1.Effect of evolocumab early use after PCI on blood lipids and inflammatory factors in patients with ultra-high-risk ASCVD

Objective To investigate the effects of evolocumab in the early postoperative period after percutaneous coronary intervention(PCI)on blood lipids and inflammatory cytokines in the patients with ul-tra-high-risk atherosclerotic cardiovascular disease(ASCVD).Methods A total of 65 patients with ultra-high-risk ASCVD treated by PCI in this hospital from October 2022 to March 2023 were selected as the study subjects and divided into the observation group(n=33)and control group(n=32)according to the used drugs.The patients in the control group received the treatment of oral rosuvastatin 10 mg;on the basis of the treatment in the control group,the observation group was treated with subcutaneous injection of evolocumab 140 mg immediately after PCI.The fasting blood samples were collected before surgery,on postoperative 3 d,1,3,6 months,and the levels of inflammation and blood lipid indexes were detected and recorded.The occur-rences of cardiovascular adverse events(MACE)and other complications in postoperative 6 months were re-corded.Results The levels of LDL-C,Lp-PLA2 and lipoprotein a on postoperative 3 d,1,3,6 months in the observation group were gradually decreased,moreover were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of IL-6,homocysteine and hs-CRP on postoper-ative 3 d,1,3,6 months in the two groups were decreased,moreover the levels of various indexes in the obser-vation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence rate of MACE in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05);the difference in the incidence rates of other complications between the two groups was not statistically significant(P>0.05).According to the Log Rank test,the sur-vival rate of the observation group was higher than that of the control group,and the difference was statistical-ly significant(P<0.05).Conclusion The early application of evolocumab after PCI could reduce the blood lipid level and inflammatory cytokines levels of the patients with ultra-high-risk ASCVD,and increase their survival rate.
2.Association between Y Chromosome microdeletions and tes-ticular development in male pediatric patients with congenital reproductive system abnormalities.

OBJECTIVES:
To analyze the distribution of Y chromosome azoospermia factor (AZF) microdeletions and their association with testicular development in male pediatric patients with congenital reproductive system disorders, including hypospadias, cryptorchidism, and disorders of sex development (DSD).
METHODS:
A prospective cohort study was conducted on pediatric patients admitted to the Department of Urology of Shanghai Children's Hospital from November 2021 to December 2023. The observation group included boys with hypospadias, cryptorchidism, or DSD, while the control group comprised boys with phimosis, indirect inguinal hernia, or hydrocele. Blood samples were collected for AZF microdeletion analysis using multiplex PCR to detect 15 sequence-tagged sites. Testicular ultrasound was performed to record testicular position and volume. Propensity score matching (PSM) was used to balance the groups. After matching, testicular volume differences were assessed. Stratified analyses compared testicular volume among children with AZF microdeletions, the control group, and children without micro-deletions in observation group.
RESULTS:
A total of 493 children were enrolled (observation group: 463; control group: 30). No Y chromosome microdeletions were detected in the control group. Four boys in the observation group had AZF microdeletions: one with cryptorchidism (AZFc+AZFd), one with isolated hypospadias (AZFc), and two with DSD (one with AZFb+AZFc+AZFd and one with AZFa). Ultrasonography measured 888 testicles. After PSM, testicular volume was significantly smaller in the observation group than in the control group (P<0.01). Stratified analysis revealed that among children under 9 years, those with AZF microdeletions tended to be older but had smaller testicular volumes compared to the control group and those without microdeletions in the observation group, although differences were not statistically significant (all P>0.05). Among children over 9 years, ages were comparable, but children with AZF microdeletions had smaller testicular volumes than the other two groups (statistical analysis was not performed due to small sample size).
CONCLUSIONS
The prevalence of Y chromosome microdeletions is higher in male children with congenital reproductive system disorders compared to the general population, particularly in those with DSD. Hypospadias, cryptorchidism, DSD, and AZF microdeletions may be associated with delayed testicular development in these children.
3.The clinical significance of Th17 cell heterogeneity in myelodysplastic neoplasms

