Renal interstitial fibrosis (RIF) is a common pathological change process from the development of various chronic nephropathies to the end stage, and it is an important histological manifestation of renal function decline. At present, no effective anti-fibrosis drugs have been found in clinical practice. In recent years, with the continuous development of traditional Chinese medicine (TCM) pharmacology, molecular biology, system biology, and network pharmacology, the research on regulating RIF with TCM monomer, single TCM, TCM compound, Chinese patent medicine, and TCM injection is deepening. Among them, Jianpi Yishen recipe, Shendi Bushen capsules, Jianzhong Bushen Xiaozheng decoction, Liuwei Dihuangtang, and Lycium barbarum polysaccharides can regulate transforming growth factor-β/small mother against decapentaplegic (TGF-β1/Smads), Wnt/β-catenin, and neurogenic locus notch homolog protein (Notch) signaling pathways. Wulingsan, Zhenwutang, pachymic acid ZA, pachymic acid ZC, and pachymic acid ZD, which mainly induce diuresis, can regulate the Wnt/β-catenin signaling pathway. Hirudin, curcumin, and Fuzheng Huayu recipe, which mainly promote blood circulation, can inhibit inflammation-related pathways such as p-nuclear transcription factor-κB (NF-κB), Toll-like receptor 4/p-nuclear transcription factor-κB (TLR4/NF-κB), and Janus kinase 2/signal transducer and activator of transcription 3 (JAK/STAT), so as to achieve anti-inflammatory and anti-oxidation effects and alleviate the progression of RIF. Shenshuai Xiezhuo decoction, Shenkang injection, and Shenshuai recipe, which are mainly used for invigorating Qi, removing blood stasis, and removing turbidity, can inhibit transdifferentiation of pericytes-myofibroblasts through vascular endothelial growth factor receptor (VEGFR) signaling pathway. At present, there are many studies on the regulation of the RIF signaling pathway by TCM, but there is a lack of a systematic summary. In this study, by combing the signaling pathway of TCM in the treatment of RIF, the effective target of TCM treatment is screened, and its possible mechanism is found, which provides new ideas for clinical treatment and new drug research and development.

BACKGROUND:
This study aimed to investigate programmed death-ligand 1 tumor proportion score in predicting the safety and efficacy of PD-1/PD-L1 antibody-based therapy in treating patients with advanced non-small cell lung cancer (NSCLC) in a real-world setting.
METHODS:
This retrospective, multicenter, observational study enrolled adult patients who received PD-1/PD-L1 antibody-based therapy in China and met the following criteria: (1) had pathologically confirmed, unresectable stage III-IV NSCLC; (2) had a baseline PD-L1 tumor proportion score (TPS); and (3) had confirmed efficacy evaluation results after PD-1/PD-L1 treatment. Logistic regression, Kaplan-Meier analysis, and Cox regression were used to assess the progression-free survival (PFS), overall survival (OS), and immune-related adverse events (irAEs) as appropriate.
RESULTS:
A total of 409 patients, 65.0% ( n = 266) with a positive PD-L1 TPS (≥1%) and 32.8% ( n = 134) with PD-L1 TPS ≥50%, were included in this study. Cox regression confirmed that patients with a PD-L1 TPS ≥1% had significantly improved PFS (hazard ratio [HR] 0.747, 95% confidence interval [CI] 0.573-0.975, P = 0.032). A total of 160 (39.1%) patients experienced 206 irAEs, and 27 (6.6%) patients experienced 31 grade 3-5 irAEs. The organs most frequently associated with irAEs were the skin (52/409, 12.7%), thyroid (40/409, 9.8%), and lung (34/409, 8.3%). Multivariate logistic regression revealed that a PD-L1 TPS ≥1% (odds ratio [OR] 1.713, 95% CI 1.054-2.784, P = 0.030) was an independent risk factor for irAEs. Other risk factors for irAEs included pretreatment absolute lymphocyte count >2.5 × 10 9 /L (OR 3.772, 95% CI 1.377-10.329, P = 0.010) and pretreatment absolute eosinophil count >0.2 × 10 9 /L (OR 2.006, 95% CI 1.219-3.302, P = 0.006). Moreover, patients who developed irAEs demonstrated improved PFS (13.7 months vs. 8.4 months, P <0.001) and OS (28.0 months vs. 18.0 months, P = 0.007) compared with patients without irAEs.
CONCLUSIONS
A positive PD-L1 TPS (≥1%) was associated with improved PFS and an increased risk of irAEs in a real-world setting. The onset of irAEs was associated with improved PFS and OS in patients with advanced NSCLC receiving PD-1/PD-L1-based therapy.

