AIM: To investigate the differences in morphological structure and retinal blood perfusion between the affected eye and the contralateral healthy eye using optical coherence tomography angiography(OCTA)in patients with idiopathic macular epiretinal membrane(IMEM)before and after surgery, and to evaluate the association of these parameters with functional and anatomical outcomes to inform prognostic assessment. METHODS:A prospective study was conducted at Zhejiang Provincial People's Hospital between January 2023 and December 2024. Consecutive patients diagnosed with unilateral IMEM were enrolled; the fellow eye served as an internal control. All participants underwent standardized ophthalmic evaluations, including optical coherence tomography(OCT), OCTA, and color fundus photography. Key quantitative parameters assessed included best-corrected visual acuity(BCVA), central macular thickness(CMT), foveal avascular zone(FAZ)area, vessel density in the inner capillary plexus(ICP), superficial capillary plexus(SCP), deep capillary plexus(DCP), and choroidal capillary perfusion area(CCPA). Measurements were obtained preoperatively and at 1 and 3 mo postoperatively. Correlation analyses were performed between the above parameters and postoperative BCVA and CMT.RESULTS: This study enrolled 30 patients(60 eyes)diagnosed with IMEM, comprising 14 males and 16 females, with a mean age of 65.4±10.8 y.At baseline, IMEM-affected eyes demonstrated significantly reduced BCVA, DCP density, and FAZ area, alongside significantly increased CMT and CCPA, compared with contralateral controls. Following vitrectomy with membrane peeling, CMT decreased significantly at both 1 and 3 mo(both P<0.05)postoperatively; DCP density and BCVA showed significant improvement(both P<0.05). No significant change in FAZ area was observed postoperatively(P>0.05). At 3 mo postoperatively, BCVA of the affected eye was negatively correlated with CMT(r=-0.549, P=0.022). At 1 mo postoperatively, CMT was negatively correlated with preoperative DCP and FAZ, positively correlated with preoperative CMT, and positively correlated with ICP and SCP at 1 mo postoperatively, and negatively correlated with FAZ at 1 mo postoperatively(all P<0.05). Furthermore, CMT at 3 mo postoperatively was negatively correlated with preoperative DCP(r=-0.498,P=0.042).CONCLUSION:In patients with IMEM, the affected eyes exhibit significantly reduced DCP density and FAZ area, alongside increased CMT and CCPA. Following vitrectomy with membrane peeling, CMT decreased progressively, DCP density demonstrated partial restoration, and vision improved gradually. Preoperatively, smaller CMT larger DCP, and FAZ were associated with more favorable surgical outcomes; postoperatively, smaller ICP and SCP densities—combined with a larger FAZ—also correlated with better functional recovery.

ObjectiveTo evaluate the effects of Tongnaoyin on the blood-brain barrier status and neurological impairment in acute ischemic stroke （AIS） patients with the syndrome of phlegm-stasis blocking collaterals by dynamic contrast-enhanced magnetic resonance imaging （DCE-MRI）. MethodsA total of 63 patients diagnosed with AIS in the Jiangsu Province Hospital of Chinese Medicine from October 2022 to December 2023 were enrolled in this study. According to random number table method，the patients were assigned into a control group （32 cases） and an observation group （31 cases）. The control group received conventional Western medical treatment，and the observation group took 200 mL Tongnaoyin after meals，twice a day from day 2 of admission on the basis of the treatment in the control group. After 7 days of treatment，the patients were examined by DCE-MRI. The baseline data for two groups of patients before treatment were compared. The National Institute of Health Stroke Scale （NIHSS） score and modified Rankin Scale （mRS） score were recorded before treatment and after 90 days of treatment for both groups. The rKtrans，rKep，and rVe values were obtained from the region of interest （ROI） of the infarct zone/mirror area and compared between the two groups. ResultsThere was no significant difference in the NIHSS or mRS score between the two groups before treatment. After 90 days of treatment，the NIHSS and mRS scores declined in both groups，and the observation group had lower scores than the control group （P<0.05）. After treatment，the rKtrans and rVe in the observation group were lower than those in the control group （P<0.01）. ConclusionCompared with conventional Western medical treatment alone，conventional Western medical treatment combined with Tongnaoyin accelerates the repair of the blood-brain barrier in AIS patients，thereby ameliorating neurological impairment after AIS to improve the prognosis.

