1.Osler’s view of the physician and physician’s narrative literacy in narrative medicine

In the era of evidence-based medicine, the progress of medical science and technology has enriched medical diagnostic tools and treatment methods, but it has also led to the loss of medical warmth and the alienation of the doctor-patient relationships. William Osler emphasized that while medical technology advances, attention should also be paid to the practice of narrative medicine and the development of physician’s narrative literacy. The view of the physician he advocated reminds us that the core of medicine still lies in the narrative connection between doctors and patients, as well as a deep understanding of human nature. By exploring the relationship between Osler’s view of the physician and narrative medicine as well as physician’s narrative literacy, this paper analyzed the methods of cultivating physician’s narrative literacy, providing references for modern medical education and practice, and assisting in the harmony and unity of science and technology and humanity.
2.Research progress on the association between earlier pubertal timing of children and hypertension with its mechanisms

Abstract
In China, the onset of puberty in children has been occurring increasingly earlier, coinciding with a rising prevalence of hypertension among adolescents. The association between early pubertal timing and hypertension has become a focal point of research. The study systematically examines the relationship between earlier pubertal timing of children and hypertension during both childhood and adulthood. It explores the underlying mechanisms from multiple perspectives, including sex hormone regulation, metabolic disorders, and psychological and behavioral factors. Furthermore, based on the principles of three level prevention, relevant intervention measures have been proposed to provide new evidence for advancing the prevention and control of hypertension starting in childhood.
3.Values of non-invasive myocardial work and tissue motion annular displacement in evaluating anthracycline-induced cardiotoxicity

Objective To explore the clinical values of non-invasive myocardial work (MW) and tissue motion annular displacement (TMAD) in evaluation of anthracycline therapy-related cardiac dysfunction in patients with non-Hodgkin lymphoma. Methods A total of 62 patients with non-Hodgkin lymphoma who received standardized chemotherapy based on doxorubicin. Two and three dimensional transthoracic echocardiography, along with two dimensional speckle tracking echocardiography, were performed one day before chemotherapy and at 3, 6, and 9 months after chemotherapy to assess left ventricular ejection fraction, global longitudinal strain (GLS), MW parameters, and TMAD. Logistic regression analysis was used to evaluate the risk factors for cancer therapy-related cardiac dysfunction (CTRCD). The receiver operating characteristic curve was used to assess the diagnostic values of MW- and TMAD-related parameters for CTRCD. Results Compared to baseline, GLS, global work index (GWI), global constructive work (GCW), global work efficiency (GWE), TMAD at midpoint (TMADmid), and TMADmid percentage of left ventricular long-axis diameter (TMADmid%) decreased at 3 months after chemotherapy, while global wasted work (GWW) increased at 6 months after chemotherapy (P<0.05). Logistic regression analysis showed that the relative reduction in GLS and TMADmid% at 3 months after chemotherapy were independent predictors for CTRCD (P<0.05), while MW parameters were not independent predictors for CTRCD. GLS reduction≥10.3% and TMADmid% reduction≥15.8% at 3 months after chemotherapy predicted CTRCD with 0.866 and 0.824 of area under the curve (AUC), 92% and 75% of sensitivity, and 74% and 80% of specificity, respectively. AUC of combination of two indexes improved to 0.905, with 75% of sensitivity and 90% of specificity. Conclusions In non-Hodgkin lymphoma patients, the combination of GLS and TMADmid% is helpful of predicting CTRCD early, TMAD may be a novel diagnostic index for CTRCD, and GLS has superior predictive performance than MW for CTRCD.
4.Mechanistic study of Tripterygium wilfordii multiglucoside in improving nephrotic syndrome via regulating the HIF-1α/miR-155-5p/Nrf2 pathway

