1.Traditional Chinese Medicine Regulates Oxidative Stress to Prevent and Treat Osteoporosis: A Review

Osteoporosis is a common bone metabolic disease, which is mainly characterized by the decrease in the number of bone trabeculae and the destruction of bone tissue microstructure, leading to increased bone fragility and fracture risks. This disease is common in postmenopausal women, elderly men, diabetes patients, and obese people. Due to the lack of awareness to prevent bone losses and the limitations of bone mass measurement methods, osteoporosis is only concerned when there are serious complications, which imposes a heavy burden on both patients and medical resources. Oxidative stress refers to the excessive production of highly active molecules such as reactive oxygen species and reactive nitrogen in the body subjected to harmful stimuli, leading to the imbalance between the oxidative and antioxidant systems and causing oxidative damage. Studies have shown that oxidative stress can increase the generation and activity of osteoclasts and inhibit the differentiation of osteoblasts, thus playing a role in the occurrence and development of osteoporosis. Traditional Chinese medicine (TCM) is considered an effective antioxidant that can alleviate oxidative stress-induced osteoporosis by regulating a variety of signaling pathways. Studies have shown that TCM can alleviate oxidative stress and promote bone angiogenesis and osteogenesis by regulating the phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt), nuclear factor-kappa B, and nuclear factor erythroid 2-related factor (Nrf2) signaling pathways. TCM alleviates oxidative stress and promotes osteogenesis by regulating the Nrf2, PI3K/Akt/mammalian target of rapamycin, and secreted glycoprotein Wnt/β-catenin signaling pathways. In addition, TCM regulates NF-κB, mitogen-activated protein kinase, and receptor activator of nuclear factor kappa B (RANK)/RANK ligand/osteoprotegerin signaling pathway to alleviate excessive bone resorption induced by oxidative stress. This paper systematically summarizes the literature on the prevention and treatment of osteoporosis by TCM or its active ingredients via the above-mentioned signaling pathways to reduce oxidative stress in recent years. It briefs the possible molecular mechanisms of oxidative stress regulation-related signaling pathways to cause osteoporosis. In addition, this paper discusses the effects and mechanisms of TCM on bone angiogenesis, osteogenesis, and bone resorption by reducing oxidative stress through the regulation of related signaling pathways, aiming to provide a theoretical basis for the research and clinical treatment of osteoporosis.
2.Efficacy of dronedarone combined with metoprolol on elderlv patients with atrial arrhythmia

Objective To investigate the efficacy of dronedarone combined with low dose metoprol-ol in the treatment of atrial tachycardia.Methods A total of 175 elderly patients with atrial tachy-cardia admitted in Northern Jiangsu People's Hospital during January 2020 to January 2022 were enrolled and then randomly divided into dronedarone group(n=57),metoprolol group(n=55)and combined group(dronedarone+metoprolol,n=48).The frequency,duration and symptom changes of arrhythmia were compared before and after treatment.Results After 3 and 6 months of administration,the total effective rate was significantly higher in the combination group than the dronedarone group and the metoprolol group(P<0.05).The frequency of atrial fibrillation(AF)attacks[(2.31±1.78)/48 h vs(11.56±18.68)/48 h],AF duration[(4.86±6.73)h/48 h vs(10.92±9.61)h/48 h],atrial flutter(AFL)attacks[(2.33±1.53)/48 h vs(4.33±1.53)/48 h]and AFL duration[(5.15±4.87)h/48 h vs(21.54±20.08)h/48 h]in the combined group,and AFL duration[(2.75±1.94)h/48 h vs(10.29±8.04)h/48 h]in the dronedarone group were reduced after 6 months of treatment(P<0.05).Conclusion In the treatment of atrial tachycardia,dronedarone combined with low dose metoprolol can not only obviously improve symptoms,but also significantly reduce the atrial premature and atrial tachycardia attacks,decrease the number and duration of AF and AFL attacks,and control heart rate effectively.
3.Correlation between serum PEDV S1 IgG antibody levels and neutralizing antibody levels in sows

