1.Two visual arthroplasty techniques for L5-S1 disc herniation:a half-year follow-up evaluation of clinical outcomes

BACKGROUND:Currently,spinal endoscopic technology has become the mainstream technology in minimally invasive spinal surgery.The specifications of the instruments for different operating systems are different,and the choice of specific surgical protocols needs to be combined with the actual situation of the patient and the choice of the clinical surgeon. OBJECTIVE:To compare the early efficacy of percutaneous endoscopic interlaminar discectomy for L5-S1 disc herniation under the iLESSYS Delta System and Endo-Surgi Plus System. METHODS:Totally 80 patients with L5-S1 disc herniation were treated with percutaneous endoscopic interlaminar discectomy.Patients were divided into two groups based on the endoscopic system used.Among them,37 cases received the iLESSYS Delta System(Delta group)and 43 cases received the Endo-Surgi Plus System(Plus group).Patient demographic characteristics,perioperative indicators,and complications were analyzed between the two groups.Clinical outcomes were quantified using back and leg visual analog scale scores,Oswestry Disability Index,and Japanese Orthopaedic Association scores at 1 day,1,3,and 6 months after surgery.Patient satisfaction was assessed according to modified MacNab criteria at final follow-up. RESULTS AND CONCLUSION:(1)The operative time and number of arthroplasties in the Plus group were less than those in the Delta group,and the differences were statistically significant(P<0.05).(2)Compared with the preoperative period,the visual analog scale scores,Oswestry Disability Index,and Japanese Orthopaedic Association scores of patients in both groups improved at all follow-up time points,and the difference was statistically significant(P<0.001).(3)There was no statistically significant difference in the comparison of pain visual analog scale scores,Oswestry Disability Index,and Japanese Orthopaedic Association scores of patients in the two groups(P>0.05).(4)At 6-month follow-up after surgery,the MacNab standard excellent and good rates in the Delta group and Plus group were 81%and 79%,respectively,with no significant difference(P=0.823).(5)The incidence of complications was 3%in the Delta group and 2%in the Plus group,but there was no significant difference between the two groups(P=0.914).(6)It is concluded that both iLESSYS Delta and Endo-Surgi Plus surgical systems achieved satisfactory early clinical results in the treatment of lumbar disc herniation,with Endo-Surgi Plus surgical moulding being more efficient and safer.
2.Construction of interpretable predictive model of acupuncture for methadone reduction in patients undergoing methadone maintenance treatment based on machine learning and SHAP.

OBJECTIVE:
To construct a predictive model for the reduction in methadone maintenance treatment (MMT) and evaluate the effects of different interventions and other clinical factors on methadone reduction using Shapley additive explanations (SHAP).
METHODS:
Two clinical trials of acupuncture for methadone reduction in MMT patients were analyzed, and the baseline data, MMT related information, intervention measures, the data related to dose-reduction outcomes were collected. The predictive model was constructed by means of 6 machine learning algorithms including support vector machine (SVM), K-nearest neighbors (KNN), logistic regression (LR), Naive Bayes (NB), random forest (RF) and categorical-boosting (CatBoost), and 2 integration methods, blending-ensemble method (Blending) and Stacking-ensemble method (Stacking). SHAP was employed for the interpretability analysis of the optimal model.
RESULTS:
A total of 251 MMT patients were included, 128 cases in the acupuncture group and 123 cases in the non-acupuncture group. CatBoost and Stacking performed optimally in the test set. CatBoost obtained an accuracy of 0.780 0±0.060 8, a precision of 0.500 0±0.120 0, a recall of 0.818 2±0.140 2, F1 score of 0.620 7±0.114 0, and receiver operating characteristic-area under curve (ROC-AUC) of 0.857 8±0.140 2 for the subjects. In MMT patients with acupuncture as an adjunctive therapy, the top 5 important features for methadone reduction, included intervention measures, body mass index (BMI), the duration of MMT, the history of opioid use and occupation; and SHAP values were 1.25, 0.36, 0.21, 0.19 and 0.12, respectively. The SHAP feature dependence plot showed that BMI, MMT duration and the history of opioid use presented a nonlinear negative correlation with the reduction effect.
CONCLUSION
In acupuncture as adjunctive therapy for methadone reduction, the clinical factors should be considered comprehensively; and the interpretable predictive model provides a scientific basis for it, which is conducive to the improvement of clinical strategy of acupuncture for methadone reduction and the development of personalized reduction scheme.