OBJECTIVE To study the improvement effect and mechanism of Tripterygium wilfordii multiglucoside (TWM) on nephrotic syndrome in rats. METHODS The nephrotic syndrome model was established by intravenous injection of adriamycin via the tail vein. The modeling rats were randomly divided into the model group (distilled water), prednisone group (10 mg/kg), and TWM high- and low-dose groups (10 and 5 mg/kg, respectively). Additionally, blank group (distilled water) without model induction was established. Each group consisted of 9 rats. Rats in each group were administered the corresponding drugs or distilled water by gavage, once a day, for 6 consecutive weeks. The histopathological morphology of kidney tissues in rats was observed; the levels of 24-hour urinary protein (24 h-UTP) and serum biochemical indicators [albumin (ALB), blood urea nitrogen (BUN), serum creatinine (SCr), cholesterol (CHOL), and triglyceride (TG)] in rats were determined; the levels of oxidative stress indicators [superoxide dismutase (SOD), malondialdehyde (MDA)] in kidney tissue of rats were determined; expressions of hypoxia-inducible factor-1α (HIF-1α)/microRNA-155-5p (miR-155-5p)/nuclear factor erythriod 2- related factor 2 (Nrf2) signaling pathway-related mRNA and protein in the renal tissues of rats were detected. RESULTS Compared with the blank group, the rats in the model group exhibited disordered renal tissue structure, with a small amount of glomerular necrosis and edema of the renal tubular epithelial cells. 24 h-UTP, serum levels of SCr, BUN, CHOL and TG, MDA content, mRNA and protein expressions of HIF-1α and Keap1 as well as the expression of miR-155-5p in renal tissues were increased significantly ( P <0.05). Serum level of ALB, SOD level in renal tissue as well as mRNA and protein expressions of Nrf2 were decreased significantly ( P <0.05). Compared with the model group, TWM high-dose and low-dose groups exhibited significant improvements in renal injury, with notable reversals in the levels of the above quantitative indicators ( P <0.05). CONCLUSIONS TWM can alleviate oxidative stress-induced damage and thereby improve nephrotic syndrome in rats by regulating the HIF-1α/miR-155-5p/Nrf2 signaling pathway.
5.Rapid Identification of Different Parts of Nardostachys jatamansi Based on HS-SPME-GC-MS and Ultra-fast Gas Phase Electronic Nose

ObjectiveTo establish a model that can quickly identify the aroma components in different parts of Nardostachys jatamansi, so as to provide a quality control basis for the market circulation and clinical use of N. jatamansi. MethodsHeadspace solid-phase microextraction-gas chromatography-mass spectrometry(HS-SPME-GC-MS) combined with Smart aroma database and National Institute of Standards and Technology(NIST) database were used to characterize the aroma components in different parts of N. jatamansi, and the aroma components were quantified according to relative response factor(RRF) and three internal standards, and the markers of aroma differences in different parts of N. jatamansi were identified by orthogonal partial least squares-discriminant analysis(OPLS-DA) and cluster thermal analysis based on variable importance in the projection(VIP) value >1 and P<0.01. The odor data of different parts of N. jatamansi were collected by Heracles Ⅱ Neo ultra-fast gas phase electronic nose, and the correlation between compound types of aroma components collected by the ultra-fast gas phase electronic nose and the detection results of HS-SPME-GC-MS was investigated by drawing odor fingerprints and odor response radargrams. Chromatographic peak information with distinguishing ability≥0.700 and peak area≥200 was selected as sensor data, and the rapid identification model of different parts of N. jatamansi was established by principal component analysis(PCA), discriminant factor alysis(DFA), soft independent modeling of class analogies(SIMCA) and statistical quality control analysis(SQCA). ResultsThe HS-SPME-GC-MS results showed that there were 28 common components in the underground and aboveground parts of N. jatamansi, of which 22 could be quantified and 12 significantly different components were screened out. Among these 12 components, the contents of five components(ethyl isovalerate, 2-pentylfuran, benzyl alcohol, nonanal and glacial acetic acid,) in the aboveground part of N. jatamansi were significantly higher than those in the underground part(P<0.01), the contents of β-ionone, patchouli alcohol, α-caryophyllene, linalyl butyrate, valencene, 1,8-cineole and p-cymene in the underground part of N. jatamansi were significantly higher than those in the aboveground part(P<0.01). Heracles Ⅱ Neo electronic nose results showed that the PCA discrimination index of the underground and aboveground parts of N. jatamansi was 82, and the contribution rates of the principal component factors were 99.94% and 99.89% when 2 and 3 principal components were extracted, respectively. The contribution rate of the discriminant factor 1 of the DFA model constructed on the basis of PCA was 100%, the validation score of the SIMCA model for discrimination of the two parts was 99, and SQCA could clearly distinguish different parts of N. jatamansi. ConclusionHS-SPME-GC-MS can clarify the differential markers of underground and aboveground parts of N. jatamansi. The four analytical models provided by Heracles Ⅱ Neo electronic nose(PCA, DFA, SIMCA and SQCA) can realize the rapid identification of different parts of N. jatamansi. Combining the two results, it is speculated that terpenes and carboxylic acids may be the main factors contributing to the difference in aroma between the underground and aboveground parts of N. jatamansi.
6.Effect of cholesterol on distribution,cell uptake,and protein corona of lipid microspheres at sites of cardiovascular inflammatory injury

