1.Mechanism of action of hesperidin and its derivatives in the management of spinal cord injury: a review

Spinal cord injury is a central nervous system disorder caused by direct or indirect trauma to the spinal cord, with an increasing incidence year by year. Neuroinflammation, oxidative stress, and local edema following spinal cord injury can lead to motor, sensory and cognitive impairment in patients, resulting in an extremely high disability rate. Conventional treatments primarily involve surgery combined with pharmacotherapy, yet clinical outcomes remain unsatisfactory. Hesperidin, a citrus-derived flavonoid and its derivatives (neohesperidin, hesperetin, etc.) exhibit anti-inflammatory, antioxidant, anti-apoptotic, and pyroptosis-inhibitory properties. When used in the management of spinal cord injury, they can effectively improve post-injury motor dysfunction. However, hesperidin and its derivatives still face problems such as poor blood-brain-spinal cord barrier penetration and incomplete understanding of their key mechanisms, limiting the efficacy of monotherapy with hesperidin and its derivatives in the management of spinal cord injurg. To this end, the authors reviewed the research progress on mechanism of action of hesperidin and its derivatives in the management of spinal cord injury, aiming to provide a reference for its future research and clinical application.
2.Clinical characteristics of co-infection of Talaromyces marneffei and non-tuberculous Mycobacterium in HIV-negative patients

Objective To analyze the clinical characteristics of co-infection of Talaromyces marneffei(TM)and non-tuberculous Mycobacterium(NTM)in human immunodeficiency virus(HIV)-negative patients.Methods Clinical data of 8 HIV-negative patients with co-infection of TM and NTM in a hospital from 2019 to 2022 were co-llected.Clinical manifestations,auxiliary examination,treatment and prognosis were retrospectively analyzed.Results Among the 8 patients,5 were females and 3 were males,with an average age of(52.25±12.31)years old.All patients presented TM and NTM disseminated infection.The major involved organs were lung(100%),lymph nodes(87.5%),and skin(75.0%).Clinical symptoms mainly included cough and expectoration(87.5%),fatigue(62.5%),joint and lumbosacral pains(62.5%),fever(50.0%),as well as skin and soft tissue abscess(50.0%),etc.Anti-interferon-γ(INF-γ)autoantibodies were detected in 4 patients and the results were positive.All 8 patients(100%)had pulmonary lesions,with chest CT mainly showing spots,patches,and striped shadows in both lungs.Among them,7 cases(87.5%)had increased and enlarged mediastinal lymph nodes,4 cases(50.0%)had pleural thickening and pleural effusion,2 cases each(25.0%for each)were accompanied by pulmonary mass shadows,bronchial stenosis,as well as increased and enlarged hilar lymph nodes.One case each(12.5%for each)had pulmonary cavity formation,bronchiectasis,and pericardial effusion.Conclusion The co-infection of TM and NTM in non-HIV patients presents disseminated infection,with multiple clinical symptoms.Chest imaging shows a wide variety of pulmonary lesions.It is prone to miss diagnosis in clinic,and the effect is not ideal after treatment for single pathogen infection.

5.Effect of shear wave elastography in predicting pathological responses to neoadjuvant chemotherapy in patients with breast cancer

Objective:To investigate the clinical value of shear wave elastography (SWE) in predicting pathological responses to neoadjuvant chemotherapy in breast cancer.Methods:According to the postoperative pathological responses, 56 patients who received neoadjuvant chemotherapy followed by surgical excision in the Fujian Cancer Hospital from August 2019 to September 2020 were divided into responders and non-responders. The relative change rates of tumor maximum diameter(ΔD2, ΔD4) and SWE stiffness (ΔEmax2, ΔEmax4, ΔEmean2, ΔEmean4) were assessed before NAC and after different NAC cycles (t2, t4). Clinical information, including age, T, N stages, ER, PR, HER2, Ki67, and molecular subtype were also considered as the variables. The independent influencing factors of pathological responses after neoadjuvant chemotherapy were obtained by logistic regression analysis and diagnostic test was carried out.Results:There were 23 cases as responders (41.0%, 23/56), and 33 cases as non-responders (58.9%, 33/56). Results of multivariate analysis showed ΔEmax4 and HER2 index were independent influencing factors of pathological responses ( OR=1.11, P<0.001; OR=31.81, P=0.002). Area under curve of the ΔEmax4 (AUC: 0.869, 95% CI: 0.746-0.941) was higher than that of HER2 (AUC: 0.690, 95% CI: 0.545-0.834). The combination of ΔEmax4 and HER2 gave the best prediction of pathological responses (AUC 0.930, 95% CI: 0.829-0.981). the sensitivity, specificity, diagnostic accuracy, postive predictive value, and negative predictive value were 78.26%, 96.97%, 75.23%, 94.73%, and 86.49%, respectively. Conclusions:ΔEmax4 and HER2 are independent predictors of pathological responses after neoadjuvant chemotherapy for breast cancer. Combined ΔEmax4 and HER2 can improve the predictive diagnostic efficacy of pathological responses to chemotherapy for breast cancer.
6.Application of REDCap Software in Radiotherapy Quality Control Platform REDCap

