The Type III Secretion System (T3SS) serves as a pivotal virulence apparatus for numerous Gram-negative bacterial pathogens, enabling them to infect both animal and plant hosts. Functioning as a molecular syringe, the T3SS directly translocates bacterial effector proteins from the bacterial cytoplasm into the interior of eukaryotic host cells. These effectors are central weapons that precisely manipulate a wide spectrum of host cellular physiological processes, ranging from cytoskeletal dynamics to immune signaling, to establish a favorable niche for bacterial survival and proliferation. Among the diverse arsenal of T3SS effectors, the YopJ family constitutes a critical group of virulence factors. Members of this family are characterized by a conserved catalytic triad structure—a hallmark of the CE clan of cysteine proteases that has been evolutionarily repurposed to confer acetyltransferase activity. A defining and intriguing feature of these enzymes is their stringent dependence on a host-derived eukaryotic cofactor, inositol hexakisphosphate (IP6), for allosteric activation. This requirement acts as a sophisticated molecular safeguard, ensuring enzymatic activity only within the appropriate host environment, thereby preventing detrimental effects on the bacterium itself. While seminal studies on individual members such as Yersinia’s YopJ and Salmonella’s AvrA have provided deep mechanistic insights, a systematic and integrative understanding of the structure-function relationships across the entire family remains fragmented. Key questions persist regarding how a conserved catalytic core has diverged to recognize distinct host substrates in different kingdoms of life. To address this gap, this article provides a systematic review of the YopJ family, focusing on three interconnected aspects: their structural features, their catalytic mechanism, and their divergent immunosuppressive strategies in animal versus plant hosts. By conducting a comparative analysis of the sequences and resolved three-dimensional structures of three representative members (e.g., HopZ1a, PopP2, AvrA), we elucidate regions of significant variation embedded within the conserved core catalytic architecture. These variable regions, often involving surface loops and substrate-binding interfaces, are crucial determinants of target specificity and functional specialization. The functional divergence of this effector family is most apparent when comparing their modes of action in different hosts. In animal hosts, YopJ-family effectors primarily sabotage innate immune signaling pathways. They achieve this by acetylating key serine and threonine residues within the activation loops of critical kinases in the MAPK and NF‑κB pathways. This post-translational modification blocks the phosphorylation and subsequent activation of these kinases, leading to potent suppression of inflammatory cytokine production. Conversely, in plant hosts, the strategy broadens to dismantle the two-tiered plant immune system. YopJ homologs target a more diverse set of substrates, including immune-associated receptor-like cytoplasmic kinases (RLCKs), microtubule networks via tubulin acetylation (which disrupts cellular trafficking and signaling), and transcription factors central to defense gene regulation. This multi-target approach effectively suppresses both Pattern-Triggered Immunity (PTI) and Effector-Triggered Immunity (ETI). In conclusion, this synthesis aims to deepen the mechanistic understanding of YopJ family-mediated pathogenesis by integrating structural biology with cellular function across host kingdoms. Elucidating the precise molecular basis for substrate selection—how conserved platforms achieve target diversity—is a major frontier. Furthermore, this knowledge provides a vital theoretical foundation for developing novel anti-virulence strategies. Targeting the conserved IP6-binding pocket or the catalytic acetyltransferase activity itself represents a promising avenue for designing broad-spectrum inhibitors that could disarm this critical family of bacterial effectors, potentially offering new therapeutic approaches against a range of pathogenic bacteria.

As the core vehicle for preserving and transmitting traditional Chinese medicine(TCM) academic thought and clinical experience, the establishment of a robust quality evaluation system for TCM clinical case reports is a crucial component in the current standardization and modernization of TCM. Based on the practical experience of constructing the China Clinical Cases Library of Traditional Chinese Medicine by the China Association of Chinese Medicine, this study conducted a comprehensive analysis of critical challenges, including insufficient authenticity and unfocused evaluation criteria. It proposed a three-dimensional evaluation framework grounded in the structure-process-outcome logic, encompassing three dimensions of authenticity and standardization, characteristics and advantages, application and translational impact. This framework integrated 12 key evaluation indicators in a systematic manner. The model preserved the academic characteristics of TCM syndrome differentiation and treatment, while aligning with modern scientific research standards, achieving a balance between individualized TCM experience and standardized evaluation. Concurrently, this study provided theoretical foundations and methodological guidance for evaluating the quality of TCM clinical cases, contributing significantly to the inheritance of TCM knowledge, evidence-based practice, and the reform of talent evaluation mechanisms.
3.Study on the value of deep learning reconstruction based magnetic resonance on the image quality and lesion diagnostic efficacy of 3D LAVA sequences

