1.Trends and Age-Period-Cohort Model Analysis of Inci-dence and Mortality of Hepatitis B Virus-Related Liver Cancer Globally and in China from 1992 to 2021

[Purpose]To analyze the changing trends of the disease burden of liver cancer related to hepatitis B virus(HBV)globally and in China from 1992 to 2021.[Methods]Based on the Global Burden of Disease database in 2021,indicators such as the age-standardized incidence rate and mortality rate of HBV-related liver cancer globally and in China from 1992 to 2021 were collected.The Joinpoint regression model was used to analyze the changing trends of epidemiologi-cal characteristics,and the age-period-cohort model was adopted to analyze the impacts of age,period,and cohort factors on the incidence and mortality risks of HBV-related liver cancer globally and in China.[Results]From 1992 to 2021,the age-standardized incidence rate and age-standardized mortality rate of HBV-related liver cancer globally generally showed a trend of decreasing.The average annual percentage changes were-0.31%and-0.61%,respectively,and all the down-ward trends were statistically significant(both P<0.05).During the same period,the average annu-al percentage changes of the age-standardized incidence rate and age-standardized mortality rate of HBV-related liver cancer in China were-0.45%and-0.90%,respectively,and all the downward trends were statistically significant(both P<0.05).The results of the age-period-cohort model anal-ysis showed that from 1992 to 2021,the annual net drift rates of the incidence of HBV-related liver cancer globally and in China were-0.71%(95%CI:-0.84%~-0.57%)and-0.73%(95%CI:-1.01%~-0.44%),respectively.The annual net drift rates of the mortality were-1.15%(95%CI:-1.28%~-1.02%)and-1.42%(95%CI:-1.69%~-1.14%),respectively,all showing an over-all decline.The age effect showed that the risk of HBV-related liver cancer incidence in both the global and Chinese populations began to increase after 30 years of age,peaking in the 70~74 age group,while the risk of mortality surged after 40 years of age and peaked in the population aged 80 and above.The period effect indicated that the incidence and mortality risks were the highest from 1997 to 2001 and the lowest from 2017 to 2021.The cohort effect revealed that the inci-dence and mortality risks gradually decreased in populations born after 1962,with the 2007-2011 birth cohort having the lowest risks.The results of the Wald x2 test showed that there were statistically significant differences in the changing trends of the age,period,and cohort effects on the incidence and mortality of HBV-related liver cancer globally and in China(all P<0.05).[Con-clusion]From 1992 to 2021,the age-standardized incidence rate and age-standardized mortality rate of HBV-related liver cancer showed a downward trend both globally and in China.The disease burden of HBV-related liver cancer in China was higher than the global level.The age-period-cohort model has revealed the historical changes in the incidence and mortality of HBV-related liver cancer.The current and future situation of the disease burden of HBV-related liver cancer is not optimistic.It is recommended to implement precise stratified interventions for populations of different ages,periods,and birth cohorts,and actively transform the prevention,treatment,and management strategies for HBV-related liver cancer.
2.Methacrylated dermal extracellular matrix hydrogel promotes repair of abdominal wall defects

