【Objective】 To conduct the laboratory quality assessment between 12 blood stations in Hebei province, analyze the results and explore the accuracy and comparability of testing, so as to improve the level of testing ability and quality management. 【Methods】 With reference to the external quality assessment rules of National Center for Clinical Laboratories and combined with the instructions of quality assessment samples, daily testing process of the laboratories were assessed. The quality indicators include blood cell count (WBC, RBC, Hb, HCT, MCV, MCH, MCHC and PLT), biochemical items (TP) and coagulation parameters (FIB and FⅧ). 【Results】 There are still problems in laboratories in terms of personnel operation, instrument maintenance and the impact of different reagent batches, especially in biochemical items and coagulation parameters. The pass rate of biochemical items was the lowest, only 72.75%, and that of blood cell count was the highest, reaching 98.75%. 【Conclusion】 With the progress of the project, the quality monitoring level of daily blood sampling tests in the quality control laboratory of each blood station has been improved. However, it is still necessary for each laboratory to improve the testing ability and quality management to a higher level in Hebei.

Objective To explore the mechanism of hyperbaric oxygen (HBO) combined with bone marrow mesenchymal stem cells (BMSCs) transplantation to reduce the inflammatory response of spinal cord injury (SCI) in rats,and whether it is related to nuclear factor-kappa B (NF-κB) signaling pathway or not.Methods One hundred and sixty male adult SD rats were randomly divided into 4 groups according to the random number table:sham-operated group,SCI group,BMSCs transplantation group and BMSCs transplantation+HBO group (n=40).The rats from the latter three groups were established the SCI models by modified Allen dropping;and 1 mL BMSCs (3 × 106 cells) suspension was given by tail vein injection one h after SCI into the rats of the BMSCs transplantation group and BMSCs transplantation+HBO group;rats in the BMSCs transplantation+HBO group were given hyperbaric oxygen treatment.Rats were evaluated for locomotor function of hind limbs with Basso-Beattle-Bresnahan (BBB) scale one,3,7,14 d after SCI.Three d after SCI,the expression levels of NF-κB,IκB-α,tumor necrosis factor (TNF)-α and interleukin (IL)-1β in the spinal cord tissues were determined by Westem blotting;and enzyme-linked immuno sorbent assay (ELISA) was employed to detect the TNF-α and IL-1β levels.The number of NF-κB p65 positive cells in the spinal cord tissues was determined by immunohistochemistry.The histopathological changes of injured spinal cords were observed by HE staining.Results BBB scores of BMSCs transplantation group and BMSCs transplantation+HBO group were significantly higher than those of SCI group 3,7 and 14 d after SCI (P＜0.05),and those in the BMSCs transplantation+HBO group increased more significantly.The contents of NF-κB,TNF-α and IL-1β in the spinal cord tissues of BMSCs transplantation group were significantly lower than those in the SCI group 3 d after SCI,and the content of IκB-α protein was significantly increased (P＜0.05);the contents of NF-κB,TNF-α and IL-1β in the spinal cord tissues of BMSCs transplantation+HBO group were significantly lower than those in the BMSCs transplantation group,and the content of IκB-α protein was significantly increased (P＜0.05).The concentrations of TNF-α and IL-1β in the spinal cord tissues of BMSCs transplantation group were significantly lower than those in the SCI group (P＜0.05);the concentrations of TNF-α and IL-1β in the BMSCs transplantation+HBO group were significantly lower than those in BMSCs transplantation group (P＜0.05).The number of NF-κB p65 positive cells in the BMSC transplantation group was significantly smaller than that in SCI group (P＜ 0.05);the number of NF-κB p65 positive cells in BMSCs transplantation+HBO group was significantly larger than that in the BMSCs transplantation group (P＜0.05).As compared with those in the SCI group,the inflammatory response and edema were reduced,the area of spinal cord empty area decreased,and the damage area was improved in the BMSCs transplantation group and BMSCs transplantation+HBO group.Conclusion HBO combined with BMSCs transplantation has a synergistic effect after SCI,which might be partially due to down regulation of signal pathway of NF-κB and reduce of inflammatory reaction,and it might play a role in the treatment of spinal cord secondary injury.

Objective To develop an intelligent moxibustion instrument based on MCU,on the basis of moxibustion principles of traditional Chinese medicine.Methods A mixed-signal system-on-chip MCU C8051F020 was used to design four kinds of moxibustion heads to heat,light,and to upload detected temperature to the host microcontroller.Sixteen output ports were designed of which temperature and moxibustion time can be individually set.Results The temperature range of moxibustion head was 30-60℃,with error ≤±3 ℃.The timing range was 10-90 min,and full-scale timing error was ≤±1 min.The average infrared all normal emission rate of moxibustion head was 0.89.Conclusions The test parameters meet the requirements for moxibustion instrument,and the instrument is safe,reliable and simple to operate.

