1.Effect of removing residual silicone oil on digestive endoscopes between silicone oil removal detergent and multi-enzyme detergent

OBJECTIVE To observe the effects of silicone oil removal detergent and multi-enzyme detergent on re-moving the residual silicone oil from digestive endoscopes so as to provide bases for clinical cleaning of digestive endoscopes.METHODS Totally 60 colonoscopes that were used in the endoscopy center of Shenzhen Hospital of Southern Medical University in Jul.2024 were chosen as the research subjects and were randomly divided in to the experimental group with 30 colonoscopes(the silicone-removing multi-enzyme detergent group)and the control group with 30 colonoscopes(the multi-enzyme detergent group),the two groups were respectively treated with sil-icone-removing multi-enzyme detergent and multi-enzyme detergent during the cleaning process.The average bac-terial colony counts of rinsed endoscopes,residual water droplet counts of the cleaned endoscopes and absorbance values of residual methyl silicone oil in the biopsy forceps channels of the endoscopes were observed and compared between the two groups so as to evaluate the effect on removing silicone oil.RESULTS The median average bacteri-al colony counts were 3.05(1.62,4.68)× 103 CFU/piece in the experimental group,36.70(34.66,38.10)× 103 CFU/piece in the control group(Z=-6.654,P<0.001).The median counts of residual water droplets on the en-doscopes were 7.00(5.00,16.50)in the experimental group,31.50(12.00,43.00)in the control group(Z=-3.940,P<0.001).The median absorbance value of residual methyl silicone oil in the biopsy forceps chan-nels of the endoscopes was 6.85(5.58,7.85)× 10-3 in the experimental group,36.50(35.50,43.65)× 10-3 in the control group(Z=-6.655,P<0.001).CONCLUSION The silicone-removing multi-enzyme detergent shows more remarkable effect on removing the residual silicone oil on the endoscopes than the multi-enzyme detergent,it improves the cleaning effect of the endoscopes and is worthy to be verified and promoted in the hospital.
2.Antimicrobial resistance surveillance in the bacterial strains isolated from pediatric intensive care units in China:results from 2020 to 2022

Objective This study aimed to investigate the antimicrobial resistance profiles of bacterial strains isolated from pediatric intensive care units(PICU)in China for better antimicrobial therapy.Methods Clinical isolates were collected from 17 institutions,including tertiary care children's hospitals and pediatric department of tertiary general hospitals in China from January 1,2020 to December 31,2022.Antimicrobial susceptibility testing was carried out according to a unified protocol using Kirby-Bauer method or automated systems.Results were interpreted according to the breakpoints released by the Clinical and Laboratory Standards Institute(CLSI)in 2020.Results A total of 10 688 isolates were collected,including gram-positive organisms(39.2%)and gram-negative organisms(60.8%).The top three organisms were S.aureus(13.6%,1 453/10 688),A.baumannii(10.0%,1 067/10 688),and coagulase-negative Staphylococcus(9.9%,1 058/10 688).Multi-drug resistant organisms(MDROs)were very common in children.The prevalence of methicillin-resistant Staphylococcus aureus(MRSA),carbapenem-resistant Enterobacterales(CRE),carbapenem-resistant E.coli,carbapenem-resistant K.pneumoniae(CRKP),carbapenem-resistant A.baumannii(CRAB),and carbapenem-resistant P.aeruginosa(CRPA)was 41.1%,19.4%,8.8%,30.9%,67.4%,and 28.8%,respectively.Overall,more than 50%of Enterobacteriales isolates were resistant to cephalosporins,while nearly 25%of Enterobacteriales isolates were resistant to carbapenems.MDROs were highly resistant to commonly used antibiotics.More than 80%of CRE and CRAB strains were resistant to all beta-lactam antibiotics.CRE and CRAB showed low resistance rates to tigecycline and polymyxin.CRPA showed lower resistance rates to piperacillin,beta-lactamase inhibitor combinations than the resistance rates to third and fourth generation cephalosporins.All of the Staphylococcus and Enterococcus isolates were susceptible to vancomycin and tigecycline.None of PRSP strains isolated from meningitis and nonmeningitis samples were resistant to rifampicin,vancomycin,or linezolid.The prevalence of β-lactamase-negative ampicillin-resistant(BLNAR)strains was 43.3%in Haemophilus influenzae.Conclusions MDROs were prevalent in PICU.It is necessary to establish an effective multidisciplinary team(MDT)to control the antimicrobial resistance.

