1.Latent profile analysis and influencing factors of parental fear of progression in school-age children with asthma

Objective:To explore the current situation of fear of progression among parents of school-age children with asthma, and to analyze the latent profile categories and influencing factors, so as to provide the reference for the formulation of prevention and intervention.Methods:Using convenient sampling method, school-age children with asthma and their parents treated in the Affiliated Children's Hospital of Xiangya School of Medicine, Central South University from March to August 2023 were selected as the study objects. The General Data Questionnaire, Parent Fear of Progression Scale, Childhood Asthma Control Test, and Social Support Rating Scale were used to investigate school-age children with asthma and their parents. Latent profile analysis was performed on the fear of disease progression among parents of school-age children with asthma, and χ2 test and binary Logistic regression were used to explore the influencing factors. Results:A total of 210 parents of the children were included with 51 fathers and 159 mothers.The age distribution was 20-30 years old with 42 parents, 31-40 years old with 112 parents, 41-50 years old with 56 parents. In 210 children with asthma, there were 123 males and 87 females, with 141 cases aged<10 years old and 69 cases aged ≥10 years old. The fear of progression among parents of school-age children with asthma could be divided into two potential profile categories: "psychological dysfunction group (46.7%, 98/210)" and "psychological health good group (53.3%, 112/210)". The results of binary Logistic regression analysis showed that asthma family history, combined allergic rhinitis, child-parent relationship, marital status, asthma symptom control level, and social support level were the influencing factors of fear of progression among parents of school-age children with asthma( OR values were 0.925-10.010, all P<0.05). Conclusions:The fear of progression among parents of school-age children with asthma is at a moderate level, and there are obvious different classification characteristics. In the future, targeted and precise prevention and intervention should be implemented according to the category characteristics of fear of progression among parents of school-age children with asthma.
2.Visualization analysis of pulmonary rehabilitation research based on Web of Science and China National Knowledge Infrastructure

Objective:To analyze the related literature of pulmonary rehabilitation research at home and abroad, understand its research focus and development trend, and provide a reference and basis for future pulmonary rehabilitation research.Methods:Literature related to pulmonary rehabilitation published in the database of Web of Science and China National Knowledge Infrastructure from January 2015 to February 2022 was retrieved, and the contents such as the number of articles published annually, authors, research institutions, and high-frequency keywords were visually analyzed by bibliometrics and CiteSpace software.Results:The number of articles published each year showed an obvious upward trend from 2015, and the number of Chinese documents was higher than that of English documents from 2019 to 2020. MARTIJN A SPRUIT was the author with the largest number of articles published in English, with a total of 65 articles published, with close cooperation among scholars. Che Guowei was the author with the largest number of articles published in Chinese literature, with 13 articles published in total. The cooperation among the authors was relatively lacking and scattered. The main research institutions abroad were universities, and the institution with the highest number of papers was Maastricht University in the Netherlands, with a total of 85 articles. The research institutions cooperated closely and formed a cooperation network. Hospitals were the main research institutions in China, and the thoracic surgery Department of West China Hospital of Sichuan University had the highest total number of publications, with 14 articles. The cooperation among institutions was not close, and there was a lack of cooperation with universities and scientific research institutions. Through keyword co-occurrence and clustering and timeline view analysis, it was concluded that the research hotspots in this field were pulmonary rehabilitation of patients with different chronic respiratory diseases, the influence of pulmonary rehabilitation on lung function and quality of life of patients with chronic obstructive pulmonary disease, and the intervention methods of pulmonary rehabilitation. The future trend was predicted as the clinical application value of pulmonary rehabilitation in the treatment of lung cancer.Conclusions:At present, pulmonary rehabilitation research is in a stable development period. However, the cooperation between domestic authors and institutions is not close enough compared with foreign countries. In the future, China should strengthen the communication and cooperation between research teams, and learn from foreign research results to further develop the application of pulmonary rehabilitation in lung cancer patients, to promote the development of pulmonary rehabilitation research in China.
3.Clinical and genetic characteristics of 9 rare cases with coexistence of dual genetic diagnoses.

