To explore the advantages of traditional Chinese medicine (TCM) and integrative TCM-Western medicine approaches in the treatment of diabetic microvascular complications (DMC), refine key pathophysiological insights and treatment principles, and promote academic innovation and strategic research planning in the prevention and treatment of DMC. The 38th session of the Expert Salon on Diseases Responding Specifically to Traditional Chinese Medicine, hosted by the China Association of Chinese Medicine, was held in Beijing, 2024. Experts in TCM, Western medicine, and interdisciplinary fields convened to conduct a systematic discussion on the pathogenesis, diagnostic and treatment challenges, and mechanism research related to DMC, ultimately forming a consensus on key directions. Four major research recommendations were proposed. The first is addressing clinical bottlenecks in the prevention and control of DMC by optimizing TCM-based evidence evaluation systems. The second is refining TCM core pathogenesis across DMC stages and establishing corresponding "disease-pattern-time" framework. The third is innovating mechanism research strategies to facilitate a shift from holistic regulation to targeted intervention in TCM. The fourth is advancing interdisciplinary collaboration to enhance the role of TCM in new drug development, research prioritization, and guideline formulation. TCM and integrative approaches offer distinct advantages in managing DMC. With a focus on the diseases responding specifically to TCM, strengthening evidence-based support and mechanism interpretation and promoting the integration of clinical care and research innovation will provide strong momentum for the modernization of TCM and the advancement of national health strategies.

Objectives:The aim is to analyze the cost structure and coefficient of variation for hospitalized patients with malnutrition based on Diagnosis-Related Groups(DRG),providing a reference for the further application and promotion of DRG.Method:Data were collected from patients admitted to Ningxia Hui Autonomous Region People's Hospital between March 2023 and August 2023.A diagnostic system based on artificial intelligence was used to identify malnourished patients. The composition of hospitalization costs for these individuals was described and analyzed,as was the coefficient of variation for various costs within DRG groupings.A multivariate regression analysis was conducted to identify the factors that influence patient hospitalization costs. Results:The average age of hospitalized patients with malnutrition was(68.12±16.43)years,with an average length of stay of(14.55±8.47)days,with an average hospitalization cost of(32 128.89±35 345.61)yuan.Among patients within the same DRG group, the coefficient of variation for various costs was found to be lower in the malnutrition group than in the normal group. This suggests that when assessed individually, the malnutrition group exhibited a higher degree of homogeneity in their cost structures. The factors influencing total hospitalization costs were found to be:length of hospital stay(P=0.001),nutritional monitoring fees(P=0.020),number of chronic diseases(P=0.003),and Karnofsky Performance Status(KPS)score(P=0.038).Hospitalization costs were positively correlated with both length of stay and nutritional assessment fees, but negatively correlated with the number of chronic diseases and KPS scores. Conclusions:Malnutrition has a profound impact on health outcomes, medical expenses, length of hospital stay, and disease severity. The implementation of the DRG system aims to standardize and improve the nutritional diagnosis and treatment process by categorizing different stages of malnutrition. This approach can minimize variations within DRG groups, making it easier to allocate medical resources more precisely and efficiently.Furthermore,it is a valuable reference tool for promoting DRG payment reform in different regions.

Chinese Journal of Digestive Endoscopy 2025;42(5):391-395
Objective:To assess the effectiveness of the combined biological therapy and endoscopic balloon dilation (EBD) for preventing intestinal stricture recurrence in patients with Crohn disease, and to identify the risk factors for post-EBD stricture recurrence. Methods:This retrospective cohort study enrolled Crohn disease patients who underwent EBD with or without biological therapy at the Department of Gastroenterology, the First People's Hospital of Changzhou, from January 2016 to December 2023. The patients were divided into the biologics-EBD group and the EBD monotherapy group, and recurrence rates of intestinal stenosis between the groups were compared. The Kaplan-Meier method was employed to estimate the stricture-free survival, with intergroup differences assessed via log-rank test. The Cox proportional hazards regression model was utilized to perform a multivariate analysis of the survival data of the included cases for the independent risk factors for the recurrence of intestinal strictures after EBD in Crohn disease. Results:In accordance with the inclusion and exclusion criteria, a total of 47 cases were ultimately included in the study, with 29 cases in the biologics-EBD group and 18 cases in the EBD monotherapy group. The baseline data of the two groups were comparable. Stricture recurrence occurred in 6/29 (20.7%) biologics-EBD patients versus 10/18 (55.6%) EBD monotherapy patients. Kaplan-Meier curves demonstrated superior cumulative stricture-free survival in the biologics-EBD cohort ( P=0.014). Cox proportional hazards regression model confirmed elevated recurrence risk with EBD monotherapy ( HR=5.360, 95% CI: 1.340-21.449, P=0.018) and small intestinal strictures ( HR=7.746, 95% CI: 1.908-31.446, P=0.004). Conclusion:The combination of biologics with EBD for the treatment of intestinal strictures in Crohn disease can effectively prevent the recurrence of intestinal strictures. Regarding small intestinal strictures in Crohn disease, it is suggested to combine biologics for treatment to prevent the high recurrence rate of strictures.

Copy number variation encompassing SNP plays an important role in IVD and precision medi-cine.As the most commonly used method,FISH could not overcome the probe cross-reactivity which is common when to detect SNP.Here we developed a quantitative and qualitative method on copy number variation encompassing SNP.In this study, the rs76711854 was used as an example to establish a quanti-tative method by advantage of probe cross-reactivity.The fragment encompassing rs76711854 and its downstream to 9 514 bp were amplified by PCR.The allelic genotypes were verified by Sanger sequen-cing.Different probes with or without cross-reactivity to be used via quantitative real-time PCR and digit-al PCR.The different clusters(2D)and fluorescence intensity layers(1D)exist by adding probe with cross-reactivity.The A/G ratio measured by digital PCR is 2︰1,which is verified by probe targeting to the SNP.The copy-number variant exists in the 9kb-long fragment upstream to the SNP of prostate cancer cell line but not in human endometrial adenocarcinoma cell line Ishikawa. The data suggest that there is a multi-copy variation at this locus in DU145 cells. The method applied here is based on one single cross-reactivity probe via digital PCR.