Gastric cancer with peritoneal metastasis (GCPM) is a common and lethal manifestation of advanced gastric cancer, with a median survival of only 5-11 months. This consensus was developed by 30 experts from Asia (China, Japan, Korea, and Singapore) using the Delphi method and the GRADE evidence grading system. A total of 29 statements were formulated, covering the diagnosis and assessment of GCPM, indications for laparoscopic exploration and NIPS (normothermic intraperitoneal and systemic treatment), treatment regimens, prevention and management of complications, criteria for conversion surgery, and postoperative intraperitoneal therapy. The consensus aims to standardize clinical practice and improve the prognosis of patients with GCPM.

6.Analysis of factors influencing postpartum depression in elderly multipara undergoing repeat cesarean section

Objective:To investigate the factors influencing postpartum depression (PPD) in elderly multipara undergoing repeat cesarean section.Methods:A convenience sampling method was used to select 160 elderly multipara who underwent repeat cesarean section and completed follow-up at the Department of Obstetrics, First Affiliated Hospital of Zhengzhou University, from June 2021 to June 2023. The Edinburgh Postnatal Depression Scale (EPDS) was employed to assess PPD incidence, classifying participants into PPD and non-PPD groups. Binary Logistic regression analysis was conducted to identify factors influencing PPD.Results:The incidence of PPD among the 160 elderly multipara was 33.13%. Binary Logistic regression analysis indicated that monthly household income, postpartum sleep disturbances, and newborn weight were influencing factors for PPD in elderly multipara undergoing repeat cesarean section.Conclusions:Monthly household income, postpartum sleep disturbances, and newborn weight are significant factors influencing the occurrence of PPD in elderly multipara undergoing repeat cesarean section.
7.A sericin hydrogel scaffold for sustained dexamethasone release modulates macrophage polarization to promote mandibular bone defect repair in rats

Objective To evaluate the efficacy of a modified sericin hydrogel scaffold loaded with dexamethasone(SMH-CD/DEX)scaffold for promoting bone defect healing by stimulating anti-inflammatory macrophage polarization.Methods The light-curable SMH-CD/DEX scaffold was prepared using dexamethasone-loaded NH2-β-cyclodextrin(NH2-β-CD)and sericin hydrogel and characterized by scanning electron microscopy(SEM),Fourier transform infrared spectroscopy(FTIR),biocompatibility assessment and drug release test.THP-1 macrophages incubated with the scaffold were examined for protein expressions of iNOS and Arg-1,mRNA expressions of IL-6,Il-10,Arg-1 and iNOS,and surface markers CD86 and CD206 using Western blotting,RT-qPCR,and flow cytometry.In a co-culture system of human periodontal ligament stem cells(HPDLSCs)and THP-1 macrophages,the osteogenic ability of the stem cells incubated with the scaffold was evaluated by detecting protein expressions of COL1A1 and Runx2 and expressions of ALP,Runx2,OCN and BMP2 mRNA,ALP staining,and alizarin red staining.In a rat model of mandibular bone defect,the osteogenic effect of the scaffold was assessed by observing bone regeneration using micro-CT and histopathological staining.Results In THP-1 macrophages,incubation with SMH-CD/DEX scaffold significantly enhanced protein expressions of Arg-1 and mRNA expressions of IL-10 and Arg-1 and lowered iNOS protein expression and IL-6 and iNOS mRNA expressions.In the co-culture system,SMH-CD/DEX effectively increased the protein expressions of COL1A1 and Runx2 and mRNA expressions of ALP and BMP2 in HPDLSCs and promoted their osteogenic differentiation.In the rat models,implantation of SMH-CD/DEX scaffold significantly promoted bone repair and bone regeneration in the bone defect.Conclusion The SMH-CD/DEX scaffold capable of sustained dexamethasone release promotes osteogenic differentiation of stem cells and bone defect repair in rats by regulating M2 polarization.
8.Clinical profiles of community-acquired Pseudomonas aeruginosa infections in children