Objective To investigate the proportion of Th17 cells,Th1-like Th17 cells and FoxP3+Th17 cells in bone marrow of patients with myelodysplastic syndrome(MDS),the expression of interleukin-17A(IL-17A)in bone marrow supernatant and its clinical significance.Methods Forty MDS patients(MDS group)and 18 patients with nutritional anemia(control group)were selected.MDS patients were classified into the low blast(MDS-LB)group(19 cases)and the increased blast(MDS-IB)group(21 cases,including 11 cases of type IB1 and 10 cases of type IB2)based on morphological definition.The MDS patients were scored according to the revised International Prognostic Scoring System(IPSS-R),with 18 cases in the low-risk group(≤4.5)and 22 cases in the high-risk group(>4.5).Flow cytometry was used to detect the proportion of Th17 cells,Th1-like Th17 cells and FoxP3+Th17 cells in bone marrow of the MDS group and the control group.Enzyme-linked immunosorbent assay(ELISA)was used to detect the level of IL-17A in bone marrow supernatant of the above samples.Results The proportion of Th17 cells and the level of IL-17A were higher in patients of the MDS group than those in the control group(P<0.05).According to the median expression level of IL-17A,the MDS group was divided into the low-expression group(<13.71 ng/L,20 cases)and the high-expression group(≥13.71 ng/L,20 cases).Compared with the low-expression group,there were higher proportion of patients with blast cells<5%and low-risk patients(P<0.05)in the high-expression group.Compared with the IL-17A low-expression group,the IL-17A high-expression group had a higher proportion of patients with blast cells<5%and relatively low-risk patients(P<0.05).Compared with the low-risk patients,high-risk patients had a lower proportion of Th17 cells,IL-17A levels and Th1-like Th17 cells,and a higher proportion of FoxP3+Th17 cells(P<0.05).Compared with the MDS-LB group,the MDS-IB group had a lower proportion of Th17 cells,IL-17A levels and Th1-like Th17 cells,and a higher proportion of FoxP3+Th17 cells(P<0.05).Conclusion The proportion of Th17 cells and the level of IL-17A are significantly increased in MDS patients.The decreased proportion of Th1-like Th17 cells and the increased proportion of FoxP3+Th17 cells may be related to the increased proportion of blast cells and higher risk stratification in patients.
4.Effects of Early Postoperative Changes in Graft Viscoelasticity on Stress Distributions Within the Knee Joint Following Anterior Cruciate Ligament Reconstruction

Objective To investigate stress distributions of the knee joint at 0 and 15th day after anterior cruciate ligament reconstruction(ACLR)under a compressive force through the axis of the femoral shaft onto the proximal femur.Methods A three-dimensional(3D)finite element model of the human knee joint incorporating viscoelastic material properties was developed.The one-dimensional(1D)Prony series viscoelastic constitutive model parameters for articular cartilage,menisci,ligaments,and anterior cruciate ligament(ACL)grafts were determined by fitting experimental creep curves.The viscoelastic parameters of ACL grafts at 15th day after ACLR surgery were extrapolated.Finite element simulations were then performed to analyze the von Mises stress distributions in knee ligaments,ACL grafts,articular cartilage,and menisci under 1.5 kN vertical downward compressive load applied to the femur,with loading durations of 1 second and 600 seconds.Results At 15th day after ACLR surgery,the initial relaxation modulus and equilibrium modulus of human ACL grafts remained elevated compared to native ACL tissues,resulting in a significantly higher stress concentration within the grafts relative to healthy ACL.Despite the compromised mechanical properties of the grafts after ACLR surgery,the vertical downward compressive force applied to the femur under both short-term(1s)and prolonged(600 s)loading durations,exhibited a minimal biomechanical impact on articular cartilage and meniscal structures.Conclusions Following ACLR,vertical compressive loads during weight-bearing rehabilitation exercises such as standing demonstrate minimal impact on articular cartilage and meniscus,while promoting fibrous regeneration of the graft.This renders such exercises a prudent early-stage rehabilitation strategy.Graft preparation requires balanced consideration of elastic and viscous properties,with grafts exhibiting higher relaxation modulus and viscosity coefficient than healthy ACL proving more effective in maintaining early postoperative knee stability.
5.Inpatients′ needs on medical nursing assistants: a qualitative user persona study

