1.Relationship between serum Sirt1-6 levels and oxidative stress indicators in age-related cataract patients and their diagnostic value

AIM: To investigate the relationship between serum levels of sirtuins(Sirts)1-6 and oxidative stress markers, and to evaluate their diagnostic value for age-related cataract(ARC).METHODS:A prospective cohort of ARC patients admitted to the hospital between June 2019 and July 2021 was enrolled as the study subjects. Concurrently, age-matched healthy individuals undergoing routine physical examinations at the hospital during the same period were recruited as the control group. Baseline demographic and clinical characteristics were compared between the two groups. Serum levels of Sirt1-6 and oxidative stress markers including malondialdehyde(MDA), superoxide dismutase(SOD), glutathione peroxidase(GSH-Px), and total antioxidant capacity(TAC)were compared. Pearson correlation analysis was performed to assess the associations between serum Sirt1-6 levels and oxidative stress parameters. Multivariate Logistic regression analysis was conducted to analysis risk factors associated with ARC occurrence. Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of serum Sirt1-6 levels and oxidative stress indicators for ARC.RESULTS: The study included 127 ARC patients, comprising 68 men and 59 women, with a mean age of 66.05±2.58 y; the concurrent control group consisted of 121 patients, comprising 63 men and 58 women, with a mean age of 65.54±2.86 y. Serum levels of Sirt1, 3 and 6, SOD, GSH-Px, and TAC were significantly lower in the ARC group compared to the control group(all P<0.001), whereas MDA levels were markedly elevated(P<0.001). Pearson correlation analysis revealed that serum levels of Sirt1, Sirt3, Sirt6 in the ARC group were positively correlated with SOD, GSH-Px, and TAC, and negatively correlated with MDA. Multivariate Logistic regression analysis demonstrated that serum Sirt1, Sirt6, MDA, SOD, and GSH-Px were significantly associated with the occurrence of ARC(all P<0.001). ROC curve analysis showed that the combination of Sirt1, Sirt6, MDA, SOD, and GSH-Px yielded an area under the curve(AUC)of 0.995 for diagnosing ARC, which was significantly higher than that of Sirt1 alone(Z=4.978,P<0.001), Sirt6 alone(Z=7.487,P<0.001), MDA alone(Z=6.449,P<0.001), SOD alone(Z=5.773,P<0.001), or GSH-Px alone(Z=5.056,P<0.001), indicating superior diagnostic accuracy of the multimarker panel(P<0.05). CONCLUSION: Serum levels of Sirt1, 3, and 6 are generally reduced in ARC patients and are closely associated with oxidative stress imbalance. Specifically, decreased serum levels of Sirt1 and Sirt6, along with oxidative stress markers(elevated MDA and reduced SOD and GSH-Px)are identified as risk factors for ARC. Moreover, the combined detection of these indicators presents high diagnostic value for ARC.
2.Unfractionated heparin attenuates histone-induced coagulation activation via the Ang/Tie2 pathway