The aim of this study was to investigate the correlation between porcine epidemic diar-rhea virus(PEDV)S1 IgG antibody levels and neutralizing antibody potency in sow sera.Sera from 5 PEDV-infected farms with a clear immune background,5 non-infected farms and 5 infected farms with an unclear immune background,and sera from return-fed reserve pigs,totaling 716 copies,were collected and measured,and the correlation between PEDV S1 IgG antibodies and neutralizing antibodies was analyzed.The results showed that the PEDV S1 IgG and neutralizing antibodies of sow sera showed highly significant positive correlation,the correlation coefficient was 0.892(P<0.000 1).Previous studies have shown that the level of PEDV neutralizing antibodies in sow serum correlates with the ability of piglets'maternal antibodies to resist PEDV infection.Therefore,the a-bility of maternal antibodies against PEDV in piglets can be evaluated by detecting PEDV S1 IgG antibodies in the serum of sows.In 10 PEDV-infected farms,the neutralizing antibodies to PEDV in the sera of sows after immunization were generally high,and the S1 IgG antibodies were also high,and their S/P values were higher than 3.5 in 66.9%of the farms(347/519),and the highest anti-body levels were found in the four farms in which PED did not occur,whereas the neutralizing an-tibodies in the immunized sows in the five PEDV-uninfected farms were generally low,and their S1 IgG antibodies were also low,and only 8.1%(13/161)having S/P values higher than 3.5.The re-sults suggest that most sows in PEDV-infected farms can provide good immunoprotection to pig-lets after immunization,while pigs in PEDV-uninfected farms need further immunization if they need to achieve a higher level of immunoprotection.The present study provides a substantial clini-cal basis for the use of PEDV S1 IgG antibody levels to assess the effectiveness of PEDV antibody protection in swine herds.
4.Effect of dabigatran etexilate in elderly patients with non-valvular atrial fibrillation

Objective To investigate the clinical application of dabigatran etexilate in elderly patients with non-valvular atrial fibrillation. Methods Clinical materials of elderly non-valvular atrial fibrillation patients with dabigatran etexilate in the Department of Cardiology of the Subei People's Hospital Affiliated to Yangzhou University from January 2021 to June 2022 were collected, and according to the relevant guidelines, clinical application, laboratory indicators and clinical outcomes were statistically analyzed. Results Among 150 patients, 14 cases had no indications for medication and 12 cases had contraindication; 93 cases had insufficient dosage, 9 cases had excessive dosage, 13 cases had unreasonable frequency of administration, and 20 cases had unreasonable conversion with other anticoagulant drugs. 5.The influencing factors of surgically induced astigmatism in phacoemulsification

【Objective】 To investigate the influencing factors of surgically induced astigmatism (SIA) in phacoemulsification. 【Methods】 Cataract phacoemulsification was performed in 69 patients with regular corneal astigmatism from December 2021 to March 2022. SIA of the anterior corneal surface, posterior corneal surface and simulated keratometry (SimK) in the diameter range of 3 mm, 5 mm and 7 mm centered at the apex of the cornea was observed. Pearson correlation analysis was used to analyze SIA with age, axial length (AL), anterior chamber depth (ACD), white-to-white (WTW), and central corneal thickness (CCT). Multiple linear regression was used to analyze the influencing factors of SIA. 【Results】 The mean age of the 69 patients was (63.25±14.74) years old, and 28 (40.58%) were male. The analysis found that the |SIA| of SimK was negatively correlated with WTW (r=-0.265, P=0.028), the |SIA| of 3 mm, 5 mm, 7 mm anterior surface was negatively correlated with WTW (r=-0.320, P=0.007; r=-0.337, P=0.005; r=-0.287, P=0.017), and the |SIA| of 3 mm, 5 mm, 7 mm posterior surface was negatively correlated with AL (r=-0.390, P=0.001; r=-0.352, P=0.003; r=-0.317, P=0.008). Multiple regression analysis showed that WTW was negatively correlated with |SIA| of SimK, 3 mm, 5 mm, 7 mm anterior surface and 3 mm posterior surface (B=-0.261, P=0.047; B=-0.387, P=0.016; B=-0.323, P=0.009; B=-0.297, P=0.041; B=-0.085, P=0.049). 6.Comparative analysis of domestic classification management list for clinical use of antibiotics versus WHO AWaRe classification list of antibiotics

OBJECTIVE To provide reference and suggestions for dynamic adjustment of classification management lists for clinical use of antibiotics and the promotion of rational use of antibiotics. METHODS The latest version of provincial classification management lists for clinical use of antibiotics were aggregated into the “national list”, which was compared with 2021 WHO AWaRe classification list of antibiotics (hereinafter referred as to “AWaRe classification list”) to make a descriptive statistical analysis about the number of different classes of antibiotics in the two lists and their differences. RESULTS Based on the different classification principles, 262 kinds of antibiotic preparations in the national list were classified into non-restricted (84), restricted (83) and highly-restricted classes (95), and 258 kinds in the AWaRe classification list were classified into access (87), watch (142) and reserve classes (29); 182 kinds of antibiotic preparations were both included in the two lists. In the national list, among the non-restricted antibiotic preparations, 36 kinds belonged to access class, 30 belonged to watch class and 1 belonged to reserve class; among restricted antibiotic preparations, 7 belonged to access class, 46 kinds belonged to watch class and 3 belonged to reserve class; among highly-restricted antibiotic 82805019。E-mail:yyy211anne@163.com preparations, 9 belonged to access class, 35 belonged to watch class and 15 kinds belonged to reserve class. Among them, 91 kinds of antibiotic preparations were not recommended by WHO (20 kinds) or not included in the AWaRe classification list (71 kinds). CONCLUSIONS The classification methods of two lists are different in classification principles and grading of some similar drugs. The classification management list of antibiotics is one of the key points of antibiotics management, more research is needed in the future to provide sufficient evidence for optimizing antibiotics classification management.
7.Analysis of antibiotic use and rationality for children in primary healthcare institutions from Beijing