Objective To investigate the influence of early rehabilitation intervention in functional recovery of patients with spinal cord injury (SCI).Methods Eighty patients with SCI,admitted to our hospital from October 2009 to September 2013,were chosen in our study and divided into two groups:the treatment group,received rehabilitation treatment within one month of injury (n=40),and the control group,received rehabilitation treatment one month after injury (n=40).Both groups were given conventional treatment besides rehabilitation treatment.Scale of activities of daily living,Barthel index (BI) and functional independence measurement (FIM) were performed before treatment and three months after treatment.Results As compared with those before treatment (38.23±17.88 and 38.23± 17.88 in the treatment and control groups,23.38±15.50 and 25.50±17.28,55.98±16.76 and 62.75±14.44),the scores of activities of daily living (73.68±27.38 and 63.45±19.41),BI (53.13±28.46 and 43.63±20.25)and FIM (92.78±24.72 and 84.50±17.85) after treatment were significantly higher in both groups (P＜0.05);as compared with patients from the control group,patients from the treatment group had significantly higher scores of activities of daily living,BI and FIM after treatment (P＜0.05).Conclusion Early rehabilitation intervention can obviously promote the function recovery of SCI; rehabilitation should be involved within one month of injury.

Objective To study the value and significance of neuromuscular electrical diagnosis instrument to assess peripheral nerve injury. Methods using neuromuscular electrical instrument for determination of 135 cases of peripheral nerve injury, some patients simultaneously electromyography and nerve conduction velocity examination. Results 135 cases of peripheral nerve injury, 22 cases of normal part degeneration in 81 cases, completely denatured 32 cases, the positive rate was 83.70%(P<0.05). Among 30 cases of electromyography and nerve conduction velocity determination, 28 cases results. Conclusion It has a certain value and significance of neuromuscular electrical diagnosis instrument for peripheral nerve injury rehabilitation evaluation.

Objective To study the effects of transplantation of human adipose-derived mesenchymal stem cells (hADSCs) combined with exercise training on the functional recovery in rats after spinal cord injury(SCI) and the related mechanism.Methods One hundred male SD rats were randomly divided into combination therapy group,cell transplantation group,rehabilitation training group,injury control group and shamed-operated group (n=20).Except for the shamed-operated group,rats in the other groups were induced SCI models using a modified Allen impactor (H=25 mm) at T10.Right after SCI,hADSCs were transplanted into the rats of combination therapy group and cell transplantation group.The combination therapy group and exercise training group started treadmill training on the second day.Hind limb movement was assessed using Basse-Beattie-Bresnaban (BBB) scale before the operation and 1st,2nd,3rd,7th,14th,21th,28th days after SCI.On the 3nd 7th,14th,28th days after SCI,the relative concentration of brain-derived neurotrophic factor (BDNF) of the spinal cord was measured by enzyme-linked immunosorbent assay (ELISA).Results (1)The BBB scores of cell transplantation group and exercise training group were markedly improved since the 7th day of SCI than those in the control group (P＜0.05); the scores of the combination therapy group were significantly higher since the 3nd day of SCI than those of the cell transplantation group,rehabilitation training group and injury control group (P＜0.05).(2) As compared with that in the shamed-operated group,the BDNF expression in the injury control group decreased significantly since the 3th day of SCI (P＜0.05); the BDNF expression in the cell transplantation group and rehabilitation training group were significantly higher than that in the injury control group since the 7th day of SCI (P＜0.05),but that in the combination therapy group were significantly higher than that in the cell transplantation group and rehabilitation training group (P＜0.05).Conclusions The hADSCs transplantation combined with exercise training have a synergistic effect on functional recovery after SCI,which might be partially due to the up-regulation of BDNF expression.

Objective To observe the clinical effects of the adjustive tractor for cervical dislocation.Methods Forty-seven patients were included between September 2007 and November 2011.There were 36 males and 11 females with age ranged from 7 to 62 (mean,35 years).The mean interval from injury to admission was 8.6 h (range,0.5-72 h).There were atlanto-occipital dislocation in 2 cases,C1.2 in 2 cases,C2.3 dislocation in 5 cases,C3.4 dislocation in 2,C4.5 in 9,C5.6 in 13,C6.7 in 14.Thirty cases were complicated by fracture.No facet locking occurred in sixteen cases.Facet locking was found in ten cases and bilateral facet locking was in 21 cases.After reduction,brace or internal fixation followed.According to American Spinal Injury Association (ASIA) spinal function impairment scale standard,there were 4 cases in level A,10 cases in level B,18 in C,10 in D,and 5 in E.According to Japanese Orthopaedic Association (JOA) spinal function rating standard,the mean JOA score was 9 (range,2-14).Results All 47 cases were reduced successfully without neuronal function aggravation.Traction power ranged from 7 to 60 kg (mean,25.6 kg),the mean time of traction was 8 min (range,3-10 min).The mean follow-up was 38 (range,6-48) months.All the patients achieved normal alignment and intervertebral height.The intervertebral body fusion was observed in all of cases,the mean fusion time was 3.3 months (range,3-6 months).One patient who experienced nonunion of vertebral arch fracture didn't receive further treatment because of absence of symptoms.At last follow-up,there were 3 cases in level A,1 in level B,4 in C,8 in D,and 31 in E according to ASIA scale.The mean JOA score was 12 (range,2-17).Conclusion The adjustive tractor is simple and safe for prompt reduction.