Objective:To investigate the role of the angiopoietin (Ang)/tyrosine kinase receptor 2 (Tie2) pathway in mediating histone-induced coagulation activation in mice with acute lung injury, and the protective mechanism of unfractionated heparin (UFH).Methods:Twenty-four mice were randomly divided into three groups ( n=8 per group): Control group, Histone group, and Histone + UFH group using the random number table method. The Histone group and Histone + UFH group were administered 50 mg/kg histone via the tail vein. One hour later, UFH was given at a dosage of 400 U/kg by the same way. The Control group was administered an equivalent volume of sterile saline solution. Four hours after modeling, tissue samples were collected. HE staining was performed to observe the pathology of lung tissue. Lung lobe wet and dry weights were measured to assess the degree of pulmonary edema. Immunohistochemistry was used to observe the expression of fibrinogen (FIB) in lung tissue. ELISA was used to detect the levels of thrombin-antithrombin complex (TAT), plasminogen activator inhibitor type-1 (PAI-1), and D-dimer (D-D) in plasma. The qRT-PCR was used to measure the mRNA expression levels of tissue factor (TF), FIB, PAI-1, angiopoietin (Ang)-1, Ang-2, and Tie2 in lung tissue. Western blot was used to measure the protein expression levels of TF, FIB, Ang-1, Ang-2, and pTie2 in lung tissue. Results:The HE staining results revealed that, compared to the Control group, the Histone group exhibited thickened alveolar walls, significant neutrophil infiltration, and alveolar congestion with edema, indicating histone-induced ALI ( P<0.001). In contrast, the Histone + UFH group exhibited milder lung injury ( P<0.001), suggesting that UFH mitigated the lung damage induced by histones. The lung wet/dry weight ratio and lung water content percentage were significantly higher in the Histone group than in the Control group ( P<0.001), while UFH reduced the severity of pulmonary edema ( P<0.01). Immunohistochemistry revealed intravascular thrombus formation and fibrin deposition in the Histone group, which were reduced by UFH. ELISA results showed significantly elevated levels of TAT, PAI-1, and D-D in the Histone group ( P<0.001), and UFH decreased the levels of these parameters stimulated by histone ( P<0.05). The qRT-PCR showed increased mRNA expression of TF, FIB, PAI-1, and Ang-2, and decreased expression of Ang-1 and Tie2 in lung tissues in the Histone group, while UFH mitigated the effects of histones on mRNA expression of these parameters. Western blot analysis indicated increased protein expression of TF, FIB, and Ang-2 ( P<0.01), and decreased expression of Ang-1 and Tie2 in lung tissues in the Histone group ( P<0.01), and UFH reduced the impact of histones on the protein expression of these parameters ( P<0.05). Conclusions:Histones can cause lung injury, pulmonary edema, and coagulation activation in mice lung tissue. UFH can effectively alleviate histone induced lung injury, and coagulation activation in by histones via the Ang/Tie2 signaling pathway.
3.Study on the temporal expression of growth differentiation factor-15 and its mortality prognostic implications in patients with acute coronary syndrome

Objective:To investigate the temporal expression of Growth Differentiation Factor-15 (GDF15) in the serum of patients with Acute Coronary Syndrome (ACS) and explore the clinical significance of GDF15 in protecting cardiomyocytes in ACS.Methods:A retrospective study was conducted on 289 ACS patients admitted to the emergency departments from February to October 2023. Data on gender, age, troponin T (TnT), creatine kinase isoenzyme (CK-MB), GDF15, and B-type natriuretic peptide (BNP) within 30 minutes of admission were recorded. Differences in these indicators among different groups were compared. Receiver Operating Characteristic (ROC) curves were plotted to evaluate the diagnostic value of GDF15, TnT, and BNP for ACS. Among the patients, 15 exhibited a temporal expression pattern of GDF15, and their blood samples were re-measured using a GDF15 fluorescent quantitative immunochromatographic assay kit. Fifteen patients without temporal expression were randomly selected as controls, and their samples were also re-measured to exclude detection errors. Fifteen patients with temporal expression were included in the temporal expression group, and 15 without temporal expression were included in the non-temporal expression group. Laboratory indicators such as fasting blood glucose, glycated hemoglobin, triglycerides, creatinine, and uric acid were compared between the groups. Additionally, patient age, gender, body mass index (BMI), coronary angiography results, echocardiography, Gensini score, left ventricular ejection fraction (LVEF), and GRACE risk score were recorded to assess their correlation with GDF15 temporal expression. Statistical analysis was performed using SPSS 27 software, with continuous data expressed as mean ± standard deviation (Mean ± SD) and compared using t-tests and χ2 tests. Results:The overall trend in ACS patients showed a higher proportion of males than females (73.36% vs. 26.64%). The oldest group was the Unstable Angina (UA) group, with a mean age of (63.98 ± 15.19) years, while the youngest group was the non-ACS chest pain group, with a mean age of (54.29 ± 16.39) years. A higher proportion of patients in the UA, ST-segment elevation myocardial infarction (STEMI), and non-ST-segment elevation myocardial infarction (NSTEMI) groups had a history of smoking. The combination of GDF15 and TnT showed high diagnostic value for ACS, with an area under the ROC curve (AUC) of 0.843, consistent with previous studies. Among all ACS patients, 15 exhibited a temporal expression pattern of GDF15, where GDF15 levels peaked at 4 hours, gradually decreased, and peaked again at 24 hours. Patients in the temporal expression group had higher LVEF and left ventricular end-systolic diameter compared to the non-temporal expression group. The Gensini score was lower in the temporal expression group, and the GRACE risk score was significantly lower in the temporal expression group (00.7±14.72) compared to the non-temporal expression group (116.1±23.46), with a statistically significant difference ( P = 0.0115). There were no significant differences in general characteristics (age, gender, BMI) or clinical biochemical indicators (fasting blood glucose, glycated hemoglobin, triglycerides, total cholesterol, high-density lipoprotein, low-density lipoprotein, creatinine, uric acid) between the temporal and non-temporal expression groups ( P > 0.05). Conclusions:GDF15 demonstrates significant diagnostic and prognostic predictive value in ACS. Patients with temporally dynamic expression of serum GDF15 exhibit milder myocardial injury and a lower probability of mortality. These findings provide novel therapeutic targets and research directions for further exploring the role of GDF15 in ACS management.
4.Evaluation of the efficacy of linaclotide combined with PEG in the bowel preparation for colonoscopy of patients with chronic constipation