Objective:To explore the application value of deep learning reconstruction(DLR)of magnetic resonance(MR)on the image quality and lesion diagnostic efficacy of three-dimensional liver acceleration volume acquisition(3D-LAVA)sequences.Methods:A retrospective analysis was conducted on the image data of magnetic resonance imaging(MRI)of 45 patients who underwent MR contrast-enhanced examinations on abdomen at West China Hospital of Sichuan University from June 2023 to September 2023.For the venous phase of the LAVA sequence,conventional reconstruction and DLR were performed respectively to obtain conventional LAVA images and DLR-LAVA images at the venous phase.Two radiologists conducted subjective evaluation for the overall image quality,image clarity,artifacts,and diagnostic confidence of the two groups of images by using a 5-point scale.And then,the signal-to-noise ratio(SNR)of the images of liver,spleen,and lesion,as well as the contrast-to-noise ratio(CNR)of the liver,spleen to fat,muscle,and lesions were objectively evaluated.The two radiologists measured respectively the maximum and minimum diameters of the largest layer of the largest lesion,and compared the diagnostic efficacy of conventional LAVA and DLR-LAVA for lesions.The diagnostic efficacies of conventional LAVA and DLR-LAVA were respectively assessed by using the receiver operating characteristic(ROC)curve.Results:The consistency of the two radiologists'subjective evaluations for the image quality of the conventional LAVA and DLR-LAVA sequences was moderate and above moderate,with Kappa values ranging from 0.553 to 0.902,and P<0.001.Both radiologists found that the subjective image quality,image clarity,artifacts,and diagnostic confidence of the DLR-LAVA sequence were higher than those of the conventional LAVA sequence(Z Physician 1=4.24,5.49,3.50,4.47,Z Physician 2=4.15,3.12,3.77,4.26,P<0.05).The SNR values of the liver,spleen,and lesions in the DLR-LAVA images were all higher than those in the conventional LAVA sequence(t=-20.45,-18.58,-5.51,P<0.001).The CNR values between liver and fat,between liver and muscle,between liver and spleen,between spleen and fat,between spleen and muscle,and between lesion and liver in DLR-LAVA sequence were higher than those in the conventional LAVA sequence(t=-20.49,-19.94,-18.05,-16.74,P<0.001).There were no statistically significant differences in the measured values of the maximum and minimum diameters of the largest lesions between the conventional LAVA and DLR-LAVA sequences(P>0.05).The area under curve(AUC)values of physician 1 who used respectively the conventional LAVA and DLR-LAVA sequences were 0.897 and 0.916 in diagnosing lesions,while those of physician 2 were respectively 0.851 and 0.890.Conclusion:DLR can improve the image quality of the LAVA sequence for liver,and has a higher diagnostic value for lesions.
4.Expert consensus on whole-process management of drug traceability codes in medical institutions of Sichuan province

OBJECTIVE To provide standardized whole-process guidance on drug traceability codes for medical institutions in Sichuan province, ensuring medication safety and compliance with medical insurance supervision requirements. METHODS Based on evidence-based principles and expert consensus, Expert Consensus on Whole-process Management of Drug Traceability Codes in Medical Institutions of Sichuan Province (hereinafter referred to as the Consensus) was formulated through systematic literature review, field investigations, establishment of a multidisciplinary expert committee and multiple rounds of questionnare consultation via the modified Delphi method, and finalized through consensus meetings. RESULTS & CONCLUSIONS The Consensus clarifies key operating procedures for code verification, code assignment and code return, whole-process operational standards for drug warehouse acceptance and storage, drug warehouse outbound delivery and pharmacy acceptance check, drug distribution and dispensing in pharmacy and intravenous admixture center, medication administration in nursing units and examination departments, as well as drug return process. Key recommendations are proposed such as improving the core functions of the drug traceability system, unifying the hospital-wide traceability code database, strengthening the management of traceability codes for backup medications, establishing a management organization and institutional framework, and optimizing the architectural design and data governance requirements of the drug traceability system. The release of the Consensus will provide scientific, standardized and implementable practical guidelines for medical institutions of Sichuan province, helping to improve closed-loop management of the drug traceability system, strengthen medication safety and fulfil medical insurance fund supervision.
5.Construction and validation of a risk prediction model for hyperuricemia in perimenopausal and postmenopausal women

