1.Correlation between non-HDL-C/HDL-C and subclinical atherosclerosis in officers and seamen of naval special duty

Objective:To investigate the correlation and its clinical significance between the ratio of non-high density lipoprotein cholesterol to high density lipoprotein cholesterol (non-HDL-C/HDL-C) and subclinical arteriosclerosis (SA) in officers and seamen of naval special duty.Methods:A total of 118 officers and seamen of naval special duty with SA admitted to Xiamen Special Services Recuperation Center of the Army from June 2015 to June 2018 were selected as SA group. According to the carotid intima-media thickness (C-IMT) and plaque occurrence, they were divided into two subgroups: 60 cases in thickening group and 58 cases in plaque group. And another 60 officers and seamen of naval special duty admitted in the sanatorium in the same period for resting were selected into control group as they were healthy after examination. Routine physical examination recordings were compared among those groups, including general clinical data (cardiovascular health behavioral factors score, C-IMT, and blood glucose) and recordings on lipid [triglyceride (TG), total cholesterol (TC), HDL-C, low density lipoprotein cholesterol (LDL-C), non-HDL-C, non-HDL-C/HDL-C ratio, and atherogenic index of plasma (AIP)]. C-IMT and AIP were analyzed by correlation and multiple linear regression, and carotid plaque occurrence was analyzed by binary logistic regression.Results:①Among the control group, the thickening group, and the plaque group, variance analysis showed that TG, TC, LDL-C, non-HDL-C, AIP, non-HDL-C/HDL-C, C-IMT increased in turn significantly, while HDL-C, exercise score, and healthy diet score decreased in turn significantly ( P<0.05 or P<0.01). ②Correlation analysis showed that there was a positive correlation of non-HDL-C/HDL-C with C-IMT and AIP, respectively ( P<0.01 or P<0.05). ③Regression analysis showed that non-HDL-C/HDL-C entered the regression model with C-IMT and AIP as dependent variables ( P<0.01), and did not enter the regression model with common carotid artery plaque as the dependent variable ( P>0.05). Conclusion:There is obvious abnormal lipid metabolism in officers and seamen of naval special duty with SA. Non-HDL-C/HDL-C is positively correlated with C-IMT and AIP, which is a risk factor of officers and seamen of naval special duty with SA and can be used as a good index for evaluating officers and seamen of naval special duty with SA. Among the officers and seamen of naval special duty, the control of lipid should be focused on.

3.Effect of general anesthetic mode on postoperative cognitive dysfunction in patients undergoing laparoscopic radical hysterectomy

Objective To investigate the influence of two general anesthestic modes on postoperative cognitive dysfunction (POCD) in the patients undergoing laparoscopic radical hysterectomy .Methods One hundred ASA Ⅰ‐Ⅱ patients undergoing lapa‐roscopic radical hysterectomy were randomly allocated to the propofol group (group P) and sevoflurane group (group S) ,50 cases in each group .The anaesthesia time ,total dose of sufentanil ,total dose of vecuronium ,recovery time ,recovery time for regaining ori‐entation and complications during anesthetic recovery period were recorded .The cognitive function was assessed by the mini‐mental state examination (MMSE) on preoperative 1 d (T0 ) ,postoperative 1 d (T1 ) ,postoperative 3 d ,(T2 ) ,postoperative 7 d (T3 ) ,post‐operative 1 month (T4 )、postoperative 3 months (T5 ) and the POCD occurrence situation was evaluated by adopting the Z scoring . Results The total dose of sufentanil and vecuronium in the group S was lower than that in the group P (P<0 .05) ,the recovery time and time for regaining orientation in the group S was longer than that in the group P (P<0 .05);the incidence rates of shive‐ring ,dysphoria and upper respiratory tract obstruction in the group S were higher than those in the group P (P<0 .05) .There were no statistically significant difference in the MMSE scores between the two groups (F=0 .14 ,P=0 .709);the MMSE scores in each group had statistical differences among different time points (F=74 .46 ,P<0 .01) .The interaction effect existed between the gen‐eral anesthetic mode and time with MMSE score (F=7 .99 ,P<0 .01);the MMSE scores at T1 ,T2 in the group S were lower than those in the group P (P<0 .05) .The incidence rate of POCD at T1 ,T2 、T3 ,T4 in the group S was higher than that in the group P (P<0 .05) .Conclusion The incidence rate of POCD in the patients undergoing laparoscopic radical hysterectomy by adopting sevoflurane inhalation general anaesthesia is higher than that by adopting propofol anesthesia ,but which has no difference after postoperative 3 months .
