construct the Bayesian network model. The model consisted of nine nodes and eleven directed edges. Prolonged static posture work, prolonged forward-head desk work, and anxiety symptoms were directly related to WMSDs. Age and educational level were indirectly related to WMSDs through their influence on prolonged forward-head desk work. Depression symptoms were indirectly associated with WMSDs through their influence on anxiety symptoms. The model's prediction accuracy was 90.5%. Conclusion The prevalence of WMSDs among medical staff in Beijing City is relatively high. Prolonged static posture work, prolonged forward-head desk work, and anxiety symptoms may directly increase the risk of developing WMSDs.
6.Observation on difference of iron deposition in deep brain nuclei of systemic lupus erythematosus patients with different disease activity levels using quantitative susceptibility mapping

Objective To observe the difference of iron deposition in brain deep nuclei of systemic lupus erythematosus(SLE)patients with different disease activity levels using quantitative susceptibility mapping(QSM).Methods Fifty-four SLE patients were retrospectively enrolled and divided into moderate to severe activity group(score>9,n=25)and mild activity group(score≤9,n=29)according to SLE disease activity index(SLEDAI)score.Patients' general clinical data,as well as the mean QSM of bilateral head of caudate nuclei,putamina,globi pallidi,red nuclei,substantiae nigrae,dentate nuclei and thalami were compared between groups.Pearson or Spearman coefficients were performed to analyze correlations of the mean QSM of nuclei being significant different between groups and conventional clinical indicators.Then receiver operating characteristic(ROC)curve was drawn,and the area under the curve(AUC)was calculated to evaluate the efficacy of the mean QSM for distinguishing moderate to severe and mild activity SLE.Results Compared with mild activity group,moderate to severe activity group had higher serum ferritin,erythrocyte sedimentation rate(ESR),C reactive protein(CRP),anti-double stranded DNA(anti-dsDNA)antibody levels,also higher proportion of positive anti-ribosomal P protein(anti-P)antibodies,but lower complement C3 and C4 levels(all P<0.05).The mean QSM of bilateral putamina and thalami were significantly higher in moderate to severe activity group than in mild activity group(both P<0.05).The mean QSM of bilateral putamina was positively correlated with SLEDAI scores,ferritin levels and positivity of anti-P antibodies in SLE patients(with r or rs of 0.447,0.526 and 0.473,respectively,and all P<0.05).The AUC for distinguishing moderate to severe SLE and mild activity SLE based on the mean bilateral putamina QSM was 0.810.Conclusion There were significant differences of iron deposition in deep brain nuclei of SLE patients with moderate to severe and mild activity.The mean QSM of bilateral putamina could be used to distinguish SLE with moderate to severe activity and mild activity.
7.Clinical analysis of 6 cases of Mycoplasma pneumoniae-induced rash and mucositis in children

Objective:To summarize the clinical features, laboratory findings, treatment and prognosis of children confirmed as Mycoplasma pneumoniae-induced rash and mucositis (MIRM) in children. Methods:This retrospective study concluded 6 children diagnosed as MIRM in Department of Gastroenterology and Infectious Diseases, Shanghai Children′s Hospital, School of Medicine, Shanghai Jiao Tong University from August 2023 to April 2024. This paper described the characteristics of MIRM and analyzed the therapeutic strategy and prognosis.Results:A total of 6 children were diagnosed as MIRM including 2 boys and 4 girls with an age of onset was 6.4 (3.1, 7.5) years. Among the 6 patients, 4 patients had oral mucosal involvement among whom 2 showed crusting of the lips. Four patients had ocular involvement manifesting as conjunctival congestion and increased secretion. All patients presented with skin lesions, manifesting as target-shaped damage in 4 cases, herpes herpetiformis in 1 case and purpura-like rash in 1 case. Serological tests for Mycoplasma pneumoniae IgM and Mycoplasma pneumoniae nucleic acid test were positive in all 6 cases. Two cases received intravenous immunogloblin infusion combined with methylprednisolone, monotherapy of methylprednisolone in 4 cases. The course of glucocorticoids was 1-7 weeks, and the initial dose was 2-4 mg/(kg·d), which was gradually reduced according to the rash. The children were followed up for 3 to 9 months, no case suffered from long term ocular or cutaneous complications or recurrence of rash. All cases had good prognosis. Conclusions:Children diagnosed as MIRM present with mild symptoms and usually have good prognosis with early identification and appropriate intervention. Individualized therapy should be applied based on the severity of skin involvement.
8.SPP1 expression in SMARCA4-deficient non-small cell lung cancer and its relationship with PD-L1

AIM:To analyze the expression of se-creted phosphoprotein 1(SPP1)and programmed cell death-ligand 1(PD-L1)in SMARCA4-deficient non-small cell lung cancer,and to provide a scientif-ic basis for the study of the follow-up treatment of this rare pathological type of lung cancer.METH-ODS:The clinical and pathological characteristics of 12 patients with this disease were analyzed retro-spectively,and the patients were divided into two groups of adenocarcinomas and poorly differentiat-ed carcinomas according to their morphological characteristics,and the relationship between the expression of SPP1 and PD-L1 was analyzed in the two groups.RESULTS:SPP1 expression was detect-ed in all patients and Its expression level was signif-icantly higher in the poorly differentiated carcino-ma group compared with the adenocarcinoma group(P=0.015);PD-L1 expression was found in 6/7 patients(5 cases were not measured),compared with the adenocarcinoma group,PD-L1 was also highly expressed in the poorly differentiated carci-noma group(P=0.048)and the PD-L1 difference be-tween the two groups suggested that the results were similar to those of SPP1.CONCLUSION:SMARCA4-deficient non-small cell lung cancer has high positive expression of SPP1 and PD-L1.It was more pronounced in patients with poorly differenti-ated carcinoma.There may be a positive correla-tion between SPP1 and PD-L1 expression in SMAR-CA4-deficient non-small cell lung cancer and the mechanism of the correlation needs to be further verified in subsequent studies.

10.Dual-energy CT for diagnosing knee gouty arthritis complicated with knee osteoarthritis

Objective To observe the value of dual-energy CT(DECT)for diagnosing knee gouty arthritis(GA)complicated with knee osteoarthritis(KOA).Methods Forty-eight cases of GA(GA group),30 cases of KOA(KOA group)and 60 cases of GA complicated with KOA(GA+KOA group)were retrospectively enrolled.Clinical and imaging data were compared among 3 groups and between each 2 groups,and the efficacy of DECT for diagnosing knee GA complicated with KOA was analyzed.Results Significant differences of detection rate and CT value of bone marrow edema(BME)were found among 3 groups and between each 2 groups(all P<0.05),also of the course of disease,serum uric acid(SUA),monosodium urate(MSU)volume,bone erosion score and CT value between GA group and GA+KOA group(all P<0.05).The area under the curve(AUC)of bone erosion score and CT value for diagnosing knee GA complicated with KOA was 0.779 and 0.824,respectively,and of the combination of the above 5 indexes was 0.898,higher than that of each parameter alone(all P<0.05).Conclusion DECT was helpful for diagnosing knee GA complicated with KOA,and further combining with clinical parameters could improve its diagnostic efficacy.