Objective To develop and compare prediction models for hyperuricemia(HUA)in perim-enopausal and postmenopausal women using Lasso regression,random forest,and multivariate logistic regres-sion.Methods A multi-stage,stratified cluster sampling method was used to select 12 790 subjects from An-ning City,Yunnan Province.Prediction models for HUA were constructed using Lasso regression,random for-est,and multivariate logistic regression.The efficacy of the model was evaluated by accuracy,sensitivity,speci-ficity,F1 score,and area under the curve(AUC).Results LASSO regression analysis screened 19 variables for inclusion in the model,such as age,waist circumference,diastolic blood pressure,BMI,HDL-C,fasting blood glucose(FBG),etc.The accuracy rate was 0.701,the sensitivity was 0.703,the specificity was 0.680,and the F1 score was 0.806.The AUC(95%CI)was 0.770(0.748-0.792).The results of the random forest model show that variables such as creatinine,triglyceride-glucose index(TyG),TG,BMI,TC,Urea nitrogen(Urea),and ALT were relatively important,with an accuracy rate of 0.663,a sensitivity of 0.653,a specificity of 0.738,and an F1 score of 0.774.The AUC(95%CI)was 0.763(0.741-0.785).Multivariate logistic re-gression results showed that 11 variables including creatinine(Cr),TyG,BMI,Urea,and ALT were included in the model,with an accuracy rate of 0.705,a sensitivity of 0.707,a specificity of 0.686,an F1 score of 0.809,and an AUC(95%CI)of 0.771(0.749-0.793).Conclusion The overall performance of LASSO re-gression and multivariate logistic regression models is better.The random forest model has a strong variable screening ability and high specificity,and can be used as a supplement to provide more accurate predictions.
6.Research Advances of Deep Learning-based Raman Spectroscopy and Their Application in Detection of Microplastics

Microplastics are widely present in various environments such as water bodies,land,and atmosphere,which pose threats to the ecological environment and human health through transmission and accumulation in the food chain.The existing detection techniques for microplastics face challenges such as complex preparation procedure of samples,low efficiency in processing large batches of samples,and difficulties in handling complex samples.Therefore,there is an urgent need for rapid and efficient detection techniques suitable for complex microplastics samples in the field of environmental monitoring.Raman spectroscopy,known for its advantages such as rapidity,accuracy,high sensitivity,non-destructiveness,and non-contact,demonstrates great application potential in detection of microplastics.Deep learning,an artificial intelligence method known for its large-scale data processing,nonlinear modeling and automatic feature extraction capabilities,is receiving increasing attention in the analysis of Raman spectroscopy signals.The application of deep learning-based Raman spectroscopy has significantly improved performance indicators such as detection efficiency and accuracy.This article introduced the existing Raman enhancement techniques,summarized the deep learning methods applied in Raman spectroscopy signal analysis,reviewed the recent research and application progress of deep learning-based Raman spectroscopy in detection of microplastics,and finally discussed the challenges and future prospects of deep learning-based Raman spectroscopy in detection of microplastics.
7.Establishment and Application of TaqMan qPCR Detection Method for Human DNA Contamination in DNA Laboratory

Objective To establish a highly sensitive and specific method for detecting human DNA based on real time quantitative PCR(qPCR)technique for the rapid detection of potential DNA con-tamination sources in DNA laboratories.Methods Primers and probes were designed with Primer Ex-pressTM software using the reference sequence of human 18S rRNA gene as a template,and the opti-mal prime-probe combination was screened by matrix method.The PCR products of the target se-quence of human 18S rRNA gene were used to construct the plasmid,and a plasmid standard was used to draw the standard curve of the qPCR system.According to the Minimum Information for Pub-lication of Quantitative Real-time PCR Experiments(MIQE)guidelines,the specificity,sensitivity,re-peatability and application effect of the qPCR system were evaluated.Results The sensitivity of the qPCR system established in this study was 5.3×10-5 ng/μL,which showed good specificity for human DNA samples.The correlation coefficient of the qPCR system was-0.999,and amplification efficiency was 100%.Both the intra-batch and inter-batch variation coefficients were less than 2%.Conclusion The established human DNA detection method based on qPCR technique has good specificity,high sen-sitivity,and robust stability.It can be used for rapid detection of DNA contamination and daily moni-toring of the accumulated human DNA in the laboratory environment.