Cholesterol(CH)plays a crucial role in enhancing the membrane stability of drug delivery systems(DDS).However,its association with conditions such as hyperlipidemia often leads to criticism,overshadowing its influence on the biological effects of formulations.In this study,we reevaluated the delivery effect of CH using widely applied lipid microspheres(LM)as a model DDS.We conducted comprehensive in-vestigations into the impact of CH on the distribution,cell uptake,and protein corona(PC)of LM at sites of cardiovascular inflammatory injury.The results demonstrated that moderate CH promoted the accumulation of LM at inflamed cardiac and vascular sites without exacerbating damage while partially mitigating pathological damage.Then,the slow cellular uptake rate observed for CH@LM contributed to a prolonged duration of drug efficacy.Network pharmacology and molecular docking analyses revealed that CH depended on LM and exerted its biological effects by modulating peroxisome proliferator-activated receptor gamma(PPAR-γ)expression in vascular endothelial cells and estrogen receptor alpha(ERα)protein levels in myocardial cells,thereby enhancing LM uptake at cardiovascular inflam-mation sites.Proteomics analysis unveiled a serum adsorption pattern for CH@LM under inflammatory conditions showing significant adsorption with CH metabolism-related apolipoprotein family members such as apolipoprotein A-V(Apoa5);this may be a major contributing factor to their prolonged circu-lation in vivo and explains why CH enhances the distribution of LM at cardiovascular inflammatory injury sites.It should be noted that changes in cell types and physiological environments can also influence the biological behavior of formulations.The findings enhance the conceptualization of CH and LM delivery,providing novel strategies for investigating prescription factors' bioactivity.
7.Effects of Shuwei Decoction on Mitochondrial Autophagy and Cell Apoptosis of Interstitial Cells of Cajal in Rats with Functional Dyspepsia

Objective To investigate the effects of Shuwei Decoction on calcium homeostasis,oxidative stress,mitochondrial autophagy and cell apoptosis of interstitial cells of Cajal(ICC)in functional dyspepsia(FD)rat with liver depression and spleen deficiency syndrome;To discuss its mechanism in the treatment of FD.Methods Totally 50 SD rats were randomly divided into blank group,model group and Shuwei Decoction low-,medium-and high-dosage groups(3.78,7.56,15.34 g/kg),with 10 rats in each group.An improved composite etiology method was used to establish a rat model of FD with liver depression and spleen deficiency syndrome,and rats in the Shuwei Decoction low-,medium-and high-dosage groups were orally administered for 14 days.The gastric antrum was taken,the primary ICC was isolated and cultures,and Fluo-4 AM and Rhod-2 AM staining was used to detect the accumulation of calcium ions in the cytoplasm and mitochondria of ICC,DCFH-DA and MitoSox Red were used to detect the content of reactive oxygen species(ROS)in ICC cytoplasm and mitochondria respectively,immunofluorescence staining and flow cytometry were used to detect mitochondrial autophagy and apoptosis in ICC,Western blot was used to detect ICC mitochondrial LC3 and voltage dependent anion channel(VDAC)1 protein expression.Results Compared with the blank group,the model group exhibited significantly increased accumulation of cytoplasmic and mitochondrial Ca2+and ROS in ICC,the co-localization of TOM20 and LAMP2 markedly increased(P<0.01),along with a significant increase in cell apoptosis rate(P<0.01),the expressions of mitochondrial LC3 Ⅱ/Ⅰ and VDAC1 proteins significantly increased(P<0.01).Compared with the model group,the accumulation of Ca2+and ROS in ICC cytoplasm and mitochondria was significantly decreased in Shuwei Decoction medium-and high-dosage groups(P<0.05,P<0.01),the co-expression of TOMM20 and LAMP2 significantly decreased(P<0.01),the apoptosis rate decreased(P<0.01),and the expressions of mitochondrial LC3 Ⅱ/Ⅰ and VDAC1 proteins significantly decreased(P<0.01).Conclusion Shuwei Decoction may achieve the treatment of FD with liver depression and spleen deficiency syndrome in rats by inhibiting Ca2+overload,reducing the accumulation of ROS and excessive autophagy of mitochondria,inhibiting the apoptosis of ICC and restoring ICC function.
8.Association of plasma 8-hydroxy-2′-deoxyguanosine with left ventricular diastolic dysfunction in asymptomatic patients with type 2 diabetes