Objective: To analyze the clinical and genetic characteristics of pediatric patients with dual genetic diagnoses (DGD). Methods: Clinical and genetic data of pediatric patients with DGD from January 2021 to February 2022 in Peking University First Hospital were collected and analyzed retrospectively. Results: Among the 9 children, 6 were boys and 3 were girls. The age of last visit or follow-up was 5.0 (2.7,6.8) years. The main clinical manifestations included motor retardation, mental retardation, multiple malformations, and skeletal deformity. Cases 1-4 were all all boys, showed myopathic gait, poor running and jumping, and significantly increased level of serum creatine kinase. Disease-causing variations in Duchenne muscular dystrophy (DMD) gene were confirmed by genetic testing. The 4 children were diagnosed with DMD or Becker muscular dystrophy combined with a second genetic disease, including hypertrophic osteoarthropathy, spinal muscular atrophy, fragile X syndrome, and cerebral cavernous malformations type 3, respectively. Cases 5-9 were clinically and genetically diagnosed as COL9A1 gene-related multiple epiphyseal dysplasia type 6 combined with NF1 gene-related neurofibromatosis type 1, COL6A3 gene-related Bethlem myopathy with WNT1 gene-related osteogenesis imperfecta type XV, Turner syndrome (45, X0/46, XX chimera) with TH gene-related Segawa syndrome, Chromosome 22q11.2 microduplication syndrome with DYNC1H1 gene-related autosomal dominant lower extremity-predominant spinal muscular atrophy-1, and ANKRD11 gene-related KBG syndrome combined with IRF2BPL gene-related neurodevelopmental disorder with regression, abnormal movement, language loss and epilepsy. DMD was the most common, and there were 6 autosomal dominant diseases caused by de novo heterozygous pathogenic variations. Conclusions: Pediatric patients with coexistence of double genetic diagnoses show complex phenotypes. When the clinical manifestations and progression are not fully consistent with the diagnosed rare genetic disease, a second rare genetic disease should be considered, and autosomal dominant diseases caused by de novo heterozygous pathogenic variation should be paid attention to. Trio-based whole-exome sequencing combining a variety of molecular genetic tests would be helpful for precise diagnosis.

4.MaxEnt model-ArcGIS-based evaluation of habitat suitability of Hedysari Radix

AIM To evaluate the habitat suitability of Hedysari Radix based on MaxEnt model and ArcGIS.METHODS Using the MaxEnt model to screen the ecological factors affecting the distribution of Hedysari Radix,an evaluation model was thus established.ArcGIS software was used to evaluate the ecological suitability of Hedysari Radix to obtain the data about the highly suitable area,the moderate suitable area,the low suitable area and non-suitable area for its growth in China.RESULTS Hedysari Radix found its 1.29×106 km2 suitable area in China,among which the highly suitable area was 5×104 km2,mainly in Gansu Province,the moderately suitable area was 3.38×105 km2,and the low-suitable area was 9×105 km2,occupying 4.03%,26.20%and 69.77%of all,respectively.The main ecological factors affecting the distribution of Hedysari Radix were determined to be altitude,precipitation in the hottest quarter,solar radiation in September and December,seasonal temperature variation deviation and basic saturation of upper soil(0-30 cm).CONCLUSION With its result complying well with the literature records,this study provides theoretical basis for the introduction and cultivation of Hedysari Radix,and sustainable utilization of resources as well.
5.Protective Mechanism of Total Flavonoids of Lavender Against Skin Photoaging in Mice

