1.Research hotspots and prospects of integrating traditional Chinese medicine culture into ideological and political education from 2013 to 2022

By analyzing information such as the number of publications,authors,institutions and keywords in the literature on the integration of traditional Chinese medicine(TCM)culture into ideological and political education in the past 10 years,it was found that the overall number of publications showed an increasing trend. As the main research fronts,TCM colleges and universities had not yet formed teams in cooperation with other universities. The research hotspots were relatively concentrated,but the research horizons were not broad enough. It is suggested that in the future,policy should be taken as the guide to build a cross-regional interdisciplinary cooperation system,horizontally and vertically extend research subjects,innovate and construct diversified teaching models,continue to deeply explore and organically integrate ideological and political education elements,continuously improve the evaluation system of educational functions,and further enhance the timeliness of integrating traditional Chinese medicine culture into ideological and political education. So as to make greater contributions to cultivating talents in the new era and promoting the inheritance and innovation of TCM culture.
2.Advantages of Traditional Chinese Medicine in Treating Dominant Disease: Allergic Rhinitis

In response to the Opinions of the CPC Central Committee and the State Council on Promoting the Inheritance, Innovation, and Development of traditional Chinese medicine(TCM) and the spirit of the National Conference on TCM, Chinese Association of Chinese Medicine organized experts in Otorhinolaryngology Head and Neck Surgery of traditional Chinese and western medicine to discuss the clinical advantages of TCM and integrated traditional Chinese and western medicine in the treatment of allergic rhinitis (AR) and they reached a basic consensus. In recent years, the prevalence of AR has been on the rise, threatening the quality of life of patients and giving rise to a heavy burden to both the patients and the society. AR is resulted from immune imbalance rather than reduced immunity or hyperimmunity, and the imbalance is similar to the Yin-yang disharmony in TCM. In the treatment of this disease, western medicine features rapid onset. However, it is cost-intensive and causes severe surgical trauma, and the recurrence is common. TCM boasts diverse methods for AR, which can be used in all stages of this disease. It has advantages in controlling symptoms such as nasal congestion, runny nose, or dysosmia in the attack stage, preventing recurrence in the remission stage, and treating refractory AR or steroid-resistant AR. In particular, acupuncture enjoys a reputation in treatment of AR, which has been supported by evidence-based medicine and recommended by guidelines. While treating local symptoms of AR, TCM regulates the psychosomatic conditions, which facilitates chronic disease management and long-term follow-up. We should integrate the advantages of TCM and western medicine, give full play to the unique nonnegligible and irreplaceable advantages of TCM, formulate a comprehensive diagnosis and treatment scheme for learning and promotion, and summarize the research outcomes to promote the theoretical innovation of TCM on AR from the perspective of integrated traditional Chinese and western medicine.
3.Analysis of the clinical predictive value of lactate on the prognosis of patients with acute-on-chronic liver failure combined with infection

Objective:To explore the predictive value of lactic acid for the adverse prognostic outcomes in patients with acute-on-chronic liver failure combined with infection.Methods:A retrospective analysis was conducted on the clinical data of 208 cases of ACLF combined with infection who were hospitalized from January 2014 to March 2016. Patients were divided into a survival group ( n = 83) and a mortality group ( n = 125) according to the results of a 90-day follow-up. The clinical data were statistically analyzed between the two groups. Multivariate logistic regression with two categorical variables was used to analyze the independent risk factors for 90-day disease mortality and establish a new prediction model. The receiver operating characteristic curve (ROC curve) was used to evaluate the predictive value of lactic acid, the MELD score, the MELD-Na score, lactic acid combined with the MELD score, lactic acid combined with the MELD-Na score, and the new model. Results:The 90-day mortality rate of 208 cases of ACLF combined with infection was 60.1%. There were statistically significant differences in white blood cell count, neutrophil count, total bilirubin (TBil), serum creatinine (Cr), blood urea nitrogen (BUN), blood ammonia, the international normalized ratio (INR), lactic acid (LAC), procalcitonin, the MELD score, the MELD-Na score, hepatic encephalopathy (HE), acute kidney injury (AKI), and bleeding between the two groups. Multivariate logistic regression analysis showed that TBil, INR, LAC, HE, and bleeding were independent risk factors for 90-day mortality in patients with ACLF combined with infection. After the establishment of MELD-LAC, MELD-Na-LAC, and a new prediction model, the ROC curve revealed that the AUC (95% confidence interval) of MELD-LAC and MELD-Na LAC were 0.819 (0.759 ~ 0.870) and 0.838 (0.780 ~ 0.886), respectively, and was superior than the MELD score [0.766 (0.702 ~ 0.823)] and MELD-Na score [0.788 (0.726 ~ 0.843)], with P < 0.05, while the new model had an AUC of 0.924, the sensitivity of 83.9%, specificity of 89.9%, and accuracy of 87.8%, which was higher than LAC, MELD score, MELD-Na score, MELD-LAC, and MELD-Na-LAC ( P < 0.01). Conclusion:Lactic acid is an independent risk factor for mortality in patients with ACLF combined with infection, and it improves the clinical predictive value of MELD and MELD-Na for the prognosis of mortality.
4.Value of serum complement C3 in the diagnosis of liver fibrosis associated with primary biliary cholangitis

