Objective To explore the effectiveness of the generative large language model MedGo in nursing decision-making for elderly patients with multimorbidity. Methods A quasi-randomized controlled trial study was conducted involving 6 junior nurses, 6 senior nurses and the MedGo model from January 1, 2025 to March 31, 2025 at the Emergency Internal Medicine Ward of Shanghai East Hospital Affiliated to Tongji University. Clinical data of 120 elderly patients with multimorbidity were analyzed to compare the performance of the three groups in four tasks (nursing diagnosis assessment, nursing intervention formulation, complication identification, and complication prevention) from three evaluation dimensions: decision-making time consumption, decision accuracy, and decision-making quality. Results In terms of decision-making time, the senior nurse group completed all four tasks faster than the junior nurse group (P<0.01), and the MedGo group completed all four tasks faster than the junior nurse group (P<0.001) and the senior nurse group (P<0.001). In terms of decision-making accuracy, senior nurse group scored higher than junior nurse group in all four tasks (P<0.001), while the MedGo group outperformed the senior nurse group only in complication identification (P<0.001). In terms of decision-making quality, the MedGo group scored higher than junior nurse group (P<0.001) and senior nurse group (P<0.001) in all four tasks. Conclusions The MedGo model demonstrates advantages of high efficiency, accuracy, and quality in nursing decision-making for elderly patients with multimorbidity; senior nurses outperform junior nurses in decision-making, providing diverse references for clinical nursing decision-making.

Purpose To explore the diagnostic value of two-dimensional speckle tracking echocardiography(2D-STE)in differentiating left ventricular hypertrophy(LVH)caused by Fabry disease from other etiologies.Materials and Methods A total of 23 patients clinically confirmed Fabry disease with LVH(Fabry disease group)in Peking University First Hospital from August 2014 to February 2023,retrospectively.23 patients with hypertensive LVH(hypertensive LVH group)and 23 with hypertrophic cardiomyopathy(HCM)(HCM group)were also included.Conventional echocardiographic parameters and 2D-STE-derived left atrial strain and left ventricular longitudinal strain were analyzed and compared among the three groups.LASSO regression was used to select variables and construct a diagnostic model to differentiate hypertensive LVH,HCM and Fabry disease group. Results The Fabry disease group showed significantly reduced left ventricular global longitudinal strain and segmental left ventricular longitudinal strain in the basal anterior wall, basal anterolateral wall and mid inferior wall compared to the hypertensive LVH group(P<0.05).There were no significant differences in left atrial strain, left ventricular global longitudinal strain and segmental left ventricular longitudinal strain between the Fabry disease and HCM groups(P>0.05).Compared to the conventional echocardiography-based model, the combined model did not significantly improve diagnostic accuracy for differentiating the three etiologies(Z=-1.314--0.594,all P>0.05).Conclusion Compared to patients with hypertensive LVH,HCM and Fabry disease group exhibit varying degrees of decrease in global and segmental longitudinal strain of the left atrium and left ventricle.Additional measurements of left ventricular longitudinal strain and left atrial strain do not provide significant incremental diagnostic value over conventional echocardiography in distinguishing among Fabry disease, hypertensive LVH and HCM.