Objectives:To investigate clinical characteristics, outcomes and antimicrobial resistance of community-acquired Pseudomonas aeruginosa (CAPA) infections in Chinese pediatric patients. Methods:This retrospective study was conducted at 6 tertiary hospitals in China during January 2016 to December 2018. The clinical and microbiological data of CAPA infected hospitalized children in Hainan and in other regions were collected and compared, and the antimicrobial resistance patterns, clinical characteristics and antibiotic therapy were analyzed. Between different groups were compared using the Chi-square test and Mann-Whitney U test. Results:Among 91 patients, 63 cases were males, 28 cases were females, and 74 cases were from Hainan province, 17 cases were from other regians. The age of consultation was 22.5 (5.4, 44.0) months. Twenty-four cases (26%) had underlying diseases. Fever (79 cases (87%)) and cough (64 cases (70%)) were common initial symptoms. Other concomitant symptoms included wheezing 8 cases (9%), diarrhea 3 cases (3%) and vomiting 4 cases (4%). Twenty-eight cases (31%) had organ infections, including pneumonia 22 cases (24%), skin infection 5 cases (5%), meningitis, intra-abdominal infection and upper urinary tract infection each 1 case (1%). The resistance rate of CAPA isolates to cefepime (4% (4/90)), amikacin (1% (1/90)), ciprofloxacin (2% (2/90)) and levofloxacin (1% (1/89)) was low, and to ceftazidime, piperacillin, piperacillin-azobactam, carbapenem was 12% (11/90), 3/16, 18% (10/56) and 6% (5/90), respectively. Antimicrobial combination therapy accounted for 52% (47/91) of empirical therapy and 59% (52/88) of definite therapy. Two cases (2%) were hopeless discharged, and 3 cases (3%) died during hospitalization. The worse prognosis of CAPA infection is significantly different among children in other regions and in Hainan (4/17 vs. 1% (1/74), χ2=9.74, P<0.05). Conclusions:The invasive CAPA-infection has regional difference in incidence and prognosis in China. Clinical symptoms and signs are non-specific. CAPA strains isolated from pediatric patients display low level of resistance to most of the common antipseudomonal antibiotics. The proportion of poor prognostic outcome is lower in Hainan than in other regions.
9.Effectiveness and safety of narrow-band intense pulsed light on erythromelanosis follicularis faciei et colli

Objective:To evaluate the efficacy and safety of narrow-band intense pulsed light in the treatment of erythromelanosis follicularis faciei et colli (EFFC).Methods:The patients with EFFC, diagnosed at the Department of Cosmetic Laser Surgery, Hospital for Skin Disease, Chinese Academy of Medical Sciences, and Peking Union Medical College, from January 2017 to December 2022, were retrospectively evaluated. There were 28 patients, including 24 males and 4 females. They ranged in age from 15 to 35 (21.0±4.2) years. All patients received three sessions of narrow-band intense pulsed light treatments, with intervals of 1-2 months. A follow-up visit was completed 1-2 months after three treatments, and efficacy was evaluated. Two dermatologists assessed the clinical improvement in erythema, pigmentation, and follicular papules on a 4-point scale (0=0-25% improvement, 1=26%-50% improvement, 2=51%-75% improvement, and 3=76%-100% improvement). Adverse events were also recorded.Results:A total of 28 patients received three sessions of narrow-band intense pulsed light treatments and were followed-up. After three treatments, the mean improvement scores for erythema, follicular papules, and pigmentation were (2.02±0.65), (1.38±0.59) and (0.80±0.61) respectively. Among these patients, 67.9% (19/28) experienced over 50% improvement in erythema, 39.3% (11/28) showed over 50% improvement in follicular papules, and 7.1% (2/28) demonstrated over 50% improvement in pigmentation. No blisters and scars were formed.Conclusions:Narrow-band intense pulsed light is a safe and effective treatment for patients with EFFC, particularly for those presenting with erythema.
10.Effects of combined exercise training on exercise ability and inflammatory factors in elderly patients with chronic obstructive pulmonary disease complicated with sarcopenia

Objective To investigate the effects of combined exercise training on exercise ability and inflammatory factors in elderly patients with chronic obstructive pulmonary disease(COPD)complicated with sarcopenia.Methods Sixty elderly patients with COPD complicated with sarcopenia who were treated in Hangzhou Third People's Hospital from July 2022 to May 2023 were selected as study objects,and were divided into control group and treatment group according to random number table method,with 30 cases in each group.The control group received routine treatment of COPD,while the treatment group received combined exercise training on the basis of routine treatment for 12 weeks.The appendicular skeletal muscle mass index(ASMI),grip strength,walking speed,6 min walking distance(6MWD),COPD assessment test(CAT),forced expiratory volume in one second(FEV1)/forced vital capacity(FVC)、serum interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were compared.The correlation between serum IL-6、TNF-α and ASMI,grip strength、walking speed were analyzed.Results After treatment,6MWD in both groups was significantly longer than before treatment,and CAT score was significantly lower than before treatment(P<0.05).The ASMI,grip strength and walking speed of patients in treatment group were significantly higher than before treatment,and the serum IL-6 and TNF-α were significantly lower than before treatment(P<0.05).The ASMI,grip strength and walking speed of treatment group were significantly higher than those of control group,6MWD was significantly longer than that of control group,CAT score,serum IL-6 and TNF-α were significantly lower than those of control group(P<0.05).There was no significant difference in FEV1/FVC between two groups before and after treatment(P>0.05).Pearson correlation analysis showed that serum IL-6、TNF-α were negatively correlated with ASMI、grip strength and walking speed.Conclusion Combined exercise training can improve muscle mass and strength,improve exercise ability and clinical symptoms,and inhibit inflammatory response in elderly patients with COPD complicated with sarcopenia.