ObjectiveTo investigate the value of serum complement C3 level in determining the stage of liver fibrosis in primary biliary cholangitis (PBC). MethodsClinical data were collected from 108 patients with PBC who attended Tianjin Second People’s Hospital and underwent liver biopsy from January 2012 to October 2022. The degree of liver fibrosis (S0-4) was assessed according to the Scheuer scoring system, with ≥S2 defined as significant liver fibrosis, ≥S3 defined as progressive liver fibrosis, and S4 defined as liver cirrhosis. The independent samples t-test was used for comparison of normally distributed continuous data between two groups, and a one-way analysis of variance was used for comparison between multiple groups; the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups, and the Kruskal-Wallis H test was used for comparison between multiple groups; the chi-square test or the Fisher’s exact test was used for comparison of categorical data between groups. The area under the ROC curve (AUC) was used to evaluate the efficacy of complement C3 in the diagnosis of liver fibrosis in patients with PBC. The Spearman correlation analysis was used to investigate the correlation between complement C3 and liver fibrosis stage. ResultsAmong the 108 patients with PBC, there were 87 (80.6%) female patients and 102 patients (94.4%) with positive autoantibody. As for the stage of liver fibrosis, there were 5 patients (4.6%) in S0 stage, 41 (38.0%) in S1 stage, 23 (21.3%) in S2 stage, 25 (23.1%) in S3 stage, and 14 (13.0%) in S4 stage. There was a significant difference in the level of complement C3 between the patients with different liver fibrosis stages (H=42.891, P<0.001). The level of complement C3 gradually decreased with the aggravation of liver fibrosis, with a negative correlation between them (r=-0.565, P<0.001). Liver stiffness measurement (LSM), aspartate aminotransferase/alanine aminotransferase ratio, aspartate aminotransferase-to-platelet ratio index, and fibrosis-4 were negatively correlated with complement C3, with a correlation coefficient of -0.439 (P<0.001), -0.323 (P=0.001), -0.206 (P=0.033), and -0.291 (P=0.002), respectively. The multivariate logistic regression analysis showed that complement C3 level was an independent predictive factor for significant liver fibrosis, progressive liver fibrosis, and liver cirrhosis, while LSM was an independent predictive factor for significant liver fibrosis and progressive liver fibrosis. The ROC curve analysis showed that complement C3 had an AUC of 0.731, 0.832, and 0.968, respectively, in the diagnosis of significant liver fibrosis, progressive liver fibrosis, and liver cirrhosis, with a corresponding cut-off value of 1.445, 1.235, and 1.005, respectively, and complement C3 combined with LSM had an AUC of 0.811, 0.941, and 0.976, respectively, in the diagnosis of significant liver fibrosis, progressive liver fibrosis, and liver cirrhosis. There was a significant difference in AUC between complement C3 combined with LSM and complement C3 alone in the diagnosis of significant liver fibrosis (Z=2.604, P=0.009), and there was also a significant difference in AUC between complement C3 combined with LSM and complement C3 alone in the diagnosis of progressive liver fibrosis (Z=3.033, P=0.002); there was no significant difference in AUC between complement C3 combined with LSM and complement C3 alone in the diagnosis of liver cirrhosis (Z=1.050, P=0.294), while There was a significant difference in AUC between complement C3 combined with LSM and LSM alone in the diagnosis of liver cirrhosis (Z=2.326, P=0.020). ConclusionSerum complement C3 level has a certain clinical value in assessing the degree of liver fibrosis in patients with PBC, and complement C3 combined with LSM can further improve the efficacy of complement C3 or LSM in the diagnosis of liver fibrosis in PBC.
5.Analysis of the effect of mechanical thrombectomy for acute anterior circulation ischemic stroke in over-time window

