1.Interventional effect and mechanism of Bifidobacterium in chronic liver disease

Compared with traditional therapies for chronic liver disease (CLD), Bifidobacterium has the characteristics of multi-target intervention, high biosafety, and good host compatibility and provides new strategies for intervention of CLD progression in terms of microecological regulation. Various studies have shown that Bifidobacterium regulates liver homeostasis and exerts a therapeutic effect on CLD by regulating intestinal flora, maintaining antioxidation, promoting energy consumption, alleviating inflammation, improving glycolipid metabolism, and exerting an antitumor effect. This article systematically reviews the studies on Bifidobacterium in the treatment of CLD in China and globally, explores their different mechanisms, and elaborates on the interaction between related signaling pathways (such as the nuclear factor erythroid 2-related factor 2 signaling pathway and the adenosine monophosphate-activated protein kinase signaling pathway) and the liver, in order to provide a basis for probiotic intervention in liver pathology, as well as new ideas for the comprehensive treatment of CLD.
2.Effect and mechanisms of highly active umbilical cord mesenchymal stem cells on aging spleen in elderly tree shrews

BACKGROUND:Spleen has the functions of blood storage,hematopoiesis,and immunity.With the increase of age,the structural degeneration and functional decline of spleen lead to the impairment of immune system function,thus accelerating the aging process of the body.The treatment of spleen aging in tree shrews with highly active umbilical cord mesenchymal stem cells has not been reported. OBJECTIVE:To explore the intervention effect and mechanism of highly active umbilical cord mesenchymal stem cells on spleen aging in tree shrews. METHODS:Highly active umbilical cord mesenchymal stem cells were isolated,cultured,and obtained from the umbilical cord tissue of newborn tree shrews by caesarean section.The differentiation abilities of adipogenesis,osteogenesis,and chondrogenesis were detected by three-line differentiation kit.Cell cycle and surface markers were detected by flow cytometry.The second generation of highly active umbilical cord mesenchymal stem cells were transfected with Genechem Green Fluorescent Protein with infection complex values of 100,120,140,160,180,and 200,respectively,to screen the best transfection conditions.After transfection,the fourth generation of highly active umbilical cord mesenchymal stem cells was injected into the tail vein of tree shrews in the elderly treatment group.The young control group and the aged model group were not given special treatment.After 4 months of treatment,the spleen tissue was taken and the structure of the spleen was observed by hematoxylin-eosin staining.β-Galactosidase staining was used to detect the activity of aging-related galactosidase.Immunohistochemical staining was used to detect the expression levels of p21 and p53 proteins.Ki67 and PCNA immunofluorescence staining was used to detect cell proliferation activity.Immunofluorescence staining was used to detect the expression levels of spleen autophagy protein molecules Beclin 1 and APG5L/ATG5.Reactive oxygen species fluorescence staining was used to detect the content of reactive oxygen species in spleen tissue.CD3 immunofluorescence staining was used to detect the change of the proportion of total T lymphocytes.The secretion levels of interleukin 1β and transforming growth factor β1 in spleen were detected by enzyme linked immunosorbent assay.The distribution of highly active umbilical cord mesenchymal stem cells labeled with green fluorescent protein in spleen tissue was observed by DAPI double staining of nucleus. RESULTS AND CONCLUSION:(1)Highly active umbilical cord mesenchymal stem cells grew in a short spindle shape with fish-like growth,with a large proportion of G0/G1 phase,and had the potential to differentiate into adipogenesis,osteogenesis,and chondrogenesis.(2)Multiplicity of infection=140 and transfection for 72 hours were the best conditions for labeling tree shrews highly active umbilical cord mesenchymal stem cells with Genechem Green Fluorescent Protein.(3)Compared with the aged model group,in the aged treatment group,the spleen tissue cells of tree shrews were arranged closely,and the area of white pulp was increased(P<0.01);the boundary between red pulp and white pulp was clear;the proportion of germinal centers did not show statistically significant difference(P>0.05).The activity level of galactosidase related to spleen tissue aging was decreased(P<0.001),and the expression levels of aging protein molecules p21 and p53 were down-regulated(P<0.001).The expression levels of proliferation-related molecules Ki67 and PCNA were up-regulated(P<0.001,P<0.05);expression levels of autophagy-related molecules Beclin 1 and APG5L/ATG5 were up-regulated(P<0.001),and the content of reactive oxygen species decreased(P<0.001),and the proportion of CD3+T cells increased(P<0.05).The secretion level of interleukin 1β in the aging-related secretion phenotype decreased(P<0.001);no significant difference was found in transforming growth factor β1 level(P>0.05).Compared with the young control group,the above indexes were significantly different in the elderly treatment group(P<0.05).(4)Green fluorescent cells labeled with green fluorescent protein were observed in spleen tissue of tree shrews the elderly treatment group by frozen tissue section observation.The results show that intravenous infusion of highly active umbilical cord mesenchymal stem cells can migrate to spleen tissue,inhibit the production of reactive oxygen species,down-regulate the expression of aging-related proteins,induce autophagy,promote cell proliferation,reduce chronic inflammation,and then improve the structure and function of spleen tissue.
3.Immunomodulatory effect of umbilical cord mesenchymal stem cells on type 2 diabetes mellitus

