1.Characterization of vaginal flora in pregnant women during the second trimester using 16S rRNA full-length gene sequencing

Objective:To characterize the vaginal flora of pregnant women during the second trimester using full-length 16S rRNA sequencing.Methods:A total of 142 pregnant women were systematically sampled from a pregnancy cohort. Vaginal swabs were collected for full-length 16S rRNA gene sequencing,and bioinformatics analysis was performed to characterize the vaginal microbiota and identify associated influencing factors.Results:Among the 142 pregnant women,the most frequently detected species were Lactobacillus iners(83.10%,118/142)and Lactobacillus crispatus(49.30%,70/142). The majority of samples(90.85%,129/142)were classified as Lactobacillus-dominant vagitypes,with the Lactobacillus iners vagitype accounting for 48.59%(69/142)and the Lactobacillus crispatus vagitype accounting for 38.73%(55/142). The vaginal microbiota was clustered into five community state types(CSTs):Ⅰa,Ⅰb,Ⅲa,Ⅲb,and Ⅳ. The most prevalent CSTs were Lactobacillus iners-dominated CST-Ⅲ(51.41%,73/142)and Lactobacillus crispatus-dominated CST-Ⅰ(24.65%,35/142). No samples were classified as CST-Ⅱ or CST-Ⅴ. A significant negative correlation was observed between Lactobacill and vaginosis-associated bacteria. Age,alcohol consumption,smoking,and vaginal treatments showed significant associations or trends toward significance with various Alpha diversity indices. Vaginal douching was associated with CST clustering,while obstetric history(primiparity,previous miscarriage history)was associated with vagitype classification. However,no significant associations were identified between maternal baseline characteristics and Beta diversity indices. Conclusions:Full-length 16S rRNA gene sequencing reveals that the vaginal microbiota of pregnant women is dominated by Lactobacillus iners and Lactobacillus crispatus. Maternal age,lifestyle factors such as smoking and alcohol consumption,and obstetric history are significantly associated with variations in vaginal microbiota composition.

The reform of payment based on disease diagnosis related grouping (DRG) is an important measure to deepen the medical security system. Its implementation will have a profound impact on the operation and management of public TCM hospitals from four aspects: medical service efficiency, cost, quality, and medical resource allocation. At the same time, there are also difficulties in reform, such as the contradiction between personalized diagnosis and treatment in TCM and standardized payment in DRG 2.0, compatibility issues between TCM medical record data and DRG 2.0 grouping system, limitations of special case negotiation mechanism, and difficulty in cost control and accounting. In response, this study formulated targeted strategies and recommendations intended to facilitate the effective implementation of the DRG 2.0 payment system, and bolster the operational and administrative efficacy of public TCM hospitals, culminating in a favorable scenario of tripartite synergy among "healthcare providers, insurers, and patients".
3.Application of FOCUS-PDCA in reducing the homepage defect rate of inpatient medical records

Objective FOCUS-PDCA was applied to reduce the defect rate on the front page of medical records,and provide high quality data of medical records for medical institutions and administrative departments at all levels.Methods Start-ing from the increase of problems on the home page of a hospital in the"post-epidemic period",FOCUS-PDCA management mode was used,according to nine implementation steps,quality management tools such as Gantt chart,fishbone map and Plato were used to focus on,analyze and solve problems,and constantly repeated problems.The number of defects and the rate of defects on the front page of hospitalization records from January to March 2023 were taken as the control group to compare the changes before and after the implementation of the project.Results From January to March 2023,the number of defects on the homepage was 1 734,and the defect rate was 15.14%.After the implementation of the project,the number of defects on the homepage of Octo-ber to December 2023 was 890,and the defect rate was 6.54%,and the difference was statistically significant(P<0.05).Conclusion The method of applying FOCUS-PDCA to reduce the defect rate on the front page of inpatient medical records is ef-fective,which plays an important role in promoting the improvement of the quality of data such as basic patient information,hos-pitalization process information,and diagnosis and treatment information.

Antibody-drug conjugates(ADCs)are a new type of targeting antibodies that conjugate with highly toxic anticancer drugs via chemical linkers to exert high specificity and efficient killing of tumor cells,thereby attracting considerable attention in precise oncology therapy.Cetuximab(Cet)is a typical antibody that offers the benefits of good targeting and safety for individuals with advanced and inoperable cutaneous squamous cell carcinoma(cSCC);however,its anti-tumor activity is limited to a single use.Cisplatin(CisPt)shows good curative effects;however,its adverse effects and non-tumor-targeting ability are major drawbacks.In this study,we designed and developed a new ADC based on a new cytotoxic platinum(Ⅳ)prodrug(C8Pt(Ⅳ))and Cet.The so-called antibody-platinum(Ⅳ)prodrugs conjugates,named Cet-C8Pt(Ⅳ),showed excellent tumor targeting in cSCC.Specifically,it accurately delivered C8Pt(Ⅳ)into tumor cells to exert the combined anti-tumor effect of Cet and CisPt.Herein,metabolomic analysis showed that Cet-C8Pt(Ⅳ)promoted cellular apoptosis and increased DNA damage in cSCC cells by affecting the vitamin B6 metabolic pathway in tumor cells,thereby further enhancing the tumor-killing ability and providing a new strategy for clinical cancer treatment using antibody-platinum(Ⅳ)prodrugs conjugates.
8.Finite element analysis of femoral neck fracture treated by internal fixation of femoral neck system under nonanatomical reduction