Objective To analyze the therapeutic effect of mechanical thrombectomy (MT) in patients with acute anterior circulation ischemic stroke (AACIS) in over-time window.Methods From January 2018 to November 2018,40 patients with AACIS treated with MT were enrolled in our hospital.Patients were divided into in-time-window group (onset-treatment time ≤6 h,n=30) and over-time-window group (onset-treatment time>6 h,n=10) according to the time of onset-treatment.The therapeutic effect was evaluated by modified thrombolysis in cerebral infarction (mTICI).The National Institutes of Health Stroke Scale (NIHSS) scores were compared before and after treatment to evaluate the improvement of neurological deficits.The modified Rankin scale (mRs) score,complication rate,and mortality were compared at 90 days postoperatively to assess prognosis.Results The success rate of patients in the in-time-window group was 83.33%(25/30),and the success rate of patients in the over-time-window group was 80.00% (16/20).There was no significant difference in the success rate between the two groups (P>0.05).The NIHSS scores at the time of discharge were lower than those at admission (P<0.05).There was no significant difference in NIHSS scores between the two groups (P>0.05).Within 90 days after surgery,no deaths occurred in either group.There was no significant difference in the prognosis rate and complication rate between the two groups (P>0.05).Conclusion MT can effectively improve the recanalization rate of patients in the treatment of patients with AACIS over-time-window,and has similar short-term and long-term treatment effects as patients in the in-time-window.
6. Investigation and analysis on characteristics of a cluster of COVID-19 associated with exposure in a department store in Tianjin

Objective:
To describe the epidemiological characteristics of a cluster of COVID-19 cases reported in Baodi district of Tianjin as of 18 February, 2020, which might be associated with the exposure in a local department store, and provide suggestions for prevention and control strategy development.
Methods:
The basic characteristics, time and area distributions, clinical manifestations, epidemiological history and transmission mode of the COVID-19 cases associated with the department store exposure were analyzed.
Results:
A total of 40 COVID-19 cases were associated with the department store exposure, accounting for 75.47% of the total confirmed cases (53 cases) reported in Baodi district. The cases were mainly at the age of 60 years or older (35.00%) and farmers (40.00%). The main clinical manifestations included fever (95.00%), cough (35.00%), and diarrhea (15.00%). The proportion of confirmed severe cases was 32.50%. The incidence curve showed that the incidence peak occurred on 31 January, 2020. Among the 40 cases, 6(15.00%) were department store employees, 19(47.50%) were customers and 15(37.50%) were close contacts (secondary cases). The first case occurred on 21 January, 2020, this case was a department store employee who had a purchasing history at whole sale markets in other provinces and cities before the onset, and 3 employees were still on duty after symptom onsets. The median of the incubation period of customer cases was 6 days, and the median of the interval between onset and medical treatment of customer cases was 7 days.
Conclusion
This was a cluster epidemic of COVID-19, which might be associated with the exposure in the department store. By now, the current prevention and control measures have achieved satisfied effects.
7.Application of blood lactate level in evaluating the prognosis of liver failure patients

As lactate is elevated due to lactate metabolic disorder in liver failure, recent studies have shown that blood lactate has a high value in predicting the prognosis of liver failure. With reference to the research findings in recent years, this article introduces the prediction model of LiFe score and reviews the advances in the clinical application of blood lactate level in patients with liver failure in China and foreign countries. It is believed that a high lactate level is an independent risk factor for poor prognosis of liver failure patients, and it is proposed that lactate can be used as an important indicator to evaluate the severity and prognosis of liver failure and further optimize the scoring system for the prognosis of related liver diseases.
8. Endoscopic minimally invasive treatment of gastric stromal tumors