ObjectiveTo study the protective effect of total flavonoids of lavender on skin photoaging induced by ultraviolet B (UVB) in mice and to explore its mechanism from the perspective of nuclear factor E2-related factor 2 (Nrf2) antioxidant pathway. MethodEighty-four female KM mice were randomly divided into seven groups, namely blank group, model group, solvent group, vitamin E (0.013 g·kg-1) group, as well as low-, middle-, and high-dose (0.25, 1.25, 2.50 g·kg-1) groups of total flavonoids of lavender. The naked skin on the back of mice was irradiated with UVB for inducing optical damage. Thirty minutes before irradiation, the skin was coated with the total flavonoids of lavender. After continuous irradiation for one week, the skin moisture and elasticity on the back of mice were evaluated, and the effects of total flavonoids of lavender on histopathological changes in mouse skin were investigated by hematoxylin-eosin (HE) and Van Gieson (VG) staining. The levels of malondialdehyde (MDA), superoxide dismutase (SOD), total antioxidant capacity (T-AOC), nitric oxide synthase (NOS), and glutathione peroxidase (GSH-Px) after skin homogenization were detected by colorimetry, the inflammatory factors interleukin-1 (IL-1), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in skin tissue by enzyme-linked immunosorbent assay (ELISA), and the mRNA expression levels of Nrf2, Kelch-like epichlorohydrin-associated protein 1 (Keap1), BTB-CNC homology 1 (Bach1), heme oxygenase-1 (HO-1), quinone oxidoreductase 1 (NQO1), and glutamate-cysteine ligase catalytic subunit (GCLC) by real-time polymerase chain reaction (Real-time PCR). ResultCompared with the blank group, the model group exhibited significantly increased appearance score (P<0.01), reduced skin moisture and elasticity (P<0.01), pronounced pathological changes in the skin tissue like epidermal thickening, scabbing, small abscess, and severe injury, elevated MDA, NOS, IL-1, IL-6 and TNF-α (P<0.05, P<0.01), lowered SOD, T-AOC, Nrf2, Keap1, NQO1 and GCLC mRNA expression (P<0.05,P<0.01), and up-regulated Bach1 mRNA expression (P<0.01). Compared with the model group, the total flavonoids of lavender at the low, middle, and high doses all remarkably reduced the appearance score (P<0.01), enhanced the skin moisture and elasticity (P<0.01), diminished the MDA, NOS, IL-1, IL-6, and TNF-α (P<0.05, P<0.01), increased SOD, T-AOC, Nrf2, Keap1, NQO1, HO-1 and GCLC mRNA expression (P<0.05, P<0.01), and down-regulated the expression of Bach1 mRNA (P<0.01). ConclusionThe protective effect of the total flavonoids of lavender against skin photoaging in mice is significant, which may be related to its activation of Keap1/Nrf2/ARE signaling pathway, regulation of oxidative stress, and improvement of inflammatory response.
6.Liuwei Dihuangtang Alleviates Myelin Injury in Anterior Cingulate Cortex and Depression-like Behaviors of Rats with Diabetes Mellitus and Depression via Promoting Autophagy and Regulating Microglial Phenotype

ObjectiveTo observe the effect of Liuwei Dihuangtang (LWDHT) on depression-like behaviors of rats with diabetes mellitus and depression (DD) and explore its mechanism. MethodThe diabetes mellitus (DM) model was induced by the high-fat diet and tail vein injection of low-dose streptozotocin (STZ) in 50 male Sprague-Dawley rats of SPF grade. Then the DD model was induced by chronic unpredictable mild stress (CUMS) for 28 days in DM rats. Fifty DD rats were randomly divided into model group, fluoxetine group (10 mg·kg-1·d-1), and low-, medium-, and high-dose LWDHT groups (3.375, 6.75, 13.5 g·kg-1·d-1), with 10 rats in each group. Another 10 healthy rats were assigned into a control group and received normal saline by gavage. After four weeks of drug intervention, the forced swimming assay was carried out to assess the depression-like behaviors of rats. The expression levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), interleukin-4 (IL-4), and interleukin-10 (IL-10) in the anterior cingulate cortex (ACC) were detected by enzyme-linked immunosorbent assay (ELISA). Immunofluorescence was used to detect the expression of myelin basic protein (MBP) in ACC and the co-localization of ionized calcium-binding adapter molecule 1 (Iba1) with intracellular microtubule-associated protein 1 light chain 3 (LC3). The protein expression levels of MBP, myelin proteolipid protein (PLP), myelin oligodendrocyte glycoprotein (MOG), Beclin-1, LC3, p62, and microglia (MG) phenotypic protein-related inducible nitric oxide synthase (iNOS), and arginase 1 (Arg1) were detected by Western blot. ResultCompared with the control group, the model group showed shortened swimming time and prolonged immobility time (P<0.01). Compared with the model group, the medium- and high-dose LWDHT groups showed reduced immobility time (P<0.05, P<0.01). Compared with the control group, the model group showed decreased protein expression of MBP, PLP, and MOG in the ACC region (P<0.01). Compared with the model group, the fluoxetine group and the medium- and high-dose LWDHT groups showed up-regulated protein expression of MBP, PLP, and MOG (P<0.05, P<0.01). Compared with the control group, the model group showed decreased MBP fluorescence intensity in the ACC region (P<0.01). Compared with the model group, the fluoxetine group and the medium- and high-dose LWDHT groups showed increased MBP fluorescence intensity in the ACC region (P<0.05, P<0.01). Compared with the control group, the model group showed increased expression of iNOS (P<0.01) and slightly increased Arg1 protein expression. Compared with the model group, the medium- and high-dose LWDHT groups and the fluoxetine group showed down-regulated iNOS expression and up-regulated Arg1 protein expression (P<0.05, P<0.01), but there was no significant difference between the fluoxetine group and the medium-,high-dose LWDHT groups. Compared with the control group, the model group showed increased expression levels of proinflammatory factors IL-1β and TNF-α in the ACC region (P<0.01) and slightly increased expression levels of anti-inflammatory factors IL-4 and IL-10. Compared with the model group, the fluoxetine group, and the medium- and high-dose LWDHT groups showed down-regulated expression of IL-1β and TNF-α (P<0.05, P<0.01) and up-regulated expression of IL-4 and IL-10 (P<0.05, P<0.01). Compared with the control group, the model group showed reduced expression levels of Beclin-1 and LC3Ⅱ (P<0.01) and increased expression level of p62 (P<0.01). Compared with the model group, the fluoxetine group and the medium- and high-dose LWDHT groups showed up-regulated Beclin-1 and LC3Ⅱ expression (P<0.01) and down-regulated p62 expression (P<0.01). Compared with the control group, the model group showed decreased LC3+Iba1+ cells in the ACC region (P<0.01). Compared with the model group, the fluoxetine group and the medium- and high-dose LWDHT groups showed increased LC3+Iba1+ cells (P<0.05, P<0.01). ConclusionLWDHT can alleviate the depression-like behaviors in DD rats presumedly by promoting MG autophagy, regulating MG phenotypic changes, and increasing MG clearance of myelin sheath fragments. Meanwhile, MG phenotypic transformation also inhibits ACC inflammation in DD rats, improves the local microenvironment of oligodendrocyte proliferation and differentiation, and ultimately promotes the repair and remyelination of damaged myelin sheath.

