BACKGROUND:Liuwei Dihuang Wan takes"three tonifying and three reducing effects"as its compatibility feature to nourish yin and tonify the kidneys, while Zuogui Wan takes"seeking yin in yang"as its compatibility feature to nourish yin and tonify the kidneys by promoting yang. Both of them belong to the same method of nourishing yin and tonifying the kidneys, and have better curative effects at the symptomatic and cellular molecular levels. OBJECTIVE:To observe the effects of Liuwei Dihuang Wan and Zuogui Wan in bone metabolism, and to explore their mechanism of action in the osteoprotegerin(OPG)/receptor activator of nuclear factor-κB ligand(RANKL)osteoblastic pathway. METHODS:Thirty-two Sprague-Dawley rats were randomized into model,Liuwei Dihuang Wan,Zuogui Wan,and sham operation group, with eight rats in each group.Osteoporosis models were prepared using removal of both ovaries in the first three groups.Starting at 30 days postoperatively, rats in the Liuwei Dihuang Wan group were gavaged with Liuwei Dihuang Wan 1.125 g/kg/d;rats in the Zuoqui Wan group were gavaged with Zuogui Wan 2.25 g/kg/d;and rats in the sham operation group and the model group were gavaged with saline 10 mL/kg/d.After 12 weeks of gavage, the rat tibia was taken to measure bone mineral density. The serum levels of estrogen, bone alkaline phosphatase, and cAMP/cGMP were measured using ELISA,and the expression of OPG/RANKL in the femur was detected using western blot. RESULTS AND CONCLUSION:Compared with the sham operation group, the model group showed a decrease in bone mineral density and levels of estrogen and bone alkaline phosphatase(P<0.05)and an increase in cAMP/cGMP level(P<0.05).Compared with the model group, the Liuwei Dihuang Wan group and the Zuogui Wan group significantly increased bone mineral density(P<0.05)and bone alkaline phosphatase levels(P<0.05);the Zuogui Wan group significantly decreased cAMP/cGMP levels(P<0.05)and upregulated OPG expression(P<0.05);the Liuwei Dihuang Wan group upregulated OPG expression and downregulated RANKL expression(P<0.05);and both groups were unable to significantly increase estrogen levels(P>0.05).To conclude,Zuogui Wan,which seeks yin from yang, can effectively increase the expression of OPG but cannot downregulate the expression of RANKL.However,Liuwei Dihuang Wan,which has three tonifying and three reducing effects, can bidirectionally regulate the expression of OPG and RANKL.This result suggests that Liuwei Dihuang Wan can significantly inhibit osteoclastic function compared with Zuogui Wan,and further research is needed to verify this conclusion.

Chinese Pediatric Emergency Medicine 2025;32(3):191-196
Objective:To assess the nutritional status and risk of children with cyclic vomiting syndrome(CVS)and to provide a clinical evidence for nutritional support. Methods:A total of 160 children diagnosed with CVS and 160 healthy controls were prospectively enrolled in the study, who were admitted to Beijing Tsinghua Changgung Hospital from April 2021 to February 2024.Nutritional status was assessed by anthropometric indices(height, weight, and body mass index),and it was expressed in standard deviation score(Z score).Nutritional risk screening tool STRONGkids was applied to nutritional risk screening, and the laboratory assessment(albumin,pre-albumin,hemoglobin,urea nitrogen and creatinine)were measured. Results:Among the 160 cases, there were 73 males and 87 females, including 18 cases of mild seizures and 142 cases of moderate and severe seizures, with the average onset age of(47.54±2.96)months and an average diagnosis age of(74.89±3.68)months.The average duration from the first attack to diagnosis was(27.35±2.36)months,and the average course of the disease was(41.35±2.63)months.The malnutrition rate of children in CVS was 36.9%(59/160),of which 18.1%(29/160)was severely malnourished. There was a statistically significant difference in the weight-for-age Z-score(WAZ),height-for-age Z-score(HAZ),and body mass index-for-age Z-score(BAZ)between the CVS group and the control group( P<0.001).We found a statistically significant difference in laboratory indicators(albumin,pre-albumin,hemoglobin,urea nitrogen and creatinine)between the two groups(all P<0.001)and hemoglobin was positively correlated with WAZ,HAZ and BAZ( r=0.346,0.250,0.277,all P<0.01).Using the STRONGkids nutritional risk screening tool, there were 43 cases with moderate nutritional risk and 117 cases with high nutritional risk, and children with high nutritional risk were likely to have lower WAZ,HAZ and BAZ than the children with moderate nutritional risk, and there was a significant difference in WAZ,HAZ,BAZ,hemoglobin, and creatinine between the two groups( P<0.05).And there was a correlation between the degree of CVS attack and nutritional risk grouping( r=0.543, P<0.001). Conclusion:Children with CVS have a high incidence of malnutrition, and the serum albumin, prealbumin, hemoglobin, urea nitrogen and creatinine levels are of great value for nutritional assessment. The STRONGkids score method helps to evaluate nutritional risk in children with CVS.