Chinese Journal of General Surgery 2025;34(6):1157-1170
Background and Aims:Aortic aneurysm,a major cardiovascular disease with high mortality and disability rates, has emerged as a critical global public health challenge.Elevated body mass index(BMI)has been confirmed as an independent risk factor for aortic aneurysm. However, the long-term trends and heterogeneity of the disease burden attributable to high BMI in China-across sex, age, and region-remain insufficiently studied. This study, based on the Global Burden of Disease(GBD)database, aimed to analyze the changes in the disease burden of high BMI-related aortic aneurysm in China from 1990 to 2021 and compare these trends with those in global and socio-demographic index(SDI)-stratified regions. Methods:Data from GBD 2021 were used to extract mortality rates and disability-adjusted life years(DALY)due to aortic aneurysm attributable to high BMI in China from 1990 to 2021.Age-standardized mortality rate(ASMR)and age-standardized DALY rate(ASDR)were calculated.Long-term trends were assessed using the estimated annual percentage change(EAPC)and Joinpoint regression models. An autoregressive integrated moving average model was applied to project trends from 2022 to 2036.Results:During the study period, deaths and DALY from aortic aneurysms attributable to high BMI in China increased nearly sevenfold.ASMR rose from 1 to 3 per 100 000 population(EAPC=3.91),and ASDR increased from 23 to 74 per 100 000(EAPC=4.11),both showing a marked upward trend.Males consistently bore a higher burden across all age groups, particularly among those aged≥65 years.Decomposition analysis revealed that the increased burden in males was mainly driven by epidemiological improvements, while that in females was primarily attributable to population aging.Compared with the United States and global trends,China exhibited a faster increase in high BMI-related aortic aneurysm burden, which is projected to remain elevated over the next 15 years. Conclusion:The disease burden of aortic aneurysm attributable to high BMI continues to rise in China, with pronounced differences across sex and age.Targeted weight management, health interventions, and early screening strategies are urgently needed for high-risk populations to curb the upward trend and optimize public health resource allocation.

BACKGROUND:The imbalance between proliferation and apoptosis of chondrocytes plays an important role in the occurrence and development of osteoarthritis. Previous studies have found that hsa-miR-3202 is involved in regulating the proliferation and apoptosis of various cells. However,no studies have explored the correlation between hsa-miR-3202 and osteoarthritis.OBJECTIVE:To investigate the expression of hsa-miR-3202 in osteoarthritic chondrocytes and its effect on the proliferation and apoptosis of chondrocytes. METHODS:(1) MicroRNAs differentially expressed in osteoarthritic chondrocytes were screened by biogenic analysis. Based on the current research situation at home and abroad,hsa-miR-3202 was selected for follow-up studies, and its target genes were predicted by gene ontology (GO) functional enrichment and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment. (2) Human normal chondrocyte cell lines C28/I2 in logarithmic growth phase were selected and randomly divided into four groups for culture:in normal group, cells were cultured in normal medium for 24 hours, the medium was then changed to normal medium for another 6 hours of culture, and changed to normal medium for subsequent culture;in lipopolysaccharide group, cells were cultured in lipopolysaccharide-containing medium for 24 hours, the medium was then changed to normal medium for another 6 hours, and changed to normal medium for subsequent culture;in lipopolysaccharide+NC group, cells were cultured in lipopolysaccharide-containing medium for 24 hours, and then transfected with has-miR-3202 mimics control for 6 hours, and the medium was change to normal medium for subsequent culture;in lipopolysaccharide+hsa-miR-3202 mimics group, cells were cultured in lipopolysaccharide-containing medium for 24 hours and then transfected with has-miR-3202 mimics for 6 hours, and the medium was changed to normal medium for subsequent culture. After further 48 hours of culture, the expression level of hsa-miR-3202 was detected by fluorescence quantitative PCR and cell apoptosis was detected by flow cytometry. After further culture of 0-72 hours, cell proliferation activity was detected by cell counting kit-8. RESULTS AND CONCLUSION:Bioinformatics analysis results indicated that hsa-miR-3202 was significantly down-regulated in osteoarthritic chondrocytes. GO functional enrichment and KEGG pathway enrichment showed that the function of hsa-miR-3202 target gene was closely related to cell growth and apoptosis. The results of in vitro cell experiments showed that compared with the normal group, the expression level of hsa-miR-3202 and proliferation ability of chondrocytes were significantly decreased in the lipopolysaccharide group (P<0.05),while the apoptotic rate was significantly increased (P<0.05). Compared with the lipopolysaccharide group, the expression level of hsa-miR-3202 and proliferation ability of chondrocytes were significantly increased in the lipopolysaccharide+hsa-miR-3202 mimics group (P<0.05),while the apoptotic rate was significantly decreased (P<0.05). To conclude, the expression of hsa-miR-3202 is down-regulated in osteoarthritic chondrocytes to inhibit cell proliferation and promote cell apoptosis, thus affecting the occurrence and development of osteoarthritis.