BACKGROUND:Synthetic polymers,such as polypropylene and polyester,used for the treatment of abdominal wall defects not only lack biodegradability and bioactivity but also fail to meet the demands of complex and irregular wounds.Therefore,finding bioactive materials with low immunogenicity and good histocompatibility has become a hot spot in the repair of abdominal wall defects. OBJECTIVE:To prepare methacryloyl modified dermal extracellular matrix hydrogel and explore its potential application in abdominal wall defect. METHODS:(1)The porcine dermis was acellular with 0.25%trypsin and 1%Triton X-100 in turn to obtain the dermal extracellular matrix.After pepsin digestion and methacrylic anhydride modification,the methacrylated dermal extracellular matrix hydrogel was formed by photocrosslinking.The microscopic morphology of the hydrogel was observed by scanning electron microscope,and its rheological properties,swelling properties and other physical and chemical properties were tested.(2)Mice fibroblasts(L929)were inoculated into methacrylated dermal extracellular matrix hydrogel to detect the cell compatibility.(3)Totally 12 SD rats were randomly divided into two groups(n=6)to create abdominal wall defect model with peritoneum preserved.The defect site of the polypropylene group was filled with polypropylene material,and the hydrogel group was filled with methacrylated dermal extracellular matrix hydrogel.The wound skin of both groups was covered with polypropylene material.The wound healing was observed and histological analysis was carried out. RESULTS AND CONCLUSION:(1)Enzymatic hydrolysis had a good decellularization effect on porcine dermis after decellularization,and the original glycosaminoglycans and collagen were well retained.Scanning electron microscope observation revealed that the dermal extracellular matrix hydrogel presented loose and porous structure.The aperture was between 70 and 120 μm.The swelling ratio was(16.88±3.24)%and the water absorption was(94.24±1.11)%.The rheological property test showed that the methacrylated dermal extracellular matrix hydrogel was stable and had shear thinning characteristics,with injectability.(2)CCK-8 assay and live/dead staining showed that methacrylated dermal extracellular matrix hydrogel had good cell compatibility.(3)The results of animal experiments showed that the skin wound healing rate of the experimental group was higher than that of the control group at 7,10,and 14 days after operation(P<0.05).Hematoxylin-eosin and Masson staining of skin and muscle tissue exhibited that compared with the polypropylene group,the skin wound epithelialization,hair follicle formation,collagen fiber arrangement,and neovascularization were better in the hydrogel group 14 days after surgery.The skin wound new tissue structure was similar to the normal tissue at 28 days after surgery,and scar hyperplasia was less.A small amount of muscle regeneration was observed on day 28 after operation.(4)The results show that the methacrylated dermal extracellular matrix hydrogel can promote wound skin healing and muscle tissue regeneration in rats with abdominal wall defect.

4.Analysis of the current situation of defeat in men undergoing IVF-ET therapy and its influencing factors

Objective:To investigate the defeat of men undergoing in vitro fertilization and embryo transfer (IVF-ET) and analyze its influencing factors. Methods:A total of 202 male patients undergoing IVF-ET in Reproductive Medicine Hospital in Changsha from May 2021 to September 2021 were selected by random sampling. General data questionnaire, Chinese version of defeat scale, social support scale, Olson marital quality questionnaire (marital satisfaction, communication), self-rating depression scale, mental resilience scale were used.Results:The defeat score of 202 men in IVF-ET was 16.25±9.93. Marital status, marital quality, education, communication, social support, depression and resilience were the main influencing factors of male defeat after IVF-ET (all P<0.05). Conclusion:Male who have received IVF-ET treatment also have a certain degree of defeat. While paying attention to the psychological status of women who have received IVF-ET, we should also pay attention to the psychological health of the male who have high defeat in IVF-ET treatment and carry out targeted psychological intervention measures to alleviate their defeat, which will be beneficial to the treatment outcome.

6.Application effect of modified early warning scoring system in treatment of patients with multiple injuries in emergency department

Objective:To explore the application effect of modified early warning scoring system in treatment of patients with multiple injuries in emergency department.Methods:Using the convenient sampling method, a total of 126 patients with multiple injuries who were admitted to Emergency Department of Heping Hospital Affiliated to Changzhi Medical College from January 2018 to December 2019 were selected as the research objects. The patients were divided into the control group and the intervention group by chronological order , with 63 cases in each group. The control group was given the traditional multi-injury routine first aid and monitoring plan, while the intervention group applied a modified early warning scoring system on the basis of the control group. The success rate of first aid and the degree of deterioration of patients were compared between the two groups.Results:After the implementation of modified early warning scoring system, the success rate of first aid of patients in the intervention group was higher than that of the control group and the degree of disease deterioration was lower than that of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The modified early warning scoring system has a significant application effect in emergency patients with multiple injuries, which can improve the success rate of treatment of patients and reduce the degree of disease deterioration.
7.Preoperative localization of 18F-FDG PET/MR in refractory epilepsy patients with negative MRI