OBJE CTIVE To inv estigate the antibiotic use and rationality for children in community health service institutions from Beijing ,and to provide reference for promoting rational use of antibiotics in primary healthcare institutions. METHODS All the prescriptions for children from primary healthcare institutions in 2019 were extracted from prescription review system of community health service institutions in Beijing. The use of antibiotics was described according to the related indicators of the World Health Organization/International Network for Rational Use of Drug (WHO/INRUD). The structure of antibiotics use was analyzed according to the anatomical therapeutic chemical (ATC)classification as well as the WHO AWaRe classification and diagnosis. RESULTS A total of 288 primary healthcare institutions and 10 422 prescriptions for children were included. The number of institutions in high-income areas ,middle-income areas and low-income areas were 119,80 and 89 respectively,and the number of prescriptions involved were 2 430,2 163 and 5 829 respectively,including 1 447 prescriptions involving antibiotics (13.9%). Among 1 447 prescriptions,the rate of prescriptions involving combined use of antibiotics was 1.4%(20 pieces);the rate of prescriptions involving antibiotics injection was 9.7%(141 pieces);4.8% antibiotics prescriptions were rated as unreasonable (69 pieces). The three most commonly used antibiotics were the macrolides (40.2%),the second-generation cephalosporins (26.5%) and the third-generati on cephalosporins (23.4%). The proportion of antibioti cs prescriptions from groups of access ,caution, reserve and not recommended were 9.1%,92.1%,0.3%,and 0, respectively. The rate of antibiotics prescriptions fortonsillitis was the highest (31.9% ). Among 69 irrationalantibiotics prescriptions ,main of them were irrational drug use (56 pieces,81.2%). CONCLUSIONS The rate of antibiotics prescriptions for children in primary healthcare institutions from Beijing is lower than the standard of WHO antibiotics prescription rate (20.0%-26.8%),but the use rate of antibiotics at caution grade is too high.
8.Analysis of Provincial Antibiotic Classification Management Lists in China

OBJECTIVE:To provide re ference for scientific management and rational use of antibiotics. METHODS :Provincial antibiotic classification management lists were retrieved from official websites of provincial health administrative departments , CNKI and other search engines from Apr. 2012 to Mar. 2020. The difference and adjustment of those lists were analyzed descriptively. RESULTS & CONCLUSIONS : Since April 2012, 30 provinces released provincial antibiotic classification management lists ,among which only 10 provinces updated the lists. In the provincial classification management lists ,the list of Jiangsu included the most antibiotics (208 kinds),that of Xinjiang involved the least (101 kinds). Among the 253 antibiotics included in the provincial lists ,134 antibiotics had two management levels ,and 19 antibiotics had three management levels. The 10 antibiotics with the most times of adjustment in the provincial lists mainly include nitimidazoles ,β-lautam compound preparations and quinolones. There was the phenomenon that the same antibiotics were simultaneously included or withdrawn from the lists ,and the grading management level was increased or decreased in the provincial adjustment. Provincial classification management lists updated slowly in China ,and the quantity and classification of drugs selected in the list were quite different. It is suggested that provincial health administrative departments should continuously optimizeand improve classification management list of antibiotics , and form a long-term mechanism of dynamic adjustment of list and inter-provincial evidence sharing ,so as to promote the rational clinical use of antibiotics ,and curb bacterial resistance.
9.The CT imaging diagnosis and surgical treatment of mixed epithelial and stromal tumors of the kidney