Objective To explore the application of linaclotide combined with compound polyethylene glycol electrolyte powder(PEG)in bowel preparation for colonoscopy of patients with chronic constipation.Methods A toltal of 190 patients with chronic constipation who underwent electronic colonoscopy in Beijing NO.6 Hospital from January to October 2023 were selected as the objects.The patients were divide into linaclotide group(n=63),3L PEG group(n=64),and 4L PEG group(n=63)by random number table method.The application effects of three groups were compared.Results Boston bowel preparation scale score was higher than 3L PEG and 4L PEG groups,there were significant difference among groups(P<0.05).The qualified rate of patients in linaclotide group was better than that of 3L PEG group and 4L PEG group,and the qualified rate of patients in 4L PEG group was better than that of 3L PEG group,there were significant difference among groups(P<0.05).Conclusion Linaclotide combined with PEG can not only improve the intestinal cleaning quality of patients with constipation,but also do not increase the adverse reactions of patients.
5.Research progress on the cGAS-STING signaling pathway in glioma

Glioma is the most common primary malignant brain tumor in the adult population,characterized by high incidence and low sur-vival rates.Current standard treatment options exhibit limited efficacy.In recent years,the crucial role of cGAS-STING signaling in tumor im-munity has garnered significant attention.This article systematically reviews the molecular mechanisms of cGAS-STING signaling in glioma and its potential tumor-suppressive effects,providing an in-depth exploration of various STING agonists and their delivery systems targeting this pathway.This review also summarizes the synergistic effects of combining cGAS-STING pathway activation with radiotherapy,chemo-therapy,and immune checkpoint inhibitors,providing substantial references for the development of multimodal comprehensive treatment strategies.
6.Risk factors of recurrent laryngeal nerve injury in microwave ablation for thyroid nodules:a study based on malignant risk stratification for nodule

Objective:To investigate the independent risk factors of recurrent laryngeal nerve(RLN)injury after microwave ablation(MWA)for thyroid nodules of different malignant stratification.Methods:The medical records of 240 patients,who underwent microwave ablation for thyroid nodules in the department of ultrasound,Beijing Friendship Hospital Affiliated to Capital Medical University from September 2022 to August 2024,were retrospectively selected.All thyroid nodule cases were categorized based on the American College of Radiology Thyroid Imaging Reporting and Data System(TI-RADS)classification criteria and whether occurred RLN injury during the ablation procedure.A total of 54 patients with RLN injury and 65 patients without RLN injury,who were classified as TI-RADS 4a or higher than that,were divided into the high-risk group,and 35 patients with RLN injury and 86 patients without RLN injury,who were classified below TI-RADS 4a,were divided into the low-risk group.And then,a series of parameters included the benign and malignant nodules,the upper diameter of nodules,the left and right diameters of nodules,anteroposterior diameters of nodules,the aspect ratio(>1,≤1),overall echo,calcification,location,cystic solidity,and ablation parameters were analyzed.The risk factors of RLN injury of two groups were analyzed by using single factor and multi factor analysis.Results:There were not significant differences in the benign and malignant nodules,the upper diameters of nodules,the left and right diameters of nodules,anteroposterior diameters of nodules,the volume of nodules,overall echo,calcification,ldiametersocation,and cystic solidity between high and low-risk groups(P>0.05).In high-risk group,the distance between nodules and esophageal groove of trachea was less or equal to 2mm,and the increase of nodule volume were independent risk factors for RLN injury(OR=4.199,1.002,P<0.05),respectively.In the low-risk group,the nodule,which location was on the Zuckerkandl tubercle(Z-nodule),was risk factor that significantly increased RLN injury(OR=3.296,P<0.05).Conclusion:For nodules with differently malignant risk,the anatomical location,volume parameters and optimized ablation plan should be paid special attention before surgery,so as to reduce the risk of RLN injury.
7.Correlation between plasma Hcy and the degree of coronary artery calcification in the elderly