Objective:To explore the accuracy of 18F-fluorodeoxyglucose (FDG) PET/MR in preoperative localization of refractory epilepsy patients with conventional MRI negative. Methods:From August 2016 to December 2018, 57 refractory epilepsy patients (36 males, 21 females, age (24.0±10.3) years) with conventional MRI negative who underwent surgery in Xuanwu Hospital were retrospectively enrolled. All patients received interictal 18F-FDG PET/MR before surgery and the epileptogenic foci were determined by using visual and semi-quantitative methods. Patients were followed up for 1 year and the surgical outcome was evaluated according to Engel classification. The sensitivity, specificity and accuracy of 18F-FDG PET/MR in locating epileptogenic foci were calculated according to surgical resection and followed-up results as the " gold standard" . Results:Of 57 patients, 51(89.5%, 51/57) showed single or multiple hypo-metabolism focus on 18F-FDG PET/MR, and 6(10.5%, 6/57) showed no abnormal metabolism changes. The microstructure abnormality was found in 18 patients (31.6%, 18/57) on 18F-FDG PET/MR images. Follow-up results were obtained from 46 patients, and 84.8%(39/46) with seizure improvement (Engel Ⅰ-Ⅲ). The sensitivity, specificity and accuracy of 18F-FDG PET/MR in preoperative localization of epileptic foci was 90.0%(27/30), 3/16 and 65.2%(30/46), respectively. Conclusion:18F-FDG PET/MR is helpful for the detection of epileptic foci in patients with MRI-negative refractory epilepsy, and can provide reliable information for further surgical treatment.
8.A novel compound heterozygous mutation causing 3-methylcrotonyl-CoA carboxylase deficiency.

OBJECTIVETo explore the molecular mechanism for a boy suspected with 3-methylcrotonyl-CoA carboxylase deficiency by neonatal screening.
METHODSPCR and Sanger sequencing were used to identify potential mutations of MCCC1 and MCCC2 genes. SIFT and Polyphen-2 software was used to predict the effect of variant on the protein function and conservation of the variant across various species. Human Splicing Finder and Swiss-PdbViewer4.1.0 were applied to analyze the possible mechanism of the variant.
RESULTSFor the proband, a compound heterozygous mutation was discovered in the MCCC1 gene, namely c.539G>T (p.G180V) and c.704_711del (p.A235Vfs*4), which were inherited from his father and mother, respectively. The two mutations have disrupted the protein conformation, which in turn may impact the function of MCC protein.
CONCLUSIONThe compound heterozygous mutations of the MCCC1 gene may contribute to the 3-methylcrotonyl-CoA carboxylase deficiency manifested by the patient.

9.Early effect of tonsillotomy on children′s snoring with tonsil hypertrophy

Objective To explore short-term effect of tonsillotomy on children′s obstructive sleep apnea hypopnea syndrome (OSAHS) with tonsil hypertrophy by Ellman double-frequency radio-frequency machine .Methods Totally 30 cases children′s OS-AHS with tonsil hypertrophy were had tonsillotomy by Ellman double-frequency radio-frequency machine .They were compared with 30 cases children′s OSAHS with tonsil hypertrophy by traditional tonsillectomy .Short-term effect of tonsillotomy were ana-lyzed by wound bleeding ,operation time ,postoperative pain ,difficulty eating ,PSG testing ,immune function ,growth and develop-ment .Results Postoperative symptom in 1 weeks and 6 months ,compared with the traditional tonsillectomy ,30 cases of OSAHS patients were found that there were less bleeding ,shorter operative time ,mild postoperative pain ,no difficulty in feeding ,improving sleep-disordered breathing ,no decline in immune function ,good growth and development .Conclusion Short-term effect of tonsillot-omy on children′s OSAHS with tonsil hypertrophy is good by Ellman double-frequency radio-frequency machine ,it is a minimally invasive ,safe and effective surgical method children OSA HS .
10.Studying on the Interaction of Mental Health, Internet Addiction Disorder, Nociceptive Behavior and Sleep Quality among College Students in Lanzhou University

Objective:To study the status and relationship of mental health,internet addiction disorder,nociceptive behavior and sleep quality among college students,and to provide evidence for healthcare. Methods:A total of 2 868 college students were chosen from Lanzhou University and surveyed with questionnaires on the status of mental health, internet addiction disorder, nociceptive behavior and sleep quality. Results:Sleep quality and health risk behaviors in girls were better than boys (P<0.05) . Psychological state in medical students was best. Sleep quality score in engineering students was the highest. Health risk behaviors in science students had highest score. Internet addiction behavior in engineering students had the highest score. Mental health score in three or more students was less than that in junior students,while sleep quality and internet addiction behavior score was higher (P<0.01) . Conclusion:Mental health has a negative influence on nociceptive behavior and internet addiction disorder,while it has a positive effect on sleeping quality. A negtive correlation lies between sleeping quality and nociceptive behavior or internet addiction disorder. The departments should pay attention to the status of mental health,internet addiction disorder,nociceptive behavior and sleep quality among college students,and the appropriate prevention is necessary.