Objective:To compare the dose-response effects of single-fraction ultra-high dose rate (FLASH) and conventional dose rate (CONV) whole abdominal irradiation (WAI) with X-rays on acute radiation-induced intestinal injury in mice, in order to identify optimal dose parameters and potential mechanisms. Methods:A total of 186 male C57BL/6J mice were randomly assigned to a non-irradiation group ( n=6), FLASH irradiation groups ( n=90), and CONV irradiation groups ( n=90). Acute radiation-induced intestinal injury models were established using single-fraction WAI with 11, 12, 13, 14, and 15 Gy X-rays (200 Gy/s for FLASH and 4 Gy/min for CONV). Changes in body weight, stool characteristics, and disease activity index (DAI) scores were assessed at 9 d post-irradiation. At 7 d post-irradiation at 11, 12, and 13 Gy, the intestines were collected for macroscopic examination and length measurement. The small intestine was selected for HE staining and quantitative analysis of intestinal crypt number and mucosal epithelial thickness. The survival of mice was assessed at 15 d post-WAI across all dose groups. Results:After single-fraction WAI at 11, 12, and 13 Gy, the body weight was higher in the FLASH group than that in the CONV group ( t=10.17, 12.65, 10.16, P<0.05). The DAI scores for the FLASH group were 1.00±1.10, 3.17±0.75, and 2.83±1.17, respectively, which were lower than those of the CONV group (4.33±0.52, 7.00±0.00, 8.60±0.55; t=8.70, 11.71, 14.99, P<0.05). However, after WAI at 14 Gy and 15 Gy, there were no significant differences in body weight and DAI between the FLASH group and the CONV group ( P>0.05). At 7 d after single-fraction WAI at 11, 12, and 13 Gy, mice in the FLASH group exhibited less intestinal congestion, edema, and shortening compared with the CONV group. The difference between the FLASH and CONV groups were statistically significant in small intestine length at 11 and 13 Gy ( t=4.42, 3.78, P<0.05), and in colorectal length at 11 and 12 Gy ( t=3.97, 3.12, P<0.05). Small intestine HE staining revealed superior preservation of intestinal architecture in the FLASH group compared with the CONV group, characterized by longer villi, increased crypt numbers, thicker mucosal epithelium, and enhanced structural integrity. The differences in crypt number and mucosal epithelial thickness were statistically significant ( tcrypt=13.10, 23.80, 11.90; tmucosal=5.75, 2.64, 7.74; P<0.05). At 15 d post-irradiation, the survival rate in the 15 Gy FLASH group was higher than that in the CONV group (50% vs. 10%, χ2=5.39, P<0.05), with a median survival extension of 6 d ( HR=0.340, 95% CI: 0.115 4-0.999 9). No significant survival differences were observed between the FLASH group and the CONV group at 11, 12, 13, and 14 Gy ( P>0.05). Conclusions:FLASH irradiation significantly alleviated acute radiation-induced intestinal injury from medium single-fraction WAI with 11, 12, and 13 Gy X-rays compared with CONV irradiation, and showed potential to improve mouse survival after single-fraction WAI at 15 Gy. This effect is likely associated with the preservation of intestinal crypts and exhibits a dose-dependent relationship.

Objective To measure the range of cervical spine mobility in flight trainees and analyze its correlation with flight task performance. Methods A total of 55 participants were enrolled(30 in the experimental group,25 in the control group). An independently developed AI-based digital cervical spine function assessment system was used to measure rotational ranges in yaw, pitch, and roll directions, with comparative analysis performed against contralateral rotation.A subjective scoring scale was designed, and flight instructors of the experimental group were invited to provide performance ratings to explore the relationship between cervical mobility and flight task performance. Results Significant differences were observed between the experimental and control groups in left rotation(P<0.01),right rotation(P<0.05),and flexion(P<0.05).Within the experimental group, significant differences were found between left and right rotation(P<0.01)and between flexion and extension(P<0.01).Correlation analysis between flight performance scores and cervical mobility revealed a significant association between situational awareness and yaw range(r=-0.415,P<0.05).Conclusion The experimental group exhibited significant differences in certain cervical mobility parameters compared to the control group, suggesting that occupational characteristics may influence cervical mobility. The correlation between yaw range and situational awareness implies a potential link between head-neck yaw capability and vestibular-visual system functions, though further validation is required.

