1.Treatment of pathological myopic foveoschisis by pars plana vitrectomy with fovea-sparing internal limiting membrane peeling and silicone oil tamponade

AIM: To investigate the clinical efficacy of pars plana vitrectomy(PPV)combined with fovea-sparing internal limiting membrane(ILM)peeling and silicone oil(SO)tamponade for treating pathological myopic foveoschisis(PMF).METHODS:This study is a retrospective observational analysis of 10 cases(10 eyes)diagnosed with PMF that underwent PPV with fovea-sparing ILM peeling and SO tamponade between January 2023 and November 2024. The best-corrected visual acuity(BCVA), central foveal thickness(CFT), foveoschisis(FS), and the detachment and reattachment of FS and macular fovea were assessed preoperatively and at 1 wk, 1 and 3 mo postoperatively.RESULTS:Among the 10 cases of PMF patients(10 eyes), the complete reattachment rate was 30%(3 eyes), while partial reattachment was observed in 70%(7 eyes). At 3 mo postoperatively, BCVA(LogMAR)was significantly improved to 0.957±0.393 compared with 1.432±0.509 before surgery(P<0.05), and both CFT(437.9±180.4 vs. 207.5±76.1 μm)and FS(686.5±172.2 vs. 290.7±86.6 μm)showed significant decreases(P<0.05). No complications such as macular hole, retinal detachment, silicone oil emulsification, or endophthalmitis were observed during the surgery or throughout the follow-up period.CONCLUSION:PPV with SO tamponade and fovea-sparing ILM peeling has been demonstrated to facilitate both visual acuity improvement and anatomical reattachment in cases of PMF.
2.Value of transcranial Doppler in the assessment of stroke in patients undergoing extracorporeal membrane oxygenation

Stroke is one of the most common and serious neurological complications associated with extracorporeal membrane oxygenation (ECMO) therapy, and close monitoring, early recognition, and timely intervention can reduce the incidence rate of stroke and the mortality rate of patients. Transcranial Doppler (TCD) and transcranial color Doppler ultrasonography (TCCD) have emerged as the preferred modalities for monitoring cerebral blood flow in ECMO patients due to their bedside applicability, ability to provide real-time dynamic assessments, and noninvasive safety. This article summarizes the spectral characteristics of cerebral blood flow in patients undergoing various ECMO modalities and elaborates on the latest research advances and clinical significance of TCD/TCCD in predicting stroke events, monitoring cerebral microembolic signals, reflecting the change in intracranial pressure, confirming brain death, and providing prognostic evaluation.

3.Clinical and hemodynamic features of patients with left innominate vein compression

Objective To investigate the hemodynamic features of left innominate vein (LIV) compression and their association with clinical manifestations, and to assess the diagnostic value of ultrasound. Methods A total of 32 patients with LIV compression and 67 healthy controls (HCs) were enrolled, and according to the symmetry of the diameters of bilateral transverse sinuses, sigmoid sinuses, and internal jugular veins (IJV), the HCs were divided into symmetric group with 32 individuals and asymmetric group with 35 individuals. All subjects underwent CTV/MRV and ultrasound examination to measure the diameter, velocity, and flow volume of IJV and vertebral vein, and the differences between groups were analyzed. The receiver operating characteristic (ROC) curve was plotted to assess diagnostic efficacy. Results The patients with LIV compression mainly had the neurological symptoms such as cerebral tinnitus, dizziness, and tinnitus. Compared with the HCs, the patients with LIV compression had a significantly lower total flow volume of IJV and vertebral vein on the ipsilateral side [(406.42±177.05)ml/min vs (742.55±276.41)ml/min, P0.001)], and the parameters of the ipsilateral side were lower than those of the contralateral side. There was no significant difference in the flow volume of IJV between the contralateral side of patients with LIV compression and the dominant side of the HCs in the asymmetric group. Total flow volume had an area under the ROC curve of 0.849 in predicting LIV compression, with a sensitivity of 77.60%, a specificity of 78.10%, and a cut-off value of 525.73 ml/min. Conclusion This study systematically reveals the blood flow characteristics of the bilateral IJV-vertebral vein system in patients with LIV compression,i.e.,LIV compression leads to a reduction in flow on the ipsilateral side and a compensatory increase on the contralateral side,and the asymmetry of bilateral blood flow may be the cause of the symptoms such as cerebral tinnitus and tinnitus.Ultrasound can be used to measure the total flow volume of IJV and vertebral vein, and it can effectively identify this disease and thus has a good value in screening.