Objective:To explore the predictive value of mitochondrial oxidative stress biomarkers 8-hydroxy-2′-deoxyguanosine (8-OHdG) and mitochondrial DNA (mtDNA) copy number in asymptomatic type 2 diabetes mellitus (T2DM) patient with left ventricular diastolic dysfunction.Methods:This was a cross-sectional study. T2DM patients without cardiovascular symptoms who were admitted to the Department of Endocrinology, the Third Affiliated Hospital of Soochow University between April 2018 and May 2022 were enrolled. According to the HFA-PEFF score, the enrolled patients were divided into three groups: the T2DM group (HFA-PEFF score ≤1), the left ventricular diastolic dysfunction suspected positive group (HFA-PEFF score 2-4), and the left ventricular diastolic dysfunction positive group (HFA-PEFF score ≥5). Multivariate logistic regression was performed to evaluate the association of plasma 8-OHdG level and mtDNA copy number with left ventricular diastolic dysfunction in patients with T2DM. Receiver operating characteristic curves were constructed to evaluate the predictive ability of plasma 8-OHdG level and its combination with baseline clinical data for left ventricular diastolic dysfunction in T2DM patients, and stratified analysis was performed by sex, age, diabetes duration and hemoglobin A1c levels.Results:A total of 163 T2DM patients without cardiovascular symptoms, aged (54.0±8.7) years, including 93 males (57.1%), were enrolled. Compared with T2DM group, patients in left ventricular diastolic dysfunction suspected positive and positive groups (44.59 (27.72, 55.58) μg/L vs. 93.23 (59.58, 129.80) μg/L vs. 101.91 (71.39, 137.39) μg/L, P<0.05) had significantly higher plasma 8-OHdG levels, while mtDNA copy number showed no statistically significant differences among the three groups (both P>0.05). Multivariate logistic regression analysis showed that after adjusting for confounder factors, elevated plasma 8-OHdG level were independently associated with both suspected positive left ventricular diastolic dysfunction ( OR=1.036, 95% CI 1.019-1.053, P<0.001) and positive left ventricular diastolic dysfunction ( OR=1.035, 95% CI 1.018-1.053, P<0.001). mtDNA copy number showed no significant association with T2DM accompanied by left ventricular diastolic dysfunction ( P>0.05). Stratified analysis indicated that elevated plasma 8-OHdG level was significantly associated with left ventricular diastolic dysfunction in different age, sex, diabetes course and hemoglobin A1c levels subgroups (all P<0.05). The receiver operating characteristic curves indicated that the combination of baseline clinical data with 8-OhdG level ( AUC=0.871, 95% CI 0.814-0.928, P<0.001) is more effective in predicting asymptomatic left ventricular diastolic dysfunction in T2DM patients than 8-OHdG ( AUC=0.783, 95% CI 0.704-0.861, P<0.001) or baseline clinical data ( AUC=0.736, 95% CI 0.647-0.826, P=0.001) alone. Conclusion:The mitochondrial oxidative stress biomarker 8-OHdG combined with baseline clinical data has good predictive value for asymptomatic left ventricular diastolic dysfunction in T2DM patients, and is expected to be a biomarker to identify diabetes mellitus with asymptomatic diastolic dysfunction.
9.Study on the pollution and influencing factors of legionella pneumophila in industrial circulating cooling tower