Objective:To investigate the impact of different CT reconstruction kernels on the quantitative analysis of small pulmonary vessels in patients with chronic obstructive pulmonary disease (COPD) and high-risk patients. Methods:This study was a cross-sectional study. Clinical and imaging data of 73 COPD and high-risk patients visiting the China-Japan Friendship Hospital between March and April 2024 were retrospectively analyzed. All patients underwent high-resolution CT of the chest and pulmonary function tests, with the ratio of forced expiratory volume in one second to forced vital capacity (FEV 1/FVC) obtained. The raw CT data were reconstructed using different kernels: the FC86 group used the adaptive iterative dose reduction(AIDR) 3D standard lung sharp reconstruction algorithm, the FC18 group used the AIDR 3D standard Body standard reconstruction algorithm, the advanced intelligent clear-IQ engine(AiCE) Lung group used the AiCE deep learning reconstruction algorithm for lung, and the AiCE Body group used the AiCE deep learning reconstruction algorithm for body. Image signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) and image noise were calculated. The pulmonary vessel segmentation & Measurement software was used to segment and extract pulmonary arteries and veins of four groups in thin-slice whole-lung CT imaging, obtaining the ratio of small pulmonary arteries (%V artery<5) and the ratio of small pulmonary veins (%V vein<5). The One-way repeated measures ANOVA or Friedman test was used to compare the differences in SNR, CNR, image noise, %V artery<5, and %V vein<5 among the four groups, followed by Bonferroni post hoc or Bonferroni-Dunn test with P-value correction to analysis differences between subgroups. The correlations between %V artery<5 and FEV 1/FVC, as well as between %V vein<5 and FEV 1/FVC were analyzed using Spearman rank correlation analysis in all four groups. Results:The overall differences in image noise, SNR, and CNR in the AiCE Lung, AiCE Body, FC18, and FC86 groups were statistically significant ( P<0.001). Except for the difference in CNR values between the AiCE Lung group and the FC18 group, which was not statistically significant ( P=0.192), all differences were statistically significant ( P<0.016 7). The overall differences in %V artery<5 values and %V vein<5 values in the AiCE Lung, AiCE Body, FC18, and FC86 groups were statistically significant ( P<0.001). The %V artery<5 and %V vein<5 values in the FC18 group were lower than those in the AiCE Lung, AiCE Body, and FC86 groups ( P<0.016 7), and the rest of the differences were not statistically significant ( P>0.016 7). %V artery<5 and %V vein<5 were positively correlated with FEV 1/FVC in all 4 groups ( P<0.05), with the highest correlation coefficient between %V vein<5 and FEV 1/FVC in the AiCE Body group ( r=0.501, P=0.001). Conclusions:DLR-AiCE-based kernel reconstruction optimizes image quality and significantly affects the results of quantitative parameters of small pulmonary vessels. The reconstruction kernel prioritized for quantitative analysis of small vessels within the lungs in COPD based on the CT scanner in this study is AiCE Body.