4.Study on the similarities and differences of orthostatic hypotension in patients with Parkinson′s disease and multiple system atrophy

Objective:To summarize and compare the characteristics of orthostatic hypotension (OH) in patients with Parkinson′s disease and multiple system atrophy (MSA).Methods:The active standing test data of 210 Parkinson′s disease patients (Parkinson′s disease group) and 85 MSA patients (MSA group) admitted to the Department of Neurology, Xuanwu Hospital, Capital Medical University from January 2021 to March 2022 were retrospectively analyzed. Demographic information, clinical data, Hoehn-Yahr staging, and Unified Parkinson′s Disease Rating Scale (UPDRS), Non-Motor Symptoms Questionnaire (NMSQ), Montreal Cognitive Assessment Scale and Mini-Mental State Examination scores were collected. The comparative analysis of OH was conducted according to the changes of heart rate and blood pressure during the active standing test.Results:Among the 85 patients with MSA, 52 were found with MSA parkinsonism variant (MSA-P) and 33 with MSA cerebellar variant (MSA-C). The 210 Parkinson′s disease patients were aged (61.5±11.0) years, with 116 males (55.2%). The 85 MSA patients were aged (60.1±6.8) years, with 44 males (51.8%). Compared with the Parkinson′s disease group, the Hoehn-Yahr staging [2.0(2.0, 3.0) vs 3.0(2.0, 3.0), Z=-5.278, P<0.001], NMSQ[ 25.0(11.0,46.5) vs 45.0(24.0,70.0), Z=-3.632, P<0.001] and UPDRS scores [50.0(32.0,68.0) vs 65.5(44.5,78.5), Z=-3.073, P=0.003] in the MSA group were higher. The incidence of OH in the MSA group was higher than that in the Parkinson′s disease group [63.5% (54/85) vs 25.7%(54/210), χ 2= 37.284, P<0.001], but there was no statistically significant difference between the MSA-P and MSA-C groups . Compared with the Parkinson′s disease group, the MSA group had a higher incidence of classical OH [54.1%(46/85) vs 12.9%(27/210), χ 2=55.316, P<0.001] and neurogenic OH [36.5%(31/85) vs 9.0%(19/210), χ 2=32.326, P<0.001],but there was no statistically significant difference in the incidence of initial OH and delayed OH between the two groups. The incidence of severe OH in the MSA group was also higher than that in the Parkinson′s disease group [57.6%(49/85) vs 16.7%(35/210), χ 2=49.894, P<0.001], but there was no statistically significant difference in the incidence of pre-clinical OH and mild OH between the two groups. Conclusions:The incidence, time change, severity and pathophysiological basis of OH in Parkinson′s disease and MSA patients are different. Different types of OH may help to distinguish MSA from Parkinson′s disease.
5.Research progress on perioperative monitoring of cerebral autoregulation and neurovascular coupling for carotid artery stenting

Cerebral autoregulation (CA) is the ability of blood vessels to keep cerebral blood flow (CBF) relatively constant across a wide range of blood pressure or cerebral perfusion pressure. Ischemic cerebrovascular disease has been associated with CA impairment, but the mechanism is unclear. While their association is a focus, research on perioperative CA and neurovascular coupling (NVC) monitoring for carotid artery stenting has also made progress. This paper focuses on research progress on perioperative CA and NVC monitoring in patients undergoing carotid artery stenting, and discusses the value of CA and NVC for monitoring autoregulation of cerebral blood flow in the perioperative period of carotid artery stenting.
6.Research progress of yeast microcapsules as oral drug delivery carrier

As a natural drug delivery carrier with rough and porous surface and hollow core, yeast microcapsules have good safety, high targeting and high stability, and have excellent application prospects in oral drug delivery systems. Yeast cells can be treated and washed with acid-base and organic solvents to obtain loose and porous yeast microcapsules. Yeast microcapsules can encapsulate drugs through electrostatic interactions, passive diffusion, hydrophobic interaction and other methods. The surface of yeast microcapsules is mainly composed of β-glucan, which can maintain stability in the gastrointestinal environment; it can be recognized by the surface-related receptors of immune cells, thus activating the immune response, and can be transported to the lesion site with the movement of lymphocytes after being ingested. Yeast microcapsules are safe and very suitable for delivering vaccines, anti-inflammatory drugs, and anti-tumor drugs. They can not only achieve oral delivery of the aforementioned drugs, but also enhance drug efficacy and improve drug targeting. In the future, more research on systemic transport mechanisms or the development of more efficient combination drug delivery systems can be carried out to fully exhibit the clinical value of yeast microcapsules.
7.Correlation between hemodynamic characteristics or dynamic cerebral autoregulation and postoperative hyperperfusion in patients undergoing carotid endarterectomy