Turning to critical illness is a common stage of various diseases and injuries before death. Patients usually have complex health conditions, while the treatment process involves a wide range of content, along with high requirements for doctor′s professionalism and multi-specialty teamwork, as well as a great demand for time-sensitive treatments. However, this is not matched with critical care professionals and the current state of medical care in China. Telemedicine, which shortens the distance of medical professionals and the gap of disease diagnosis and treatments in various regions through electronic information, can effectively solve the current problem. Therefore, there is an urgent need to develop a standardized, high-quality visualization telemedicine round system .Therefore, experts have been organized to search domestic and foreign literature on telemedicine round for critically ill patients and to form this consensus based on clinical experiences so as to further improve the level of critical care treatments in regions.
9.Effect of 3D printing extravascular stent implantation on hemodynamics of large vessels in patients with nutcracker syndrome

Objective:To explore the effect of 3D printing extravascular stent implantation on the hemodynamics of the relevant large vessels in patients with nutcracker syndrome(NCS),and to discuss the clinical safety of this new procedure.Methods:A total of 30 NCS patients admitted to Tangdu Hospital Affiliated to Air Force Military Medical University from May to December 2023 were prospectively included. All the patients were received laparoscopic 3D printing extravascular stent implantation for NCS treatment. Whether the lumbago,abdominal pain,hematuria,proteinuria and varicocele had improved after the stent implantation were observed. The inner diameter,blood flow velocity of left renal vein,abdominal aorta,superior mesenteric artery and the diameter of inferior vena cava were measured by ultrasound 1 day before surgery and 8 days after surgery,respectively.In addition,the carotid-femoral pulse wave velocity(cfPWV)was measured by a newly developed method,by which the arterial stiffness could be detected based on Doppler ultrasound. The blood flow Doppler spectra of each subject were recorded at the right common carotid artery and the right common femoral artery,respectively,then based on that,the cfPWV was automatically calculated by this new technique. The above internal diameters,blood flow velocity and cfPWV were compared before and after the laparoscopic 3D printing extravascular stent implantation.Results:Among the 30 patients,the postoperative blood flow velocity at the left renal vein compression site was significantly lower than that before surgery[(50.7 ± 14.8)cm/s vs(122.1 ± 24.1)cm/s, P<0.001)],and the clinical symptoms,including lumbago and abdominal pain,hematuria,proteinuria and varicocele were significantly improved(all P<0.05). There were no significant changes in the internal diameter,blood flow velocity of abdominal aorta,superior mesenteric artery and the diameter of inferior vena cava,and cfPWV before and after operation(all P>0.05). Conclusions:The 3D printing extravascular stent implantation through laparoscope can effectively improve left renal vein compression in patients with NCS,and cause no significant change in large arterial hemodynamics,suggesting that this surgical method is a safe and reliable treatment for patients with NCS.

Thoracolumbar spine fracture often leads to severe pain, functional impairments, and neurological deficits, for which open reduction and internal fixation can effectively restore the spinal structural stability. Open decompression and reduction with internal fixation can help relieve spinal cord compression and improve spinal function in cases of concomitant cord injury. Although spinal stability can be restored through surgery, patients often face chronic pain and functional impairments postoperatively. A postoperative rehabilitation program is critical in optimizing therapeutic outcomes, reducing complications, and minimizing the risk of secondary injuries. However, current rehabilitation methods, such as physical therapy, functional training, and pain management, are confronted with problems in clinical practice, including significant variation in efficacy, poor patient adherence, and prolonged rehabilitation period. There is an urgent need for a unified rehabilitation strategy to address these problems. To this end, the Spinal Trauma Group of the Orthopedic Physicians Branch of the Chinese Medical Association and the Spine Health Professional Committee of the Chinese Human Health Technology Promotion Association organized experts from relevant fields to formulate Evidence-based guidelines for rehabilitation treatment after internal fixation of thoracolumbar spine fracture in adults ( version 2025) by integrating evidences from clinical researches and advanced rehabilitation concepts at home and abroad. A total number of 14 recommendations concerning the rehabilitation treatment with multimodal analgesia, psychological intervention, deep vein thrombosis prevention, core muscle and extremity exercise, appropriate use of braces, early weight-bearing, device-aided rehabilitation exercise, neuroregulatory therapy, rehabilitation team were put forward, aiming to standardize the post-operative rehabilitation process following internal fixation, promote the functional recovery, and enhance patients′ quality of life.