BACKGROUND:Umbilical cord mesenchymal stem cells present immunomodulatory function and multidirectional differentiation potential.Umbilical cord mesenchymal stem cells can actively improve the chronic low-grade inflammation state in type 2 diabetes mellitus and which is important for preventing the development of type 2 diabetes mellitus progression.OBJECTIVE:To review the research progress of umbilical cord mesenchymal stem cells in the treatment of type 2 diabetes mellitus through immunomodulatory effect.METHODS:Using"umbilical cord mesenchymal stem cells,UC-MSCs,mesenchymal stem cells,MSCs,type 2 diabetes mellitus,T2DM,immunity,insulin resistance,inflammation"as Chinese and English search terms,articles were searched on CNKI,Web of Science and PubMed databases.Relevant reviews,research papers and theses were screened and 88 papers were collected for summarization based on the inclusion and exclusion criteria.RESULTS AND CONCLUSION:Umbilical cord mesenchymal stem cells can effectively delay the occurrence and development of type 2 diabetes mellitus by reducing inflammatory response,inducing macrophages to M2 polarization,increasing the sensitivity of target organs to insulin,improving insulin resistance,and alleviating the dysfunction of islet beta cells through immune regulation.However,before umbilical cord mesenchymal stem cell transplantation can become a routine treatment,large-scale basic and applied basic research is needed to determine the optimal treatment regimen and efficacy.
4.Development of a risk prediction model for cancer-related cognitive impairment in lung cancer patients from the perspective of precision health in nursing science

Objective:Predictive modelling of risk of cancer-related cognitive impairment (CRCI) in lung cancer patients from the perspective of precision health in nursing science.Methods:Prospectively collected lung cancers treated in the Department of Respiratory Medicine of Affiliated Nanjing Brain Hospital, Nanjing Medical University from October 2023 to April 2024 as study subjects by a convenience samphing method. Lasso regression was used to screen the characteristic variables and construct the prediction model, and the predictive ability was evaluated by the AUC of the subjects′operating characteristics; Bootstrap resampling (1 000 times) internal validation of the model; the Hosmer-Lemeshow goodness-of-fit test was performed and the calibration curve was plotted to evaluate the calibration of the model; the clinical validity of the model was evaluated by decision cure analysis (DCA).Results:A total of 142 patients with lung cancer were included, 94 males and 38 females. The incidence of CRCI in lung cancer patients was 69.7%(99/142). Lasso regression showed that age(≥65), education, tumor stage, serum albumin, and PLR were independent risk factors for CRCI (coefficients of 0.372 048 72, - 0.361 265 78, 0.068 728 00, - 0.039 940 32, 0.001 639 92 respectively). The model AUC was 0.874 (95% CI: 0.815-0.933), with a sensitivity of 0.768, and a specificity of 0.860; the H-L goodness-of-fit test showed good agreement ( χ2 = 4.51, P>0.05), and Bootstrap re-sampling internal validation showed an AUC of 0.826. Calibration curves showed good agreement and accuracy between the model predicted probabilities and the actual observed probabilities. DCA showed that the model had clinical benefit when the threshold probability was approximately>25%. Conclusions:The CRCI column-line diagram risk model constructed in this study has good predictive efficacy and can effectively predict the occurrence of CRCI in lung cancer patients.
5.Implementation and impact of perioperative management based on ERAS philosophy in the robot-assisted surgery for thoracolumbar fracture