Objective:To investigate the CT features and surgical treatment of mixed epithelial and stromal tumor of the kidney (MESTK).Methods:From April 2015 to August 2018, 13 patients with MESTK confirmed by pathology at the First Affiliated Hospital, College of Medicine, Zhejiang University, were enrolled in this study and their clinical data were analyzed retrospectively. These patients included 3 males and 10 females, with age ranging from 22 to 80 years old and the median age of 37 years old. Two patients complained of lumbar discomfort with urinary urgency and another 2 patients presented with hematuria. Nine patients were asymptomatic and their renal lesions were detected by abdominal ultrasonography for physical examination or other reasons. There were 2 cases had a history of polycystic kidney disease, 1 patient took contraceptive for 2 years, and 3 cases took antihypertensive agents for 5-10 years. The other patients denied any administration of hormone or other medicine for long time. The urine routine test was normal for these patients except for 2 patients did not take this test before operation. The creatinine value ranged from 45 to 486 μmol/L, with the median value of 67 μmol/L. The scope of glomerular filtration rate (GFR) was 6.98 to 132.28 ml/min, with the median value of 109.28 ml/min. The preoperative computer tomography (CT) showed single neoplasm in 11 patients and no obvious neoplasm was found in 2 patients who had polycystic kidney disease. Moreover, these lesions presented cystic neoplasm for 8 cases, cystic-solid neoplasm for 2 cases and solid neoplasm for 3 cases. Long diameter was measured about 2.5 to 7.5 cm, with the median of 5 cm, and calcification was found in 4 cases. These tumors manifested mild to moderate enhancement and delayed-enhancement for tumor parenchyma and intracapsular septum. The CT attenuation value was from 20 to 55 Hounsfield unit (HU). Base on the R. E.N.A.L. score system, these renal lesions got 4 to 10 points, with the median value of 7 points. Before operation, just 1 case was diagnosed as angiomyolipoma and 2 cases were diagnosed as polycystic kidney disease. No definitive diagnosis was made for the other 10 cases by CT imaging. In addition, the tumor staging based on preoperative imaging was made at T1a for 8 cases and T1b for 3 cases. Among these cases, 2 patients with polycystic kidney disease underwent open radical nephrectomy due to recurrent hematuria and uremia. One patient underwent laparoscopic radical nephrectomy due to perirenal fat adhesion. Partial nephrectomy was performed in remaining 10 patients by open surgery for 6 patients, robotic surgery for 3 patients and laparoscopic surgery for 1 patient. These renal tumors and 0.5-1.0 cm surrounding normal renal parenchyma were removed during operation.Results:In this study, 8 patients had blood loss of 30 to 100 ml during open operation, and 5 patients had blood loss of 20 to 100 ml during laparoscopic or robotic surgery. The warm ischemic time during partial nephrectomy was around 17 to 40 min, with the median value of 20 min. All of 13 cases were diagnosed as MESTK by postoperative pathology and the surgical margin was negative. There were no serious complications or special treatment after operation. The follow-up time ranged from 12 to 41 months, with the median time of 21 months. Obvious signs of tumor recurrence or metastasis were not detected in 13 cases during follow-up.Conclusions:CT images of MESTK mostly presented cystic or cystic-solid lesions, and solid lesions were relatively rare. This disease always showed mild to moderate enhancement and delayed enhancement of septal or tumor parenchyma. If there is coarse calcification in the lesions, it is more likely to be diagnosed as MESTK. Partial nephrectomy is preferred and radical nephrectomy should be considered when perirenal fat adhesion is suggested by the preoperative CT images.
10.Systematic Review of Health and Economic Burden Caused by Antibiotic Resistance in China

OBJECTIVE:To provide scientific basis for evaluating the burden caused by antibiotic resistance (AbR)and reference for policy making on crubing AbR in China. METHODS :Databases including CNKI ,Wanfang database ,VIP,PubMed, Scopus,Medline and EconoLite from Jan. 1st 2016 to Aug. 10th 2020 were searched to collect studies on burden caused by AbR in China . After independent literature screening anddata extraction ,Newcastle-Ottawa scale (NOS)was used to evaluate the literature quality ,and a descriptive analysis was conduced to evaluate the health and economic burden caused by AbR. RESULTS :A total of 27 Chinese and English literatures were included. The NOS scores of i ncluded literatures were 4-6,and all of them were retrospective case-control study ; the patients were divided into case group (resistance infection ) and control group (susceptible infection or non-infection ); mortality,length of stay and medical expenditure were commonly applied as the measurement indexes. In the included studies ,the mortality of patients infected with AbR bacteria was 0.7-12.0 times that of patients infected with susceptible bacteria ;the mean or medium value of total length of stay was 0.9-2.5 times that of patients infected with susceptible bacteria ;the mean or medium of total medical expenditure was 1.0-2.7 times that of patients with susceptible bacteria infection. The differences in these indicators were greater between patients infected with AbR infections and those without becterial infections. CONCLUSIONS :Bacterial drug resistance could increase the health and economic burden. However ,the existing relevant studies were mainly single center researches,the sample representation was insufficient ;the research design did not adjust for time-dependent bias ;the repeatability was low ,and the perspective of evalution was limited. It is urgent to carry out multicenter studies with higher quality to comprehensively evaluate the health and economic burden caused by AbR in China.