Gastrointestinal stromal tumor (GIST) is one of the most common tumors originating from gastrointestinal mesenchymal tissues, accounting for about 0.2% of all gastrointestinal tumors. It can occur anywhere in the gastrointestinal tract. Because gastrointestinal stromal tumors have the biological characteristics of non-directional differentiation and potential malignancy, it is very important to resect them completely in time.Traditional surgical excision has a good prognosis in the past, but it has some shortcomings, such as large trauma, slow postoperative recovery and more complications. In recent years, with the continuous development of digestive endoscopy technology, many new endoscopic techniques have emerged for the treatment of gastric stromal tumors, such as endoscopic mucosal dissection (ESD), endoscopic full-thickness resection (EFTR), endoscopy and laparoscopic combined surgery, but the safety and effectiveness of endoscopic surgery for gastric stromal tumors are still controversial in the industry.
9.The risk factors of perhypertension in postmenopausal women in Inner Mongolia

Objective To investigate the risk factors of prehypertension among postmenopausal women.Methods2 592[(43±12)years old] health women were collected as the research object in Saihan District of Hohhot in Inner Mongolia Autonomous Region in April 2015,including 697[(58±6)years old] postmenopausal subjects and 1 895[(37±8)years old] premenopausal subjects.T test was used to compare means of blood pressure, fasting blood glucose (FBG), blood lipid, and body massive index (BMI) between postmenopausal group and premenopausal group and to compare prevalence of prehypertension, hyperglycemias, dyslipidemias, overweight, and obesity between two groups.Logistic Regression was implemented to analyze the relationship between different risk factors and prehypertension among postmenopausal women.Results Compared with premenopausal women, the systolic pressure, diastolicpressure, BMI, FBG, triacylglycerol (TG), low density lipoprotein (LDL) in postmenopausal women were significantly higher(P<0.05).Prevalence of prehypertension, impaired fasting glucose (IFG), diabetes, TG abnormalities, LDL abnormalities, overweight, and obesity in postmenopausal women were significantly higher than in premenopausal women(P<0.05).Age 55 to 59, 60 to 64 and above 65 years overweight, obesity, IFG and diabetes were independent risk factors of prehypertension among postmenopausalwomen.Conclusions Age 55 yearsand above, overweight, obesity, IFG and diabetes are the independent risk factors of prehypertension among postmenopausal women.
10.Effects of GIK on liver injury induced by lipopolysaccharide in rats

Objective To investigate if glucose-insulin-potassium (GIK)would relieve the liver injury induced by endotoxemia in rats.Methods Sixty SD male rats,weight 200-250g,were randomly divided into three groups (n = 20):control group (group C),lipopolysaccharide group (group LPS,LPS 8 mg/kg)and Glucose-insulin-potassium group(group GIK,8 mg/kg LPS+GIK 4 ml·kg-1 ·h-1 ).All the rats were injected with 20 mg/kg ketamine intraperitonealy before trial. Erythrocin was daubed on the wound to avoid infection.The rats of group LPS and group GIK were injected LPS 8 mg/kg intraperitoneal,then,rats in group LPS and group GIK received saline(4 ml·kg-1 ·h-1 )or GIK(Glucose 200 g/L,Insulin 60 IU/L,KCL 60 mmol/L)infusion continuously. Liver and serum samples were collected on before injection,3 days after injection and 5 days after in-jection.Serum concentrations of ALT and AST were measured.TNF-αlpha of liver homogenate was detected by ELISA.The severity of liver damage was assessed by an approprite histopathological sco-ring system and apoptosis of parenchymal cells were assessed by TUNEL immunofluorescence assay. Results Compared with group control,the level of serum ALT and AST in group LPS and group GIK were significantly higher at 3 days after injection.The level of hepatic TNF-α,the hepatic damage score and the index of hepatic apoptosis in group LPS and group GIK were significantly higher on 3 days after injection and 5 days after injection.(P<0.05).Compared with group LPS,the level of hepatic TNF-αand the hepatocyte apoptosis rates decreased significantly in group GIK on 3 days after injection.The level of serum ALT and AST,hepatic TNF-α,the hepatic damage score and the hepatocyte apoptosis rates decreased significantly in group GIK at 5 days after injection(P <0.05).Conclusion Intraperitoneal injection of endotoxin can cause liver injury in rats,resulting in the liver hepatdysfunction and hepatocyte damage.GIK has protective effects on LPS induced liver injury in rats.