Objective:To study the application effect of perioperative management based on enhanced recovery after surgery(ERAS)philosophy in the robot-assisted surgery for thoracolumbar fracture.Methods:This was a prospective random control trial(RCT).A total of 80 patients with thoracolumbar fracture admitted to Jiangsu Provincial People's Hospital from January 2020 to January 2025 were selected.They were divided into control group and ERAS group according to random number method,with 40 cases in each group.All of them adopted robot-assisted surgery for thoracolumbar fracture as the main therapeutic scheme.After surgery,the control group adopted routine perioperative management,and the ERAS group adopted perioperative management based on the ERAS philosophy.The postoperative recovery time of physiological function(the time for removing urinary catheter,time for eating,time for exhaling,time for getting out of bed,and length of hospital stay)between two groups were compared.The self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were applied to assess the levels of anxiety and depression of the patients in two groups before surgery and at the 1st d,5th d and 7th d after surgery.In addition,the pain score of visual analogue scale(VAS),the incidence of postoperative complication,the scores of psychological state before and at the 7th day after surgery,and patient's satisfaction for diagnosis and treatment during the length of hospital stay as the evaluation of Nursing Scale for Nursing Staff(NSNS)before discharge were compared between the two groups.Results:The time for removing urinary catheter,time for eating,time for exhaling,time for getting out of bed,and length of hospital stay of ERAS group were shorter than those of control group,and the differences were significant(t=3.508,9.171,5.423,10.983,6.675,P<0.05).Before surgery,there were not significant differences in VAS score,SAS score and SDS score between the two groups(P>0.05).The VAS scores at the 1st d,5th d and 7th d after surgery in ERAS group were significantly lower than those in control group(t=5.433,5.882,5.060,P<0.05).At the 7th d after surgery,the scores of SAS and SDS in ERAS group significantly decreased,which were significantly lower than those in control group(t=14.327,13.390,P<0.05).The incidence of postoperative complications in ERAS group was 7.50%(3/40),which was lower than 32.50%(13/40)in control group(x2=7.812,P<0.05).Satisfaction of patients in ERAS group was 95.00%,which was significantly higher than 77.50%in control group(x2=5.165,P<0.05).Conclusion:The perioperative management based on ERAS philosophy can contribute to promote the recovery of postoperatively physiological function,and relieve postoperative pain,and improve postoperatively negative emotional state,and reduce the incidence of postoperative complications,and enhance patient's satisfaction.
6.Optimization of extraction process for Bletillae Rhizoma and evaluation of its anti-oxidant,tyrosinase inhibitory activities

AIM To optimize the extraction process for Bletillae Rhizoma,and to evaluate its anti-oxidant,tyrosinase inhibitory activities.METHODS With ultrasound time,ethanol concentration and solid-liquid ratio as influencing factors,the total extraction content of gastrodin,protocatechualdehyde,p-hydroxybenzaldehyde,1,4-bis[4-(gluconoxy)benzyl]-2-isobutylmalate-2-glucoside,1,4-bis[4-(gluconoxy)benzyl]-2-isobutylmalate,yam Ⅲ,dihydropinosin and 3'-O-methylyam Ⅲ as an evaluation index,the extraction process was optimized by Box-Behnken response surface method.Subsequently,the extract's scavenging effects on DPPH,ABTS+free radicals,and inhibitory ability on tyrosinase were determined.RESULTS The optimal conditions were determined to be 49 min for ultrasound time,55%for ethanol concentration,1∶30 for solid-liquid ratio,and 2 times for extraction frequency,the total extraction content was 13.18 mg/g.The extract demonstrated the IC50 of 10.12,314.07 and 1.70 μg/g on DPPH,ABTS+free radicals and tyrosinase,respectively.CONCLUSION This simple,reliable and stable method can be used for the extraction for Bletillae Rhizoma with strong anti-oxidant,tyrosinase inhibitory activities.
7.Research progress of lateral occlusion scheme selection during natural dentition reconstruction

Occlusal reconstruction is an important method of restoring a normal occlusal relationship to the natural dentition.The cor-rect choice of lateral occlusion scheme is necessary for physiological occlusal reconstruction.The main lateral occlusion schemes for the occlusal reconstruction of the natural dentition are canine protection and group function.This article reviews the effects of lateral occlu-sion schemes on the stomatognathic system and factors affecting the choice of lateral occlusion schemes in the occlusal reconstruction of natural dentition.According to the literature review,the lateral occlusion schemes affect the physiological functions of the stomatognath-ic system.Both lateral occlusion schemes have less impact on the temporomandibular joint.Group function produces greater EMG activi-ty,jaw movement velocity,and jaw lateral freedom than canine protection.The lateral occlusion schemes of the natural dentition should be selected according to the degree of lateral movement,types of static occlusion,and age of patients.For patients with Angle's ClassⅢ malocclusion,skeletal Class Ⅱ,Ⅲ malocclusion,and the elderly,group function is recommended,while canine protection is more recommended for young adults.For patients who eventually need to achieve canine protection,it is recommended to design group func-tion in the early stage of lateral movement,and gradually transition to canine protection with the increase of lateral movement.Thus,clinicians need to fully understand the influencing factors of lateral occlusion selection during occlusal reconstruction in natural denti-tion,providing a basis for the development of personalized diagnostic and treatment plans.