AIM:To investigate the clinical effects of tirofiban and butylphthalide combined with stent thrombectomy in the treatment of patients with acute cerebral infarction in the ultra-time win-dow.METHODS:A total of 100 patients with ACI beyond time window admitted to our hospital from January 2021 to January 2023 were randomly grouped into the control group(50 cases,treated with stent thrombectomy)and the study group(50 cases,treated with tirofiban and butylphthalide combined with stent thrombectomy),clinical data of patients were collected,and the clinical efficacy,blood lipid levels,coagulation function,neurologi-cal function,quality of life,and incidence of ad-verse reactions were compared between the two groups.RESULTS:Compared with the control group,the total effective rate of the study group obviously increased(P＜0.05).Before treatment,there was no obvious difference between the two groups in triacylglycerol(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),prothrom-bin time(PT),activated partial thromboplastin time(APTT),D-dimer(D-D),fibrinogen(FIB),NIHSS score and Barthel index scale(BI)score of patients with ACI beyond time window(P＞0.05).After treat-ment,the TG,TC,LDL-C,FIB,D-D,and NIHSS score of ACI patients in both groups decreased(P＜0.05),while HDL-C,PT,APTT,and BI score increased(P＜0.05),and the above indicators improved more ob-viously in the study group(P＜0.05).There was no obvious difference in the total incidence of adverse reactions between the two groups(P＞0.05).CON-CLUSION:The combination of tirofiban and bu-tylphthalide with stent thrombectomy in the treat-ing ACI beyond time window has a definite effect,which can improve the blood lipid level,coagula-tion function and neurological function of patients,and help to improve the quality of life of patients,with good safety.

Objective:To evaluate the clinical value and safety of an intraperitoneal chemotherapy port technique in patients with gastric cancer and peritoneal metastases undergoing intraperitoneal chemotherapy.Methods:This was a retrospective, descriptive case analysis. From November 2022 to October 2024, patients diagnosed with gastric cancer and peritoneal metastases at Wuxi Branch of Ruijin Hospital, Shanghai Jiao Tong University School of Medicine with an expected survival >3 months, underwent laparoscopic exploration combined with implantation of an intraperitoneal chemotherapy port [PORT-A-CATH II system (Model 21-4055-24)] implantation. The procedure was as follows: (1) after laparoscopic exploration, a 4-cm skin incision was made at a predetermined site and a subcutaneous pocket created by dissecting to the muscle fascia and removing subcutaneous fat as needed to position the port septum 0.5-1.0 cm from the skin surface; (2) under direct laparoscopic visualization, the abdominal cavity was punctured and a guidewire inserted, followed by an 8.5 Fr sheath, through which a catheter with three trimmed side holes was placed after removal of the sheath; (3) the catheter length in the abdominal cavity was adjusted to 25–30 cm and the catheter trimmed, and connected to the port base, ensuring it extended beyond the connector's visible hole; (4) the whole port was placed within the subcutaneous pocket, and non-absorbable sutures used to create a double purse-string suture at the catheter's abdominal entry, forming an anti-reflux ring; (5) non-absorbable sutures were used to securely fix the port to the fascia through its four base holes and the exposed catheter segments on the fascia sutured and buried; (6) patency was confirmed by injecting saline and followed by intermittent skin closure provided there was no bleeding; and (7) the catheter tip was positioned in the pelvic cavity under laparoscopic guidance. Postoperatively, the patients underwent normothermic intraperitoneal and systemic treatment. The port infusion protocol involved disinfecting the skin (>10 cm diameter) around the port, confirming the puncture site, inserting a Huber needle vertically at 90° to the port base, infusing 100 mL saline to ensure patency, followed by continuous infusion of 1000 mL paclitaxel solution, and sealing with 20 mL saline before removing the needle. No saline flushing was required between chemotherapy infusions. The primary outcomes were the incidence and management of complications post-port implantation.Results:The study cohort comprised 225 patients with gastric cancer and peritoneal metastases. Using standardized port implantation and postoperative puncture procedures, the complication rate during follow-up was 14.2% (32/225), including effusion in 14 patients (6.2%), port infection in 10 (4.4%), incision dehiscence in four (1.8%), port inversion in two (0.9%), hematoma in one (0.4%), and catheter rupture in one (0.4%). Seventy-five percent (24/32) of patients with complications recovered and continued using the port after conservative treatments (e. g., aspiration of effusions, antibiotic therapy, incision management), whereas the remaining 25.0% (8/32) with complications required surgical removal of the port because the treatment was ineffective. The presence of preoperative ascites ( P=0.019) and peritoneal cancer index score>15 ( P=0.038) were significantly associated with development of complications. Conclusions:Our standardized procedure for intraperitoneal chemotherapy port implantation is safe and feasible for patients with gastric cancer and peritoneal metastases, having a low overall complication rate. Most complications can be successfully managed with conservative treatment, the device thus providing reliable support for intraperitoneal chemotherapy.

