1.Evidence-based practice of discharge preparation services for hip replacement patients

Objective To construct an evidence-based practice program of discharge preparation services for hip replacement patients based on the best evidence,and to evaluate its clinical effects,aiming to provide references for the implementation of relevant interventions.Methods Knowledge to Action Framework was used as a guide to summarise the best evidence on discharge preparation services for hip replacement patients and to select services based on clinical context.We selected 38 hip replacement patients and 17 healthcare professionals from the De-partment of Orthopaedic Surgery at a tertiary A hospital in Shanghai in September-October 2023 to conduct a baseline review,identify facilitating and hindering factors,and develop a practice protocol.We also selected 38 hip replacement patients and 17 healthcare professionals from November-December 2023 on this ward for evidence-based practice.Nurses'rates of completion of review indicators,levels of knowledge of discharge preparation ser-vices,and discharge planning competencies were compared before and after practice.Patients'readiness for dis-charge,hip function,and unplanned readmissions were also compared before and after the practice.Results After evidence-based practice,nurses'completion rates of review indicators,knowledge of discharge preparation services,and discharge planning competencies were higher than baseline levels(P<0.05).Hip replacement patients showed a significant improvement in readiness for discharge and hip function(P<0.001).Conclusion The implementation of an evidence-based practice program of discharge preparation services improves nurses'knowledge and skills,and also contributes to the discharge readiness and hip function of hip replacement patients.

Digital health education is a crucial means to enhance the accessibility and effectiveness of medical services.The"new digital gap"faced by the elderly population makes them a marginalized group in digital health education and disease management.This paper takes cognitive load theory as its starting point,outlines the theoretical foundation of cognitive load,and systematically reviews the factors that influence cognitive load in digital health education for the elderly and propose corresponding optimization inspirations.The aim is to provide references for optimizing digital education models and program designs for the elderly.

Diabetes, hypertension, and dyslipidemia, collectively referred to the " Three Highs, " represent increasingly prevalent metabolic risk factors in China. Many individuals experience all three conditions concurrently, significantly heightening the risk of cardiovascular disease and mortality. Although the National Metabolic Management Center(MMC) has been established for over eight years and has its unique features, the awareness, treatment, and control rates of these diseases in China remain low, and the efficiency of community management is insufficient. According to the previous two editions of management guidelines and the most recent domestic and international diagnostic and treatment guidelines, this paper conducts an in-depth analysis of the operational experience and management strategies of the MMC. Its aim is to improve the efficiency of grassroots MMC mode management for " Three Highs" patients and ensure that patients receive more standardized management.

7.Research progress on the risk prediction model of delirium after hip fracture

The risk of delirium after hip fractures is high and the prognosis is poor. This article introduces, analyzes, and compares common risk prediction models for postoperative delirium after hip fractures, including risk stratification models, risk scoring models, nomogram models, and simple prediction models, so as to provide a reference for medical and nursing staff to select and apply models.
8.Characteristics of methadone maintenance treatment clinic patients and influencing factors for HBsAg positivity based on Bayesian network model

Objective:To understand the characteristics and explore the influencing factors of HBsAg positivity in methadone maintenance treatment (MMT) clinic patients.Methods:A face to face interview and medical record review were conducted in 1 040 patients at three MMT clinics in Guangxi from September to November in 2014. The questionnaire information included general demographic characteristics, drug use history, MMT status, sexual behaviors, and health status, etc. Blood samples were collected from the patients at the same time for the detections of the level of HBsAg, anti-HBs and anti-HCV. By using χ2 test, unconditional logistic regression model and Bayesian network model the influencing factors for HBsAg positivity in MMT clinic patients and the complex network relationship among these factors were explored. Results:A total of 1 031 MMT clinic patients were surveyed, the HBsAg positive rate was 11.35 % (117/1 031). The anti-HCV positive rate was 71.77 % (740/1 031), among the anti-HCV positive patients, the HBsAg positive rate was 10.27 % (76/740). After adjusting for the confounding factors, anti-HBs positive persons might not be HBsAg positive ( OR=0.05, 95 %CI: 0.03-0.09), and anti-HCV positive persons might not be HBsAg positive too ( OR=0.30, 95 %CI: 0.17-0.52) compared with anti-HBs negative and anti-HCV negative persons, respectively. The persons with family history of hepatitis B virus infection were more likely to be HBsAg positive compared those with no such family history ( OR=5.30, 95 %CI: 2.68-10.52). Bayesian network model analysis results showed that family history of hepatitis B virus infection and anti-HBs were directly related with HBsAg positivity. Anti-HCV, intravenous drug use in the past three months and other drug using during treatment were indirectly related with HBsAg positivity. Conclusions:Anti-HBs, family history of hepatitis B virus infection, anti-HCV, intravenous drug use in past three months and other drug use during treatment were related with the HBsAg positivity in MMT clinic patients. So, it is necessary to enhance health education, improve health awareness and decrease high risk behaviors to reduce the rate of HBV infection.
9.Factors influencing the incidence of post-stroke depression

Objective To investigate the impact of daily life ability, self-efficacy and social support on post-stroke depression. Methods This was a cross-sectional questionnaire survey. A convenience sample of stroke patients from neurology department was created. Data were collected from patients about their level of daily life ability, self-efficacy, social support and depression measured. Inferential statistics multiple regression analysis was used to estimate the relationships between the predictors and post-stroke depression. Results A total of 144 (96.00%) of 150 stroke patients completed the questionnaires. The level of depression was 14.19 ± 9.25 with 25.00% (36/144) of incidence of post-stroke depression. Self-efficacy scored 40.45 ± 11.41, social support scored 28.34 ± 8.02, which negatively predicted depression (r=-0.834 and-0.736, P<0.01 respectively). Self-efficacy had the most significant impact on depression (t=-6.433, P < 0.01), however, the impact from daily life ability scored 76.74 ± 22.12, which had not statistically significant impact on depression (t=-1.773, P=0.083). Conclusions During treatment and nursing management of post-stroke depression, attention should be paid to the psychiatric variable such as self-efficacy and social support.
10.Qualitative research on professional attitudes of new entry nurses with master degree in level three class A hospitals

Objective To explore the professional attitudes of new entry nurses with master degree, and to provide evidence for the career development of this group of nurses. Methods By adopting the phenomenology research method of qualitative research, 8 new entry postgraduates nurses was conducted the in-depth interviews. Data were analyzed by using category analysis. Results Three themes were extracted, namely professional identity, working pressure and seeking professional development. Conclusions The new entry postgraduate nurses hope to show their value, so managers should strengthen their management and promote the recognition of their own value. At the same time, managers can provide emotional support and create a good environment for them, in order to facilitate their development quickly and confront work with positive attitudes.