This study aimed to comprehensively examine the association of gallstones, cholecystectomy, and cancer risk. Multivariable logistic regressions were performed to estimate the observational associations of gallstones and cholecystectomy with cancer risk, using data from a nationwide cohort involving 239 799 participants. General and gender-specific two-sample Mendelian randomization (MR) analysis was further conducted to assess the causalities of the observed associations. Observationally, a history of gallstones without cholecystectomy was associated with a high risk of stomach cancer (adjusted odds ratio (aOR)=2.54, 95% confidence interval (CI) 1.50-4.28), liver and bile duct cancer (aOR=2.46, 95% CI 1.17-5.16), kidney cancer (aOR=2.04, 95% CI 1.05-3.94), and bladder cancer (aOR=2.23, 95% CI 1.01-5.13) in the general population, as well as cervical cancer (aOR=1.69, 95% CI 1.12-2.56) in women. Moreover, cholecystectomy was associated with high odds of stomach cancer (aOR=2.41, 95% CI 1.29-4.49), colorectal cancer (aOR=1.83, 95% CI 1.18-2.85), and cancer of liver and bile duct (aOR=2.58, 95% CI 1.11-6.02). MR analysis only supported the causal effect of gallstones on stomach, liver and bile duct, kidney, and bladder cancer. This study added evidence to the causal effect of gallstones on stomach, liver and bile duct, kidney, and bladder cancer, highlighting the importance of cancer screening in individuals with gallstones.

4.Study on the inhibition of cell cycle progression and proliferation ability of lung adenocarcinoma cells by regulating the TGF-β1/Smads signaling pathway by morin

Objective To investigate the inhibitory effect of morin on the cell cycle progression and prolif-eration ability of lung adenocarcinoma cells by regulating the transforming growth factor β1(TGF-β1)/Smads signaling pathway.Methods Human lung adenocarcinoma cell line A549 was used for in vitro experiments.Different concentrations of morin(0,30,90,150 μg/mL)were used to treat human lung adenocarcinoma cell line A549.Cell survival rate was detected by CCK-8 method,cell cycle distribution was detected by Hoechst 33342 staining method,cell proliferation ability was detected by clone formation assay,cell invasion ability was detected by Transwell invasion assay,cell apoptosis was detected by TUNEL staining method,and the expres-sion levels of TGF-β1/Smads signaling pathway related proteins were analyzed by Western blot.Results CCK-8 experimental results showed that the survival rate of A549 cells in the morin-treated group significantly decreased with the increase in morin concentration(P<0.05).The results of Hoechst 33342 staining showed that the proportion of G1 phase cells in the morin-treated group significantly increased,and the proportion of S phase and G2/M phase cells significantly decreased(P<0.05).The results of the colony formation assay showed that the number of colonies formed in the morin-treated group was significantly less than that in the control group,and the colony formation rates in the 90 μg/mL and 150 μg/mL groups were reduced by approximately 50%and 70%respectively(P<0.05).Transwell invasion assay results showed that the number of invasive cells in the morin-treated group was significantly reduced(P<0.05).TUNEL staining results showed that the proportion of apoptotic cells in the morin-treated group significantly increased(P<0.05).Western blot results showed that the expression levels of TGF-β1 and p-Smad2/3 in the morin-treated group were significantly reduced(P<0.05),and the expression level of Smad7 was significantly in-creased(P<0.05).Conclusion Morin blocks A549 cells in the G1 phase by regulating the TGF-β1/Smads signaling pathway,thereby inhibiting cell proliferation,reducing cell invasion ability,and inducing cell apopto-sis.This indicates that morin has potential application value in the treatment of lung adenocarcinoma.
5.Application progress of objectification of tongue and pulse diagnosis in the diagnosis and treatment of malignant tumor

The objectification of tongue and pulse diagnosis in TCM transforms tongue and pulse images into quantitative data through modern technology, and shows multi-dimensional application value in tumor diagnosis and treatment. In terms of tongue diagnosis, the characteristic tongue images of tumor patients are related to pathological indicators such as serum factors and tongue coating microflora, which can assist in diagnosis, syndrome differentiation and efficacy evaluation. The machine learning model preliminarily realizes the recognition of tongue images of tumors such as gastric cancer. In terms of pulse diagnosis, the abnormal pulse condition collected by the sensor is related to the abnormal vascular structure and hemodynamics. The Jin's "chongbo" index and pulse graph parameters can be used for tumor malignancy judgment and efficacy monitoring. The deep learning algorithm improves the accuracy of pulse analysis. However, there are some problems in the current research, such as small sample size, inconsistent equipment standards, and insufficient tongue and pulse information fusion. In the future, through multi-modal data integration, machine learning optimization and large sample verification, it is necessary to promote the combination of tongue and pulse objectification technology and TCM syndrome differentiation theory, build a standardized diagnosis model, and provide objective support for accurate diagnosis and treatment of tumors.
6.Upregulation of miR-1287-5p promotes paclitaxel resistance in human esophageal cancer cell line KYSE30