Alzheimer's disease (AD) is the major form of dementia in the elderly and is closely related to the toxic effects of microglia sustained activation. In AD, sustained microglial activation triggers impaired synaptic pruning, neuroinflammation, neurotoxicity, and cognitive deficits. Accumulating evidence has demonstrated that aberrant expression of deubiquitinating enzymes is associated with regulating microglia function. Here, we use RNA sequencing to identify a deubiquitinase YOD1 as a regulator of microglial function and AD pathology. Further study showed that YOD1 knockout significantly improved the migration, phagocytosis, and inflammatory response of microglia, thereby improving the cognitive impairment of AD model mice. Through LC-MS/MS analysis combined with Co-IP, we found that Myosin heavy chain 9 (MYH9), a key regulator maintaining microglia homeostasis, is an interacting protein of YOD1. Mechanistically, YOD1 binds to MYH9 and maintains its stability by removing the K48 ubiquitin chain from MYH9, thereby mediating the microglia polarization signaling pathway to mediate microglia homeostasis. Taken together, our study reveals a specific role of microglial YOD1 in mediating microglia homeostasis and AD pathology, which provides a potential strategy for targeting microglia to treat AD.

Objective: To explore the clinical characteristics and diagnosis and treatment plans of neurofibromatosis type 1 (NF1), and to provide references for clinical diagnosis and treatment. Methods : The clinical manifestations and treatment of an 8-year-old female patient with NF1 was reported. A literature review was conducted to summarize the clinical characteristics and therapeutic strategies of NF1. Multiple NF1s occurred on the right cheek, orbit, and eyelid, and recurred after surgical resection. The tumor caused ptosis, incomplete closure, and vision loss in the upper eyelid of the right eye. After a multidisciplinary assessment determined that radical resection was not feasible, selumetinib sulfate targeted therapy was adopted (25 mg, Po, bid), 28 days constitute one treatment course, and 14 courses have been completed, combined with symptomatic ocular treatments, such as Befusu. Result: The follow-up showed that the tumor volume did not continue to increase (stable disease), the uncorrected vision of the right eye improved (0.05 vs 0.1), and no drug-related adverse reactions occurred during the treatment period. The literature review summarizes the diverse clinical manifestations of NF1, with café-au-lait macules, multiple neurofibromas, and Lisch nodules being hallmark features. Currently, surgical intervention remains the most commonly employed and primary therapeutic approach for NF1; however, for patients who do not meet the criteria for surgery, alternative treatment strategies should be considered. MEK inhibitors, such as selumetinib, demonstrate significant efficacy in inhibiting the growth of NF1-associated plexiform neurofibromas, with tumor volume reductions of at least 20% observed in 70% of pediatric patients in the SPRINT clinical trial. Furthermore, these inhibitors exhibit favorable long-term safety profiles. Conclusion Café-au-lait macules, multiple neurofibromas, and Lisch nodules are hallmark features of NF1. Selumetinib is safe and effective for NF1 in the head and neck of children, and it is the preferred treatment option for patients who are not suitable for surgery. Long-term follow-up monitoring of tumor changes and drug safety is required.

