Purpose:
This study aimed to evaluate the safety and efficacy of gemcitabine and cisplatin (GP) regimen in combination with immune checkpoint inhibitor sintilimab as neoadjuvant therapy for muscle-invasive bladder cancer (MIBC) patients and the feasibility of the following selective bladder sparing surgery.
Materials and Methods:
Patients with histopathologically confirmed urothelial carcinoma without distant metastases (T2-4a, N ≤ 1, M0, American Joint Committee of Cancer 8th) and with adequate organ function will be enrolled. The therapeutic regimen was sintilimab 200 mg once on day 8, gemcitabine 1,000 mg/m2 and cisplatin 35 mg/m2 once on days 1 and 8, every 21 days for four cycles. The primary endpoint was pathologic complete response (pCR, pT0N0) rate. The secondary end points were ypT < 2 rate, R0 resection rate, event-free survival, and safety.
Results:
From May 4, 2020, to May 20, 2023, 55 patients were enrolled. Forty-six patients were evaluated for efficacy. Among the 42 patients who underwent surgery, 16 patients (38.0%) achieved pCR. Thirty-three patients (78.6%) achieved pT < 2. With a median follow-up of 15.7 months, the 1-year event-free survival was 91.3%. Notwithstanding the poor pathological baseline characteristic of a high T3-T4a proportion (39.1%), a promising bladder preservation (including 22 patients transurethral resection of bladder tumor, 5 patients partial cystectomy, and 4 surveillances) rate was achieved (67.4%). The most common grade ≥ 3 treatment-related adverse events was neutropenia (n=15, 27.3%), which was related to chemotherapy. There were no grade 3 immune-related adverse events.
Conclusion
Neoadjuvant GP plus sintilimab is a promising regimen for MIBC patients, with relatively high pT < 2 rate and triggering the emerging roles for the multi-disciplinary team decision-making for bladder sparing surgery.

ObjectiveTo establish a highly specific and sensitive triplex TaqMan quantitative PCR method for the detection of mouse hepatitis virus (MHV), mouse parvovirus (MPV), and reovirus type 3 (Reo-3) in laboratory mice. MethodsRecombinant plasmid standards were constructed using conserved genomic regions of the three target viruses. Specific primers and probes were designed, and a triplex TaqMan quantitative PCR (qPCR) system was optimized through preliminary experiments. Sensitivity was evaluated using 10-fold serial dilutions (101-107 copies/μL). Intra- and inter-assay repeatability were assessed via reproducibility experiments. ResultsA strong linear correlation (R 2>0.99) was observed between copy number and Ct value at template concentrations of 102-107 copies/μL. Intra-assay and inter-assay coefficients of variation (CV) were both below 5%. No cross-reactivity was observed with mouse cytomegalovirus (MCMV), vesicular stomatitis virus (VSV), Sendai virus (SeV), or pneumonia virus of mice (PVM). ConclusionThe triplex TaqMan quantitative PCR method established in this study has high sensitivity, good repeatability, and strong specificity. It enables the rapid, specific, and sensitive detection of MHV, MPV, and Reo-3, allows the simultaneous detection of multiple pathogens in a single tube, and can be applied to the detection of clinical samples from laboratory animals and to epidemiological investigations, thereby providing effective scientific and technological support for the prevention and control of viral infection and the interruption of transmission.

Objective The relationship between blood pressure variability(BPV)and short-term neurological prognosis in patients with aneurysmal subarachnoid hemorrhage(aSAH)was investigated. Methods The study conducted a retrospective analysis of clinical data from aSAH patients who underwent surgical treatment and were admitted to the ICU at Beijing Tiantan Hospital,Capital Medical University, from January 2023 to April 2024.BPV was quantitively assessed by calculating the standard deviation(SD),successive variation(SV),coefficient of variation(CV),and range of mean blood pressure(MBP).Patients were divided into two group based on discharge GOS scores:good prognosis[Glasgow Outcome Scale(GOS)4-5]and poor prognosis(GOS 1-3)groups.Comparative analyses were performed to evaluate differences in BPV metrics between the two groups, followed by multivariable logistic regression modeling to adjust for potential confounding factors and elucidate the association between BPV and clinical prognosis. Results A total of 150 patients were included, with 59 in the poor prognosis group and 91 in the good prognosis group. The poor prognosis group exhibited significantly elevated levels of MBP-SD[(9.85±3.20)mmHg vs.(8.04±2.31)mmHg,P<0.001],MBP-SV[(10.37±3.85)mmHg vs.(8.07±2.33)mmHg,P<0.001],MBP-CV(10.00±3.30%vs.8.19±2.33%,P<0.001),and MBP-range[(39.60±13.56)mmHg vs.(32.44±9.78)mmHg,P<0.05]compared to the good prognosis group.Cohen’s d values indicated moderate effect sizes for BPV differences(0.65,0.72,0.63,and 0.61,respectively).Multivariable logistic regression showed that MBP-SD(OR=1.22,95%CI:1.08-1.39,P=0.002)and MBP-SV(OR=1.19,95%CI:1.05-1.35,P=0.007)were independently associated with poor prognosis. Conclusion Elevated MBP-SD and MBP-SV within the first 24 hours postoperative period are independent predictors of unfavorable short-term neurological outcomes in aSAH patients.
