1.Rapid Qualitative Analysis Methods and Their Application in Implementation Science

Implementation science (IS) aims to systematically analyze and address the real-world gaps from evidence to practice and the influencing factors of the context. It is necessary to carry out qualitative research to gather relevant implementation outcomes. Nevertheless, traditional qualitative analysis has issues such as consuming a great deal of time and energy, and it is unable to promptly provide the crucial data required for implementation science research. The Rapid Qualitative Analysis (RQA) method, through semi-structured interviews and the adoption of techniques such as immediate data condensation and matrix analysis, can effectively shorten the cycle of qualitative data collection and data processing. RQA can promptly identify social determinants of health such as structural barriers, facilitators, and the behavioral characteristics of target groups. It provides a real-time basis for public health decision-making, the interpretation of complex social phenomena, and the process and effectiveness evaluation of research projects. Although RQA is difficult to conduct in-depth theoretical analysis based on grounded theory, its efficiency and flexibility make it the preferred tool for large-scale and time-sensitive research. Thus, it has been widely applied in implementation science research. This paper sorts out the core concepts and commonly used technical methods of RQA, as well as the differences between RQA and traditional qualitative analysis. It also explores the applications of RQA in intervention optimization, process evaluation, and implementation outcome evaluation. By integrating specific cases, this paper clarifies its application value in the field of implementation science. In the future, it is advisable to explore the integration of RQA with technologies such as artificial intelligence and big data, in order to bridge the gap between the transformation of scientific research achievements into practice. Under circumstances of limited resources or tight time constraints, RQA can be used to efficiently conduct implementation science research, providing convenient and scientific methodological and technical support for accelerating evidence-based practice.
2.Challenges and Recommendations for Implementing Key Technologies in Decentralized Clinical Trials of Traditional Chinese Medicine

Traditional Chinese medicine (TCM) clinical trials face challenges such as low participant compliance, insufficient geographical coverage, and cost-effectiveness imbalances. Decentralized clinical trials (DCT), enabled by digital technology for remote data collection and monitoring, offer a new direction for TCM clinical trial research. This article systematically reviews three novel clinical trial design models. Combining the holistic concept and indivi-dualized treatment characteristics of TCM, it analyzes the challenges currently faced in TCM DCT practice, including the digitization and standardization of TCM theory, data security, privacy protection and patient engagement difficu-lties, insufficient ethical review and regulatory system adaptation, inadequate personnel training, and a shortage of interdisciplinary talent. Addressing these challenges, the article proposes methodological recommendations for DCT implementation that align with the principles of TCM diagnosis and treatment. These recommendations include promoting the intelligentization and standardization of TCM practices, constructing a full-chain data security and privacy protection system, improving the ethical framework and clarifying regulatory responsibilities, and cultivating and building interdisciplinary talent and capabilities, which provide theoretical and technical references for establishing standardized DCT practices in TCM.

4.Real-world study of glycerin enema for preoperative bowel preparation in benign anorectal diseases

Objective To investigate the effect of glycerin enema in preoperative intestinal preparation for anorectal benign diseases.Methods A total of 74 patients with benign anorectal diseases admitted to the anorectal department of our hospital from September 2023 to December 2023 were selected as the study ob-jects.According to different preoperative intestinal preparation methods,they were divided into glycerol group(n=30)and compound polyethylene glycol electrolyte powder(PEG-EP)group(n=44).The glycerol group was treated with glycerol enema to clean the intestine,and the PEG-EP group was treated with PEG-EP ene-ma before operation.Adverse reactions,defecation frequency,intestinal cleanliness,intestinal preparation tol-erance,first postoperative defecation time and bowel quality were observed and compared between the two groups.Results Propensity score was used for 1∶1 matching,and 25 patients were included in the two groups after PSM.The main adverse reactions were abdominal pain in the glycerol group and nausea,abdomi-nal distension and diarrhea in the PEG-EP group.There was no significant difference in the incidence of ad-verse reactions between the two groups(P>0.05).Compared with the PEG-EP group,the number of bowel movements after bowel preparation in the glycerol group was less,the intestinal preparation tolerance rate was higher,and the time of first postoperative bowel movement was earlier,with statistical significance(P<0.05).Conclusion Glycerin enema has more advantages than oral PEG-EP in preoperative intestinal prepara-tion for anorectal benign diseases.
5.NUMB endocytic adaptor protein (NUMB) mediates the anti-hepatic fibrosis effect of artesunate (ART) by inducing senescence in hepatic stellate cells (HSCs).