Objective To discover the mechanism by which miR-1287-5p affects the sensitivity of human esophageal cancer cell line KYSE30 to paclitaxel.Methods KYSE30 cells were divided into control group,group of transfected with miR-1287-5p mimic and inhibitor groups.RT-qPCR was used to detect the transfection rate.A paclitaxel-resistant cell line was established and cell viability and half inhibitory concentration(IC50)were detected by CCK-8 assay.The scratch test,Transwell chamber test and TUNEL method were used to evaluate the migration,invasion and apoptosis of cells.Western blot and immunocytochemistry(ICC)were used to detect the expression and localization of Akt and GSK-3β in cells.Results The expression level of miR-1287-5p in the miR-1287-5p mimic group was significantly higher than that in the miR-NC group and control group(P<0.05),while the ex-pression level of the miR-1287-5p inhibitor group was significantly lower than that in miR-NC group and control group(P<0.05).The cell survival rate of the miR-1287-5p mimic group was higher than that of the miR-NC group and the miR-1287-5p inhibitor group(P<0.05).The cell migration rate and number of invasive cells in the miR-1287-5p mimic group were higher than those of the miR-NC group(P<0.05),and the apoptosis rate was lower than that of the miR-NC group(P<0.05).While in miR-1287-5p,the cell migration rate was lower and invasive cells were less in the inhibitor group and the apoptosis rate was increased as compared to the miR-NC group(P<0.05).The expression of Akt and GSK-3β in the miR-1287-5p mimic group was significantly increased than in the miR-NC group(P<0.05),while the expression in the miR-1287-5p inhibitor group was significantly inhibited than in miR-NC group(P<0.05).Conclusions Up-regulation of miR-1287-5p can promote the migration and invasion of paclitaxel-resistant cell lines,inhibit cell apoptosis and thereby promote the development of paclitaxel resistance.
7.Application of Holistic Nursing Model Based on Delphi in Patients with AMI

Objective To explore the application value of a holistic nursing model based on the Delphi method in patients with acute myocardial infarction(AMI).Methods A total of 110 AMI patients treated at Diqing Shangri-La People's Hospital in Yunnan Province from January 2019 to January 2020 were enrolled and divided into a control group and an observation group,with 55 patients in each,based on their admission order.The control group received routine nursing care,while the observation group received holistic nursing model intervention based on Delphi in addition to the routine nursing care.After 6 months of nursing,the left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),left ventricular ejection fraction(LVEF),general self-efficacy scale(GSES)score,complications,and changes in nursing satisfaction were compared and analyzed between the two groups.Results(1)After nursing,the LVEF significantly increased and the LVEDV and LVESV significantly decreased in both groups;the increase in LVEF was more significant,and the decrease in LVEDV and LVESV was more pronounced in the observation group(P<0.05);(2)The GSES scores of patients in both groups improved compared to before nursing,with a more significant improvement in the observation group(P<0.05);(3)The incidence of adverse events during hospitalization was lower in the observation group than in the control group(P<0.05);(4)The satisfaction rate was higher in the observation group than in the control group after nursing(P<0.05).Conclusion The application of a holistic nursing model based on Delphi in AMI patients can effectively improve cardiac function and self-management efficacy,reduce the incidence of complications,and enhance nursing satisfaction,demonstrating high feasibility.