Objective To observe the value of subchondral bone(SB)intravoxel incoherent motion diffusion weighted imaging(IVIM-DWI)for quantitatively evaluating knee joint cartilage degeneration.Methods Totally 125 patients with knee cartilage degeneration were prospectively enrolled and divided into mild group(n=65,Outerbridge grades Ⅰ-Ⅱ)and moderate-severe group(n=60,Outerbridge grades Ⅲ-Ⅳ)based on arthroscopic findings.Additionally,30 healthy young subjects were recruited as control group.The apparent diffusion coefficient(ADC),true diffusion coefficient(D),pseudo diffusion coefficient(D*)and perfusion fraction(f)values of medial condyle of SB of femur,lateral condyle of femur,medial tibial plateau and lateral tibial plateau,as well as the mean values of each parameter of the above 4 regions were obtained based on knee joint IVIM-DWI and compared within and among groups.Spearman correlation analysis was used to observe the correlations between quantitative parameters and Outerbridge grading.The area under the receiver operating characteristic curve(AUC)was calculated to evaluate the efficacy of each quantitative parameter alone and their combination for assessing knee cartilage degeneration.Results Significant differences of D*values were found among different parts of knee joint SB in moderate-severe group,also of D value,D* value and f value among different parts of knee joint SB in mild group(all P＜0.05).Significant differences of IVIM-DWI parameter were found among different parts of knee joint SB in control group(all P＜0.05).ADC value of knee joint SB in moderate-severe group,mild group and control group decreased successively,while D value increased successively(all P＜0.001).D* values of knee joint SB in moderate-severe group was higher than that in mild group and control group(both P＜0.05).No significant difference of f value of knee joint SB was found between moderate-severe group and mild group(P＞0.05),while both the latter two were higher than that in control group(both P＜0.05).ADC value,D value and f value of knee joint SB were positively correlated(rs=0.671,0.634,0.416,all P＜0.01),while D* value of knee joint SB was negatively correlated with Outerbridge grading(rs=-0.729,P＜0.01).AUC of combination of parameters for distinguishing mild group from control group,and mild group from moderate-severe group was 0.859 and 0.808,respectively.Conclusion SB IVIM-DWI could be used for quantitatively evaluation of knee cartilage degeneration.

Sweet syndrome (acute febrile neutrophilic dermatosis) is a relatively rare inflammatory di-sease, which is characterized by the sudden appearance of tender erythematous skin lesions, often accompanied by pyrexia and elevated neutrophil count. The pathogenesis is not clear yet. Recently, multiple studies have found the association between Sweet syndrome and infections, autoimmune diseases, malignant tumors and the application of multiple drugs. According to different causes, Sweet syndrome can be divided into three types: classical (or idiopathic) Sweet syndrome, malignancy-associated Sweet syndrome and drug-induced Sweet syndrome. Classical Sweet syndrome usually presents in women between the age of 30 to 50 years and may be related to infection, inflammatory bowel disease, or pregnancy. The clinical symptoms typically respond promptly after corticosteroid therapy. The major diagnostic criteria of classical Sweet syndrome include sudden painful erythematous skin lesions, histopathologic evidence of a dense neutrophilic infiltrate without evidence of leukocytoclastic vasculitis; minor criteria include pyrexia over 38 ℃, association with hematologic or visceral malignancies, inflammatory diseases, pregnancy or preceded by infection, prompt response to systemic glucocorticoid or potassium iodide treatment, abnormal laboratory values (three of four: erythrocyte sedimentation rate >20 mm/h, positive C-reactive protein, >8.0×10⁹/L leukocytes, >70% neutrophils). The presence of both major criteria and two of the four minor criteria are required to diagnose classical Sweet syndrome. As for the malignancy-associated Sweet syndrome, skin lesions can be found precede, follow, or at the same time with the diagnosis of hematologic malignancy or a solid tumor. At present, ¹⁸F-fluorodeoxyglucose (¹⁸F-FDG) is commonly used as a positron emission tomography computed tomography (PET/CT) imaging agent for diagnosing and screening malignant tumors. Therefore, most of the case reports on the ¹⁸F-FDG PET/CT manifestations of Sweet syndrome are malignancy-associated. Even classical Sweet syndrome is often accompanied by inflammatory bowel disease, autoimmune diseases, etc. Therefore, for patients with suspected or confirmed Sweet syndrome, it is necessary to take ¹⁸F-FDG PET/CT examination to clarify the general condition, whether it is for patients with malignant signs such as elevated tumor markers values and weight loss, or for patients with classical Sweet syndrome to exclude underlying inflammatory diseases. ¹⁸F-FDG PET/CT is often able to detect the solid tumor early, and assess the degree of hematologic malignancy and inflammatory disease. This study reported a classical Sweet syndrome case associated with inflammatory bowel disease, which was confirmed with skin and intestinal histological examination. The clinical manifestations, laboratory values, ¹⁸F-FDG PET/CT manifestations of the patient related diseases were reported, which was to improve nuclear medicine physicians' understanding of Sweet syndrome. Early diagnosis and treatment can often achieve excellent clinical effect.