Objective:To investigate the hemodynamic characteristics and dynamic cerebral autoregulation(dCA) of patients with severe carotid stenosis before carotid endarterectomy(CEA), and to analyze their correlations with hyperperfusion after CEA.Methods:A total of 63 patients with unilateral severe carotid artery atherosclerotic stenosis who underwent CEA were consecutively recruited prospectively in Xuanwu Hospital, Capital Medical University from January 2021 to August 2021. According to postoperative hyperperfusion, patients were divided into hyperperfusion group (13 cases) and non-hyperperfusion group (50 cases). The general clinical data and hemodynamic parameters were compared between the two groups. The dCA was evaluated by the transfer function analysis that measured the fluctuation amplitude of mean cerebral blood flow velocity(CBFV) with blood pressure change, the time difference of phase with blood pressure change, and the correlation between mean CBFV and blood pressure change. The value of dCA for predicting postoperative hyperperfusion was analyzed through the ROC curve and the area under the curve(AUC).Results:①The proportion of hypertension in hyperperfusion group was higher than that in non-hyperperfusion group ( P<0.05). ②The peak systolic velocity (PSV) at the stenosis lesion of the internal carotid artery, the end-diastolic velocity (EDV) at the stenosis lesion of the internal carotid artery and the ratio of PSV at the stenosis lesion of the internal carotid artery to the distal internal carotid artery in the hyperperfusion group were higher than the non-hyperperfusion group.And the PSV of the ipsilateral middle cerebral artery in the hyperperfusion group was lower than the non-hyperperfusion group (all P<0.05). ③In the very low-frequency and low-frequency region, the phase in the hyperperfusion group was lower than that in the non-hyperperfusion group (all P<0.05), while there was no significant difference in the high-frequency regions( P>0.05). In the three regions of very low-frequency, low-frequency and high-frequency, there were no significant differences in the gain and coherence between the two groups(all P>0.05). ④The best cutoff value of phase in the very low-frequency was 33.28 for predicting hyperperfusion after CEA (AUC=0.766, 95% CI=0.629-0.904, P=0.03), with the specificity of 0.700, and sensitivity of 0.846. Conclusions:There are differences in hemodynamics and dCA between the hyperperfusion group and the non-hyperperfusion group after CEA. The impaired preoperative dynamic cerebral autoregulation is an independent predictor of postoperative hyperperfusion.
8.Hemodynamic evaluation of the efficacy for superficial temporal artery-middle cerebral artery bypass with vascular ultrasonography

Objective:To evaluate the hemodynamic changes and short-term (one year) outcomes after superficial temporal artery(STA)-middle cerebral artery (MCA) bypass by vascular ultrasonography.Methods:Operation group included a total of 41 hemispheres of 38 patients who underwent STA-MCA bypass for severe stenosis or occlusion of MCA or severe stenosis or occlusion of internal carotid artery(ICA), or Moyamoya disease at Xuanwu Hospital between August 2017 and June 2020. The following examinations were performed in all patients: cranial MRI, CT perfusion imaging, and cerebral angiography. All of the patients were retrospectively followed up for one year postsurgery. Normal group included a total of 40 hemispheres of 20 healthy people who had been examined by vascular ultrasonography of carotid artery and intracranial artery. Inner diameter, peak systolic velocity (PSV), end-diastolic velocity (EDV), mean velocity (MV), resistance index (RI), pulsatility index (PI) and flow of STA, PSV, EDV, MV, PI, RI of external carotid artery (ECA) and PSV, EDV, MV, PI, RI of MCA at 1 week, 6 months and 12 months after STA-MCA bypass of operation group were compared with normal group.Results:①Inner diameter, PSV, MV and flow of STA were increased significantly in operation group at 1 week, 6 and 12 months than normal group (all P<0.05). The flow of STA was decreased significantly from 1 week to 12 months after operation (all P<0.05), but Inner diameter, PSV and MV were only decreased gradually from 1 week to 12 months after operation (all P>0.05). ②STA/ECA PI and RI decreased significantly after operation compared with normal group (all P<0.01). Conclusions:Vascular ultrasonography is a non-invasive examination which can objectively evaluate the extracranial and intracranial hemodynamic changes after STA-MCA bypass, and provide reference effectiveness of the operation. The flow of STA has decreased gradually from 1 week to 12 months after operation to achieve the homeostasis, but it is still higher than normal.STA/ECA PI and RI can be a steady indirect pointer to show the fluency of bypass.
9.Clinical characteristics of carotid web-related ischemic stroke