In order to digitize the radiotherapy quality control data, convenient to carry on the dynamic quantitative analysis to the equipment condition, this paper presents a low-cost open radiotherapy quality control platform based on REDCap. The platform uses the REDCap data acquisition system as a carrier to achieve the digital storage of this data, anytime, anywhere data entry, and can be easily derived for data analysis by creating REDCap research projects and inputting radiotherapy quality control data. It is proved that REDCap can be used in the construction of radiotherapy quality control platform.
7.Porting Radiotherapy Software of Varian to Cloud Platform

To develop a low-cost private cloud platform of radiotherapy software. First, a private cloud platform which was based on OpenStack and the virtual GPU hardware was builded. Then on the private cloud platform, all the Varian radiotherapy software modules were installed to the virtual machine, and the corresponding function configuration was completed. Finally the software on the cloud was able to be accessed by virtual desktop client. The function test results of the cloud workstation show that a cloud workstation is equivalent to an isolated physical workstation, and any clients on the LAN can use the cloud workstation smoothly. The cloud platform transplantation in this study is economical and practical. The project not only improves the utilization rates of radiotherapy software, but also makes it possible that the cloud computing technology can expand its applications to the field of radiation oncology.
8.A Software Platform of Treatment Planning System for Radiotherapy Snake Robot

A software platform integrated of radiation treatment planning system and navigation system has been developed for a newly-developed snake robot. This platform is developed in a module-plugin form based on 3D Slicer medical image visualization and analysis application platform and an open-source toolkit SlicerRT. The platform realized function modules of image access and visualization, image segmentation, radiation path planning, treatment planning evaluation and optical navigation, and has provided the snake robot an extensible software experiment platform. It is beneficial for the brachytherapy research work and experiments.
9.Recent advances of gold nanoparticles for cancer radiotherapy

Based on a review of currently published 40 papers ( 20 published in recent 5 years, 20 published in recent 10 years) on gold nanoparticles for cancer radiotherapy, the general characteristics, theoretical studies, in vitro experiment, in vivo experiment,and the clinical prospects of targeted radiotherapy with gold nanoparticles were reviewed. Three key aspects guarantee further investigation for the full understanding of the radiosensitization effect of gold nanoparticles:the cellular localization and tissue distribution of nanoparticles and influential factors;the micro dose enhancement effect of gold nanoparticles, and the molecular biological mechanism. More cross disciplinary collaboration, research, development and translation are needed before gold nanoparticles are put into clinical trials.
10.Commissioning of Axesse accelerator for volumetric modulated arc therapy

Objective To test the accuracy and reliability of Axesse accelerator for volumetric modulated arc therapy (VMAT).Methods The accuracy and reliability of Axesse accelerator for VMAT were tested in a stepwise manner, from the simple to the complex and from the part to the whole.For the parts of the system, the stability of dosimetric output and the position accuracy of multi-leaf collimator (MLC) were tested.For the process of the system, the variable VMAT dose rates and gantry speed modulation, the MLC speed and dose rate modulation, and dosimetric verification in patients were tested.Results Compared with fixed gantry irradiation, the variation in dosimetric output was below 1.0% for rotary irradiation including the slide rotary irradiation of the dynamic MLC.The MLC position error of 0.5 mm was visible using the electronic portal imaging system of Axesse, iViewGT 3.40.The MLC position accuracy was within 1 mm for fixed gantry irradiation and rotary irradiation.In the range of the dose rate applied in clinical practice, the testing results of variable VMAT dose rates and gantry speeds as well as variable dose rates and MLC speeds showed that the variation between different strip-field beam intensities was below 2.0%.Using a gamma criterion of 3 mm/3%, the pass rates in dosimetric verification of patients with cervical cancer, prostate cancer, and breast cancer were 96.52%, 95.72%, and 98.83%, respectively.Conclusions The Axesse system can precisely control MLC motion, variable dose rates, and gantry speeds in VMAT.The Axesse system is accurate and reliable for VMAT.