9.Clinical and pathological characteristics analysis of benign pulmonary nodules clinically highly suspected as malignant: A retrospective cohort study

Objective To discuss the main pathological types and imaging characteristics of pulmonary nodules that are highly suspected to be malignant in clinical practice but are pathologically confirmed to be benign. Methods A retrospective analysis was performed on the clinical data of patients with pulmonary nodules who were initially highly suspected of malignancy but were subsequently pathologically confirmed to be benign. These patients were treated at the First Affiliated Hospital of Xiamen University from December 2020 to April 2023. Based on the outcomes of preoperative discussions, the patients were categorized into a benign group and a suspicious malignancy group. The clinical data and imaging characteristics of both groups were compared. Results A total of 232 patients were included in the study, comprising 112 males and 120 females, with a mean age of (50.7±12.0) years. Among these, 127 patients were classified into the benign group, while 105 patients were categorized into the suspicious malignancy group. No statistically significant differences were observed between the two groups regarding age, gender, symptoms, smoking history, or tumor history (P>0.05). However, significant differences were noted in nodule density, CT values, margins, shapes, and malignant signs (P<0.05). Further analysis revealed that in the suspicious malignancy group, solid nodules were predominantly characterized by collagen nodules and fibrous tissue hyperplasia (33.3%), followed by tuberculosis (20.4%) and fungal infections (18.5%). In contrast, non-solid nodules were primarily composed of collagen nodules and fibrous tissue hyperplasia (41.2%) and atypical adenomatous hyperplasia (17.7%). Conclusion Benign pulmonary nodules that are suspected to be malignant are pathologically characterized by the presence of collagen nodules, fibrous tissue hyperplasia, tuberculosis, atypical adenomatous hyperplasia, and fungal infections. Radiologically, these nodules typically present as non-solid lesions and may exhibit features suggestive of malignancy, including spiculation, lobulation, cavitation, and pleural retraction.
10.Correlation between HLA-I and PD-L1 Expression Levels and Clinicopathological Features and Cellular Immune Infiltration in Bladder Cancer

Objective To investigate the correlation between the expression of human leukocyte antigen class I(HLA-I)and programmed cell death ligand 1(PD-L1)with clinicopathological features and cellular immune infiltration.Methods A total of 150 patients with bladder cancer diagnosed and treated in Anhui Provincial Hospital from May 2020 to April 2023 were retrospectively selected as the study objects.The positive expression rates and positive scores of HLA-I and PD-L1 were compared between cancerous tissues and adjacent tissues.The positive scores of HLA-I and PD-L1 in cancer tissues of patients with different clinical characteristics were compared,and the correlation between HLA-I,PD-L1 and clinical characteristics of patients with bladder cancer was analyzed by Kendall's tau-b method.Logistic regression model was used to establish the combined model parameters of HLA-I and PD-L1 positive scores,and receiver operating characteristic(ROC)curve was drawn to analyze the HLA-I and PD-L1 positive scores and the area under the curve(AUC),sensitivity and specificity of the combined diagnosis of bladder cancer.Results The positive expression rate of HLA-I in cancer tissues was lower than that in paracancer tissues[38.67%(58/150)vs 81.33%(122/150)],while the positive expression rate of PD-L1 was higher than that in paracancer tissues[57.33%(86/150)vs 14.00%(21/150)],and the differences were statistically siginficant(χ2=56.889,61.377,all P<0.05).The HLA-I positive score of cancer tissues was lower than that of paracancer tissues[2.00(1.00,3.00)vs 3.00(3.00,5.00)],while the PD-L1 positive score was higher than that of paracancer tissues[3.00(2.00,5.00)vs 2.00(1.00,2.00)],and the fifferences were statistically significant(Z=-8.409,-6.346,all P<0.05).There was no significant difference in HLA-I and PD-L1 positive scores among different sex,age and tumor diameter(ZHLA-1=-1.834,-0.622,-0.543;ZPD-L1=0.811,0.812,0.919,all P>0.05).The difference of HLA-I and PD-L1 positive scores among different pathological stages,lymph node metastasis,differentiation degree,CD4+,CD8+and CD68+were statistically significant(ZHLA-1=-7.034~3.814;ZPD-L1=-4.479~3.257,all P<0.05).Kendall's tau-b correlation analysis showed that HLA-I was negatively correlated with pathological stage,lymph node metastasis,degree of differentiation,and positively correlated with negative infiltration of CD4+,CD8+and CD68+(r=-0.528~-0.286,all P<0.05).PD-L1 was positively correlated with pathological stage,lymph node metastasis,degree of differentiation and negatively correlated with negative infiltration of CD4+,CD8+and CD68+(r=-0.243~0.334,all P<0.05).ROC curve analysis showed that the positive scores of HLA-I and PD-L1 and the AUC values of the combined diagnosis of bladder cancer were 0.773,0.702 and 0.856,respectively.Sensitivity was 61.30%,57.30%and 82.00%.The specificity was 81.30%,86.00%and 73.30%.Conclusion The expression of HLA-I and PD-L1 is abnormal in patients with bladder cancer,and their expression is affected by the positive infiltration of immune cells.Observing the positive expression of HLA-I and PD-L1 is beneficial to provide guidance for clinical diagnosis and treatment.