Objective To investigate the association between plasma homocysteine(Hcy)level and coronary artery calcification(CAC)and its clinical predictive efficacy.Methods A total of 172 patients who underwent coronary CT angiography North China University of Science and Technology Affiliated Hospital from April 2019 to May 2021 and CAC score(CACS)>0 were enrolled.According to the CACS value,the subjects were divided into mild calcification group(n=136)and severe calcification group(n=36),and the clinical characteristics of two groups were compared and analyzed.Multivariate Logistic regression model was used to screen the independent influencing factors of CAC severity,and a prediction model was constructed based on the Hcy detection value.The clinical diagnostic value was evaluated by the receiver operating characteristic(ROC)curve.Results There were significant differences in Hcy,white blood cell count,triglyceride and magnesium ion levels between two groups(P<0.05).Multivariate Logistic regression analysis showed that Hcy,white blood cell count and magnesium ion level were independent risk factors for the progression of CAC.Furthermore,a regression model based on Hcy was constructed and ROC curve was fitted to evaluate its predictive efficacy.The results suggested that the predictive model had the best performance when the critical value of Hcy was set at 27.4μmol/L:the sensitivity was 55.6%,the specificity was 97.1%,and the area under the curve was 0.765.Conclusion Hcy serves as an independent risk factor for the severity of CAC and can effectively predict the progression of CAC with high accuracy.
8.Research Progress on Catgut Embedding Therapy for Abdominal Obesity

Abdominal obesity represents the predominant obesity phenotype in China.Compared to peripheral obesity,individuals with abdominal obesity are more prone to metabolic disorders,cardiovascular diseases,and higher mortality rates.Catgut embedding therapy at acupoints has demonstrated significant clinical efficacy in preventing and treating abdominal obesity.However,standardized clinical guidelines for its application in abdominal obesity management have not yet been established.This review comprehensively summarizes the clinical applications of catgut embedding therapy for simple abdominal obesity and abdominal obesity with various complications,aiming to provide clinical evidence and theoretical guidance for its use in preventing and treating abdominal obesity.

Under the influence of long-term space flights and the confined space environment,it is very easy to induce space depression syndrome,mainly manifested as decreased emotional stability,sleep disorders,mental fatigue,etc.,which seriously affect the living conditions and working abilities of astronauts.The current treatment methods mainly focus on psychological support and drug intervention.Acupuncture has a good effect in treating depression.Therefore,starting from the TCM pathogenesis and modern medical pathogenesis of aerospace depression syndrome,we conducted literature retrieval from databases such as the Chinese Medical Classic,China National Knowledge Infrastructure(CNKI),Wanfang,VIP,PubMed,and Web of science.For the included literature,we adopted the stratified evidence scoring method and combined the TCM mechanism and modern medical mechanism of the effect of acupuncture.A prescription for acupuncture points was constructed to provide a basis for selecting acupuncture points for the prevention of aerospace depression syndrome through acupuncture.
10.Impacts of corneal topography-guided phacoemulsification with transparent corneal incision on efficacy and complications of cataract patients

AIM: To investigate the effect of corneal topography-guided phacoemulsification with transparent corneal incision on cataract patients.METHODS:A total of 92 cataract patients(92 eyes)admitted to our hospital from February 2021 to February 2023 were prospectively selected and randomly divided into two groups: the control group(46 eyes)received the conventional 11:00 clear corneal incision scheme, and the study group(46 eyes)received the steepest meridian clear corneal incision scheme. The uncorrected visual acuity, corneal surface morphology indicators, including surface regularity index(SRI), surface asymmetry index(SAI), and cylinder(CYL), subjective dry eye symptoms questionnaire scoring scale(SDES), tear film break-up time(BUT), and complications were compared between the two groups.RESULTS:All patients completed the follow-up. The uncorrected visual acuity of the study group was significantly better than that of the control group at 1 wk, 1 and 3 mo after surgery(all P<0.05); there were statistical significance in the SAI and CYL of both groups of patients at 3 mo after surgery(all P<0.05); the fluctuation levels of SDES and BUT in the study group were significantly lower than those in the control group at different time points after surgery(all P<0.05); and there was no statistical difference in complications between the two groups(P>0.05).CONCLUSION:Under the guidance of corneal topography, phacoemulsification through the transparent corneal incision of the steepest meridian of the cornea can improve the uncorrected visual acuity of cataract patients, restore the corneal surface morphology, and have few complications.