The Singapore Health Services cluster (SingHealth) radiology film archives are a valuable repository of local radiological cases dating back to the 1950s. Some of the cases in the archives are of historical medical interest, i.e. cerebral angiography in the workup of patients with hemiplegia. Other cases are of historical social interest, being conditions seen during earlier stages of Singapore's development, i.e. bound feet. The archives form a unique portal into the development of local radiology as well as the national development of Singapore. A selection from the archives is published in commemoration of the International Day of Radiology in 2020, as well as the 200th anniversary of the Singapore General Hospital in 2021. This pictorial essay comprises gastroenterology, musculoskeletal and obstetrics and gynaecology cases from the archives.

10.Effects of care and nursing practice plan based on timing theory in primary caregivers of infants with infantile spasms

Objective:To explore the effect of the care and nursing practice plan based on the timing theory in primary caregivers of infants with infantile spasms.Methods:From July 2019 to June 2020, 80 infants with infantile spasm and 80 primary caregivers in the Department of Neurology of Hunan Children's Hospital were selected as the research object. All infants with infantile spasms were numbered according to the time of admission. The odd-numbered infants were admitted to the First Department of Neurology as the observation group, and the even-numbered children were admitted to the Second Department of Neurology as the control group, with 40 infants and 40 primary caregivers in each group. The control group carried out conventional treatment and nursing, and the observation group implemented a care and nursing practice plan based on the timing theory on the basis of the control group. Before and after the intervention, the Caring Ability Inventory (CAI) , Family Caregiver Task Inventory (FCTI) , Caregiver Burden Inventory (CBI) , and Gesell Developmental Schedules (GDS) were used to evaluate the caring ability, caregiving ability, care load of the primary caregivers of the two groups of infants, and the cognition function of the two groups of infants.Results:After the intervention, the total CAI scores of the primary caregivers in the observation group were higher than those before intervention and those in the control group after the intervention, and the total scores of FCTI and CBI were lower than those before intervention and those in the control group after the intervention, and the differences were statistically significant ( P<0.05) . After the intervention, the developmental quotient (DQ) values of the five energy areas of GDS in the observation group were higher than those before the intervention, and the DQ values of the three energy areas of gross motor, fine motor, and language were higher than those of the control group after the intervention, and the differences were statistically significant ( P<0.05) . Conclusions:The care and nursing practice plan based on the timing theory can improve the caring ability and caregiving ability of the main caregivers of children with infantile spasms, reduce the care load, and improve the cognitive function of the children.