Objective:To investigate the pollution status of legionella pneumophila of industrial circulating cooling towers, and explore the factors affecting the positive rate and quantity of legionella pneumophila, in order to provide data support for the risk assessment and hierarchical management of legionella pneumophila in workplaces.Methods:In February, May, August and October 2023, the cooling water samples were collected from a total of 20 industrial circulating cooling towers of 9 enterprises in an industrial park in Shanghai. The water temperature, free residual chlorine, total residual chlorine, turbidity, chroma, pH, conductivity, chemical oxygen consumption, iron content and total plate count in the water samples were detected. Meanwhile, the legionella pneumophila in the water samples were qualitatively and quantitatively analyzed, and the influence of water quality indexes on the positive rate and number of legionella pneumophila detected was analyzed by logistic regression and linear regression respectively.Results:The positive rates of legionella pneumophila in the first to fourth quarters of 20 industrial circulating cooling towers of 9 chemical enterprises were 35% (7/20), 45% (9/20), 45% (9/20), and 85% (17/20). The serotypes of legionella pneumophila detected in the cooling tower were mainly legionella pneumophila type 1 (95.2%, 40/42), and the number of legionella pneumophila ranged from 0.2 to 316.8 cfu/ml. Circulating cooling water temperature ( OR=1.36, 95% CI: 1.12-1.64, P=0.001) and conductivity ( OR=1.02, 95% CI: 1.00-1.03, P=0.026) were independent risk factors affecting the detection of legionella pneumophila. There was a positive correlation between chemical oxygen consumption and the detected amount of legionella pneumophila ( β=8.08, P=0.002) . Conclusion:The positive rate of legionella pneumophila in industrial circulating cooling water system is high, and the detected quantity fluctuates greatly, which may have health risks, suggesting that it is necessary to pay attention to the water quality management such as water temperature, conductivity and chemical oxygen consumption of industrial circulating cooling water and the monitoring and control of legionella pneumophila.
10.Spousal correlations of blood lipid based on a family design

Objective:To explore the spousal correlations of total cholesterol(TC),total triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),and high-density lipoprotein cholesterol(HDL-C),and to investigate the reasons behind these spousal correlations.Methods:Participants and data were from the baseline survey of family-based cohort studies in Fangshan,Beijing and Tulou,Fujian.The ori-gin of spousal correlations were explored from perspectives of convergence,assortative mating,social ho-mogamy.Pearson's correlation and generalized linear models(GLM)were used to estimate the spousal correlation.Convergence was assessed by Pearson's correlation between the phenotypic differences be-tween couples and the duration of marriage,with GLM used for further validation.Pearson's correlation of genetic risk scores(GRS)and couple-specific Mendelian randomization(MR)were calculated to assess the genetic correlation and possible causal relationships between spouses.Two-independent-sample t-tests were used to compare GRS consistency across subgroups divided by education attainment,couple-specific MR and Q statistics used to test assortative mating in subgroups and intergroup differences.Results:In the study,342 couples(287 couples from Fangshan and 55 couples from Fujian)were included,with the average age of(64.91±8.76)years.Spousal correlations of TC,TG,HDL-C,and LDL-C showed statistically significant associations both before and after adjusting for covariates,with effect sizes of 0.229(95%CI:0.125-0.327),0.257(95%CI:0.155-0.354),0.179(95%CI:0.074-0.280),and 0.181(95%CI:0.076-0.282).For convergence,for each additional year of marriage,ΔTC increased by 0.016 mmol/L(95%CI:0.001-0.033 mmol/L),and ΔLDL-C increased by 0.017 mmol/L(95%CI:0.002-0.031 mmol/L).For assortative mating,GRS correlations and results of couple specific MR didn't show any statistical significance.For social homogamy,no differences in GRS or assortative mating were found between subgroups stratified by education attainment.Conclusion:The blood lipid in participants exhibit spousal phenotypic correlations,however,no effects of convergence,assortative mating or social homogamy were observed.More independent studies with larger sample sizes are warranted to further validate these findings in the future.