Objective:To understand the experience of inpatients′ needs on medical nursing assistants, and explore the characteristics and population differences, in order to provide a reference for optimizing the inpatient companion management system.Methods:The descriptive qualitative study was adopted for semi-structured in-depth interviews of 19 inpatients hospitalized in Zhongshan Hospital Fudan University from December 2023 to January 2024 based on purposive sampling. Information on personal characteristics, status and experience of dependence on medical nursing assistants was collected. Colaizzi seven-step analysis method was used to construct the fact labels and model dimensions. Manually extract the characteristics of inpatients′ needs on medical nursing assistants, and construct a user persona to complete the visualization.Results:A total of 19 inpatients′ were interviewed, including 12 males and 7 females with an average age of (63.16 ± 11.56) years. Four dimensions were extracted including cognitive characteristics, health status characteristics, family support characteristics, and psychological characteristics. Combined with personal traits and experience, three types of typical user persona were constructed including independent type, mutual assistance and cooperation type, complete dependence type.Conclusions:Group heterogeneity existed in the demand of inpatients on medical nursing assistants. According to the user persona, the manager should formulate personalized and precise caregiver management strategies.

9.Expression and significance of PIK3CA,p-AKT and PTEN in sinonasal squamous cell carcinoma

Objective To investigate the expression of PIK3CA,phosphorylated protein kinase B(p-AKT)and phosphatase and tensin homologue deleted on chromosome 10(PTEN)in sinonasal squamous cell carcinoma(SNSCC).Methods The expressions of PIK3CA and PTEN in head and neck squamous cell carci-noma(HNSCC)were analyzed through the data set of HNSCC in the cancer genome map of UCSC Xena data-base.The immunohistochemical SP method was used to measure the expression of PIK3CA,p-AKT and PTEN in 43 cases of SNSCC tissues,20 cases of normal inferior concha tissues.The relationship between the expressions of PIK3CA,p-AKT and PTEN protein with the clinicopathological features and prognosis of the patients with SNSCC was analyzed.Results The results of bioinformatic analysis showed that PIK3CA mR-NA expression in HNSCC tissues was higher than that in paracancerous tissues(P<0.01),while the PTEN mRNA expression was lower than that in paracancerous tissues(P<0.05).The immunohistochemical detec-tion results showed that the positive expressions rates of PIK3CA and p-AKT proteins in normal nasal mucosa tissues were significantly lower than those in SNSCC tissues,while the positive expression rate of PTEN pro-tein in SNSCC tissues was significantly higher than that in normal inferior nasal concha mucosa tissues,and the differences were statistically significant(P<0.01).The expressions of PIK3CA and p-AKT protein were related to the clinical stage,differentiation degree and primary site(P<0.05),but were not related to age,gender,smoking and drinking(P>0.05);the PTEN protein expression was not related with the clinical stage,differentiation degree,primary site,age,smoking and drinking(P>0.05).The Spearman analysis showed that the expression of PIK3CA in SNSCC tissues was positively correlated with p-AKT protein ex-pression(r=0.664,P<0.01),and PIK3CA was negatively correlated with PTEN protein(r=-0.414,P<0.01).The expression of p-AKT was negatively correlated with PTEN protein(r=-0.453,P<0.01).The Kaplan-Meier analysis showed that the median survival time of the patients with PIK3CA and p-AKT protein positive expression was shorter than that of the patients with negative expression(P<0.01).There was no statistically significant difference in median survival between the patients with PTEN protein positive expres-sion and those with negative expression.Conclusion The overexpressions of PIK3CA and p-AKT accompa-nied by the loss of PTEN expression participate in the development and progression of SNSCC,moreover the PIK3CA and p-AKT expressions are related to the poor prognosis of the patients.
10.Clinical significance of HOXB4 gene expression levels in myelodysplastic syndromes

Objective To investigate the expression of HOXB4 gene in patients with myelodysplastic syn-dromes(MDS)and its clinical significance in disease progression.Methods mRNA expression of HOXB4 gene in bone marrow mononuclear cells was detected by real-time fluorescence quantitative PCR(RT-qPCR),and the difference in HOXB4 expression was compared between 49 patients with MDS(MDS group)and 35 patients without MDS(group C).The relationship of mRNA expression of HOXB4 with disease characteristics and clinical prognosis was explored in MDS patients.Results mRNA expression level of HOXB4 gene was higher in MDS group than that in group C(P<0.05).The patients were divided into a high-and a low-expression group according to the median expression level of HOXB4.Leukocyte count was lower in the high-expression group in the low-expression group at the time of initial diagnosis.The proportion of patients with subtypes of primitive cellular hyperplasia,poor prognostic staging and leukemic transformation was higher in the high-expression group than in the low-expression group.Conclusions mRNA expression level of HOXB4 gene has certain relation with AML transformation in MDS patients.