Objective:To analyze the clinical characteristics and management strategy of carotid web (CW)-related stroke and improve the understanding of CW.Methods:The clinical data of 6 patients with CW-related stroke treated at Xuanwu Hospital, Capital Medical University, from January 2018 to September 2022 were retrospectively analyzed. Relevant data included clinical manifestations, bronchial magnetic resonance imaging (MRI), CT angiography (CTA)/digital subtraction angiography (DSA), carotid ultrasonography, and transcranial color-coded Doppler sonography (TCCD). Treatments and follow-ups were also reported.Results:All 6 patients were middle-aged or young, without traditional risk factors for cerebrovascular disease. The male-to-female ratio was 2∶4. All patients had recurrent strokes, with a score of 2-4 on the National Institutes of Health Stroke Scale at the time of onset. Head MRI showed 5 patients with cerebral infarction in the middle cerebral artery supply area; 1 patient had no intracranial infarction. No significant stenosis of the vessels at the site of the CW was observed in any of the 6 patients. Four patients had ipsilateral stenosis or occlusion of the middle cerebral artery, 1 patient had microembolic signals in the middle cerebral artery, and 1 patient had no significant abnormalities in the intracranial artery. All 6 patients were initially diagnosed by ultrasound. Diagnoses were confirmed by CTA/DSA in 5 cases; the other patient did not show any significant abnormalities. All 6 patients underwent a carotid endarterectomy after a pathological examination to confirm the diagnosis. Postoperatively, regular antiplatelet aggregation and statin lipid-lowering therapies were administered. A follow-up of 0.5 to 5.2 years showed no stroke recurrence in any patient.Conclusions:CW-related stroke is a rare cause of cryptogenic stroke and has a high recurrence rate. For stroke patients who do not have traditional risk factors for cerebrovascular disease while repeatedly experiencing embolic events of the internal carotid artery system, attention should be paid to the combination of the carotid and cerebral arteries, and comprehensive hemodynamic characteristics should be checked for the presence of a CW. Carotid artery ultrasound and TCCD combined with other imaging methods can improve the detection rate of a CW. Simple antiplatelet aggregation therapy cannot effectively prevent stroke recurrence. For patients with confirmed CW-related cerebral infarction, surgical resection can be performed, and the treatment effect can be evaluated using ultrasonography.
10.Estimating risk of atherosclerotic stroke with nomogram based on multi parameter characteristics

Objective To construct an nomogram model for predicting the risk of ischemic stroke in patients with carotid stenosis,and verify its predictive efficacy. Methods Retrospective collection of 720 patients hospitalized in the First Hospital of Jilin University and Xuanwu Hospital from August 2018 to January 2021.The patients were divided into symptomatic group and asymptomatic group according to their clinical manifestations and imaging examination.The clinical data and carotid ultrasound characteristics were collected,and the risk factors of ischemic stroke in patients with carotid artery stenosis were analyzed by LASSO regression analysis and multivariate Logistic regression analysis.The nomogram model for predicting the risk of ischemic stroke in patients with carotid stenosis was constructed using R4.2.2 "rms" package.Bootstrap method was used to repeatedly sample 1 000 times for internal verification.Consistency index,H-L goodness of fit test and calibration curve were used to evaluate the prediction efficiency of the nomogram model. Results Of 720 patients,295 were classified as symptomatic group,and 425 were classified as asymptomatic group.Multivariate logistic regression analysis showed that hypertension,diabetes,smoking,the degree of carotid stenosis,plaque echogenicity,morphology and thickness were independent risk factors for ischemic stroke in patients with carotid artery stenosis (P<0.05).The nomogram model was constructed based on the above seven risk factors.The ROC curve analysis results showed that the AUC of the nomogram model predicting the risk of ischemic stroke in patients with carotid stenosis was 0.801 (95%CI 0.769-0.833).Bootstrap method was used for internal verification,and the results showed that the consistency index was 0.793.The calibration curve analysis showed that the calibration curve of the nomogram model is close to the ideal curve. Conclusion The nomogram model based on vascular risk factors and carotid ultrasound characteristics in this study has a high degree of differentiation and calibration in predicting the risk of ischemic stroke in patients with carotid stenosis,which is helpful for clinicians to stratify the risk of patients.