Objective:To report the long-term outcomes of Chinese rectal cancer patients after adopting a Watch and Wait (W&W) strategy following neoadjuvant therapy (NAT).Methods:This multicenter, cross-sectional study was based on real-world data. The study cohort comprised rectal cancer patients who had achieved complete or near complete clinical responses (cCRs, near-cCRs) after NAT and were thereafter managed by a W&W approach, as well as a few patients who had achieved good responses after NAT and had then undergone local excision for confirmation of pathological complete response. All participants had been followed up for ≥2 years. Patients with distant metastases at baseline or who opted for observation while living with the tumor were excluded. Data of eligible patients were retrospectively collected from the Chinese Wait-and-Watch Data Collaboration Group database. These included baseline characteristics, type of NAT, pre-treatment imaging results, evaluation of post-NAT efficacy, salvage measures, and treatment outcomes. We herein report the long-term outcomes of Chinese rectal cancer patients after NAT and W&W and the differences between the cCR and near-cCR groups.Results:Clinical data of 318 rectal cancer patients who had undergone W&W for over 2 years and been followed up were collected from eight medical centers (Peking University Cancer Hospital, Fudan University Shanghai Cancer Center, Sun Yat-sen University Cancer Center, Shanghai Changhai Hospital, Peking Union Medical College Hospital, Liaoning Cancer Hospital, the First Hospital of Jilin University, and Yunnan Cancer Hospital.) The participants comprised 221 men (69.4%) and 107 women (30.6%) of median age 60 (26-86) years. The median distance between tumor and anal verge was 3.4 (0-10.4) cm. Of these patients, 291 and 27 had achieved cCR or near-cCR, respectively, after NAT. The median duration of follow-up was 48.4 (10.2-110.3) months. The 5-year cumulative overall survival rate was 92.4% (95%CI: 86.8%-95.7%), 5-year cumulative disease-specific survival (CSS) rate 96.6% (95%CI: 92.2%-98.5%), 5-year cumulative organ-preserving disease-free survival rate 86.6% (95%CI: 81.0%-90.7%), and 5-year organ preservation rate 85.3% (95%CI: 80.3%-89.1%). The overall 5-year local recurrence and distant metastasis rates were 18.5% (95%CI: 14.9%-20.8%) and 8.2% (95%CI: 5.4%-12.5%), respectively. Most local recurrences (82.1%, 46/56) occurred within 2 years, and 91.0% (51/56) occurred within 3 years, the median time to recurrence being 11.7 (2.5-66.6) months. Most (91.1%, 51/56) local recurrences occurred within the intestinal lumen. Distant metastases developed in 23 patients; 60.9% (14/23) occurred within 2 years and 73.9% (17/23) within 3 years, the median time to distant metastasis being 21.9 (2.6-90.3) months. Common sites included lung (15/23, 65.2%), liver (6/23, 26.1%), and bone (7/23, 30.4%) The metastases involved single organs in 17 patients and multiple organs in six. There were no significant differences in overall, cumulative disease-specific, or organ-preserving disease-free survival or rate of metastases between the two groups (all P>0.05). The 5-year local recurrence rate was higher in the near-cCR than in the cCR group (41.6% vs. 16.4%, P<0.01), with a lower organ preservation rate (69.2% vs. 88.0%, P<0.001). The success rates of salvage after local recurrence and distant metastasis were 82.1% (46/56) and 13.0% (3/23), respectively. Conclusion:Rectal cancer patients who achieve cCR or near-cCR after NAT and undergo W&W have favorable oncological outcomes and a high rate of organ preservation. Local recurrence and distant metastasis during W&W follow certain patterns, with a relatively high salvage rate for local recurrence. Our findings highlight the importance of close follow-up and timely intervention during the W&W process.
10.Application of Magnetic Resonance Imaging and Transrectal Ultrasound Image Fusion Targeted Transperineal Biopsy Technique Using Electromagnetic Needle Tracking Under Local Anaesthesia

Objective To explore the effect and safety of magnetic resonance imaging and transrectal ultrasound (mpMRI-TRUS) image fusion targeted transperineal biopsy technique using electromagnetic needle tracking under local anesthesia. Methods We retrospectively analyzed the clinical and pathological data of 81 patients with mpMRI-TRUS image fusion targeted transperineal prostate biopsy using electromagnetic needle tracking under local anesthesia. Visual analog scale (VAS) and visual numeric scale (VNS) were used to evaluate the pain level and satisfaction of patients during prostate biopsy (VAS-1 and VNS-1), one hour after puncture (VAS-2 and VNS-2), and one day after surgery (VAS-3 and VNS-3). The perioperative clinical data and tumor positive rate of postoperative biopsy were recorded. Results The average prostate volume of 81 patients was 53.39±29.46 cm3. The PSA values of patients with PI-RADS scores of 2, 3, 4, and 5 were 9.14±2.31, 9.95±4.10, 14.77±6.36, and 32.17±24.39 ng/ml, respectively. The scores of VAS-1, VAS-2, and VAS-3 were 1.70±0.73, 1.16±0.58, and 0.53±0.55, respectively; the scores of VNS-1, VNS-2, and VNS-3 were 2.74±0.44, 3.69±0.46, and 3.84±0.37, respectively. The average surgical time was 17.47±3.44 minutes. Postoperative pathological results showed that the tumor positive rate of targeted prostate biopsy was 64.20%. According to the PI-RADS score for subgroup analysis, the tumor positive rates of patients with PI-RADS scores of 2, 3, 4, and 5 were 21.43%, 44.44%, 61.11%, and 96.77%, respectively. After transperineal prostate biopsy, gross hematuria occurred in 19.75% patients, and urinary retention occurred in 3.70%. The latter were relieved after symptomatic treatment. All patients did not experience complications, such as perineal puncture area hematoma, urinary tract infection, hematospermia, hematoma in perineal puncture area, urinary tract infection, hematospermia, vagus nerve reaction, or septic shock. Conclusion For suspected prostate cancer patients, mpMRI-TRUS image fusion targeted transperineal biopsy technique using electromagnetic needle tracking under local anesthesia is a feasible and easily tolerated surgical procedure. It has good safety and high tumor positive-detection rate, indicating that this technique is worthy of further clinical promotion and application.