Developing and identifying effective medications and targets for treating hepatic fibrosis is an urgent priority. Our previous research demonstrated the efficacy of artesunate (ART) in alleviating liver fibrosis by eliminating activated hepatic stellate cells (HSCs). However, the underlying mechanism remains unclear despite these findings. Notably, endocytic adaptor protein (NUMB) has significant implications for treating hepatic diseases, but current research primarily focuses on liver regeneration and hepatocellular carcinoma. The precise function of NUMB in liver fibrosis, particularly its ability to regulate HSCs, requires further investigation. This study aims to elucidate the role of NUMB in the anti-hepatic fibrosis action of ART in HSCs. We observed that the expression level of NUMB significantly decreased in activated HSCs compared to quiescent HSCs, exhibiting a negative correlation with the progression of liver fibrosis. Additionally, ART induced senescence in activated HSCs through the NUMB/P53 tumor suppressor (P53) axis. We identified NUMB as a crucial regulator of senescence in activated HSCs and as a mediator of ART in determining cell fate. This research examines the specific target of ART in eliminating activated HSCs, providing both theoretical and experimental evidence for the treatment of liver fibrosis.

6.Explore the Mechanism of Gegen Qinlian Decoction in Ameliorating Nonalcoholic Fatty Liver Disease Through Effect HepG2 Cells Based on Transcriptomics

Objective To explore the potential mechanism of Gegen qinlian decoction(GGQLD)containing serum in ameliorating nonalcoholic fatty liver disease(NAFLD)in human hepatocellular carcinoma HepG2 cells based on transcriptomics.Methods An in vitro model of NAFLD was constructed by free fatty acid(FFA)-induced fat accumulation in HepG2 cells,and cells were treated with different proportions of GGQLD and pioglitazone-containing serum.The lipid deposition in each group was detected by oil red O staining,and the lipid content in each group was evaluated by triglyceride level.Transcriptome technology was used to detect the differentially expressed genes between the intervention groups,and GO annotation analysis,KEGG enrichment analysis and protein interaction(PPI)network analysis were performed to verify the differentially expressed genes by RT-PCR.Results Compared with normal control group,the number of red lipid droplets in the model control group increased,and the triglyceride content increased significantly(P<0.01).Compared with model control group,the content of red lipid droplets in the GGQLD medium dose group showed a decreasing trend,and the intracellular triglyceride content decreased significantly(P<0.05).A total of 608 differentially expressed genes were identified by transcriptome analysis,of which 163 differentially expressed genes were up-regulated and 445 differentially expressed genes were down-regulated.GO enrichment analysis showed that the differentially expressed genes were mainly involved in the regulation of MAP kinase phosphatase activity.KEGG analysis showed that the differentially expressed genes were mainly involved in MAPK signaling pathway.RT-PCR results showed that GGQLD up-regulated the expression level of MAP2K6 mRNA and down-regulated the expression levels of FOSL1,CTSL,DUSP5,DUSP1,JUN,HSPA6,IL1A,IL11 and RELB mRNA,which may be mainly involved in MAPK signaling pathway.Conclusion GGQLD has the effect of improving NAFLD,which may be related to MAPK signaling pathway.
7.Validation and application of Chinese Clinical Context Assessment for Community Health in evidence-based hypertension medication management