Objective To investigate the effect of infrasound on the growth of bone marrow mescenchymal stem cells (BMSCs) and seek the most reasonable duration for using infrasound. Methods The primary BMSCs were obtained from Sprague-Dawley rats through whole bone marrow adherent cultivation.The cells of passage 3 were divided into trial groups treated with infrasound for 10 min,30 min or 60 min,and control groups which were not treated with infrasound but exposed to air for the same durations.The vitality of cell proliferation was measured using the CCK8 method.Apoptosis and the cell cycle were analysed with flow cytometry (FACS). Results After cultivation for 72 h,the optical density (OD) values for BMSCs treated with infrasound for 10 min,30 min and 60 min were 1.480 ± 0.030,1.348 ± 0.030,1.493 ± 0.030 respectively and after 96 h they were 1.774 ± 0.030,1.731 ± 0.030 and 1.833 ± 0.030.All of which were significantly greater than in the control groups ( 1.479 ± 0.030,1.267 ±0.030 and 1.227 ± 0.030 after 72 h and 1.567 ± 0.030,1.563 ± 0.030 and 1.632 ± 0.030 after 96 h).With the extension of the treatment duration,the OD values of the BMSCs increased or increased after decreasing,and the OD value for BMSCs treated with infrasound for 60 min was the highest.The FACS results indicated no effect of infrasound on apoptosis of BMSCs when the treatment duration was 30 min,but that cell apoptosis could be inhibited when the treatment duration was 10 min or 60 min.The early phase apoptosis rates were 1.07％ ± 0.12％ and 0.97％ ±0.21％ in the trial groups treated for 10 min and 60 min respectively,and 1.43％ ± 0.06％ and 3.33％ ± 0.15％ in the respective control groups,a highly significant difference.The results of cell cycle analysis showed infrasound could disturb the cell division of BMSCs significantly when the treatment duration was less than 30 min,but there was no significant effect when the treatment duration was 60 min. Conclusions Infrasound can promote cell proliferation and disturb cell division,but it did not cause apoptosis of BMSCs.When the treatment duration was 60 min,cell proliferation improved steadily,the apoptosis rate decreased,and the cell cycle did not change.So 60 min is the most reasonable treatment duration for infrasound.

Objective To study the effects of rehabilitation training combined with the transplantation of bone marrow mesenchymal stem cells-derived neural stem cells (BMSC-D-NSCs) on the expression of Nogo-A and NgR protein in rats after spinal cord injury (SCI).Methods The spinal cords of eighty Sprague-Dawley rats were injured using a modified Allen′s impactor (H = 25 mm) at T10. The injured rats were randomly divided into a combination therapy group which was given rehabilitation training and cell transplants, a cell graft group, a rehabilitation training group and a control group. At the 7th day post SCI, BMSC-D-NSCs were transplanted into the injured spinal cords of the rats in the combination therapy and cell graft groups. Hindlimb movement was assessed using the BassoBeattie-Bresnahan (BBB) scale every week, and protein was extracted from the injured spinal cord tissue for Nogo-A and NgR determination by Western blotting at the 1st, 3rd and 7th day after cell transplantation.Results The average BBB score of the rats in the combination therapy group was significantly higher than that of the other groups from 2 weeks post transplantation. The scores in the rehabilitation training group were significantly higher than in the control group from the 5th week post transplantation. Western blotting showed high expression of Nogo-A and NgR protein 24 h post surgery, but these declined with time. For Nogo-A there was a significant difference among the groups at all three time points. In the combination therapy group the expression declined to a minimum by the 7th day. For NgR protein there was no significant difference between the 1st and 3rd day in any group.Conclusions Rehabilitation training combined with BMSC-D-NSC transplantation can have a synergistic effect on functional recovery from SCI. It can down regulate the expression of Nogo-A and NgR protein.

OBJECTIVETo evaluate safety and immunogenicity of inactivated hepatitis A vaccine in HBsAg carriers and healthy children.

METHODSOne hundred and twenty-one healthy children and ten HBsAg carriers, aged 1-10 years HAV susceptible were enrolled in the study. The inactivated hepatitis A vaccine was produced by Tangshan Biogenetic Company. The dosage of the vaccine was 1000 U/Dosage and 500 U/Dosage. The vaccination schedule was six month apart for two injections. The serum anti-HAV level was detected with EIA at one month after first injection and at one and six month after the booster injection, respectively.

RESULTSThe anti-HAV appeared in all the children. One month after the booster injection, the serum anti-HAV level in children vaccinated 500 U/Dosage was 4684.9 mIU and 4535.6 mIU, respectively and in the children vaccinated 1000 U/Dosage, 5399.8 mIU and 7347.1 mIU, respectively. The anti-HAV level was not statistically different between the two groups of children. There was no adverse reaction after the vaccination. The anti-HAV level was still high one year after first injection.

CONCLUSIONSThe data indicated that the safety and immunogenicity of the domestic inactivated hepatitis A vaccine were excellent in both groups of children.