BACKGROUND:Irreducible femoral neck fracture was difficult to obtain anatomic reduction.As a new type of internal fixation,the femoral neck system is still blank for the treatment of non-anatomical reduced femoral neck fractures. OBJECTIVE:To explore the biomechanical stability of femoral neck system internal fixation under nonanatomical reduction in the treatment of femoral neck fractures based on finite element analysis. METHODS:CT data of the hip joint of a healthy female adult were obtained.Anatomical reduction of femoral neck fracture models with Pauwels angles of 30°,50°,and 70° were established using Mimics 21.0,Geomagic Wrap 2021,and SolidWorks 2020.The fracture proximal ends of the three anatomical reduction models were shifted upward by 2 mm along the fracture line,and three positive buttress models with different Pauwels angles were obtained.In the same way,three negative buttress models were acquired by shifting downward by 2 mm.SolidWorks 2020 was used to make the femoral neck system internal fixation,and the nine femoral neck fracture models were assembled with the femoral neck system.Then Ansys 19.0 was used for finite element analysis.The displacement distribution and maximum displacement,stress distribution and maximum stress of the femur and femoral neck system were recorded under 2100 N stress. RESULTS AND CONCLUSION:(1)When Pauwels angles were 30°,50°,and 70°,the maximum stresses of the femoral neck system appeared to be concentrated at the junction of the sliding hip screw and anti-rotation screw.The maximum femur stresses appeared to be concentrated in the medial cortex of the femur.The maximum displacement was concentrated at the upper of the femoral head and femoral neck system.(2)When Pauwels angles were 30° and 50°,the maximum displacement and maximum stress of the femoral neck system and femur were:negative buttress>anatomical reduction>positive buttress.(3)When Pauwels angle was 70°,the maximum displacement and maximum stress of the femoral neck system were:negative buttress>anatomical reduction>positive buttress;the maximum displacement and maximum stress of the femur were:negative buttress>positive buttress>anatomical reduction.(4)With the increase of Pauwels angle,the biomechanical advantage of the positive buttress was weakening.However,it was better than a negative buttress.When Pauwels angle was 30°,positive buttress was more stable than anatomical reduction.When Pauwels angle was 50°,the biomechanical difference between positive buttress and anatomical reduction became smaller.When Pauwels angle was 70°,the stability of anatomical reduction was slightly better than positive buttress.(5)If it was difficult to achieve anatomical reduction of femoral neck fracture during operation,but the positive buttress had been displaced within 2 mm,the femoral neck system could be used to offer stable mechanical fixation.It is necessary to avoid negative buttress reduction.
9.Analysis of tissue distribution of metabolites in Crocus sativus L. corms based on DESI mass spectrometry imaging technique

Objective To explore the distribution characteristics of endogenous metabolites in Crocus sativus L. corms from different origins. Methods A method based on desorption electrospray ionization mass spectrometry imaging and optimized sample pretreatment was developed for directly visualize metabolites in C. sativus corms. Results In situ characterization of metabolites such as flavonoids, organic acids, amino acids, carotenoids, and cyclic enol ether terpene glycosides was achieved. L-Citruline, phenylacetylglycine, sativol, and geniposide were specifically distributed in the corms. Apigenin 7-(6''-O-acetyl)-glucoside, isorhamnetin-3-O-β-D-Glucoside, dhurrin 6'-glucoside, and Apigenin 7-O-diglucuronide were mainly distributed in the terminal bud. For compounds distributed in the corms, the highest abundance was found in corms from Shanghai, followed by Zhejiang and the lowest from Anhui. Conclusion The distribution of metabolites in different parts of C. sativus corms from different origins and the same origin varies significantly. Flavonoids and flavonoid derivatives such as isorhamnetin-3-O-β-D-Glucoside and apigenin derivatives are mainly distributed in the terminal buds, in addition, the natural plant protection agent dhurrin 6'-glucoside is also mainly distributed in the terminal corms, whereas amino acids, which are used as energy and material supplies, are mainly accumulated in the corms.
10.Prognosis of epilepsy after operation for low-grade insular glioma

Objective To explore the influencing factors and prognosis of epilepsy after operation for insular low-grade gliomas.Methods The clinical data of 120 patients with insular low-grade gliomas with pre-operative seizure symptoms in the Chinese glioma Genome Atlas Project(CGGA)database from July 2008 to June 2014 were retrospectively analyzed.The seizure control and survival prediction information of the patients were followed up one year after operation.The risk factors affecting post-operative seizures were analyzed by Logistic multiple factor regression analysis,Kapla-Meier curve was used to analyze the effect of postoperative epilepsy control factors on progression free survival(PFS)and overall survival(OS).Results Before operation,36 cases(30.0%)had simple focal seizures,53 cases(44.2%)had focal seizures with different degrees of cognitive impairment,and 31 cases(25.8%)had generalized seizures.During the follow-up after one year of surgical treatment,72 patients(60%)had a complete improvement in epilepsy symptoms(Engel grade Ⅰ),33 patients(27.5%)had a significant improvement(Engel grade Ⅱ),11 patients(9.2%)had a significant improvement(Engel grade Ⅲ),and 4 patients(3.3%)had no improvement or deterioration(Engel grade Ⅳ).Larger tumor resection degree,no putamen involved,IDH1 mutation are favorable factors for postoperative control of epilepsy.Improvement of seizures was a favorable prognostic factor for PFS and OS.Conclusion Epilepsy is the most common symptom of insular gliomas.The improvement of postoperative epilepsy symptoms is related to the degree of tumor resection,involvement of putamen and IDH1 mutation status.Complete improvement of epilepsy symptoms can prolong the progression free survival time and overall survival time of patients.