Objective To translate the Context Assessment for Community Health(COACH)scale into Chinese,thereby providing a standardized measurement tool for context assessment in clinical evidence-based practice.Methods The scale was translated following the Brislin translation model,ultimately forming the Chinese version of the COACH scale.Using convenience sampling,584 healthcare professionals were recruited from 3 tertiary hospitals in Jiangxi,Henan,and Liaoning provinces and 2 community hospitals in Liaoning province from May to September 2024 to evaluate the reliability and validity of the scale.In October 2024,the Chinese version of COACH was applied to 44 healthcare professionals in a hypertension specialist ward of a provincial tertiary hospital.Results Of the 584 distributed questionnaires,519 valid questionnaires were collected(the effective response rate of 88.87%).The Chinese version consists of 7 dimensions with 42 items.The overall Cronbach's α coefficient was 0.975;the McDonald's omega coefficient was 0.979;split-half reliability was 0.808,and the test-retest reliability was 0.917.The scale-content validity index/average was 0.950,and the scale-content validity index/universal agreement was 0.980,with item-content validity index ranging from 0.775 to 1.000.Exploratory factor analysis extracted 7 factors,accounting for 79.260%of the cumulative variance,with factor loadings ranging from 0.569 to 0.954.Confirmatory factor analysis showed good model fit.The preliminary application results showed that the total score of the scale was 185.18±17.82.The correlation coefficients between dimensions and total score ranged from 0.116 to 0.905.Conclusion The Chinese version of the COACH scale demonstrates good reliability and validity,making it suitable as a context measurement tool for implementation research at different stages in clinical and public health settings.

10.Effects of olfactory three needle on apolipoprotein E and pathological substrates in the hippocampus of mice with Parkinson's disease dementia

Objective:To explore the mechanism of olfactory three needle therapy on Parkinson's disease dementia(PDD)by observing its effects on expression of apolipoprotein E(ApoE)、glial fibrillary acidic protein(GFAP)and related core pathology substrates in the hippocampus of PDD model mice. Method:Male C57BL/6 mice were randomly divided into four groups:control group(Control),sham opera-tion group(Sham),model group(Model)and acupuncture electrotherapy group(AE),with 10 mice in each group.The PD model was established by injecting 6-OHDA into the medial forebrain tract(MFB)and PDD mice were selected.After successful modeling and selection,the AE group received"olfactory three needle"electro acupuncture treatment.After 14 days of intervention,Morris water maze test and shuttle box test were used to evaluate the learning and memory ability of mice in each group.Hematoxylin-eosin(HE)staining was used to observe the pathological changes of hippocampal CA1 region.Western blotting was used to detect the expression of α-syn,Aβ and ApoE proteins in hippocampal CA1 region.The co-location rate of ApoE and GFAP in hippocampal CA1 region was observed by double immunofluorescence markers. Result:Compared with Model group,the AE group exhibited a shortened escape latency in water maze(P<0.01),increased platform crossing(P<0.05),increased active escape times of shuttle box(P<0.05),and reduced the average total time of electric stimulation(P<0.01).In the Model group,the neurons in the hippocampal CA1 area were sparsely arranged and showed signs of degeneration and necrosis;and cell nuclei were small,hyperchromatic and had unclear structures,indicating the appearance of nuclear pyrosis.In contrast,the AE group showed significant improvements in neuronal pathology,with most cells regularly arranged,round and large nuclei,lightly stained and clearly shaped.Compared with the Model group,the expression levels of α-syn,Aβ,ApoE protein and the co-localization rate of ApoE and GFAP in hippocampal CA1 region in AE group were decreased(P<0.01,P<0.01,P<0.01,P<0.05). Conclusion:The"Olfactory three needle"acupuncture can improve the cognitive ability and restore the morpho-logical structure and function of neurons in PDD mice.The mechanism may be related to the inhibition of ApoE expression in astrocytes and the reduction of α-syn and Aβ deposition in hippocampal CA1 region.