Objective:To investigate the effects of differential torque of maxillary posterior teeth with clear aligner on anterior teeth 3D mechanics.Methods:4 maxillary models were designed for differential torque of posterior teeth on the basis of 3D me-chanical test model with first premolar extraction. The stereolithography models were made with the same size as the standard model by 3D printing.According to the torque of the 6 keys to normal occlusion of Andrews, the standard torque of Andrews was used in group A1.The posterior teeth with lingual-crown torque of 5° were used in groups A2,and those with buccal-crown torque of 5° and 10° were respecitvely used in group A3 and A4.4 sets of corresponding clear aligners were fabricated with a 0.75 mm thick thermoplastic material. Each group had 12 pairs of the same clear aligners. The anterior teeth underwent 0.25 mm activation for en-mass retraction. The six-axis force/torque transducer measurement system was used to measure the force applied by the clear align-ers on the anterior teeth in the 3D directions. Results:In buccolingual direction:in group A1,the 4 incisors received lingual force,in groups A2-A4,the anterior teeth received labial force, and the force on the central incisors was greater than that on later-al incisors. In mesiodistal direction:in group A1-A4,4 incisors received mesial force, and the force on lateral incisors was greater than that on central incisors;in group A1-A4,the canines received distal force;the force on canine in group A4 was greater than that in A2 or A3 groups(P<0.05).The force in the vertical direction on the anterior teeth among the 4 groups(P>0.05).Conclu-sion:When en-mass retraction of the anterior teeth with clear aligner, attention should be taken to the torque of the incisors and the tip of the canines. When the posterior teeth with standard torque, the activation of labial-crown overcorrection of the anterior teeth should be increased. When the posterior teeth with lingual-crown torque or buccal-crown torque, the activation of labial-crown over-correction of the anterior teeth should be appropriately reduced.Increasing the mesial overcorrection design of canines may avoid the extrusion of anterior teeth and the deepening of o-verbite due to the distal inclination of canines.

Objective To explore the value of dual-energy CT Bone Marrow Edema in quantitatively evaluating the timing of rib fracture evolution. Methods Sixty patients with thoracic trauma were scanned by dual-energy CT.Using CT Bone Marrow Edema technique, bone marrow CT values were standardized and quantified in both the Bone Marrow Edema area at the rib fracture site and in normal areas 1 cm on both sides of the fracture. The increments of standardized CT values for Bone Marrow Edema and VNCa were obtained for three stages of healing. The numerical variables were statistically described, and both the standardized CT value increments and VNCa standardized CT value increments were compared between the three stages and between each pair of stages.Diagnostic efficacy for stages with significant differences was evaluated using the area under the receiver operating characteristic(ROC)curve(AUC),and Cut-offvalues were calculated. Results There were significant differences in standardized CT value increments and VNCa standardized CT value increments across the three stages of Bone Marrow Edema(H=10.788,p=0.005;F=115.787,p=0.000).The standardized CT value increment of Bone Marrow Edema showed significant differences between the cartilaginous callus stage(fibrous callus stage)and the bony callus-remodeling stage(H=54.958,p=0.003),while the other pairwise comparisons showed no statistical significance(H=-25.603,p=0.183;H=29.354,p=0.113,respectively).However,the VNCa standardized CT value increments showed statistical significance across all three pairwise comparisons(all p=0.000).The ROC curve for differentiating the cartilaginous callus stage(fibrous callus stage)from the bony callus-remodeling stage using Bone Marrow Edema standardized CT value increment had an AUC of 0.652,with a Cut-offvalue of 81.575 Hu.The ROC curve for distinguishing the hematoma inflammation stage from the cartilaginous callus stage(fibrous callus stage)using VNCa standardized CT value increment had an AUC of 0.668,with a Cut-offvalue of 55.700 Hu.The ROC curve for differentiating the cartilaginous callus stage(fibrous callus stage)from the bony callus-remodeling stage using VNCa standardized CT value increment had an AUC of 0.905,with a Cut-offvalue of 37.625 Hu.Conclusion Dual-energy CT Bone Marrow Edema can quantitatively evaluate the timing of rib fracture evolution, and the differences in standardized CT value increments at different stages can provide a theoretical basis for forensic identification of fractures at different time periods. The Cut-offvalues of standardized CT value increments can,to some extent, predict the time stage of a fracture, providing quantitative evidence for forensic experts in the identification of rib fractures.
