1.Differences in scapular kinematics between healthy individuals and rotator cuff tear patients based on biplane X-ray

BACKGROUND:Due to the complex movement of the scapula,which is a six-degree-of-freedom activity in three-dimensional space,it is difficult to measure it accurately using traditional methods.The image and model matching technology based on dual-plane X-ray is a three-dimensional measurement method that has gradually developed and matured in recent years.Two high-speed cameras are used to project and shoot from orthogonal directions.Compared with a single perspective,this method has advantages in observation range and reduction of out-of-plane errors,and is suitable for the study of scapula kinematics.OBJECTIVE:X-ray biplane and image-model registration technology were used to explore the differences in scapular kinematics between normal individuals and patients with rotator cuff tears,providing a basis for the treatment and rehabilitation of rotator cuff tear patients.METHODS:From April 2023 to January 2024,10 patients with normal shoulders and 10 patients with rotator cuff tears who met the inclusion criteria were enrolled from Guanghua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine.The subjects underwent a shoulder CT scan to create a 3D model and a local scapular coordinate system.X-ray biplane images were taken during shoulder abduction with two C-arm machines.The 2D and 3D images were registered to compare scapular kinematic differences between the two groups at 0°,15°,30°,45°,60°,75°,and 90° of abduction,including scapular rotation angle and displacement distance.RESULTS AND CONCLUSION:(1)During shoulder abduction,the scapula of both groups showed upward rotation,but the upward rotation of the rotator cuff tear group was greater than that of the control group,and the difference was significant when the abduction was 30°-90°(P<0.01).At the same time,the scapula internal rotation of both groups gradually increased,but the rotator cuff tear group was significantly greater than the control group when the abduction was 45°-90°(P<0.01).In addition,the anteroposterior tilt of the scapula of the two groups was significantly different when the abduction was 15°-90°(P<0.01).The scapula posterior tilt of the control group gradually increased during abduction,while the scapula of the rotator cuff tear group tilted forward except for a slight posterior tilt at 15°-30° abduction.(2)In terms of displacement,the upward displacement of the rotator cuff tear group was less than that of the control group during abduction,and the difference was significant at 15°-90°(P<0.05),but there was no significant difference in lateral and anterior-posterior displacement between the two groups(P>0.05).(3)Rotator cuff tear can cause scapular dyskinesis,characterized by increased upward rotation,internal rotation,and abnormal forward tilt during shoulder abduction.Identifying and addressing scapular dyskinesis is crucial for treating rotator cuff tear.

3.Analysis of Animal Model of Type 2 Diabetes Mellitus Based on Clinical Characteristics of Traditional Chinese and Western Medicine

Based on the etiology and clinical diagnostic criteria of type 2 diabetes mellitus (T2DM), identification and typing of treatment from the perspective of traditional Chinese and western medicine, the criteria for evaluating the clinical compatibility of traditional Chinese and western medicine in animal models of T2DM were set up. The literature was reviewed to sort out and analyze the existing commonly used modeling methods, summarize the mechanism, compare the advantages and disadvantages, and calculate the consistency between the animal model and the clinical symptoms, syndromes, and indicators from the perspective of traditional Chinese and western medicine. The authors found that spontaneous animal models and high-fat diets combined with multiple low-dose streptozotocin (STZ) induction models were more in line with modern medical pathogenesis of T2DM. However, it fails to form some special syndromes required for traditional Chinese medicine (TCM) research. In addition, there are many methods of combining the etiology and pathogenesis of TCM, which can be divided into three categories: intervention carried out by drug administration, behavioral stimulation, or environmental changes according to TCM, or use of hormones according to clinical evidence and combination of the two methods mentioned above. All of them can successfully establish different types of animal models. However, different methods of establishing syndrome models have their own advantages and disadvantages, and there is no unified standard for the stability and evaluation of syndrome models. As for the clinical consistency criteria of traditional Chinese and western medicine established in this paper, the animal model with 100% consistency has not been calculated due to the conditions of incomplete symptoms and syndromes described in the studies and different selection indicators. Consequently, the establishment of a simple, easy-to-use, and affordable T2DM animal model with both traditional Chinese and western medicine disease characteristics and the improvement of the Chinese and western medicine evaluation system for different evidence types are of great significance for the future development of TCM research on T2DM.

Metastasis serves as an indicator of malignancy and is a biological characteristic of carcinomas. Epithelial-mesenchymal transition (EMT) plays a key role in the promotion of tumor invasion and metastasis and in the enhancement of tumor cell aggressiveness. Prostaglandin E synthase 3 (p23) is a cochaperone for heat shock protein 90 (HSP90). Our previous study showed that p23 is an HSP90-independent transcription factor in cancer-associated inflammation. The effect and mechanism of action of p23 on lung cancer metastasis are tested in this study. By utilizing cell models in vitro and mouse tail vein metastasis models in vivo, the results provide solid evidence that p23 is critical for promoting lung cancer metastases by regulating downstream CXCL1 expression. Rather than acting independently, p23 forms a complex with RNA-binding motif protein 14 (RBM14) to facilitate EMT progression in lung cancer. Therefore, our study provides evidence for the potential role of the RBM14-p23-CXCL1-EMT axis in the metastasis of lung cancer.
6.Assessment of periappendiceal fat density attenuation for the severity of appendicitis

Objective To investigate the correlation between the attenuation of periappendiceal fat density and its relative CT value(rCT)with the severity of appendicitis,and whether rCT can evaluate the severity of acute appendicitis(AA).Methods A retro-spective selection was made of 79 patients who underwent whole-abdominal CT scans and were pathologically diagnosed with A A(45 with noncomplex A A,34 with complex AA)and 30 normal control group(NCG).Appendicitis-associated signs were recorded and the CT number of periappendiceal fat(CTa),CT number of mesenteric fat gap(CTm),CT number of abdominal anterior wall subcutaneous fat(CTaw),CT number of abdominal posterior wall subcutaneous fat(CTpw),CT number of retroperitoneal fat(CTrp),CT number of gluteal subcutaneous fat(CTgl),and CT number of psoasmajor muscle(CTps)were measured.The ratio of CTa to the CT values of the other 6 parts were calculated,defined as the rCT.Statistical analysis was performed using SPSS 20.0 soft-ware,and the differences were considered statistically significant at P<0.05.Results Compared among the three groups,statistical differences were found between every two groups in CTa,rCTm,rCTaw,rCTpw,rCTrp,rCTgl,and rCTps(P<0.05),and the rCT was the lowest in complex AA,followed by noncomplex A A.Receiver operating characteristic(ROC)curve analysis showed that rCTm,rCTaw,rCTpw,rCTrp,rCTgl,rCTps had diagnostic predictive value for both noncomplex AA and complex AA(P<0.05).Conclusion The attenuation of periappendiceal fat density is associated with the severity of appendicitis,and rCT has certain diagnos-tic predictive value for assessing the severity of appendicitis.The diagnosis of noncomplex AA and complex AA is based on the 95% confidence interval of rCT.
7.Influencing factors for sarcopenia in elderly hospitalized patients and construction and validation of a risk prediction model

Objective:To investigate the influencing factors for sarcopenia in hospitalized patients,to construct a risk prediction model for sarcopenia in elderly hospitalized patients,and to provide a quantitative tool for early screening and intervention of sarcopenia based on the integration of multi-dimensional indicators.Methods:A retrospective analysis was performed for the data of 2105 elderly patients who were hospitalized in The First Affiliated Hospital of Chongqing Medical University from March 2016 to June 2023.The least absolute shrinkage and selection operator analysis was used for the screening of variables,and the logistic regression analysis was used to investigate the influencing factors for sarcopenia.A predictive model was constructed,and internal and external validation was performed.The Shapley additive explanations model was used to analyze feature contributions,and a nomogram model was constructed to visualize and interpret the results.Results:The 1259 patients from March 2016 to December 2020 were randomly divided into a training set with 882 patients and an internal test set with 377 patients at a ratio of 7∶3,and the 846 patients from January 2021 to June 2023 were established as the external validation set.A total of 489 cases of sarcopenia(55.44%)were detected in the training set.The logistic regression analysis based on the training set showed that asthenia,dependence on Activity of Daily Living,malnutrition,and in-creasing age were risk factors for sarcopenia(odds ratio[OR]>1,P<0.05),and male sex,normal body mass index,and overweight were protective factors against sarcopenia(OR<1,P<0.05).The model had an AUC of 0.876(95%CI=0.854-0.899)in the training set,0.883(95%CI=0.849-0.918)in the internal test set,and 0.750(95%CI=0.717-0.783)in the external validation set,suggesting that the model had good performance.The decision curve analysis showed that the nomogram model had a good clinical value.Conclu-sion:The predictive model for sarcopenia has good performance and holds promise for clinical application.
8.Correlation between corneal biomechanics and Schlemm canal and trabecular meshwork morphology in myopia

Objective:To evaluate the correlation between corneal biomechanical parameters and Schlemm canal (SC) and trabecular meshwork (TM) in patients with myopia.Methods:Eighty-one consecutive patients who underwent refractive surgery at Wuhan EyeGood Ophthalmology Hospital from May to December 2021 were enrolled.Patients were divided into two groups according to spherical equivalent (SE) and axial length (AL), including 39 patients with 39 eyes in low to moderate myopia group (SE: -0.5 to -6.0 D, AL<26 mm) and 42 patients with 42 eyes in high myopia group (SE≤-6.0 D, AL≥26 mm).Another 20 cases (20 eyes) from the same period of health examinations were included as a healthy control group, with 10 males and 10 females.All subjects' right eye data were included for analysis.The corneal biomechanical parameters were measured with Corvis ST.The morphology of the SC and TM was observed with Zeiss Cirrus HD-OCT 5000.The differences in corneal biomechanical parameters, nasal SC (SC-N) area, temporal SC (SC-T) area, nasal TM (TM-N) width and thickness and temporal TM (TM-T) width were compared between the two groups.Correlations between corneal biomechanical parameters and SC, TM and biomechanically corrected intraocular pressure (bIOP) in myopic patients were analyzed using Pearson linear correlation analysis.The study followed the Declaration of Helsinki, and the study protocol was reviewed and approved by the Ethics Committee of Wuhan EyeGood Ophthalmology Hospital (No.AGQGSS-06).All patients were aware of the purpose and significance of this study and voluntarily signed the informed consent form.Results:The distance between the two peaks of the cornea at highest concavity (PD) was (5.49±0.28)mm in high myopia group, which was higher than (5.36±0.28)mm in low to moderate myopia and (5.05±0.29)mm in healthy control group, and the radius of curvature at highest concavity (HCR) and the stress-strain index (SSI) values in high myopia group were lower than those in low to moderate myopia and healthy control groups, with statistically significant differences (all P<0.05).The SC-T area and SC-N area were significantly higher and TM-T thickness was lower in high myopia group than those in low to moderate myopia and healthy control groups (all P<0.05).PD was positively correlated with SC-N area in myopic patients ( r=0.26, P=0.02).bIOP was negatively correlated with PD, the amount of corneal displacement at highest degree of concavity (DA), inverse concave radius (IR) and deformation amplitude ratio (DA ratio) ( r=-0.79, -0.81, -0.45, -0.50; all P=0.00) and positively correlated with stiffness parameters at applanation 1 (SP-A1) and SSI ( r=0.45, 0.46; all P=0.00). Conclusions:PD values are greater in myopic patients.The greater the PD, the greater the SC-N area.The greater the refractive error, the longer the AL, and the thinner the TM thickness.
9.Research progress on fecal microbiota transplantation for the treatment of hypertension

Hypertension is an important risk factor that causes cardiovascular diseases and premature death in China.The pathogenesis of hypertension is complex and affected by numerous factors.Gut microbiota are involved in regulating many physiological processes of human body,making it a key factor in human health.Many studies have shown that there are close relationships between gut microbiota dysbiosis and the onset and progression of hyper-tension.Restoring gut microbiota homeostasis can reduce blood pressure.Thus,gut microbiota has become a poten-tial therapeutic target for hypertension.Therefore,this article mainly reviews the connection between gut microbiota and hypertension,as well as the study progress of adjusting gut microbiota homeostasis for the prevention and treat-ment of hypertension.
10.Clinicopathological characteristics of HER2 low expression gastric adenocarcinoma

Purpose To explore the relationship between the proportion of HER2 low expression and clinicopatho-logical characteristics of gastric adenocarcinoma patients.Methods Clinical data from 3 779 gastric adenocarcinoma patients who underwent radical resection were collected.HER2 expression was categorized using immunohistochemistry combined with FISH,and the clinicopathological characteristics and prognosis of HER2 low expression gastric adeno-carcinoma patients were analyzed.Results Among the 3 779 gastric adenocarcinoma cases,1 251 cases(33.10%)showed HER2 low expression.Compared with HER2 negative expression,HER2 low expression patients were more likely to be older males,present with well-differentiated Lauren's intestinal-type adenocarcinoma,and have less nerve invasion.Furthermore,compared to HER2 negative expression,they showed higher Ki67 proliferation index and more advanced clinical stage.Meanwhile,compared with patients with HER2 over expression,those with low HER2 expres-sion were more likely to be younger females,present with well-differentiated Lauren's diffuse-type adenocarcinoma,and have more nerve invasion.Furthermore,compared to HER2 over expression,they showed lower Ki67 proliferation index and more advanced clinical stage.There was no significant difference in prognosis between HER2 low expression,HER2 negative,and HER2 over expression groups.Conclusion HER2 low expression have a relatively large propor-tion in gastric adenocarcinoma patients,and HER2-targeted antibody-drug conjugates may provide an effective treatment option for these patients with HER2 low expression.
